Welcome and Introduction Welcome to Sherwood Forest Hospitals NHS Foundation Trust.

This publication is intended to help you, your carer and your relatives and friends understand how your discharge or transfer from hospital takes place. We hope you can help us so that this happens as efficiently and smoothly as possible.

The right place for your care Your local hospital works in partnership with other hospitals in the region and your local authorities. Together they support each patient to receive the right treatment at the right place and by the right professional.

An acute hospital trust provides consultant led health services within the National Health Service.

Acute care is for patients who require emergency, medical or surgical services within a hospital environment. Sherwood Forest Hospitals provides care for patients who require this level of care.

Hospitals are the right place to be when you are in need of specific medical or surgical treatment.

However, when your treatment has been completed, it is important that your stay is not delayed for the following reasons: ■ Beds are needed for people who are very unwell, and who may be waiting in the Accident and Emergency department for a bed to become available

■ Some people find it harder to return home the longer they stay in hospital.

It is good practice to start planning for your discharge as soon as you come into hospital or even before you arrive. This means that we can begin to: ■ Assess what your needs are likely to be when you are ready to leave ■ Involve relevant staff who can help in meeting those needs (e.g. social worker, occupational therapist, physiotherapist, district nurse) ■ Make arrangements for equipment or services which need to be in place when you are medically ready to leave ■ Make sure that your carer is given any information, help or advice that is needed to aid your recovery.

All staff will work with you and your carer or relatives, as a team, in order to discuss your discharge with you.

■ P eople awaiting surgery, both urgent and nonurgent, may have their operations cancelled if a bed is unavailable

We need to discuss any concerns you have at the earliest opportunity, e.g. your safety at home, managing your personal care or domestic arrangements, your ability to move around and any equipment you may need.

■ T here is a risk of acquiring infections in hospital, so leaving at the earliest opportunity means this is less likely

You will be given an expected date for discharge based on when it is expected that your treatment will be completed. It is important that everyone

involved is aware of this date so that we can all work towards it, and so that the necessary arrangements can be made.

Relatives or carers coming to collect you can meet you in the Discharge Lounge or a nurse can take you to the main reception area.

These may include:

Finally, we understand that any hospital stay can be stressful and we need to support you and your carer during your time here. We will discuss with you the sort of help you and your carer may need when you return home, to make the transition as easy as possible.

■ Transport home. Patients are normally expected to arrange their own transport ■ S uitable clothing and footwear if you are not already using them in hospital ■ Access to a key to your property ■ Adequate basic food stocks ■ Adequate heating in your home ■A dequate support for your carer or family members.

We aim for patients to be discharged before 11am. If this is not possible it is likely you will move to the Discharge Lounge so we can get the bed ready for the next patient. The Discharge Lounge is a comfortable and safe place to wait and is staffed by nurses.

Once the doctors have written up your discharge medicines, the Pharmacy department will dispense the medicines for you to take home and deliver them to the Discharge Lounge. If you have any questions about your medicines please ask to speak to a pharmacist or a member of the nursing team.

Your Transfer of Care Practitioner is a member of the team who may, if necessary, meet you on the ward to begin planning your discharge from hospital, together with the ward staff.

For most patients, being discharged from hospital to their home is straightforward.

Some may need information and advice or may need the support of Social Services.

Some people have ongoing health and social care needs which require a more detailed assessment. Staff from health and social care will work together to plan and deliver services to support them after discharge.

This guide explains some of these processes and explains the role of the hospital and Social Care team in the hospital discharge process.

Leaving hospital You will be transferred from hospital when your consultant led team decide that you are clinically ready to leave hospital and that you no longer require an acute hospital bed.

For the majority of patients, you will go home from hospital with no additional support required. In some cases you may require some extra support to regain your independence. Your hospital team will help you, your family and carers to access these services if you need them. A more detailed description is available in the information

for patients and carers leaflet ‘Planning additional support to improve your recovery outside of hospital’, the content of which is also contained in this booklet.

Your safe and timely transfer from hospital will also allow new patients who need acute hospital treatment to be admitted without delay.

We fully support your transfer out of hospital for the following reasons: 1. To support you to regain your independence 2. Some people find it harder to return home the longer they stay in hospital 3. Acute beds are needed for people who are very unwell, and delays in transfer may result in patients waiting in the Emergency department for a bed to become available.

4. Although we work hard to ensure the highest standards of care, there is a risk of acquiring infections in hospital. Leaving hospital as soon as you are medically fit means this is less likely and reduces the risk of any complication.

When you leave Hospital It is important to know that the majority of patients will be transferred home directly from hospital.

If you no longer require consultant led care and there is a delay in the start of your care package or preferred community based placement, you cannot choose to remain in an acute hospital bed.

Continuing NHS Care If you have complex, ongoing healthcare needs, the team involved in your care must first consider whether you meet the eligibility criteria for continuing NHS Care.

Once this assessment is completed, a decision will be made with you and your family about a suitable place to transfer.

The information below explains how we can help when you need a little more care and support.

Our pledge Our aim is to help adults who need support to return home confidently and independently. We will work with you to plan your discharge home and retain your independence.

We will use every available resource to provide care close to home but it may be necessary in some cases to use facilities a little further away from your home within the Mansfield, Ashfield, Newark and Sherwood areas.

What happens next?

The multi-disciplinary discharge team will work with you and the Discharge Coordinator on your ward to plan your care needs for the next six weeks. This may be a convalescent bed in one of the community homes which offer this service. Your nurse will discuss the care home with you to ensure we plan the best support >> to meet your needs.

Options for your discharge support will include:

In a small number of cases some patients may be assessed as being unable to manage independently. This may require a longer term care option in a care home. An independent representative (advocate) can help you and your family (if appropriate) to discuss the options. It may help to move into a short term placement whilst suitable long term options are explored with you. Our discharge teams will provide support and advice about care options and will be happy to arrange an advocate for you if you prefer. We aim to ensure that your needs and wishes are met by fully discussing these with you and providing support to help you consider your options.

Community hospital You may require rehabilitation for a few weeks as an in-patient in a community hospital that has available beds. This may not be your nearest community hospital if they do not have a bed available. However, if they do we will always try to keep you close to home. It is important that we do not delay your rehabilitation and recovery so transfer to the community hospital as early as possible is vital.

Convalescent bed in the community It may be that your needs do not require the services provided by a community hospital (whose assessment indicates that the patient does not require high level nursing support). In this case a bed within a community care home that is able to provide convalescent beds will provide rehabilitation and support to improve your confidence and independence for a few weeks before transferring you home.

Before you go home, you will receive a comprehensive assessment of your physical health and social care needs. This will determine what additional support you might benefit from to enable you to be more independent at home.

This care at home (or in your residential home) will be coordinated by your local GP and the PRISM Integrated Care Team (made up of community Matron, social worker, physiotherapist and occupational therapists) who have access to a full range of services including district nursing, specialist long term conditions expertise, mental health support and self-care resources, to help you to confidently and safely 8

The staff responsible for your care will plan your discharge with you from the day you are admitted. This is important because we need to make sure that you receive the right care in the right place at the right time. Sometimes hospital isn’t the best place for your care and the homes that we work with provide safe and effective support.

Supporting people to leave hospital quickly and safely is a top priority so that: ■ You get the full benefit of your treatment ■ You get the best possible recovery and recuperation ■ We work with the community teams to ensure you are fully supported reducing the risk of being readmitted to hospital ■ You avoid picking up any other illnesses ■ We can ensure that you remain as independent as possible. Staff will work closely with you, your family or carers (if you would like them to be involved in planning your discharge) to work out what is best for you. We will use all the different services that are available to help you

recover your health and independence ■ I n a small number of cases it may be that some patients, with the support of the nursing team, decide that a long term care option is better for their health and well-being. In these cases an assessment of your nursing or social care needs will be completed to identify what support you need and any financial support which may be available ■M any people benefit from convalescent care following medical treatment. We will help people leave hospital safely as soon as their medical treatment is finished. Staff will work closely with you to help you to plan what is best for you, and offer nursing and social care in the community to help you remain independent at home after your period of convalescence ■ If you should need longer term care don’t worry. We can discuss the options and offer independent support to make the process easier.

Other people who can help you to plan your discharge: Sometimes it can be difficult to make decisions about going home and there are lots of things you may need to discuss with someone independent.

There are independent advocates who can work with you to discuss your options and help you to

collect information to help you make a decision.

POhWER is a charity and membership organisation that provides information, advice, support and advocacy to people who experience disability, vulnerability, distress and social exclusion.

Age UK Nottingham & Nottinghamshire offers an advocacy service within King’s Mill Hospital and there are other community advocacy services which our Patient Experience Team can put you in touch with.

The hospital will support you over a seven day period to make a suitable choice of home in line with the hospital policy.

What happens if I can’t decide on a long term placement?

If you are unable to find a suitable placement within 14 days, we can offer you the opportunity to move into a convalescent bed. This will allow a little more time to make your decision. It is important that you discuss any difficulties with the nurse who can arrange for more support in deciding on a placement.

It is important that you are cared for in the best environment to meet your needs once the treatment provided by the hospital is completed.

If you have any queries please speak to the Senior Nurse on duty or the Discharge Coordinator.

Therapies You may be referred to a therapist to assist in planning for your discharge from hospital.

This could be someone from a number of different professions; a dietician, occupational therapist, physiotherapist or a speech and language therapist. They will discuss with you and assess what you are able to do; and what you might need more help with. They will then discuss with you what treatment, help or support is available to improve your independence. The therapists will discuss their findings with the other members of the hospital team, and make a plan with you and your family for your discharge from hospital.

Dementia Lead Nurse Staff working at Sherwood Forest Hospitals have been trained in Dementia Awareness as part of their mandatory training. Staff working in specific

areas have undertaken further training in order to care for patients with dementia.

Dementia patients will receive a ‘This Is Me’ document for their relatives/carers to complete.

This document acts as a “Patient Passport” and should accompany the person wherever they are transferred to. The information within this document will assist the hospital staff to provide individualised care and to address specific care needs.

During your hospital stay we have a Dementia Lead Nurse who is available to provide support for patients’ relatives and carers.

If you require any additional support during your stay in hospital please ask the ward staff to contact the Dementia Lead Nurse.

The Friends and Family Test What is the Friends and Family Test?

The NHS wants to ensure that you have the best possible experience of care. The Friends and Family Test is a way of gathering your feedback about this experience and helping to drive improvement in hospital services.

When you receive care as an inpatient or in an Accident and Emergency (A&E) department, you will be given the opportunity to give your feedback by answering a simple question about your experience.

You can respond to the text message ‘STOP’. This will normally ensure you are no longer contacted in this way but if you do experience any problems please contact our Patient Experience Team who will ensure that this is addressed (see contact details below).

How will the results be used?

The results will provide a way for you to easily compare NHS hospitals so that you know where you and your family can get the best possible care.

Your hospital will gather the results and analyse them rapidly to see if any action is required. It will publish the results and you will also be able to see results for other hospitals to see how they compare.

The information will also give the NHS invaluable information on what patients think of services, which can be used to help make improvements if required.

Where will the results be published?

How will it work?

The results will be published on the NHS Choices website (www.nhs.uk). Hospitals may also publish their results in their annual reports and quality accounts.

When you are discharged, or within the 48 hours that follow, you will be asked to answer the following question: “How likely are you to recommend our ward/A&E department to friends and family if they needed similar care or treatment?”

You will be invited to respond to the question by choosing one of six options, ranging from ‘extremely likely’ to ‘extremely unlikely’.

It is really important to us that you tell us why you gave your answer, so please answer any followup questions.

Your answer will not be traced back to you, and your details will not be passed on to anyone, so please tell us exactly what you think. A member of your family or a friend is welcome to help you give your feedback to the question if you are unable to.

Does this replace the existing compliments or complaints procedure, or other forms of feedback used by the hospital?

No, you can still pass your compliments or complaints to your hospital in the normal way. Hospitals can continue to use existing ways of gathering feedback, in addition to the Friends and Family Test.

Going home from hospital?

Help with personal care and daily living tasks After a spell in hospital you may find that the every day living tasks you used to take for granted are now a bit more challenging. Our Homecare Service can help. It provides personal care and support with things such as getting up and going to bed, washing, dressing, grooming, cooking a meal, going to the toilet; in fact all of the things we tend to take for granted and need to do every day. The homecare service can also escort you, including to your follow up appointments if you

are feeling a bit anxious about going out on your own. We can also provide a shopping service and collect your pension if needed.

Information & Advice Often after a spell in hospital it is more difficult to cope with every day tasks. If you find that this is true, you may qualify for extra financial help. Our team of advisors are there to provide expert advice and information including welfare benefit calculations, and help to complete forms. They also have the ability to answer an extremely wide range of queries; from where can I get a tip-up-kettle to how can I get my house adapted to suit me. Whenever you want to find anything out from a trustworthy source – give us a call.

Products to make life easier When you come out of hospital we can arrange products designed to make it easier for you to get around your home such as rollators. We are able to arrange stair lifts, which will make all parts of your home accessible again.

Many older people worry about summoning help in an emergency, so we now can arrange a personal alarm. If you are in trouble, are feeling unwell or have a fall, you just push the button on a pendant or simple wrist watch type device and help can be on its way in moments. These alarms are perfect to provide peace of mind both for you and your family; who will know that you can summon help whenever you need it 24/7.

In addition help can be provided by our local district council who can fit a key safe to the outside of a property. These are designed to store keys in an outdoor environment allowing family members and care workers convenient access to a property.

What is NHS-funded nursing care?

NHS-funded nursing care is care provided by a registered nurse for people who live in a care home. The NHS will pay a flat rate contribution directly to the care home towards the cost of this registered nursing care.

Who is eligible for NHS-funded nursing care?

You may be eligible for NHS-funded nursing care if: ■ you are not eligible for NHS continuing healthcare but have been assessed as needing care from a registered nurse ■ y ou live in a care home registered to provide nursing care

How will my needs be assessed?

You should be assessed for NHS continuing healthcare before a decision is made about whether you are eligible for NHS-funded nursing care.

Most people don’t need a separate assessment for NHS-funded nursing care. However, if you do need an assessment or you haven’t already had one, your clinical commissioning group (CCG) can arrange an assessment for you. Find your local CCG.

Outcome of the assessment If you’re eligible for NHS-funded nursing care, the NHS will arrange and fund nursing care provided by registered nurses employed by the care home.

Services provided by a registered nurse can

include planning, supervising and monitoring nursing and healthcare tasks, as well as direct nursing care.

If you’re not eligible for NHS-funded nursing care and you don’t agree with the decision about your eligibility, you can ask your CCG to review the decision.

What is the rate of payment for NHS-funded nursing care?

NHS-funded nursing care is paid at the same rate across England. For 2015/16, the rate is £112.00 a week (standard rate).

Before October 1 2007, there were three different levels or bands of payment for NHSfunded nursing care – low, medium and high.

If you moved into a care home before October 1 2007, and you were on the low or medium bands, you would have been transferred to the standard rate from that date.

If you moved into a care home before October 1 2007, and you were on the high band, NHSfunded nursing care is paid at a higher rate. For 2015/16, the higher rate is £154.14 a week.

You’re entitled to continue on this rate unless: ■ you no longer have nursing needs ■ you no longer live in a care home that provides nursing ■ your nursing needs have reduced and you’re no longer eligible for the high band, when you would change to the standard rate of £112.00 a week, or ■ y ou become entitled to NHS continuing healthcare instead Content supplied by:

Paying for care What help is available?

Our factsheets explain the rules about local authority funding of care and explains what happens if your partner needs residential care in a care home.

Paying for permanent residential care Most people will be expected to pay something towards the costs of their accommodation and personal care from their income and capital.

If the local authority is involved in arranging your placement, the amount you will have to pay will be worked out via a means test, which is based on nationally set guidelines.

Your local authority will firstly do a care needs assessment to identify what help you need. Then they will carry out a means test to work out how much you have to pay towards your care home fees. Currently, if you have assets of more than £23,250, you will need to pay the full cost of your care. Your home won’t be included in this if a partner or close relative still lives there. Your income is also taken into account.

From April 2015, your local authority must provide a personal budget showing how much 14

they’ve calculated they should pay towards your eligible needs (the ones identified in the care needs assessment).

The Care Act The Care Act is the biggest change in social care in more than 60 years. It is being introduced in two parts in April 2015 and April 2016. It will change the way the social care system works.

There will be a care cap from April 2016. This means no-one will spend more than £72,000 of their own money on their care needs. The amount that you and the council spend on your needs will be added up in your care account.

Once this reaches £72,000, the council will pay for all your eligible needs.

From April 2016, the upper capital limit will also increase from £23,250 to £118,000.

Paying for temporary care in a care home Local authority charging rules differ when a stay in a care home is temporary, rather than permanent.

Some people go into a care home on a temporary basis to give themselves or their carers a break‚ or while they are recuperating from an illness.

Others enter a home temporarily for a ‘trial’ period to decide whether they want to live permanently in that home.

Temporary care may also be part of an ‘intermediate care’ package‚ which should be free of charge.

Paying for care if you have a partner Being part of a couple can affect a care home resident’s eligibility for local authority assistance with care home fees and both partners’ eligibility for social security benefits during the period in care.

At Age UK, we have produced a factsheet that addresses this issue and includes sections on the valuation of jointly owned capital, temporary admissions and where both members of a couple go into a care home.

Government guidance on the rights of a spouse in a care home to pass 50% of their private or occupational pension to their partner living at home are also included.

Direct payments When your personal budget is paid directly to you this is called a Direct Payment. Direct Payments allow you to arrange your care as you wish.

The Motability Scheme The Motability scheme allows some disabled people getting DLA, PIP or War Pensioners’ Mobility Supplement to obtain a car, powered wheelchair or scooter.

The scheme uses some or all of your mobility benefit payments to cover the cost of “contract hire” or “hire purchase” of an appropriate vehicle. You can also pay an extra amount of money if you want a more expensive vehicle.

With hire purchase, the price of the car will be agreed directly with the Motability dealer, and you will own the vehicle outright at the end of the agreement.

With contract hire, you won’t own a vehicle, but you will get a new car every three years,

full insurance for the driver and passengers, servicing, maintenance and repairs, vehicle excise duty, replacement tyres and breakdown cover.

Adaptations to the car can be made if needed, although there may be an extra charge for some adaptations. Under the Motability scheme, cars can also be adapted for people in wheelchairs.

If you don’t need or want a car, you can transfer your allowance to lease a scooter or powered wheelchair.

Support from Dementia Support Workers Dementia Support Workers can offer advice on accessing professional help.

We are in contact with a wide range of services and other advice centres, so we can put you in touch with specialists who can help you if your concerns are outside of our expertise. We can also refer you, or the person you care for, to services which provide for people with dementia – particularly if you are having trouble accessing this yourself.

Home visits We can arrange for a Dementia Support Worker to visit you in your own home and provide information, advice and support.

Information We can provide a wide range of advice sheets, which include specific information on different types of dementia, care issues and understanding the needs of the person with dementia.

Dementia Friends Programme Dementia Friends is a national initiative run by the Alzheimer’s Society. Funded by the government, it aims to improve people’s understanding of dementia and its effects.

Through the Dementia Friends programme, we aim to change the way the nation thinks, talks and acts to create ‘dementia friendly’ communities.

How does it work?

A group of volunteers will be trained to spread the word about what dementia is and what action individuals can take in their community, and recruit Dementia Friends.

What is a Dementia Friend?

A Dementia Friend learns about what it is like to live with dementia and then turns that understanding into action.

What is a Dementia Friends Champion?

Dementia Friends Champions are volunteers who, after attending a training course, run information sessions where they talk to people about being a Dementia Friend in their communities.

The Care Quality Commission (CQC) registers all care homes and home care agencies. You can find out which ones support specific groups of people, such as people with a learning disability or those living with dementia.

CQC’s Chief Inspector for Adult Social Care, Andrea Sutcliffe always uses ‘The Mum Test’: is a care home safe, caring, effective, responsive to people’s needs and well-led? In other words, is it good enough for my Mum (or anyone else I love and care for)?

Look for care homes and home care agencies where the staff involve people who use services and their families and carers, and treat individuals with compassion, kindness, dignity and respect.

Whether you are being cared for in your own home or in a residential setting, the staff looking after you need to be skilled, kind and supportive.

They should also be capable and confident in dealing with your particular needs. You should always feel that their support is helping you to live the life you want to.

A care home will be a home for you or your loved one. Residents should be treated as individuals with their likes and dislikes taken into account.

Think about whether a home is close enough to family, friends, and community facilities.

Look at how well-led and managed a home is. What does it have in place to ensure that it delivers high quality care? Does it promote meaningful activity and connect the home with the community?

If you or a loved one needs help with day-today care, you can contact your local council’s social services department. If you would like to organise your care yourself, you can find a care worker or personal assistant through an agency. Your local social services department should be able to provide details of approved agencies.
