Registered for the provision of care of older people and Dementia Care which includes permanent, respite, short stay, emergency respite, end of life care and day services. Our aim is to provide high quality person centred care, meeting individual needs with dignity and respect at all times. All our homes are homely with a friendly family atmosphere, providing excellent accommodation for older people and older people with Dementia. Each home is approximately 3 miles from Barnsley town centre and easily accessible off Junction 36 and 37 of the M1 and situated on easy accessible bus routes from Barnsley Bus Station. In pursuit of excellence, our priority lies with the training and development of employees and their knowledge, skills and commitment are central to the provision of high quality service provision for each individual resident. Our care plans are person centred, ensuring each individual’s care needs are identified with involvement from the resident, family and other professionals where appropriate. In accordance with the new outcomes set by the Care Quality Commission, our aim is to ensure all our staff are qualified to a minimum of Level 2 Diploma in Health Care and each new employee completes the Skills For Care Induction Programme ensuring they have the knowledge and skills to provide high quality care for the elderly. All staff undergo a Criminal Records Bureau Check prior to commencement of employment, to ensure that all our residents are safeguarded at all times.

Provision is made for pets but an assessment is carried out prior to agreement.

What will this booklet tell you? This booklet will tell you: • Who you should contact if you need some advice or help with daily living. • What services are available to help you to live independently in your own home. • The support available to carers who are looking after relatives. • What residential and nursing care is. • About the residential and nursing care homes in Barnsley for older people, the kind of care they can offer and how you can contact them.

Getting Help from Adult Social Services If you or the person you care for has a disability, a mental health problem, hearing or sight loss or is older and having difficulties managing with daily living tasks, you can contact Adult Social Services to find out about the full range of help available. You may be having problems with getting out of bed, getting dressed, going to the toilet or getting out and about in your community and need advice or support to remain independent and safe. You can contact Adult Social Services by telephoning 01226 775656 or calling in person at one of the 9 Barnsley Connects Customer Service Centres, their locations are in the contacts section at the back of this leaflet. They can put 4

you in touch with a social care worker who will talk to you to find out more about the kind of support that may be available for you. You can also email us at socialservices@barnsley.gov.uk. If you think that you need help from Adult Social Services you can ask for an assessment. This is when a social services officer talks to you and your family about your needs. The assessment takes into account: • Your health and safety • How independent you are • Your ability to manage daily living tasks • The support you receive from your family or others who are providing you with regular help (your carers)


Getting Help from Adult Social Services Continued

• Your ability to get out and about in your community The assessment will also find out if you qualify for help from Adult Social Services in arranging and paying for your care. The Government provides us with criteria to help decide who can receive help based on their needs and to make sure that people are assessed fairly. If you qualify for help it is likely that you will have to pay some money towards the cost of your care and support services, depending on your financial situation. This will be worked out with you following your assessment. It is your right to ask for an assessment of your needs, regardless of whether you are able to pay towards your care or not. If you don’t qualify for help from Adult Social Services we can provide you with a list of the many organisations and groups that offer services to people who need some support. You can get back in touch with Adult Social Services at any time if your situation becomes worse or you need some further advice. Personal Budgets If, after your assessment, it is agreed that you qualify for some help from Adult Social Services to arrange and pay for your care you will have the option of having a ‘Personal Budget’. This is money from Adult Social Services for you to spend on the care and support that you need to help you to live independently. You can receive this money directly or you can arrange for a family member or friend to manage it for

you. It must be spent on meeting your care needs and we will help you to write a Support Plan that considers what kind of help you need and how you want to receive it. Personal Budgets give people more control over how their care needs are met and more choice from a wide range of services. If you don’t want to receive a payment for you to manage your care services yourself you will still have the option of Adult Social Services arranging it for you. Support to live at home There are many different services available that may help you to stay living in your own home, as independently as possible. Some of these services can be arranged or purchased directly such as cleaning services, community meals and equipment. Alternatively you can contact Adult Social Services so that we can assess your needs and identify what services may help you. You can find out more about having an assessment and what’s involved on page 4 & 5 of this booklet. Equipment and adaptations If you are finding it difficult to wash, dress, prepare a meal or to get out and about outside of your home there is a range of equipment available for you to purchase that can make it easier

to look after yourself. Some examples of equipment that you may find useful are: • A shower chair or stool if you are finding it difficult to stand for any length of time • A trolley or a frame with a tray attached to help with keeping your balance when you are carrying things from room to room in your home • Kitchen equipment such as easy pour kettles and tin or jar openers • More specialist made to measure equipment such as a hoist The Council runs a specialist equipment demonstration centre called Enable Barnsley. They don’t sell equipment, but they have a wide variety of items on display for you to try out and they can provide you with details of how


Support to live at home Continued much things cost and where you can buy them from. You can also make an appointment to see an Occupational Therapist at Enable Barnsley who will advise you about what equipment is suitable for you. A lot of the every day living equipment that is on display is now available from local supermarkets and mail order companies or major chemists and the staff at Enable Barnsley can provide you with a list of local suppliers. Adaptations to your home Major adaptations could include a stair lift, a shower room or alterations to your kitchen. If you are a homeowner you may be eligible for a grant to help to pay for adaptations called a Disabled Facilities Grant, dependent upon your needs and your financial situation. This is intended to help older or disabled people to live as comfortably and independently in their own homes. The Council’s Equipment and Adaptations Service can also provide support and advice to people who need home adaptations, to find out if you qualify for help you will need an assessment of your situation. For more information you can contact the Equipment and Adaptations Service on 01226 775800 or email equipmentandadaptations@barnsley.gov.uk If you are a Berneslai Homes tenant you can contact them directly to request

a minor adaptation, for example a grab rail or tap lever. They may meet the cost of major alterations also but you may require a more detailed assessment by the Equipment and Adaptations team first. Useful technology The Council’s community alarm service can help older and vulnerable people to live more safely in their own homes. The alarm is linked to your telephone line via a unit and you will be given a portable pendant that you can either wear around your neck or carry with you. Pressing the button will raise the alarm with Central Call, who will then telephone you and, if you do not answer the phone, they can arrange to come out to you or contact a family member.

Support to live at home Continued The service operates 24 hours each day 365 days a year. In addition to the community alarm there is a range of equipment called Telecare that can support your safety and independence with items such as bed sensors, smoke detectors and medication reminders. You will need to pay a small charge for community alarms and Telecare equipment. To find out more contact Central Call on 01226 775671. Home Care Home Care services support people to live as independently as possible in their own home. This can include help with dressing, bathing, preparing meals and other daily living tasks. In Barnsley

there are a number of private home care agencies who provide these services. If you have been assessed by Adult Social Services as requiring some support from a home care service your care manager or social worker can help you to arrange this for you. You can find out more about how to get help from Adult Social Services on page 5 of this booklet. If you don't qualify for help from Adult Social Services or wish to make your own arrangements you can contact any of the home care agencies listed below. All have been approved by the Council to provide services in Barnsley and have been through a process of quality checking.

Getting Out and About Things to do in the day There are many free activities and clubs that are provided in communities that can be a great way of spending some time outside of your home and meeting new people. You can find out more about what is available in your area on the Council website www.barnsley. gov.uk or by telephoning Adult Social Services on 01226 775656. There are also a number of day care centres that are available to older people who are at risk of becoming isolated in their homes. Day care also provides a break for family carers. There is a charge for day care and if you want to have an assessment by Adult Social Services to find out if you qualify for some help to

pay for this service you can telephone the number on the left. Public transport Older people and eligible disabled people are entitled to a concessionary bus pass that allows free off-peak travel on buses, trains and trams across South Yorkshire. For further information you can telephone Traveline on 01709 515151 or call in person at The Information Centre in Barnsley Interchange. Blue Badge disabled parking scheme The Blue Badge is a national scheme of parking concessions for people who have severe walking difficulties and travel either as a passenger or driver. This scheme allows the badge holder to park closer to facilities and in disabled


Getting Out and About Continued parking bays where they are available. The Council assess people who may be eligible for a Blue Badge based on some national criteria. You can either call in person at any Barnsley Connects Service Centre or telephone the Blue Badge team on 01226 775600 for more details. Intermediate Care Intermediate care is provided jointly by health and adult social services. It provides a range of re-ablement services and therapies to older and vulnerable people that aim to: • Help people to remain as independent as possible in the community by

providing short-term rehabilitation programmes in order to help people to reach their individual potential and to remain living in their own homes • Prevent inappropriate hospital admission and facilitate discharge • Help to reduce the need for residential and nursing placements In Barnsley the Home Care Assessment and Re-ablement Team (HART) provides intermediate care services to people who have been assessed by Adult Social services as needing this kind of support. It is provided for a maximum of 6 weeks and there is no charge for the service.

After a stay in hospital Hospital Discharge If you are already receiving support that has been arranged with Adult Social Service please let your care manager or social worker know that you are in hospital, you can ask a friend or relative to do this on your behalf if necessary. They can arrange to have your social care services stopped until you are ready to leave hospital. After a stay in hospital most people can return to their own home although some may need ongoing health or social care support. A small number of people may have needs which cannot be met in their own homes and may need to

move into a care home or to receive NHS continuing care. If you have ongoing social or health care needs the nursing staff who have been responsible for your care in hospital can arrange for a Care Manager from the Hospital Social Work Team to visit you. They will discuss what kind of support you need when you leave hospital and will do an assessment to find out if you qualify for help from adult social services. Hospital at Home This is a scheme that offers patients who are medically stable a choice of early discharge from hospital. It provides 13


After a stay in hospital Continued intensive nursing care and therapy at home from nurses, physiotherapists and care workers. You will need to be assessed by Adult Social Services to see if you qualify for the scheme. It is a 7 day service but does not provide 24 hour care. For further information you can telephone 01226 777953. NHS Continuing Care Some people need longer term health care after a stay in hospital. Continuing

care is provided in hospital, in nursing homes or at home wherever possible. The doctor who is responsible for your care whilst you are in hospital will talk to you, your family, Adult Social Services and other health professionals about how best this care can be provided to meet your needs. Help for Carers A carer is someone who looks after a relative, partner or friend who, because of disability, illness or old age or special condition, cannot manage without help. This includes young carers, parents or others bringing up a child with a disability. Recognising that you are a carer is the first step towards getting the support that you need to help you to continue caring for your loved one. Caring can be rewarding but it can also be tiring, however there is plenty of help available to make your life easier, from finding a piece of equipment to arranging a piece of equipment to arranging to have a break from your caring duties for a short period of time to relax and unwind. There are a wide range of services available across the borough that have been developed for carers and 14

can provide you with information and advice to help you in your role. Many of these services are available in your local community and have been set up by voluntary organisations and other carers who may be in a similar situation to you. A good starting point for finding out what is available is Adult Social Services. As a carer you are also entitled to ask for an assessment from Adult Social Services that will look at your needs and how you can be supported. Respite Care Respite care services provide a break for you or your carers in a number of ways. This may be for a few hours each week, a few days or longer periods. Respite care may be provided through support at home, day care or a short stay in a

residential or nursing home. If you want to find out more about the respite services in Barnsley and whether you qualify for support contact Adult Social Services on 01226 775656.

What is a care home? There are two main types of care home: • Residential care homes which do not provide nursing care • Nursing care homes which provide nursing care A care home is what we call a home that offers accommodation and personal care. Care homes can offer different types of care, such as residential care or nursing care. There are also some care homes which provide specialist care for people

who require extra support, often due to dementia or other mental health. Care homes are owned and run by local authorities, private companies and charities. When we talk about care homes in this book, we are talking about care homes which have been inspected and registered.

Nursing and residential care homes – what is the difference? A residential care home is a care home of any size, which offers accommodation, meals and personal care. Residential homes offer care and support throughout the day and night. Staff in a residential care home can help you with washing, dressing, at meal times and going to the toilet. In a residential care home you will be cared for by suitably qualified or experienced staff. A nursing care home is similar to a residential care home and will also offer accommodation, meals and

personal care. The difference between a nursing care home and a residential care home is that a nursing care home also offers 24-hour care by a qualified nurse. Some care homes can offer both residential care and nursing care. Some care homes are dual registered. This means that the home can accommodate people needing either residential or nursing care. If you choose to move into a dual registered home for your residential care you may not need to move to another home if you develop nursing needs.

Which type of home is best for me? The choice of home is a very personal one and what suits one person may not suit another. You have the right to choose any home you like as long as Adult Services agree four key points: • There is a place available.

does someone else can pay the difference. It is important to choose the right type of care home. It is vital that the care home that you choose can offer you the level of support that you need.

• Adult Social Services can agree a contract with the owner of the care home to ensure that you are properly cared for.

If you ask us for advice or an assessment we will talk to you, your carers and your family and friends, about what level of support you need and what type of care home we think would best meet your needs.

• The home does not cost more than Adult Social Services usually pays for the care you need, or if it

• It is suitable for your care needs.

How much will I have to pay? Adult Social Services can provide financial help if you have less than £23,250 of capital assets and you need care in a residential/nursing home. If you want help with funding your care you will need to have a care assessment by a care manager or social worker. Your care manager or social worker will provide you with details about the fees for your residential or nursing home care. Before any decision is made about how much you need to contribute you will be offered a financial assessment to find out about your income, savings and any capital assets that you own. If you do not wish to provide this information about your finances Adult Social Services will assume that you have opted to pay the full cost of your care. Calculating your contribution Adult Social Services departments follow a set of national rules to work out how much you need to contribute toward the cost of your care each week. Most of your income and any capital assets of more than £14,250 are taken into account. Your income includes state benefits and private pensions. Capital assets include savings, investments and the value of any property. Everyone who is helped in this way even for a short period will have to contribute an amount towards the cost of their residential care. If you have more than £23,250 of capital assets you will be expected to pay the full costs yourself. These figures are set by the

government and change each April, the amounts quoted were correct in April 2012. If you own your own home, its value will generally be included when your capital is calculated, unless a partner or relative is still living there. You may need to sell your home to pay your contribution but you should be sure that you are happy in the care home before you sell your house or give up your tenancy. You will have up to 12 weeks to decide if you want to stay in the home. Adult Social Services can provide you with financial help while you are waiting to sell your property. If you are a tenant and are receiving housing benefit, the Council may be able to continue paying this for a while after you have moved into a home. The value of your home will not be taken into account if your partner still lives there or if you are staying in residential care for a short time. Adult Social Services will make up the difference between your contribution and the home’s fees providing that the home does not charge more than they would normally expect to pay for similar accommodation. If you wish to move into a home that charges more than Adult Social Services would normally expect you to pay you will have to find someone else, for example a relative, friend or charity (other than your spouse) to pay the ‘top up’ for as long as you are in the care home. 17


How much will I have to pay? Continued Does the fee cover everything I will need? Generally, the amount you contribute will cover all of the normal things that a care home would provide including your meals, activities etc. The home should not ask you for any more money except for personal items that you wish to buy. Everyone staying in a care home will be given a small personal allowance each week to help pay for every day things such as newspapers, hairdressing and toiletries. This is a set amount determined by the government. Claiming State Benefits You may find that you are entitled to receive Income Support/Pension Credit in a care home even if this was not the case whilst you were at home or in hospital. It is important to claim if

you are eligible. Changes to your financial circumstances If your income changes at any time whilst you are in a care home, for example an increase in your works pension, the sale of your property or an inheritance you must tell Adult Social Services by contacting the Residential Assessments Team on 01226 774812. If your capital drops to around ÂŁ23,250 you should let Adult Social Services know as you may be able to claim help with your funding. Paying for your own residential or nursing care Even if you plan to pay for your own residential or nursing care or think that you may not be able to receive financial help from us, you can still ask us for advice and a care assessment. If you choose to have a care assessment it will help you to decide what kind of care will best meet your needs. We may be able to tell you about other ways of meeting your needs as well as residential and nursing care. If you have a care assessment we may be able to help you quicker if your

savings or investments drop below £23,250 and you are able to get help from the Council with the cost of your residential or nursing care. If you have a care assessment we will also be able to provide you with advice and guidance and help you to deal with any concerns you might have about your residential and nursing care. We can also arrange for you to have a welfare benefits check with the Pensions Service to make sure you are receiving the full benefits you are entitled to.

Deciding which care home Choosing a care home is important as it may become your home. Only you can decide which home you’ll like. You should find out as much as possible about a care home as this well help you make the right choice of care home for you. You may wish to think about some of the following questions when you are deciding which care home you like. Location and building • Where is the home? • Will visitors be able to get there easily? • Are there transport links near by?

• Are facilities such as shops‚ pubs‚ parks and places of worship within easy reach? • Will it be easy for you to enter and leave the building‚ and move between rooms and floors? • Can residents have personal possessions in their rooms such as pictures, plants and furniture? Day-to-day life • Are you allowed to make choices about your daily routine? • Will you be able to rise and go to bed when you choose? • Can you choose which clothes to wear each day? 19

What are the home’s fees? • Is it clear how the fees are structured and calculated? • How are fees collected?

• Is the home right for your cultural and religious needs?

• Can the home meet your dietary needs?

• Are extra items not covered by the basic fees clearly identified and accounted for?

• Is there a choice of food‚ and when and where it can be eaten? Your care needs

• What arrangements are there for handling your personal money?

• Is the home registered to provide the level of care I need?

• How will your valuables be kept secure?

• Do the other residents seem to have a similar level of need to me?

You can either look for a home yourself or ask Adult Social Services for help. We can give you information about the range of home in Barnsley to help you decided which will suit you best.
