Introduction Hospitals are the right place to be in when you are in need of acute medical or surgical care. However when your treatment has been completed, it is important that your stay is not delayed for any reason. Therefore it is good practice to start planning for your discharge as soon as you come into hospital or even before you arrive. This means that we can begin to: • Listen to any concerns that you may have at the earliest opportunity • Assess what your needs are likely to be when you are ready to leave • Involve relevant staff who can help in meeting those needs • Give an indication of when you will be ready to leave the hospital • Make arrangements for equipment or services to be ready when you leave the hospital

Welcome to an exciting care home, designed to accommodate a variety of care needs in a pleasant and homely environment. This purpose built home is divided into independent units that can accommodate service users in either 10, 17 or 27 bed units. Each Unit has light and airy communal dining and sitting rooms providing ample choice for quiet and stimulating areas.

Berwood Court Care Home, located on Cadbury Drive, Castle Vale has established itself in the heart of the community and provides accommodation for 74 service users requiring care for both nursing and residential care needs. All bedrooms are single with en suite facilities and the home boasts 6 spacious lounge and dining rooms. All service users receive thorough assessments both prior to joining us and throughout their stay with us. The home ensures that all individual care plans are thoroughly Person Centered and that family and loved ones are as involved as they wish to be with care planning. Staff enjoy a pro active approach to their development and are supported with their learning needs, enabling all staff to achieve a minimum of NVQ level 2.

We understand that some people may not feel confident about returning home after a period of being in hospital and we work closely with Birmingham City Council’s social work teams and other healthcare teams to put in place services that will enable those people to be supported in their discharge.

We also know that following a period in hospital people may make decisions about future care needs that they may make differently if they had a longer period to consider this. This booklet will give information about schemes that are in place to assist those people.

The hospital team In the hospital you will find a multidisciplinary team that consists of doctors, nurses, therapists, pharmacists, specialist nurses, social workers and discharge liaison nurses. Some, or all, will be involved in your care.

How can I be referred to the social work team?

If, following an initial assessment by medical and nursing staff, it is identified that you may have social care needs on discharge, you will, with your consent, be referred to the social work department. The ward will complete a referral called a Section 2 Notification. This will be dealt with by the hospital-based social work team (this is only relevant to Birmingham residents) who will allocate a social worker to you. The social worker will talk with you and those involved in your care to identify the next stage in your discharge process.

The assessment process The assessment process will start with discussions with the nurses looking after you on the ward. These discussions will help clarify which members of the multidisciplinary team need to be involved in your care. We would like to know if you were receiving support from community health and social care services before your admission, or if you are aware of any issues that may impact upon your discharge from hospital.

NHS Continuing Health Care If you have complex ongoing healthcare needs the team involved in your care must first consider whether you would be eligible for NHS Continuing Health Care. If you are entitled to NHS Continuing Health Care the discharge liaison nurses will discuss with you how your ongoing needs and care will be managed.

You are welcome to have a family member or friend to be with you during the assessment process.

What next?

Hospital rehabilitation If it is identified that you would benefit from a period of hospital rehabilitation you will be referred to one of the Birmingham community hospitals at West Heath or Moseley.

Enhanced Assessment Unit Following an initial assessment it might be appropriate for you to transfer to the Enhanced Assessment Unit, so that social workers can assess and monitor your needs over a longer period. Many people find that this helps them to gain confidence after a period of ill health, and are therefore able to make informed decisions about their longer term care plan. There are therapists available to assist in the recovery process at the unit. This is available for up to 6 weeks and is free of charge.

Enablement service at home The enablement service is a service designed to support you at home, to enable you to carry out personal care needs and tasks around the home, either on your own or with a small amount of help. The service is available for a period of up to 6 weeks and is free of charge.

Enablement service in a care home This is a service designed to help you gain confidence in your ability to care for yourself, with the involvement of therapists and care workers, in a care home environment with other people. This is a service that is available for up to 6 weeks and is a free service.

24-hour care homes If it is felt that you can no longer manage at home, even with a large support package, it may be appropriate to move to a residential or nursing care home. If after assessment you are found to be eligible for a placement, you will be assisted to find a care home that can meet your needs. If the care home of your choice does not have an immediate vacancy, you will be asked to accept a place in an alternative care home, until your first choice becomes available.

This is a team of health care professionals who can continue day-today treatments for patients in their own home.

Patients using the service remain under the care of their hospital consultant but instead of being in a hospital bed, receive their dayto-day treatment from a team of healthcare professionals who travel to their home. The team is made up of fully qualified nurses, physiotherapists, occupational therapists and healthcare support workers.

Patients suitable for the QEHB@Home service will be clinically well enough to leave the hospital environment but still require further treatment. This service will only be offered by the hospital’s clinical team if it is appropriate for you.

Discharge from hospital When you are ready to leave the hospital you will be told by the nurses and doctors looking after you. If you require social care, Social Services will be formally notified by the ward sending a Section 5 Notification (Community Care Delayed Discharge Act 2003). They will then put in place whatever arrangements have been agreed with you. If there is any delay in your discharge you will be transferred to an interim care bed, in a care home, whilst arrangements are being made. If you chose not to go to the interim care bed you and your family will need to make arrangements for your immediate discharge. You cannot choose to remain in an acute hospital bed.

We aim to discharge people in the morning. We will make sure you have written information and medicines to take with you. If you are being collected we may ask you to wait in the Discharge Lounge, which is a comfortable and safe place to wait.

Discharge Lounge On the day before your discharge you will be visited by a member of staff from the Discharge Lounge. They will assist you in any way they can to help you home the next day. This will include organising transport for you where necessary, and liaising with pharmacy to ensure your medications are ordered and ready for you the next day.

The Discharge Lounge is designed to be a warm, friendly environment where patients who are fit for discharge can wait for their medication, transport or relatives/carers to collect them.

It is staffed by registered nurses at all times and refreshments are provided.

Home from Hospital – Welcome home service The Home from Hospital service is a charitable organisation that can provide support to patients within the first six weeks after they’ve left hospital.

They can: • Collect shopping and/or prescriptions • Provide a food parcel on the day of discharge for those who have no-one to shop for them • Visit for a friendly chat • Go to a bank or Post Office • Possibly escort you to hospital, the doctor, the optician or hairdresser • Give benefits advice • Help you sort out bills and other paperwork that may have gathered while you’ve been in hospital • Liaise with a wide range of statutory, voluntary and commercial services on your behalf • Provide respite care for a couple of hours • Arrange for furniture to be moved within the house • Make arrangements for pets to be re-homed or temporarily cared for While they don’t offer personal care, cleaning or laundry services, they can advise on approved services. Alzheimer’s Society Services in Birmingham If you think you may have a form of dementia, or are caring for a person who has dementia, there are many ways that the Alzheimer’s Society in Birmingham can help.

Support from Dementia Support Workers Dementia Support Workers can offer advice on accessing professional help. We are in contact with a wide range of services and other advice centres, so we can put you in touch with specialists who can help you if your concerns are outside of our expertise.

We can also refer you, or the person you care for, to services which provide for people with dementia – particularly if you are having trouble accessing this yourself.

Home visits We can arrange for a Dementia Support Worker to visit you in your own home and provide information, advice and support.

Information We can provide a wide range of advice sheets, which include specific information on different types of dementia, care issues and understanding the needs of the person with dementia.

Dementia Cafés Are places for people with dementia, their families and friends to meet other people in a similar situation for support, information and social interaction.

What should I do now?

Is a service provided by Alzheimer’s Society which uses singing to bring people together in a friendly and stimulating social environment.

Singing is not only an enjoyable activity – it can also provide a way for people with dementia, along with their carers, to express themselves and socialise with others in a fun and supportive group.

Dementia Friends Programme Dementia Friends is a national initiative run by the Alzheimer’s Society. Funded by the government, it aims to improve people’s understanding of dementia and its effects.

Through the Dementia Friends programme, we aim to change the way the nation thinks, talks and acts to create ‘dementia friendly’ communities.

How does it work?

A group of volunteers will be trained to spread the word about what dementia is and what action individuals can take in their community, and recruit Dementia Friends.

What is a Dementia Friend?

A Dementia Friend learns about what it is like to live with dementia and then turns that understanding into action.

What is a Dementia Friends Champion?

Dementia Friends Champions are volunteers who, after attending a training course, run information sessions where they talk to people about being a Dementia Friend in their communities.
