About the help we provide Sheffield City Council provides help for people in Sheffield in many different ways. If we can’t help you we may know who can, so please ask us.

What will happen? We will work with you to identify your needs and any risks to your independence. We will then assess your level of need and risk to determine if you are eligible for support from us. You are the best person to say what is right for you. We will work with you to maximise your independence. We may be able to do this without the need for an assessment or a personal budget. However, an assessment may be required in order to support you more effectively. This may result in you being allocated a personal budget. A personal budget gives you choice and control of what support you have and how you live your life. This is called Self Directed Support. A personal budget is money given to you by us so that you can arrange and pay for support. The amount of money you get in your personal budget is based on your social care needs, and the money can be used flexibly and creatively to give you more choice about the support you choose. You may be asked to contribute to your personal budget following your financial assessment.

Our priorities and criteria We aim to provide choice, control and independence for the people of Sheffield and provide support and protection to

those who need it the most. Therefore, you will be eligible for support if there is a significant or immediate risk to your safety, independence and wellbeing. If you are eligible for support you will then have a financial assessment to see if you are able to contribute to the cost of your support. For more information on this see ‘Paying for your care’ on page 7. We will also take into account the situation of your carer, if you have one, and any risks to their independence and wellbeing. What happens if I’m not eligible? If there is only some or little risk to your safety, independence and wellbeing we will provide you with information and advice about community services to help you remain independent and prevent your situation from getting worse. You are entitled to a reassessment at any time if your needs change or worsen. To request a reassessment you can contact your Social Worker directly if you have one or alternatively contact the Adult Access Team.

Your assessment The first step to getting a Personal Budget is to complete an assessment

About the help we provide questionnaire. This asks about your life, what kind of support you need and, most importantly, what you want from your life. Your Social Worker or Care Manager will visit you and can support you to complete the assessment questionnaire. You may also want to ask people who know you well to help you, these could be your family, your friends, an advocate, staff or other people paid to help you or health professionals. During this discussion the Social Worker or Care Manager will be able to offer their advice about which services you may wish to look into and also if you may need a lot of support they can help you decide if Residential or Nursing Care would be an option for you. You will also be told about your Financial Assessment. During your Financial Assessment you will be asked some basic financial information about your income, savings and benefits as you may be asked to make a financial contribution towards the amount in your budget. You’ll find more information on this in the ‘Paying for your care’ section. The answers from your assessment questionnaire are reviewed against our eligibility criteria and used to work out the amount of money you could get in your Personal Budget, this is called your Indicative Budget. Your Social Worker or Care Manager will then arrange a time to come to see you to discuss with you how you would like to complete your support plan.

What is a support plan? When you know the amount of your Indicative Budget, you will need to write a support plan to show how you will meet your needs using some or all of this money. Your support plan shows how you will use your budget to meet your needs and live the life you want. We’ll need to agree that your plan meets your needs, keeps you safe, spends the money legally, and that you have budgeted for unforeseen circumstances before you are able to get your Personal Budget. You can choose to do your own support plan, ask for help from your family, friends or an advocate, choose to pay an external agency to help you, or your Social Worker or Care Manager can help. If you’re not happy about any part of the support plan you’ll have the chance to say so. If you feel we’ve got something wrong or we’re ignoring your wishes you can ask us to look at your case again.

Mental Capacity Act - who decides when you can’t? Mental capacity means being able to make your own decisions. A new law has been introduced to help and support you when you’re unable to make decisions. You may have difficulty making decisions because of dementia, mental health problems, learning disabilities, a brain injury or stroke. The act tells you: • what to do to help you make your own decisions about something

• how to plan ahead for a time when you may loose the ability to make your own decisions in the future • how to work out if you can make your own decisions about something • what to do if there are some decisions you cannot make. Extra Care Housing in Sheffield Extra Care Housing schemes are different to residential or nursing homes. Extra Care schemes let you live independently with the reassurance that help is there if you need it. They are self contained flats and bungalows with onsite care staff who, when required, can provide the care and support you may need to improve your health and well-being. The schemes vary in size, location and facilities, and provide a range of housing options with homes to rent, buy or part buy. They are available for anyone 55 or over and cater for a variety of needs.

nursing home. The council has its own criteria to follow when we decide. In most case it will be clear which type of home is most suitable. In discussion with the assessor an agreement will be reached on the most suitable place to meet your needs. The council will only pay for a place in a nursing home if your assessment shows that your needs couldn’t be met in a residential care home.

In this section you’ll find answers to many of the questions people ask about moving into a home. We’ve made every effort to make sure this information is clear and upto-date. However, it is not intended to be a complete and authoritative statement of the law on the issues covered. The council cannot accept responsibility for any loss arising from the use of information contained in this booklet.

Residential homes offer the same care and help that you could get if a relative or friend looked after you. Staff in a care home can help you with washing and dressing, at meal times and help going to the toilet. In some homes staff can help you to gain skills that help you to be independent. A district nurse makes regular visits to care homes.

Can I choose the type of home I want to live in? The person who carries out the assessment (called the assessor) will talk to you about whether your needs will be best met in a residential care home or

Nursing homes offer 24 hour nursing care by trained nursing staff. The person in charge of the care home must be a qualified doctor or nurse, and there will always be nursing staff on duty to care for you.

Living in a residential or nursing home Which home will I like? Only you can decide which home you’ll like, so you should visit the home - perhaps with your family or friends. All homeowners prefer people to visit before they decide – and you can visit more than one home before making your choice. A list of questions to ask the home is on page 12. There’s a trial period of up to four weeks before you finally make up your mind. You can check which homes in Sheffield have vacancies by telephoning the Bed Bureau on: (0114) 273 4804. They have up-to-date information on spaces in local homes and can be contacted Monday to Friday, 9am to 4.30pm. Or you can visit our website www.sheffield.gov.uk/caresupport/ resnursing/places. If the home is full you can choose to live in a similar home while you wait. When a place is available you can choose either to move or stay where you are.

What if I don’t want to go into another home temporarily? If you’re not at risk we’ll try to give you help to stay in your own home until a vacancy occurs. Of course if you’re at risk or if your needs can only be met fully by living in a residential care home then we’ll not be able to meet them properly in your own home. Can I stay in hospital until a vacancy occurs? This is not normally possible as someone else may be waiting for urgent treatment. If you no longer need to be in hospital we’ll offer you a place in another home or help to stay in your own home until a vacancy occurs. So I’ve decided, what next? Once you have chosen your home, a member of staff from the home will visit you to chat about when and how you’ll be moving in. This is a good time to ask any final questions you have such as meal times, activities run in the home and so on.

Paying for your care Sheffield City Council will help you to pay the fees for residential care if we think this is the kind of help you need, and if we think you can’t afford to pay for it yourself. All residents in residential and nursing homes have to pay something towards the cost of their place. The amount is worked out according to a national set of rules, and will vary depending on your circumstances. Most of your income, including your state benefit, goes towards paying your fees. However you’ll be left with a weekly amount for your own use, called your Personal Allowance. From 11 April 2011 the Personal Allowance is £22.60 every week. Together with one of our Financial Assessment Visiting Officers, we’ll ask you to complete a financial information form and produce documents as evidence of your income and capital. We’ll pay the difference between your contribution and the fee for your place in the home, as long as the home doesn’t charge more than we would expect to pay. We set a price each year that we feel is reasonable to pay for care in a home. For residential care this is called the Purchaser’s Standard Fee (PSF), and if a home meets a higher environmental standard we pay an additional amount. For nursing care it is called the Purchaser’s Personal Care Rate (PPCR), and if a home meets a higher environmental standard we pay an additional amount. Details of the prices per week from 11 April

2011 for places in Sheffield are given in the table below. You may be given a provisional financial assessment whilst we’re undertaking a full assessment of your financial circumstances. The provisional financial assessment will be subject to a four weekly review pending your final financial assessment. Your final financial assessment may be different from the provisional one, and will be backdated to the start date of your service.

Paying for your care If your place at the home costs more than the amount the council will pay you’ll need to have a relative, a friend or a charity willing to pay the difference when you are living there. They’ll have to sign an agreement to pay the additional cost of your care. If at any time they’re unable to pay this amount you may be asked to move to another home. You can’t use your own money to pay the additional cost. You would be advised to inform yourself about your financial options. We recommend all new residents contact the Department for Work and Pensions (DWP) to check if they qualify for Pension Credit/Income Support to help pay towards the cost of staying in the home. You should always notify the DWP of any change in your circumstances, such as when moving into a care home. Are the charges the same for residential care and nursing care? If a nursing assessor decides you need

nursing care the nursing part of that care will be funded by NHS Sheffield. You’ll still have to pay for the accommodation and personal care you receive in the home. Unless you pay the full fee for your care yourself this won’t affect your contribution. How do you work out my capital? Capital is the value of any cash or savings you have, together with anything you own that could be sold - apart from personal possessions. Your capital does not include any money you regularly receive from a pension, charity or other organisation — this is your income. The government rules on assessing your care charges from capital have different levels that change the amount you have to pay. 1. I f you have capital less than £14,250 it will be ignored, and your charge will be assessed on your income alone. You may qualify for Pension Credit/Income Support, and we’ll pay the remainder of your fees up to the Purchaser’s Standard Fee. 2. I f you have capital over £14,250 but under £23,250 your charges will be assessed on your income and capital. In most cases you’ll qualify for Pension Credit/Income Support, and we’ll pay the remainder of your fees up to the Purchaser’s Standard Fee. As your capital reduces towards £14,250 you’ll qualify for more Pension Credit/Income Support, and this will reduce the amount the council will have to pay towards your care. So it’s important you notify both the council and the DWP of all changes in your capital and income so that you get the right help paying your fees.

Paying for your care 3. I f you have capital over £23,250 you must pay the full cost of your care. To help you pay the fees you may be entitled to Attendance Allowance/Disability Living Allowance care. If a nurse assessor decides you need nursing care you’ll also qualify for a payment from NHS Sheffield.

So how will I know who is paying what? Anyone who gets financial help from us has to sign a Placement Agreement when they move into a home. The Placement Agreement is a legal document signed by you, the council, the home, and any other person contributing to your care fees. It gives details of how much will need to be paid each week by you, by the council, and by any other person contributing to your care fees. Because this agreement is a legal contract, it’s important that you understand what

What about my Benefits?

People living in a care home generally can’t get more than the basic level of benefits for their age.

Pension Credit/Income Support — Depending on your capital and income, you may be able to get a Pension Credit/Income Support to top up your income even if you get council funding or a loan. The DWP may ignore the value of the property where you live before entering into residential care if you’re taking reasonable steps to sell it. Attendance Allowance and Disability Living Allowance — If you’re disabled, or need help with personal care, you may be able to claim Attendance Allowance or Disability Living Allowance (Care Component) as long as you pay the full fee for your place in the home and don’t get financial help from us. If the council pays part of your fees your Attendance Allowance/Disability Living Allowance (Care Component) payment will stop after four weeks, or earlier if you have already been in hospital or short term care. You should also notify the Attendance Allowance/Disability Living Allowance (Care Component) office of any change in your circumstances. You can use these benefits to pay some of your care fees. This may make it easier for you to afford to pay the fees yourself without our help. It could also mean that you’ll not use up your other monies as quickly. Meaning of words used in this booklet You may not be familiar with some of the words used in this booklet. Here’s a list of some terms used, together with an explanation of what each one means. Assessment/Assessor. If you think you need our help then a social worker will help you to find out what your needs are and how we may be able to help you to overcome your problems. Department for Work and Pensions. This is the Government department that arranges payments of benefits such as Disability Living Allowance, Attendance Allowance, Income Support and Pension Credit. Capital. This is the value of anything you own that could be sold, and any savings you have. Your capital does not include any money you regularly receive from a pension, charity or other organisation — this is your income. Criteria. This is a set of rules that we follow when deciding who qualifies for help from the council. Usually this word is used to refer to the value of these things after a person has died.

Fees. This word means the cost of a place at a residential care home or nursing home. Your fees may be made up of contributions from the Council, from your benefits, and from a friend or relative. You may also get a payment from NHS Sheffield to pay for your nursing care. Income. This is the value of any money you receive, from a pension, charity or other organisation. Legal Charge. This is an arrangement where any loan provided by the Council is legally tied to your property. This means the loan must be repaid from the value of your property when sold or from your estate after your death. Personal Allowance. This is an amount of money that you must be left with to pay for personal items whilst you are living in a residential care home or nursing home. The Personal Allowance can’t be used to help pay for your place in a home. It should be used to pay for newspapers or magazines, and for your personal care such as hairdressing and toiletries.
