Top innovation prize winners Staff scooped a major innovation award for providing a pioneering high-quality service for patients.

They won the regional Medipex NHS Innovation Awards 2015, for setting up a new in-house manufacturing unit that skilfully creates custommade devices, for children and adults who need more specialist support or stability for their bones and joints.

Previously patients had their orthoses (devises to support their feet and legs) supplied from private companies, resulting in delayed treatment for patients and higher costs for the Trust. The in-house facility has radically changed all this.

Patients can now have same day repairs, with fewer clinical appointments, while still keeping their independence and mobility. They can even choose to have their orthotics made in a range of different patterns or colours.

Medipex Ltd is the innovation hub for NHS organisations in Yorkshire and the Humber and the East Midlands. These awards exemplify excellence in the use of technology and pioneering ideas by NHS staff to improve patient care and make services more efficient.

Keep norovirus out of hospital this winter We’re saying NO to norovirus this winter and urging patients and visitors to stay away if they have diarrhoea or vomiting.

Norovirus is very contagious and can be spread through contact with an infected person, by eating or drinking contaminated food or water, or by contact with contaminated surfaces or objects. Washing your hands regularly is very successful in protecting against the virus especially before meals and after visiting the toilet.

Flu is extremely contagious and can be deadly for vulnerable patients especially those who have not had their flu jab.

Being able to diagnose the virus quickly when the patient first arrives in hospital is vital to limit the spread of infection and to start treatment without delay.

Patients with flu like symptoms presenting on the Acute Medical Unit at Doncaster Royal Infirmary (DRI) were approached and selected to participate in the clinical study by our research nurses and nurses from the Clinical Research Network.

Other participants were selected from clinical teams within the Trust through a dedicated bleep system.

That is why we are reminding anyone who has nausea, vomiting or diarrhoea not to visit hospital until they have been completely symptom-free for at least 48 hours. If they visit before then, it could mean that they unwittingly pass their norovirus onto hospital patients and staff.

Although very unpleasant, norovirus usually clears up in one or two days and most people can care for themselves with paracetamol and plenty of fluids. Symptoms include the sudden onset of projectile vomiting, watery diarrhoea, and some people may also experience headaches, mild temperature and stomach cramps.

Patients attending our hospitals during the flu season are benefiting from a new technology that helps staff to test for the flu virus in minutes from a simple throat swab.

The new technology called Alere-i was given a thorough ‘road test’ last winter (December 2014 to March 2015) by our Clinical Research team in a study involving over 200 patients.

Nicknamed the ‘winter vomiting disease’ norovirus becomes more active during the winter months. It can spread quickly in closeknit areas like hospitals, schools, nursing and residential homes.

Dr Kenneth Agwuh, Director of Infection, Prevention & Control, said: “The winter months are here again, notable for its marked increase in norovirus infection. This infection is a very contagious virus that can infect anyone, with extremely worrying consequences for patients already ill in hospital. That’s why it is so important not to visit until you have been symptom-free for at least a couple of days. An outbreak may mean we have to restrict admissions and visitors to wards to contain the virus.”

Two swabs were taken from inside the patient’s throat by the research nurses.

One sample was sent for testing to an external laboratory with a two to three day turnaround. The second swab was tested by the research nurses within the hospital using Alere-i. Results were provided in 10 minutes and confirmed whether the patient had tested positive or negative for the flu virus.

As a result of this clinical trial we are routinely testing patients showing flu like symptoms on arrival at the Acute Medical Unit at DRI and on the Assessment Treatment Centre at Bassetlaw Hospital.

Kate Carville, Head of Nursing for the Emergency Care Group, said: “Many patients arrive with breathing problems which may or may not be flu. Being able to get a quick diagnosis is excellent so that we start the patient on the right course of treatment immediately. If a patient tests positive for influenza we can isolate them and take precautions to reduce the infection spreading.”

You can help us keep norovirus out of hospital by: • Not coming to hospital if you have signs of a stomach upset, nausea, and vomiting.

• Always wash your hands thoroughly after using the toilet and before handling food.

• Wait at least 48 hours after being symptom free before visiting hospital.

Staff perform at Rugby League Internationals Part of the NHS Sport and Physical Activity challenge is to get workforces active.

We gave our staff the once in a lifetime opportunity to join CHANT Productions choir and to perform at an international rugby league match.

Over 20 members of staff joined the choir, alongside other local organisations and schools, to sing the national anthem and a mashup of songs from popular artists.

After weeks of rehearsals and singing lessons from a professional opera singer, the choir gathered at the KC Stadium in Hull, on 1 November, for the England v New Zealand match in the opening game of the autumn internationals.

“We were all naturally really nervous but when we stepped out onto the pitch and began to sing you could feel the excitement in the air.”

The choir, over 200 strong, performed during the opening ceremony of the game in front of an audience of 24,000 and 3million on TV. Louise Wheatley, Admissions Clerk who sang alongside her colleagues at the game, said: “We were all naturally really nervous but when we stepped out onto the pitch and began to sing you could feel the excitement in the air. This was a fantastic opportunity and experience.

Shaun and Vicky from CHANT Productions were brilliant to work with, really enthusiastic and encouraging.”

To top off the experience, the choir got the chance to witness the famous Hacker, performed by the New Zealand team, from pitch side and watch England win 26-12.

Patients are benefitting from a revolutionary new laboratory testing system that speeds up the identification of micro-organisms leading to faster diagnosis and treatment.

This time saving allows our microbiology team to provide a rapid same day service to doctors which has a massive positive impact on patient care.

The Matrix-Assisted Laser Desorption/ Ionisation Time of Flight (MALDI-TOF) technology, which cost £118,000, has been paid for by the Fred and Ann Green Legacy that benefits the people of the Doncaster and Mexborough areas.

“It is rare that a technology can fundamentally change well-established diagnostic testing methods. This revolutionary technology represents a shift from traditional ways of working. Thanks to the Fred and Ann Green Legacy we can now go into routine use to continue to improve the care of our patients.”

Unlike traditional laboratory methods the MALDI-TOF system does not rely on the growth of bacteria and fungi to detect strains of infections. This means that it can identify micro-organisms in less than 20 minutes compared to traditional methods which can take up to 72 hours depending on the type of infection.

The MALDI-TOF technology was trialled in our Microbiology Department for a year from September 2014, to validate its performance against the current laboratory techniques. During this time we also carried out an eight month in-house clinical study with patients with septicaemia, a lifethreatening infection of the bloodstream.

Paul Gravil, Head Biomedical Scientist Cellular Sciences, said: “Fast identification of bacteria is essential, especially with life-threatening infections such as septicaemia or meningitis, where early clinical intervention is vital.

The findings of the study have shown impressive results with patients recovering sooner due to earlier changes to antibiotic therapy and fewer patients being admitted to critical care wards.

‘Good’ for Caring and Well Led by CQC We’ve received ‘Good’ for caring and well led services by the Care Quality Commission (CQC) but we require improvement in some areas.

Published in October the CQC report recognised a number of areas of good quality care and outstanding practice and our commitment to further improve services for patients. 74 per cent of the areas inspected were judged to be ‘Good’ and there were no ‘Inadequate’ ratings given.

The report followed a series of inspections over four days in April 2015 and covered DRI, Bassetlaw Hospital, Montagu Hospital and outpatient services at Retford Hospital. The inspectors looked in detail at whether our services were safe, effective, caring, responsive and well led.

The CQC highlighted lots of areas of green reflecting good practice, and more importantly there were no red areas which meant none of our services were inadequate. However they found a small number of areas where improvements can be made in delivering safe, responsive and effective care.

Since April 2015 we have completed many of the areas highlighted in their report and we are well on our way to tackle the rest. We’ve introduced a brand new assessment and streaming service at DRIs Emergency Department to improve the experience for patients.

We’ve also improved the signage in our x-ray departments to prevent patients from entering areas where they could be exposed to radiation.

Areas of outstanding practice included partnership working between Montagu Hospital and Sheffield Hallam University to develop a tailored training package for rehabilitation nurses. The Integrated Discharge Team and the Frailty Assessment Unit were hailed as beacons of best practice.

The work of the Clinical Skills department was also highlighted as well as the support and training provided by our clinical educators in all areas where children and young people were seen in the Trust.

National recognition for clinical research nurses

Like other trusts nationally, a big challenge for us is our ability to recruit nurses and midwives due the national shortage. Our overseas recruitment drives will go some way to address this however there is more we can do.

Other areas that we are working to improve are staff training and appraisals. Since the CQC inspection, 83 per cent of our staff have received an appraisal and we are tightening up methods of monitoring and recording staff training.

A team of nurses, who support and encourage patients to take part in lifechanging clinical research, have received national recognition.

Our Clinical Research Nurses were in the final 10 for the Nursing Times 2015 Awards, and the only entry from a non-teaching hospital in the Clinical Research Nursing category.

Together with the Yorkshire and Humber Clinical Research Network we introduced a new way to support clinical research studies in every speciality. This has given patients even more opportunities to take part in research. Currently patients are involved in over 200 active research studies within the Trust. Studies range from questionnaires that examine patients’ quality of life, to more in-depth genetics studies that explore the cause of specific diseases. Patients are also involved in drugs studies which compare licensed drugs and other studies working with pharmaceutical companies to trial brand new drugs that are not currently available.

Clinical research nurses

The clinical research nurses are often the ones who first approach patients and explain the research opportunities available and what is involved. The nurses also keep the patient fully supported throughout the research and answer their questions.

Restoring voices with 30 minute procedure A specialist clinic at DRI is helping patients across the North of England to get their voices back.

Patients who have lost their voice due to vocal cord paralysis, or simply as a result of aging when the vocal cords thin and start to bow, can attend the Doncaster Joint Voice Clinic and have a product injected into their vocal cords in minutes which restores strength and volume to their voice.

She said: “This procedure has made such a big difference and given me a chance of a normal life. The procedure was quick, simple and painless and I’m very pleased with the results.”

After an initial assessment with both the speech and language therapist and the ENT surgeon, the following procedure takes 30 minutes:

Lesley added: “I’m now going to have the same procedure but with the longer lasting filler which will last for around two years.

I used to give lots of presentation talks to groups and I will now be able to get back to doing what I enjoy the most.”

• A nasal anaesthetic is sprayed into the patient’s nose.

We are one of only a handful of hospitals in the UK to make the most of advances in technology to perform the ‘office-based’ procedure using local anaesthetic in an outpatient clinic.

A speech therapist establishes the needs of each individual patient and works closely with the surgeon to show how much filler needs to be injected to get the best results for each patient.

Jane Shaw, Speech and Language Therapist, said: “The procedure is widespread in America and we have been doing it at Doncaster for around seven years with great results for our patients, who find the procedure to be truly life-changing. People often underestimate the importance of having a voice, and the impact it has on our patients.”

Mr Mark Watson, Consultant Ear, Nose and Throat (ENT) Surgeon, said: “Technically it can be a very difficult procedure in awake patients which is probably why there are so few places in the UK that offer it. Surgeons often prefer to operate in a theatre with a general anaesthetic, which could mean a day in hospital for the patient with the possibility of an overnight stay. We have gradually started to teach others to do this procedure on an ‘office’ basis and we have held courses in Manchester, Birmingham and Denmark.”

One such patient Lesley Stagg, a Probation Officer from York, had Restylane injected into her vocal cords after experiencing a loss of her voice in 2008 from vocal cord paralysis.

• The therapist inserts a small endoscope with a camera into the patient’s nose and down into their throat.

• The image is streamed to a TV screen • The surgeon applies a local anaesthetic under the skin on the neck to numb the area • Filler is injected through the windpipe into one or both vocal cords.

• After a 30 minutes wait in the clinic the patient is discharged • A follow-up check with the therapist will take place a week later.

Two types of fillers can be used: Restylane which provides a short term solution to give patients a trail period to decide whether they like the sound of their new voice and, Radiesse which gives longer lasting results of around two years.

Worksop mum supports South Yorkshire & Bassetlaw

‘Fear or Smear’ campaign Worksop mum-of-three is supporting South Yorkshire & Bassetlaw‘s Cervical Screening Programme.

Victoria Cristofis is urging all women to attend their cervical screening appointment after speaking to the Worksop Guardian in August this year of her horror at being diagnosed with life-threatening cancer.

Nationally, 1 in 5 women do not take up cervical screening invites and only 1 in 3 women aged 25 to 29 attend their appointments.

If the worst scenario happens and you were diagnosed with cancer, it’s not the end. You can beat it, as I did.”

Victoria had put off attending her cervical screening test for over a year and was devastated when doctors found abnormal cells on her cervix which were revealed to be cancerous.

A ‘Fear or Smear’ cervical screening campaign has recently been launched across South Yorkshire and Bassetlaw, aimed at women aged 25-29, because they are the first age group to be invited and the least likely to attend, the campaign promotes myth busting messages to change the stigma attached to smear appointments.

All women between the ages of 25 and 64 are eligible for a free cervical screening test every three to five years. If you are aged between 25 and 49 you will be invited to an appointment every three years.

Women aged 50 to 64 will receive their appointments every five years and if you are 65+ you will be screened if you haven’t had a smear test since the age of 50 or have had recent abnormal tests.

Along with the Trust, Victoria is asking all eligible women aged 25-64 to take five minutes to have something which could save their lives – a smear test.

Since her diagnosis, Victoria has gone on to have a full hysterectomy and is now cancerfree but wants to highlight the importance of cervical screening tests.

Worryingly the latest research from the Health and Social Care Information Centre has shown that the number of women attending screening across the region has fallen.

“I want women to know that attending your cervical screening appointment can save your life and detect abnormal cells that might develop into cancer in the future.

History Matters

The Montagu Hoard

by Garry Swann, Trust Archivist The Trust Archive has been boosted recently with a number of interesting items from Montagu Hospital’s Pharmacy.

This varied collection, consisting of 150 objects, arrived in several crates and boxes, two of them filled with polystyrene chips.

It was rather like a bran-tub as I plunged my hand into the box, not knowing what I would pull out next. Many of the items had been carefully wrapped in soft tissue paper, which was just as well, since several of them were breakable.

There were old-style glass syringes, graduated glass measures for making up medicines, and even an old-fashioned baby’s feeding bottle complete with three (rather perished) rubber teats. Three white pottery ‘Dr Nelson’s Improved Inhalers’ (picture 1) for treating congestive problems came next, then a large and heavy mortar and pestle in unglazed pottery, with pestle stand to match (2). The mortar is a mixing bowl with a pouring lip, while the pestle is a large cosh-like instrument, used to crush and mix the ingredients of the concoction being prepared. As a traditional pharmacy symbol, the mortar and pestle can still be found in pharmacy shop signs, sometimes nowadays in bright illuminated neon.

Best of all, to me, is an old mahogany and brass pill machine (3). In Victorian times, in the days before pharmacological standardisation and packaging was the norm, doctors relied for a significant part of their income on selling their own handmade pills and potions. A pill machine, therefore, was an essential piece of equipment, and its use demanded a degree of skill. Another find was a bandage roller; again, before the days of pre-packaged dressings, bandages were often made from torn-up bed-sheets and similar material. Lack of funds also resulted in bandages being washed out and re-used a regular and no doubt unpleasant task for hospital nurses. The bandage roller (4) is a simple gadget consisting of a spindle set in a wooden frame, with a handle for turning; the bandaging material was attached to the spindle and the handle rotated to wind the bandage into a tight roll. A sliding bar adjusts the frame for different widths of material.

Also included in the collection is a pharmacist’s balance (5) for weighing ingredients, superseded these days by much more accurate electronic scales. Other items include numerous laboratory thermometers, a wooden test stand complete with test tubes and litmus paper, two cast metal suppository moulds,

five early pacemakers, medicine bottles, packets of hypodermic needles, two foetal stethoscopes, a fascinating appliance called ‘Schimmelbusch’s Inhaler’ and others too numerous to mention. Some of those I have mentioned are pictured here.

All-in-all, this is a most interesting collection, and I am grateful to whoever saw fit to preserve these historic items rather than throw them away. The entire collection has now been catalogued and I hope that we shall be able to put some of the items on display.

Staff Wellbeing at Work Award We’ve achieved the ‘Bronze’ level award from the Nottinghamshire Workplace Health Scheme for inspiring our staff to live healthier lives.

Staff have been benefiting from a new staff website called the Health and Wellbeing hub, to promote healthier lifestyles. The hub is designed to give staff information and advice at a touch of a button, on healthy eating, increasing physical activity, promoting specialist support services such as stop smoking and weight management. The new hub also signposts staff to where they can get emotional support and counselling if needed.

Helen Houghton, Head of Occupational Health and Wellbeing Health, said: “I am delighted that we have received this award.

Looking after the health and wellbeing of our staff is very important, so that we can provide high quality care to our patients.

Achieving this award is the start of our ambition to create a healthy working environment for all our staff.”

A key part of the Award was encouraging a large number of staff across the Trust to become Workplace Health Champions to do their bit to support and motivate their colleagues to lead healthier lifestyles.

The Bronze Award is the first of the five award levels of the Nottinghamshire’s Wellbeing at Work award scheme. The Trust is now working towards the Silver.

The other award levels include Gold, Platinum and Maintenance.

We are encouraging people with diabetes to have their eyes screened to prevent blindness.

Diabetes can harm your eyes and is one of the most common causes of blindness.

Diabetes is caused when the body is not able to use or produce insulin and leads to raised blood glucose (sugar). If these glucose levels are not controlled, other long term conditions may develop, for example problems with your eyes.

The most serious eye condition caused by diabetes is called diabetic retinopathy.

This is where high blood sugar levels damage the retina inside your eye by causing the tiny blood vessels to bleed or leak fluid, distorting vision.

Unfortunately diabetic retinopathy often goes unnoticed until loss of vision occurs. That’s why it is important to detect it early by going for your eye screening every year – remember early detection can save your sight.

There are ways you can prevent diabetic retinopathy: • Exercise regularly • Control your blood sugar levels • Eating healthy food • Control your blood pressure • Don’t smoke.

News from the Governors Sub-committee summary The Governors have been involved in a range of work over the last few months.

The Health and Care of Adults sub-committee had a detailed presentation and discussion regarding new self-check-in kiosks which are being used in outpatient areas at DRI while the Health and Care of Young People sub-committee met to discuss its workplan for future meetings. The Communications, Engagement and Membership committee focused on member recruitment, and also discussed the potential to hold a Trust open day in 2016.

Governor ward sponsors have also worked with the nursing directorate to in relation to the new ‘15-step challenge’ and the ongoing programme of Quality Assurance Toolkit ward assessments.

At its quarterly meeting in September, in addition to the usual performance reports, the Board of Governors discussed the Care Quality Commission’s inspection process, the Patient Led Assessment of the Care Environment (PLACE) report and received the external auditors’ report on the previous year’s accounts.

A Governor Timeout was held in September 2015 and the agenda covered site development and estates, pressure ulcers and claims.

Who’s the Governor?

Mike Addenbrooke is a Doncaster Public Governor.

Tell us about your role as a governor?

Mike your career was in the quarrying and aggregate industry, tell us more about it

My two and a half years as a governor have been very enjoyable even though the learning curve has been steep. I’ve joined a number of sub-committees and support two wards that have started the new 15-step ward quality scheme. I visit these wards regularly, talking to patients and staff alike to get a feel for any issues which I feed back into the quality team. I get a real sense of contributing towards the care of patients.

I started out as a sales representative for a large quarry at Stainton. We supplied materials for the construction of the M1, M18, M62 and M180 motorways, and also Barnsley and Rotherham hospitals to name a few. Over the course of my career I worked my way up to become Commercial Director of a large Doncaster based aggregate company. Working in that environment gave me a true appreciation of quality and customer service.

You’re an avid Crown Green bowls player, how did you get into it?

The village where I live, Braithwell, has a Crown Green bowling club so it was easy, I went along, enjoyed it and now I’m on the committee. It’s a great sport that involves precision accuracy. Don’t be fooled by its calm exterior, it can be very challenging and competitive at times. If you enjoy the spending time outdoors and are looking for a healthy dose of physical activity and being more connected with the community, I would certainly recommend it to anyone.

2016 Governor Elections Governor elections start in early 2016 and we will be seeking nominations for the following governor seats: • 8 Doncaster Public Governors • 2 Bassetlaw Public Governors • 2 Staff Governors, Non-clinical • 1 Staff Governor, Medical & Dental.

Successful candidates will take up their seats on 1 April 2016.

We provide all training and there’s always someone on hand to offer advice and support. Governors serve for three years and are eligible to stand for re-election at the end of that period.
