Welcome to the Autumn/ Winter issue of the Black & Lizars patient magazine. Whatever your political persuasion, there has been a lot of debate about the importance of the NHS throughout the long Referendum campaign. This has brought into sharp focus some of the key health benefits we currently receive, and perhaps take for granted, such as the free NHS eye test in Scotland, introduced back in 2006. This free NHS test, detailed on pages 8 and 9, is quite thorough but at Black & Lizars we are delighted to be rolling-out a new generation of advanced technology such as the Optomap, an ‘ultra-wide’ retinal scanner, that takes our eye care to a completely new level. lack & Lizars is marking a year of significant growth with the opening of our new flagship practice just off the internationally acclaimed shopping venue, Princes Street in Edinburgh. We are investing some £500,000 in the upscale property in Frederick Street, on the corner of tourist hotspot Rose Street. The former Santander building will house the most technically advanced equipment and will offer hearing care as well as optical services. The prestigious new practice will be the latest of our 25-strong practice network ranging from Aberdeen to Troon to Belfast. The new flagship practice will mark the start of the next chapter of our 180 year history – including 119 years of continuous presence in the heart of Edinburgh. It will be a state of the art clinical facility, with a high profile retail ground floor and will have lift access to the larger 1st floor

clinical area. The practice benefits from the fact it has the latest technology and eye care equipment. The clinical technology is a lot more advanced than most of our competitors with wide retinal scanners built by Optos in Scotland. As well as specialising in prestige brands, the three-storey practice will have advanced equipment such as 3D OCT eye scanners, bespoke hearing and eye test areas, and the lower ground floor will house a special area for children and a sports eyewear clinic. The new practice, which will replace practices on Rose Street and George Street in Edinburgh, will preserve existing jobs and may lead to increases in staffing. It is one of the first steps in a long-term growth strategy to increase our practice portfolio to 40 within the next two years. EYE CARE IN SCOTLAND ack in 2006 when eye examinations became free for all in Scotland, the content of these examinations was also reviewed. This was fantastic news, as it resulted in a more thorough and detailed test than ever before. All patients now receive a test including set procedures which vary for different groups of patients. This means that Scotland is leading the way in eye care provision throughout the UK! You may have experienced these changes yourself, or seen a family member have a longer or more detailed test than before. Sometimes we have so much to do that it leaves less time to explain why we are doing it. I want to explain the differences and benefits of the Scottish Eye Examination here.

Why are we different?
What is different? The main noticeable difference is the use of drops to dilate pupils. This lengthens your test and leaves you with blurred vision for a short time afterwards. The NHS recommends that this is done for specific groups of patients, including anyone over 60. Dilating pupils ensures we get the best view of the inside of your eye. As pupils generally get smaller with age and we are more prone to certain eye conditions, 60 becomes the age at which this is recommended. There are other times we would advise dilation; anyone who attends with symptoms indicating a problem with their peripheral retina (sudden onset flashing lights and/or spots in their vision), if you are diabetic or attend with symptoms indicating a change at the macula
(central retina). We try and prepare you for the possibility of dilation – advising you not to drive after your appointment and allowing a good hour for your eye test. Bringing sunglasses can also help as you will be more sensitive to bright light after the drops. If you are about to turn 60 you will notice this change next time you visit us. You may also notice that when we view the inside of the eye we use a binocular microscope (slit lamp) and a small lens held in front of your eye, rather than use the hand held torch (ophthalmoscope). This ‘indirect’ method allows us to see more of your retina. The pressure of your eyes is something we measure from about the age of 40. There are different methods of measuring pressure- the most memorable being the
‘puff of air’! We are moving away from this method and towards the method used by eye specialists in hospital. This involves using a short acting local anaesthetic and shining a blue light in your eye. These drops have less effect on vision than dilating drops and it is safe to drive afterwards. This method is widely regarded as the ‘gold standard’ for eye pressure measurement. A lot of patients comment that it is nicer than the ‘air puff’ too! We also test your peripheral vision (Visual Field). Many will recognise the ‘spots of light’ test. For younger patients (including children) we tend to use different methods which don’t require longer periods of concentration. There are also some child specific tests we always do on at least their first visit. These are checks for their 3D and colour vision. These are two simple and fun tests that can tell us a lot about how your child sees the world, and if they need any help to see it better! The Scottish system was developed to improve the visual health of the nation by improving access to quality eye care. The results so far are looking good but if there was one simple piece of advice that we could give every patient it is to act on any symptoms. If you experience changes in your vision (especially if those changes happen quickly or include distortion), flashing lights, double vision, painful or red eyes you should call your local practice.

These new devices, the largest network in the UK, help to further reinforce the clinical and technological reputation of Black & Lizars, which already has one of the most extensive networks of 3D OCT

The 3D OCT scanner uses light waves to create 3D images of the eye, thus allowing us to see ‘under’ the surface of your retina. Again, this could allow us to pick up any potential problems before the patient is even aware of them. John Watt, Principal Optometrist in our Ayr practice, adds “By utilising OCT scanning, we can now detect a number of key eye conditions, such as glaucoma, age-related macular degeneration (AMD) and diabetic eye changes during a routine eye examination. Having an OCT scan taken is a quick and easy process. Nothing comes into contact with the
eye and it is completely pain free. You will be asked to look at a target and the scan will be taken in less than a minute, much like getting a photograph taken.” The above technology, taken together, now allows us to offer an ‘Enhanced Eye Examination’ for the small additional charge of £50, in these 10 practices, which also includes a longer appointment for you to view and discuss the images with your optometrist. a daY in thE liFE oF an ASSISTANT PRACTICE MANAGER iona Kelly is Assistant Manager and a fully qualified Dispensing Optician at Ayr practice. Fiona has worked at Black & Lizars for over 12 years (previously at Kilgours Optometrists, Ayr. Kilgours Optometrists, founded in 1961, joined the Black & Lizars family in December 2012). She tells us what it’s like to work in a busy, optical practice.
F
What are your main responsibilities? I would say my main responsibility is to provide all our patients with a high level of customer service and to ensure their needs are met. I also assist Clair, our Practice Manager, in the general running of the practice.
Why did you decide to become a dispensing optician? I wanted to be involved in optics from a young age. I think the fact that I wore glasses from nine years old got me interested in the profession and I always looked forward to having an eye exam.
What specialist services are offered in your practice? We offer retinal fundus photos, OCT, Optomap, Low Visual Aid assessments, diabetic screening, dry eye advice, specialist contact lens fittings and post cataract reviews. We also provide tailormade spectacle lenses, surgical loupes and recently we had a hearing booth installed.
Tell us about a typical day in the practice? A typical day consists of a variety of different tasks including helping patients to choose new frames, giving advice on spectacle and contact lenses, adjusting the fit of new spectacles and carrying out additional tests for the optometrists, for example, fields, pressures, fundus photos.
Why do people come to Black & Lizars for their glasses? I think the reputation of Black & Lizars and previously Kilgours has been gained through many years of experience and knowledge, which leads to patient satisfaction and loyalty. We have a wide variety of frames to suit all budgets, giving patients maximum choice.
How do you plan your day? We start our day with a morning meeting. Each member of the dispensing team is paired up with an optometrist for that day to ensure we are available to assist with any queries or dispenses.
What advice can you give to people who are nervous about wearing spectacles for the ďŹ rst time? I would reassure the person that the spectacles looked good and go over all the advantages to be gained from wearing

them – better vision, more confidence. There isn’t the same stigma attached to wearing spectacles, as many people in the media and showbiz have made it quite trendy. What’s the most important thing to remember when choosing new glasses? It’s important to make sure you get a frame which meets the size and style requirements of the individual. Good lens advice will also ensure that the end result meets a patient’s needs and expectations. What’s your favourite eyewear trend of the moment? I like some of the bold quirky designs that are available in ranges like ProDesign and Vanni. A lot of brands are bringing out some bigger, retro shapes which I quite like. What is your favourite part of your job? I really enjoy the patient interaction. Over the years I have built up a good relationship with a lot of patients and love to hear updates since their last visit.

Excellent communication skills are required to gain an understanding of the patients’ needs and expectations. Good numerical skills help with analysing a patient prescription. A high level of technical ability is necessary to dispense and adjust a variety of frame materials. Tell us about a memorable experience? I remember fitting on a small child’s first pair of glasses. When he put them on, he had the biggest smile on his face, turned to his mum and said “I can see!” It made my day! How do you relax in your spare time? In my spare time I like to read and spend time with friends and family. I also enjoy watching football, darts and driving fast cars!
How important is the quality of your patients’ experience to you? It is extremely important to me that our patients have a good and also enjoyable experience when they attend the practice. I quite often ‘become the patient’ to ensure that I would be satisfied with the level of

s the arrangements start for another Festive season (so soon?) we can look back on a momentous 2014. Referendum frenzy occupied the first part of it and, for golfers, the Ryder Cup soon replaced it with just as much interest! This piece has to be about eyes and optometry and the Ryder Cup is a good ‘aide memoire’ about the contrast enhancing lenses that have replaced normal sunglasses for sportsmen. Oakley, in particular, as well as a lot of other well known brands have launched these lenses and I can personally endorse the remarkable effect that they produce. Every contour becomes highlighted and the judgement of relative distance is made considerably easier. All sports participants, ski, cycle, golf, running, tennis and more, can benefit from these lenses and all Black & Lizars dispensing teams will advise on what tints would best help you in your favoured sport. In 2014 we have also seen an exciting new range of contact lenses for keratoconus. The advent of contact lenses for astigmatism has meant that people are no longer having to ‘make do’ with their contact lens vision. With the increased prescription ranges available to us, we can more easily give you the vision that you expect. Many will be familiar with the changes that occur to near vision and reading when we pass the landmark of our 40th birthday. The need for reading glasses is something that will come to most of us at some stage. The good news for contact lens wearers is that the advent of multifocal contact lenses means that many patients can now see in the distance and read with the one pair of contact lenses – meaning that they have less need for reading glasses. These lenses are even
now available in daily disposable formats for full or part-time wear. The downside of having all of these options available to you is that if you are not careful, things can start to get a little confusing. This is where the advice of an expert can be invaluable. At Black & Lizars, our optometrists are trained to the highest standards and have knowledge of, and access to, the latest contact lens products. At your routine contact lens check appointments we will monitor the lenses you use and advise if there are any better or healthier options for you. In addition to wearing the healthiest lenses the other way to reduce any future problems is to follow hygiene and wearing advice. Studies show that the vast majority of lens wearers are not compliant with hygiene advice, and this greatly increases the risk of eye infections and inflammation, potentially affecting your ability to wear contact lenses. For any lens wearer the three most important actions to take are: 1. Wash and dry your hands before handling your lenses. 2. Actively clean your lenses by rubbing and rinsing with the lens solution after wearing. 3. Replace your lenses on time, whether daily, fortnightly or monthly. If you follow this advice, and we pro-actively upgrade your lenses to the healthiest and best option for you it is likely you can continue to wear lenses for many decades to come.

Peter Stark, a patient at Black & Lizars, tells us what a difference having the right glasses makes
ally drivers have to react to constant stream of directions. “Flat over crest, one fifty into three right – don’t cut, two hundred into eight left…” Left!? He said left didn’t he!? Doesn’t look left…and an eight is a tight corner to get wrong being 80 degrees!!! Hurtling down a narrow tarmac road in the pitch dark on a night rally stage isn’t the greatest time to discover your eyes aren’t operating quite at their best! Tiredness apart, I knew it was time to visit the opticians. Having worn glasses for a matter of months, the new season was approaching and having ventured into circuit racing where you wear a full face helmet rather than an open face helmet for rally driving, it was becoming more than a passing inconvenience trying to fit ordinary spectacles beneath my helmet and in between all its padding! I’d initially gone to Black & Lizars’ Belmont Street branch in Aberdeen on recommendation from friends and having been impressed with the facilities available for their opticians to ensure each pair of glasses are exactly to their customer’s needs, it was an obvious choice to return

with my challenging requirement. I was met by the Practice Manager on arrival where I explained that this was no ordinary frame request and we discussed the issues faced when trying to wear glasses in a motorsport environment. The manager spent considerable time understanding how even putting glasses on properly beneath a racing helmet can be troublesome; keeping them in position while the helmet’s padding is fighting against them and the vibrations from the kart, which is rock-hard having no suspension! I was shown a range of sports orientated frames, however the shape of the temple tips around the ears meant these were difficult to fit in-between the padding once the helmet was in place. The manager then had the idea of trying their range of sunglass frames and we found the straighter temples and more fitting rims were most suitable. I learned that Oakley are one of few manufacturers to tint their lenses after prescribing them, so with a few frame options near our target, the manager arranged a meeting with their Oakley representative, so we could discuss the best solution. With Black & Lizars having such a close
relationship with their manufacturers it was to prove invaluable, as they were able to arrange a wide variety of options for all three of us to work through until we found the perfect solution; both in ease of fitting and remaining in position, along with comfort. MADE TO MEASURE Lucy advances knowledge of long and short sight
e would like to congratulate Dr Lucy Stevenson who has recently completed a PhD in Optometry. Lucy is an optometrist who has worked with Black & Lizars since leaving university 8 years ago. Most recently, Lucy has been working between our George Street, Edinburgh and Milngavie practices. Lucy spent 6 years researching and writing her thesis, ‘Differences in optical and physical characteristics of the eye in myopia and hyperopia’. The research involved using specialist equipmentOrbscan Corneal Topographer, IOL Master and Zywave. Collectively they measure the shape, dimensions and higher order aberrations of the eye. This research identified structural and internal differences between short and long-sighted eyes thus giving insight into the differences that exist between these two groups. By undertaking these tests at an early stage it may be possible, in the long term, to prevent the onset of short and longsighted prescriptions and detect their pattern of growth. The time and effort required to complete this PhD has allowed Lucy to

develop her analytical and clinical skills as an optometrist. Undertaking a PhD has given Lucy a depth of knowledge that can be used in her daily work, testing and advising patients on eye conditions and developing specific solutions to meet their needs.
