President’s Message

If you think a few checkpoints on the world’s largest undefended border is going to stop the traffic of fentanyl or carfentanil laced heroin into King County; guess again. For those of you that don’t know what carfentanil is, it’s an elephant tranquilizer, a synthetic opioid. Carfentanil is 10,000 times stronger than morphine and 100 times more potent than fentanyl. We need to be prepared for this ever-looming health disaster. Unfortunately, “researchers have estimated that 5 to 23 percent of all prescription opioid doses dispensed are used non-medically…particularly hydrocodone and oxycodone. Dentists prescribe 12 percent of (Immediate Release) opioids in the United States, behind only family physicians, who prescribe 15 percent.” (JADA 2011) The efforts of dentists such as Dr. Amy Cook and the WSDA House of Delegates to pass HD-12-2016 should be applauded. Here’s what the Resolution says: Therefore, be it resolved the WSDA should promote the following methods of raising awareness : 1) Dentists should register for Washington State’s Prescription Monitoring Program and check for abuse with all patients who request pain medication.

Opioid Abuse EpidemicWhat is the SKCDS Member’s role? On Saturday, January 7th, 2017, the Seattle Times reported three people died of heroin overdoses on Aurora Avenue. Easy to ignore as, “just another drug overdose story.” However, many of us have seen the devastation these drugs cause first hand on our family and friends. As dentists and citizens, we need to take notice of the epidemic of opioid abuse in the country. A situation that is bad and only going to get worse.

2) WSDA member dentists are encouraged to complete the ADA’s online continuing education course regarding opioid abuse. 3) WSDA member dentists are encouraged to communicate with each other regarding the prescriptions they have written when referring patients. 4) WSDA member dentists are encouraged to prescribe the fewest number of tablets of opioids possible and to educate patients on where to safely store and/ or dispose of left over tablets. 5) WSDA should provide opioid abuse continuing education at their annual dental convention.

Check Out Our New Website! Your Seattle-King County Dental Society has partnered with the American Dental Association to roll out a new tripartite branded website for our Society. Beginning in January, this new website launched to everyone.

We are excited about this change for several reasons: 1. You, as a member of all three organizations (SKCDS, WSDA and ADA) also known as the tripartite, will now be able to seamlessly move between the websites—especially the “Member’s Only” sections—by just signing on once. (This will become even more seamless when the WSDA also moves to this tripartite branded website in the first quarter of 2017.) This ‘single sign on’ means that you only have to remember ONE User ID and ONE password to access all three organization’s websites. 2. It will become even easier to register online for free membership or New Dentists meetings as well as enroll and pay for courses offered by any one of the three members of the tripartite. You will be able to sign up for courses 24 hours a day/7 days a week. 3. Our “Member’s Only” section is now even more robust. We invite you to log in and take a look around. We have added a Breaking News section as well as other important resources like a staffing salary and benefit survey. 4. The general public will more clearly understand that, as a member of Seattle-King, you are also a member of the American Dental Association—an affiliation that we know matters to prospective patients. 5. Our website will now tie more seamlessly to the ADA’s Mouth Healthy website—an award winning public information website for oral health. 6. Your Dental Society staff will be able to update this website easily and quickly—ensuring it is always bringing you the most up-to-date information you need. We will even be able to add alerts to the front page if we needed to do so. 7. Finally, our new format is mobile telephone and other devices compatible—giving us a clean, easy to use website wherever the member or patient is. We welcome your feedback on the new website as well as any suggestions you might have for ways in which we might further enhance it. As more and more us become tethered to our mobile phones and other electronics, it is critical that your Dental Society be as useful as possible to you.

STATE REQUIREMENTS, TRAINING AND YOU Did you know that Washington State might be raising the requirements for oral conscious sedation? Be prepared by getting your state required CE this spring!The Seattle-King County Dental Society, UW School of Dentistry and the Washington AGD are happy to present Dr. Stanley F. Malamed, speaking on “Management of the Fearful Dental Patient,” Friday, May 12, 2017. In Washington State, this course will fulfill the 7 hours of CE required every 5 years for using nitrous oxide/oral conscious sedation, or can be applied towards the hours required to obtain a permit in Level Two moderate conscious sedation. Fear of the dentist is one of our most common fears. Many patients have avoided seeking dental treatment because of these fears. In this program, Dr Malamed will review the more commonly employed management techniques for dental fear and anxiety, includingthe oral route of drug administration (e.g. triazolam [Halcion]); inhalation (N20-02); intravenous (IV), intramuscular (IM) and intranasal (IN). Techniques will be compared as to efficacy and safety, as well as their utility in the typical dental practice. Dr. Malamed, a dentist anesthesiologist, is the author of three widely used textbooks: Handbook of Medical Emergencies in the Dental Office (7th edition 2015); Handbook of Local Anesthesia (6th edition 2012); and Sedation - a guide to patient management (6th edition 2016) and two interactive DVD’s: Emergency Medicine (2nd edition, 2008) and Malamed’s Local Anesthetic Technique DVD (2004) (edition 2 - 2012). Dr Malamed has authored more than 160 scientific papers and 17 chapters in various medical and dental journals and textbooks in the areas of physical evaluation, emergency medicine, local anesthesia, sedation and general anesthesia. NEW DENTIST PROGRAM 2017

Meet, Socialize, Learn.

When the winter where turns gloomy, it’s a great time to catch up on your CDE credits. Learn the finer points of practice management while seeing old dental friends and meeting new ones. Starting on January 18th, SKCDS is offering our annual program of monthly New Dentists meetings. We will begin with a panel of practicing dentists who are working in various realms of dentistry. Dr. David Dean will talk about academia, Dr. Brittany Dean will speak on working in a community clinic, Dr. Princy Rekhi will talk about running a multi-site practice, Dr. Sara Cassidy will talk about starting a practice from scratch, and Dr. Paul Amato will address taking over a practice. These young dentists have a lot to share about the different ways you can practice dentistry. Think outside the box as we explore options besides just being an associate. On February 15th we will explore ways to increase your profitability. We will welcome three experienced practitioners who will look at TMJ, Botox and other distinctive services they offer their patients. Hear about their experiences, and what you can do to offer additional services in your practice. On Saturday, March 25 we are bringing back Boot Camp! If you have not attended Boot Camp in the past, consider checking it out! The program has been refreshed and will feature shorter, more concentrated presentations by our local experts. Lean about the basics of starting your practice and what to plan. On May 10th we will host Ben Tuinei, president of Veritas Dental Resources, to speak on third-party payers and what you can do to get the best results for your patients and practice. Effectively negotiate with third party payers and increase your annual insurance-related revenue. What do you need to know about how insurance affects your practice? Mr. Tuinei speaks regularly on insurance issues. This event will be held at the University of Washington South Campus Center. Food is included in all New Dentist meetings. SKCDS and Public Health - Our Forensics Comittee in Action In a continued effort to strengthen our relationship with Public Health - Seattle & King County (PH – SKC) Department of Health, 19 SKCDS Forensics Committee members attended orientation, on December 6, 2016, to join their Public Health Reserve Corps. Public Health Reserve Corps members are volunteers in the public health field who offer their expertise and services in case of a public health emergency or disaster. Public Health Reserve Corps works in conjunction with FEMA and the Corps members had to get clearance through an intensive background check. As part of the orientation, the Committee also toured the County morgue, given by the King County Medical Examiner, Dr. Katherine Taylor, who explained the systems and processes in place, in case of an emergency. In the past, the SKCDS Forensics Committee has worked with the Department of Public Health in times of emergencies and disasters. As an example, they worked with the Snohomish Public Health Department in the aftermath of the Oso landslide of 2014, to help identify victims’ bodies, using the victims’ dental records. Our volunteer Committee members were also recognized by the Governor’s office for their contribution of time and expertise. Harborview Medical Center is the only trauma center available to four states: Alaska, Idaho, Montana and Washington; therefore, the work of the Public Health Corps and its volunteers is even more crucial. A big thank you to the Committee members who have volunteered should an emergency or a disaster occur. Electronic Voting for 2017 – 2018 Election and Installation Set for Tuesday, April 25, 2017 The response to electronic voting was so positive last year that your Dental Society will again be emailing you an electronic ballot to the email address at which you currently receive our weekly newsletter. (For those of you for whom we do not have an email address, please see the bottom of this article.) In accordance with the Seattle-King County Dental Society Bylaws, ballots for the 2017-2018 Elective Office openings will be emailed from the Central Office no later than April 10, 2017. Your ballot is individualized and can only be opened by you. You will receive two reminder emails after that initial ballot. Please vote and make sure your voice is heard! The ballots must be cast by 5:00 p.m. on April 25, 2017, so they may be presented at the General Membership meeting to be held at Salty’s Restaurant on Alki on April 25th, beginning at 6:00 p.m. (Please see the registration form on our website at www.skcds.org and on page 15). Mentors By Al Munk DDS We all have mentors, our whole life. These are people who teach us, influence us and guide us through our lives. This process starts with our parents when we are small, then expands into all those we come into contact with as we grow older. We all remember a teacher, for example, who has long since forgotten who we are, yet continues to influence the way we are currently living our lives. Most mentors are a positive influence on our lives, but certainly the opposite is true as well. At every juncture of life, we are faced with decisions on which way to go, and often, for better or worse, there is a mentor to help us make these choices. A wrong decision can land us in jail or worse, a right decision can bring us happiness and prosperity.

EXPOSURES

In high school, one must decide which group of kids to hang out with. There are the jocks, the nerds, the “hoods”, etc. The hoods in my town were the kids that crossed the street at lunchtime and smoked. It was OK to smoke as long as you crossed the street away from school. These guys wore leather jackets and combed their hair back in waves like Elvis. There were lots of fights amongst the hoods, but nobody got shot like they do now. If these were your mentors, you learned their ways. Girls seemed to like these guys. The James Dean scenario, of the misunderstood gangster was alive and well in those days, and continues to this day. Girls are attracted to the challenges and mystery of our troubled youth. Of course I hung out with the nerds. We watched in awe as the jocks and hoods walked by with the cute girls by their side. We could only dream what this must have been like. The nerds would eventually rule the world, but we didn’t understand that then, and could only lament our misfortune at being short, smart and near-sighted. This inequity continues to this day—Madonna once said “I don’t date dentists.”

wisdom, I was sent out into the world, hoping I would one day discover what he was talking about. During college, there were once again choices to made about mentorship. My mentors were smart kids who partied hard. It was extraordinary to me that one could get straight “A”s, and still party all night. I met a few kids that could pull this lifestyle off, and I mapped my life after them. A few years of this, and I barely squeaked by, somehow got admitted to the one dental school which I applied: the good old UW. This got me out of draft, which would have provided a special mentorship all its own. Dental school certainly was a laboratory of what constitutes a good mentor. There were mean and evil mentors, who cared little about the welfare of the student, only caring about some hidden personal agenda that we would never discover. Then there were the true and good mentors, who would do anything to help you learn this complex trade. These people stood out as examples for us, and provided lifelong guidance as we embarked on our careers in dentistry. Now I find myself as mentor. I have children of my own who look to me for guidance and leadership. They are grown now but have found the difficulty of life decisions in their young adult lives. As well, I mentor other dentists in study clubs and online. The study club dentists are the ages of my children and are eager to learn the mysteries of our vast profession. I often think of how my lifetime mentors would handle a particular situation, as I attempt to pass on accumulated knowledge. Through the miracle of the internet, I can also mentor dentists in Slovenia and Yemen. They have never met me and fortunately don’t know I was just one of the high school nerds.

When it came time for me to leave for college, my father provided me with mentorship. This came, I am certain, only under duress from my mother. She insisted that he have a “talk” with me before I left. The subject of course was about sex, the topic that dominates all youthful thoughts and conversations. I was working for him at the time, and he cornered me in the store one fateful afternoon.
