Executive Director

We Only Get 18 Summers All of us have parents, some of us are parents, and others are grandparents – the thrills of experiencing our children’s lives as they develop from infants into young adults is a rite of passage that I am in the midst of. As I reflect upon my first year as Executive Director of the Seattle-King County Dental Society and then look forward towards our future, I realize that raising children is a lot like guiding our dental society.

In the movie Dave – Kevin Kline is the President and he states “I forgot that I was hired to do a job for you and that it was just a temp job at that. I forgot that I had two hundred and fifty million people who were paying me to make their lives a little better...” My friends in Scouts constantly ask me if I like my new job, my answer is that “I don’t like it, I LOVE it.” Serving the 1,714 members of the SKCDS is an honor and privilege; yet there are 3,131 total dentists in King County (or stated another way 1,417 non-members) – and I serve them all while serving our profession.

Lesson One: Treat every day on the job like it is your last, love what you do, and make other’s lives a little bit better.

I recently returned from Philmont Scout Ranch with my oldest, a fifteen-year-old Life Scout – where his crew pooled together their amassed trainings and experiences to forge the best possible outcome for our team. A Philmont trek is like a capstone project for a graduate student to pull together everything learned and apply it in a practical fashion. We were all hoping for a perfect outing – what we got was far from it. But was it a successful outing? Yes, and the obstacles they overcame as individuals and collectively as a team will serve them in college, through graduate school and into their careers. When I returned, I had hundreds of emails and many voicemails to respond to – one rose to the top of my list; a dental colleague of over 20+ years chose to rejoin our dental society because how organized dentistry has responded to COVID and mitigated thousands of dollars of unnecessary costs to her office.

Lesson Two: No organization is perfect, most organizations do many things right, with constant gradual improvements to overcome obstacles and address problems. One of those days the problem we address – might be the one that sings to your heart.

As a child I was surrounded by dentists growing up: my father Yosh, my uncle Mark, Curly Coleman, Pat Fleege, Chuck Webber, Gene Brain, Rod Dubois, Art Dugoni and a whole host of other positive role models from dentistry during my eighteen years before college. The lessons taught through experienced based learning: fishing for salmon at Westport, razor clam digging at Ocean Shores, picking matsutake mushrooms or wild huckleberries in some secret family spot and hunting for pheasants in Eastern Washington. Growing up, I learned how to fish from my father & uncle; I was taught gun safety by Rod Dubois and I was told how things were by Gene Brain. When I was just an ankle biter – I met Art Dugoni at some orthodontic meeting my father was attending. I was introduced to him in passing…but more on that later. In between dental activities, my father took time to coach my baseball teams, watched me play football, took us to UW Husky Games and even acted as a Den Leader for my Cub Scout Pack. Fast forward to my D2 year when I was representing ASDA as the Consultant at National Board Exams at the ADA Building in Chicago, and none other than Art Dugoni steps onto the elevator and strikes up a conversation with me in front of the rest of the Commission. He stepped off the elevator with us long enough to ask how my parents were – addressing them by their first names - and referencing the orthodontic meeting where he had met me as a child. Instantly, my creditability in the eyes of the other ADA Commission members shot through the roof.

Lesson Three: Dentists have been great mentors and volunteers, while we like to talk about groups, numbers, or statistics; remember that people matter most – as individuals who have real problems and need real solutions.

In 2020, we started off by volunteering in record numbers at the Seattle Center Clinic and SKCDF providing almost $9K for dentures…then the whole world was sucker punched by COVID. Dentistry in Washington was shut down for almost 3 months and the way the world worked changed before our very eyes. Dentists are resilient and when it came time to re-open, we found ourselves short of critically needed PPE – adversity forges strange bed fellows and so organized dentistry teamed with Delta Dental to secure PPE from the state’s emergency supply on the Friday before we were scheduled to reopen. In a scene akin to the Whos in Whoville celebrating in their village without Christmas - Hundreds of dentists, not just from SKCDS, but from across the state stood side by side with employees from Delta Dental, hygienists, Scouts and employees from organized dentistry – repackaging and distributing masks to ALL the dentists of Washington State. SKCDS teamed with WSDA to put together a mask fit testing program – to ensure our members and their teams could return to practice in the safest possible manner, while complying with rules that made no good sense. Statistics from the ADA Health Policy Institute (HPI) have demonstrated that the safety measures taken by dentists for their patients and teams have been effective. On top of that, 100+ SKCDS volunteers took vaccine administration training, and stepped up to reinforce the beleaguered front-line nurses and doctors to help administer the vaccine during the height of the initial pandemic.

During 2021, SKCDF acted on the clinical portion of the Memorandum of Understanding (MOU) signed with Medical Teams International (MTI) to start providing dental van care to the underserved. By partnering with the UW SOD students and faculty, we have treated 82 patients and provided over $44K of care to these individuals. SKCDS recognizes that there are still systemic inequities in our communities and profession – to that end we have invested in a series of upcoming trainings and lectures to start integrating the principles of diversity, equity, and inclusion across all aspects of our Society and Foundation – we invite all members and non-members to embark on this voyage of discovery together.

The Ambassadors of SKCDS

The Welcome Wagon, Goodwill Ambassadors, On-boarders, the Disciples of Dentistry...whatever the label, every successful organization has a way of promoting its own culture and values. Your Seattle-King County Dental Society is no different. We are the largest dental society component in Washington State, which creates a unique problem: how to bring together dentists over a large geographical area with diverse backgrounds and practice specialties. Our answer is the SKCDS Ambassador Program- a unique cadre of dental professionals willing to volunteer their time to greet new member dentists to our Society. The goal of the Ambassadors is to acclimate these newcomers to the area by offering a personal connection, whether it is to Seattle-King County on the whole or the more specific SKCDS functions. Basically, be a friend, be a connection should one be wanted or needed.

You may be wondering how much of a time commitment is being an Ambassador? We simply ask that you create time once a month, as assigned; to connect and introduce SKCDS to interested new dentist members. No cold calling is involved and you would only be contacting members that have opted-in for the program. To help you prepare, we have a network of veteran ambassadors that you can bounce questions and ideas off of, as well as share what is important to our members. This small contribution of time can help guide the direction of our dental society and even lead to new personal connections for you! MEMBER FEATURE: Dr. Chris Shyue When he was very young, Dr. Chris Shyue wanted to be a garbage truck driver…or maybe a firetruck driver…or MAYBE a backhoe operator. And then all of this changed after watching the original Top Gun movie, because now he was going to be so cool and become a fighter jet pilot! Unfortunately, that dream was shattered when he was diagnosed with nearsightedness in 4th grade, and so the cycle of future dreams began again. However, throughout his youth, he had a horrible sweet tooth and often had to visit his dentist for cavities. These visits were never particularly enjoyable (given the circumstances), so there is some irony in where his path would eventually lead him.

When Dr. Shyue studied abroad in Thailand during college, he met people who had never enjoyed the privilege of dental care. He realized how lucky he had been to be able to take dental care for granted and so, prior to starting dental school, Dr. Shyue volunteered with Medical Teams International’s Mobile Dental Clinic. This made a further lasting impression upon the future dentist, because it was gut wrenching to realize that the lack of access to care is not a unique problem in developing countries and, in fact, can be a problem in the place you call home. These experiences solidified his desire to become a dentist and help those in need. He even continues to do so in magical ways, making sure to check with each extraction patient on if they would like to take their tooth/teeth home (after proper disinfection, of course). He doesn’t want to risk anyone missing their scheduled visit from the Tooth Fairy!

Outside of his dental world, Dr. Shyue stays more than busy as he started road cycling in 2018 and, pre-COVID, enjoyed travelling and trying new food. Travel has obviously been scaled back a bit lately, so he feels fortunate to have travelled extensively during dental school and undergrad, both alone and with friends.

As for road cycling, Dr. Shyue and his friend, who originally invited him to try the sport, have completed several long-distance rides locally with Cascade Bicycle Club, including: STP (Seattle to Portland, 207 miles in one day), RSVP (Ride from Seattle to Vancouver and Party, 187.5 miles in two days), R2B2 (Ride from Redmond to Bellingham and Back, 204 miles in two days), and Gigantic Bicycle Festival (100 miles in one day). While he does not necessarily love every moment of these journeys (especially in chilly rain or unbearable heat and feeling utterly exhausted), the sense of accomplishment at the finish line has kept him signing up year after year!

In the future, Dr. Shyue has a personal goal to complete a triathlon, but the one hiccup in that plan is he needs to become a better swimmer… since right now he’s a bit more skilled at sinking. Although he does have his PADI Open Water Diver certification that he earned during his time in Thailand! Professionally, he aspires to learn implantology and be able to provide predictable care for his patients. In pursuit of this goal, he signed up for the ‘Clinical Implantology from A-Z Program’ led by Dr. Alan Yassin at the Washington Academy of General Dentistry, a ten-month training course that he recently completed! This course was an amazing learning experience for him, in which he learned the science of implantology and improved his surgical techniques, which helped him gradually expand his skills to be able to do advanced procedures, including lateral window sinus lifts. He feels he has become a better clinician, but this has also shown him that the more you learn and know, the more you realize there is to learn. As a result, he plans to continue his training with Dr. Alan Yassin in the ‘Advanced Implantology Masters’ course starting in November 2021.

Your Foundation’s Impact The goal of your Seattle-King County Dental Foundation is to eliminate dental pain and infection in King County residents. We don’t always realize it as we go through our day-to-day lives in the beautiful Evergreen State, but access to basic dental care is still considered a luxury for many and out of reach for so many more.

Recently, the SKCDF and MTI Partnership was launched, allowing for mobile clinics to serve communities that may not otherwise have access to dental care.

At a single two-day event: • 48 patients were treated • 157 procedures were performed • $21,189 was the value of the procedures • 20 referrals were given • On one of the days, we were lucky enough to include a pop-up vaccination clinic, so 30 people also received their COVID-19 vaccines!

In 2021, the Seattle-King County Dental Foundation was able to further our goals of making a difference for our community by funding the following projects: • A Dental Sealant Program, piloted by the SKCDS Access Committee, to put sealants on six-year molars in an effort to prevent tooth decay in children.

• • This program is initially being launched in the Highline School District to serve their at-risk student population.

• The launch of a Community Oral Health Advocate (COHA) program through the UWSOD to assist individuals in finding dental homes and providing education oral health.

The program also aims to train students from underrepresented minority communities to be COHA’s in their own communities, with the goal of supporting their pathways to health professions.

• A partnership between the Seattle-King County Dental Society and KidsQuest Children’s Museum to reach more diverse audiences with the message of oral health in new and creative ways.

• Contributing to the purchase of a Cone Beam Computer Tomography (CBCT) Scanner at the Swedish Community Specialty Clinic to allow for same day surgical planning and treatment for vulnerable populations.

Member Meetings and Continuing Education As we continue to navigate the ever-changing mandates in our world during this time, the Seattle-King County Dental Society remains committed to meeting the expectations of our members for quality Continuing Education programs. A silver lining for the Society on the whole is that this has permanently opened the door to a wider acceptance of virtual opportunities and the growing necessity of this medium. In the coming program season, the Society’s calendar is filled with virtual and hybrid opportunities, presented respectively by the Program, Membership, New Dentists’, and Diversity, Equity, and Inclusion Committees. At press time, the full calendar of events is still in development so be sure to read your Weekly Update and check the website for additions!

The program season launched virtually on Tuesday, October 5, 2021 with a presentation from Dr. Christine Shigaki on ‘Total Health via Innovative Dental Care’. This program encouraged all attendees to think about their patient’s care as a whole and why it is vital to adapt your practice to this thinking as all healthcare fields become more integrated. Dr. Shigaki addressed the importance of providing care for the whole person and why it is vital to adapt your practice to this thinking as all fields of healthcare become more and more integrated.

You can next join us virtually for two CDE credits on Thursday, November 4, 2021 from 6:00 – 8:00 PM when Dr. Susan Maples will be presenting‘Slaying Dragons: Acid Reflux and Diabetes Detection in the Dental Office’. This course will teach you how to recognize the oral ramifications of these conditions and aims to inspire our members to embrace a more profound role in facilitating health change behavior in their patients. However, prior to Dr. Maples presentation, we will be joined from 6:00-6:30 pm by Representative Michelle Caldier to provide a legislative update.

The topic of the winter meeting on Tuesday, January 25, 2022 will be ‘Teledentistry and the Digitization of Patient Care’ from 6:00-8:00 pm, presented virtually by Dr. Nathan Suter. Objectives of this program include: 1) Explain the use and benefits of teledentistry in the context of pre-pandemic, pandemic, and post-pandemic care. 2) Discuss the application and utility of digital technologies. 3) How implementation of teledentistry can improve access to care and potentially reduce the cost of oral healthcare. This program will be worth two CDE credits. As is tradition, the Spring Meeting with an ‘Installation of Officers and the Honoring of Life Members’. This meeting will take place on Tuesday, April 26, 2022 from 6:00 – 9:00 pm and be worth two CDE credits.

As a reminder, all of the CE Programs are free to members, regardless of the format chosen. For hybrid classes, we charge spouses and dental staff $75 to attend in-person, with non-members outside of these two designations being charged $150 for the in-person portion. For classes that are offered entirely virtually, and the virtual option of a hybrid class, we charge non-members $75. For the in-person portion of a hybrid class, we request a 72-hour advance cancellation notice from ours members to determine accurate numbers. If a member “no shows” after registering, they may be charged $75 to cover the cost of food ordered on their behalf.

You suspect OSA. What next?

Much like we screen patients for oral cancer, it is our duty to screen patients for sleep related breathing disorder (SRBD) as part of our comprehensive and annual exams. In fact, in 2017 the ADA put out a policy statement turning this into the standard of care. Some common surveys for this are the STOP BANG and the Epworth Sleepiness Scale. In addition to obesity and hypertension, dentists are able to spot other risk factors such as retrognathia, scalloped tongue, very narrow arches and enlarged tonsils. However, once you suspect SRBD, what should your next step be?

I’ve seen suggestions of either pulse oximetry or home sleep testing out of your office. This method can be full of legal and ethical pitfalls. These tests are not as accurate as an in-lab sleep study (PSG) and often under-report the patient’s condition, therefore I believe they are not the most appropriate first step.

Additionally, if someone tests negative on a home screening test at the dentist, it is then very hard to get them to move forward to the PSG. If they do suffer from SRBD, you could be left open to liability for failure to refer; worse, if you make a diagnosis of primary snoring based on one of these tests you are now practicing medicine.

The ADA policy states, “These patients should be referred, as needed, to the appropriate physicians for proper diagnosis.” Where to refer is a common question: you have three main options. You can refer to their primary care physician. While some medical insurers require this, it is often inefficient and may be another barrier to getting a study. Unless it’s required or the patient has a very close relationship with their PCP, I would skip this step. Your next choice is an ENT. This is a common first referral if the patient is a child. Some ENTs will take out tonsils and adenoids on symptoms alone, but most will require either a history of chronic infections or a positive sleep study. Your final referral choice would be directly to a sleep doctor. Ideally this would be someone who could meet with your patient face to face (either in person or via telemedicine) before creating an official diagnosis. Reach out to your local sleep doctors and get their referral form. A simple fax with a summary of what you are seeing and their Epworth and STOP BANG scores is all that’s needed to get the ball rolling. We’re lucky in King County that we have numerous resources, from big hospital-based sleep clinics to independent full sleep labs, to full telemedicine options.

SKCDS Leading on Hard Issues - An Interview with Your Leaders This is the second of a multi-part article interviewing three volunteers who are working with SKCDS and SKCDF to make a difference in our communities, profession, and the world we live in.

Diversity, Equity, and Inclusion (DEI) is not a new concept, the protests of summer 2020 shone a light on many of the inequities in our society, and highlighted the need to carefully consider how diversity, equity and inclusion are navigated in all areas of life. Great words are just that – words. And as many of us have grown up hearing, “Actions speak louder than words”. So, let’s act.

Past-President Lisa Egbert served on the DEI Task Force that developed the Society’s DEI statement and is now the co-chair of the DEI Special Committee. At the same time, the Seattle-King County Dental Foundation’s (SKCDF), President Nina Svino tasked the SKCDS Access Committee to develop an overarching strategy of projects that SKCDF could look to fund in the short, near, and long term to fill needs in our community. SKCDS Access Committee Chair, Rolf Christensen, assigned this to a work group of whom Dr. Doug Jackson was one of the authors of ‘The White Paper’ to SKCDF to recommend projects.

OGATA (O) – What one thing would you like to see done to address workforce issues in King County?

EGBERT (E)- The simple answer to addressing the severe shortage of hygienists is to expand existing programs and/or open new ones. There are far more qualified applicants to hygiene programs than there are spots available. Without support staff, there is no way to provide quality care for the people of King County at a reasonable cost. Talking to career counselors about the benefits of entering the dental profession and removing barriers to education is critical.

Encouraging people from diverse backgrounds, shortening the time needed to obtain credentials, and providing education at non-traditional times and locations are obvious strategies to get more people into these jobs.

SVINO (S) - We need to create awareness of job opportunities and education in the community and what kind of financial support can be relied on while they are training.

JACKSON (J) - Just like Washington State, the racial and ethnic demographics of King County are changing. As a health profession, the diversity of the workforce needs to keep pace with those changes. Sadly, the oral health professions, have not been keeping pace with these changes. We know anecdotally that we are nowhere close to parity for BIPOC (Black, Indigenous and People of Color) communities and their representation within the oral health professions. There is ample research demonstrating that the quality of health and healthcare for minoritized communities improves when there is greater diversity in the healthcare workforce. We need data to understand the magnitude of the disparity in representation within all oral health professions in King County and to use that data to make a bold plan for a building a future workforce that truly reflects the communities we serve.

O - Identify one area SKCDS/SKCDF is doing well in addressing diversity and contrast that to an area where we could do better?

J - As a Black cis-gendered male in academics at a predominately White institution who lives in a “liberal” city, I see points of reckoning around me every day about the importance of diversity and inclusion, the impact of race on how one navigates their way through the world, and how the time for dismantling systemic/institutional factors that have been limiting true equity is long overdue. The conclusion most have reached is that we’re doing a lot, but that we can do a lot better if this is truly among the values we hold deeply. My experiences as a longtime member of SKCDS suggest that as a dental Society and Foundation, we’ve taken diversity for granted and haven’t really talked about it in meaningful ways that address the true issues. This work is hard and sometimes it makes all of us feel uncomfortable. These are not good reasons to not start the conversation and to instead continue perpetuating the status quo.

S - I think with this past year all sectors are more aware, minority groups are speaking out and organizations are wanting this input. I feel that when we can attend health clinics and support family days in those underserved areas we can educate and engage groups that may not be properly represented.

E - Statistically, SKCDS leadership and committees reflect our membership. There are a higher proportion of women, new dentists, and people of diverse backgrounds who are very involved with the Society than in our general membership. This only tells part of the story, though. There are groups of dentists who are underrepresented in our membership and the Society needs to find ways to be more welcoming and inclusive.

J - All of us have roles to play in addressing this, whether they be at the individual level and at the organizational level. The mere fact that we’re asking the question “Where could we do better?” is a great place to start and a foundational question that I hope we ask every year. The work of equity, diversity and inclusion is not a sprint, nor is it a marathon. It’s a lifelong journey that reminds me of an African Proverb many of you may have heard, “If you want to go fast, go alone. If you want to go far, go together”. I hope we’re willing to make long-term plans to go together.

E - My favorite conversations are those that others may consider awkward because I learn the most from dentists who are honest in telling me that the Society can do better at fulfilling their needs. We are a diverse population of dentists in King County and ultimately, the goal is to provide value to individual dentists so that the lives of both dentists and patients are improved.

O - Where should organized dentistry devote our limited resources to make the greatest impact in education?

S - I feel we should set up sustainable financial scholarships for encouraging new assistants and hygienists. Without these supporting players, dentists struggle to have a thriving practice.

E - Financial and volunteer support should be directed only to programs that are increasing the number of graduates, decreasing the length of time it takes a student to complete the program, and reducing barriers to entry.

J - If we believe in the dividends that come from having an oral health workforce that reflects the community, making meaningful investments in reducing the financial barrier to oral health career training programs faced by aspiring oral health care professionals who come from economically disadvantaged backgrounds would be a powerful way of demonstrating that our Foundation’s actions speak to our values. Inviting conversations with the leaders of the area’s different oral health training programs will give the Foundation insight on how to best assist the students who face these barriers.

S - As I have also mentioned in the past, I feel an overlooked sector are working people who want to either switch careers or develop new skills. I am a strong supporter of evening and weekend classes to encompass as many individuals as possible.

O – Since the initial interview, much has occurred with regards to programs for DEI. We hope that SKCDS members and non-members with join SKCDS/SKCDF leadership on this journey – at the beginning it may be uncomfortable or awkward, but we need to have these hard discussions so that everyone participating builds upon the same knowledge base to start constructing our foundation.

To address the known workforce problems, SKCDS is advertising in two states to attract hygienists to Washington and is continually working to shape meaningful legislative changes to address barriers contributing to these issues.

I’m lucky that I have a bunch of friends. Some are old friends I’ve known for 30 or 40 years, some I’ve only recently met. You don’t have to see a good friend often to maintain the relationship. One of the great things about the internet is the level of communication it allows between people who don’t see each other much, but still have a lot to say to each other. In fact, I have one friend that I have actually only met once in person, but we have been friends for 15 years, thanks to the wonder of cyber space. He is a ninety-year old recently retired dentist who lives in Los Angeles and is perhaps the only other regular dental humor writer in the country. So naturally, he and I have to be pals. Whenever one of us has a revelation leading to a piece of writing, it is immediately sent to the other for viewing and discussion. This may lead to more inspiration and more writing which is the whole idea if you imagine yourself to be a writer. With cyber friends, it is a true meeting of the minds, without the obvious barriers present in face-to-face encounters. Age and physical appearance are much less important for successful online interactions. In fact, some people invent an online persona which differs greatly from their actual physical one. No wonder the young people are so taken with Facebook and all the other social sites that exist on the Web.

EXPOSURES

One needs to have something in common with a friend, something to talk about to pass the time. For me, the choices are dentistry, golf, skiing, guitars, and cars. I know a little bit about each of these subjects. I am also interested in theoretical physics, stream biology, logging, forest propagation and construction. I’m also obviously interested in writing but, despite popular beliefs to the contrary, I don’t find there is much to talk about with this subject. I’m not a fan of book discussions or critique groups, writing classes or workshops. I like to write but I’m just not analytical about the subject and don’t really have much to say about it. So my LA dentist/writer pal is one of the few in my entourage with writing skills.

I quite naturally have a bunch of dentist pals. We have lots to share both from the professional side as well as our other interests. If you are a dentist and you ski and golf, there is a good chance we could hit it off right now. Scratch the surface of most people and you find a wealth of interesting stuff just under the surface that you never knew existed. Online forums which are readily available can now connect you with lots of other folks in your field, some of which could become friends someday. I like to talk to trades people about their trade. I can talk plumbing and auto mechanics with the best of them and I like to learn about those new fields like computer repair which I know little about.

My friends are all men. Why is this? Billy Crystal gave the definitive dissertation on this subject in that classic film “When Harry met Sally”. He was in the car with Meg Ryan and she asked him why men and women cannot be friends. He replied that they cannot be friends because the man would eventually want to have sex with the woman. Meg Ryan responded with the question that what if the man was not physically attracted to the woman? Billy responded that he probably would want to nail her anyway. This exasperated Meg so much that the discussion ended. Go figure, the exasperation is often mutual.

Standing on the first tee with three of my good pals, the sun is coming up and the bets are being made with the expectation of bashing that first drive down the fairway—what could be better than this. It just wouldn’t be the same by myself. The subjects of discussion on the links are not profound, in fact are usually not even repeatable in mixed company, but there is no place I would rather be at that moment—with my friends. I hope you have a few as well.
