I acknowledge that the Credit Card noted above will be charged for the items indicated on this order form. If the Personalization Program was selected then I acknowledge that every January I will be automatically billed for the full yearly renewal amount, plus the cost of the update(s) for whichever albums (Babies & Children Everyday and/or Adult Everyday) I am in possession of. If the Holiday Book Program was selected then I acknowledge that each Fall I will be automatically billed for the yearly update ($50). The renewal amount covers the annual updated catalogs, image cds and resale rights fees. The update costs cover the updated pages that will be sent for the respective albums. I may cancel the program at any time. Boatman Geller will only refund the full amount of the program within 30 days of my initial personalization program arrival, pending all materials are in mint condition and shipped back to Boatman Geller at my cost. Any shipping charges incurred by the personalization program will not be refunded.
