BOILERMAKER HOSPITALITY RESPONSIBLE ALCOHOL SERVICE We are committed to providing a safe and secure environment where guests can enjoy alcohol in a responsible manner. Licensed and regulated by the State of Indiana, we will comply with all laws governing the sales and service of alcoholic beverages as well as the policies of Boilermaker Hospitality
Please initial each item below to indicate your partnership with us in providing an enjoyable and safe experience for your family and friends. _____ The “carry in” or “carry out” of any type of alcoholic beverage is prohibited. Please make alternative arrangements for any alcoholic beverages traveling with you or your guests prior to entering our facilities. _____ Beverage service is limited to a total of 5 hours. We’re happy to accommodate service time adjustments during the event when possible; however, beginning service earlier than contracted may result in ending service earlier than contracted. _____ Straight shots of any liquor or any combination of three or more liquors will not be served. _____ No one person may leave the bar with or be in possession of more than the equivalent of two alcoholic drinks at any given time. _____ No one person may leave the bar with more than the equivalent of any one alcoholic drink during the final hour of a 5 hour bar service. _____ Neither last call nor suspension of bar service for dinner will be announced. _____ Signage will be posted at each bar briefly outlining our Alcohol Service Policies. _____ Alcoholic beverages may not be removed from the event space. _____ All individuals purchasing and/or consuming alcoholic beverages must be a minimum of twenty-one (21) years of age. Anyone appearing under the age of 30 will be asked to provide identification every time they come to the bar. _____ Guests will be expected to maintain responsible standards concerning their use of alcoholic beverages. Immoderate use leading to intoxication, offensive behavior, or disorderly conduct will result in the discontinuation of service (being “cut-off”) to that individual. _____ Host of the event understands that for the safety of the event & guests, Boilermaker Hospitality and/or Purdue Athletics may require security. Security charges will be added back into final bill if deemed necessary.
_____ For the safety of your guests and in accordance with the State of Indiana liquor laws, we reserve the right to remove from the event space: Any underage person who is consuming alcohol Any person who provides an alcoholic beverage to an underage person Any person who continues consuming alcohol after service has been discontinued (“cut off”) Any person who knowingly provides an alcoholic beverage to a person to whom service has been discontinued (who is “cut off”) Any person who arrives visibly intoxicated to the event Multiple infractions of any liquor service policy or law will result in the discontinuation of all alcohol service for the remainder of the event. Should this action become necessary, payment for the event is due in full and no refund will be given for the unused beverage service time or any inconvenience or embarrassment caused. In order to allow you to enjoy your day to the fullest, we ask that you identify an alternative contact that can assist us in reminding your guests of our responsible service policies should it become necessary and you are otherwise engaged.

Boilermaker Hospitality and the employees of Boilermaker Hospitality take our responsibility, as exclusive provider of alcoholic beverages at Ross Ade Pavilion and Mackey Arena, very seriously. The form entitled “Request for Service of Alcoholic Beverages” must be completely filled out and submitted for approval at least 30 days prior to your event date. Licensed and regulated by the State of Indiana, we will comply will all laws governing the sale and service of alcoholic beverages. All individuals purchasing and/or consuming alcoholic beverages must be a minimum of twenty-one (21) years of age. We reserve the right to refuse alcohol service to anyone who cannot provide proper identification to verify age. It must be verified that a clear majority of those attending will be of drinking age. We reserve the right to remove any guest who purchases and supplies alcohol to a minor or to anyone who has already been refused service. We reserve the right to refuse alcoholic beverage service to anyone who appears intoxicated. We reserve the right to confiscate any alcoholic beverage brought in from outside sources. Our Bartenders will not serve “shots” or serve drinks made from more than two liquors (e.g. Long Island Iced Tea). Bars shall not exceed 5 hours duration. Beverages may not be self-poured. We reserve the right to remove any guest, discontinue all alcohol service or discontinue an entire event for failure to comply with the law, Levy Restaurants regulation, and/or the instruction of the Boilermaker Hospitality staff. Should any of these actions become necessary, payment for the event will remain due in full and no refund will be given for the inconvenience or embarrassment caused.
Alcohol Service Pricing Host/Cash Bars The event sponsor pays for all beverages served and is responsible for all associated fees. The current service charge (20%) and Indiana Sales Tax will be applied to all sales. There is a $100.00 fee for each bar setup at Ross Ade Pavilion and Mackey Arena. This fee covers labor for one bartender and one bar setup/tear down. Bars in the South End Zone, Tent City, or another location may have additional fees for Bar Set-Up. Additional Bartenders Boilermaker Hospitality will determine if additional bartenders are needed to meet service expectations based on the information provided on this form & event details. A charge of $60.00 for each additional bartender will be added to your invoice; please note one bartender is included with each bar setup fee. Additional fees or bartenders may be required for wine pours, toasts or speciality services. Please remember that alcoholic beverages may only be dispensed by licensed professionals. Glassware Glassware is an additional $1.00 per person for all wine pours/toasts & bar services at all locations inside Ross Ade Pavilion and Mackey Arena. Please note to service a wine pour/toast with event, glassware is mandatory. Security Security may be required for an event. Each event is evaluated on a case by case basis and if Boilermaker Hospitality and/or Purdue Athletics detemine sercurity is needed these charges would be added to event invoice; decision would be made one month before event date.
