Impact Report

Thank you for

SUPPORTINGHEALTH! Thanks to our community of donors, the PVH and MCR Foundation reached several milestones in fiscal year 2013-2014. Our foundation received both the largest individual commitment and the largest bequest gift in our organization’s history. We celebrated with our oncology team the opening of phase two of the University of Colorado Health (UCHealth) Cancer Center. And, inspired by a dynamic group of volunteers and supporters, we helped establish WISH, a women's giving circle to fund health needs of women, children and families. Along with these significant milestones, we also continued to grow support for established programs, such as employee scholarships and patient navigation.

In this report you will come across many numbers, but the real measure of our accomplishments over the past year are the stories behind the numbers. The stories tell of a community that has rallied in support of health, and of the individuals whose lives have been changed through generosity. Together, individuals, corporate partners, foundations and the public sector contributed more than $5.5 million to the foundation last year; a sum that proves beyond a doubt that northern Coloradans make health a priority. Whether it's supporting the new cancer center, foster learning and innovation among care teams or making a legacy gift through the foundation, northern Colorado has rallied behind the foundation. Whether you are a steadfast donor, or one of the more than 850 new supporters to the foundation, we are grateful to have you as partners in our mission to advance world-class health care. Thank you for your generosity and your commitment to our community. Sincerely,

Erica J. Siemers, Senior Director

Largest individual commitment  | $1,564,844 An anonymous donor made the largest individual commitment in the foundation’s history this year: $1,564,844 to support the UCHealth Cancer Center and UCHealth research programs. This gift will impact patients and their families at UCHealth for years to come. As with all gifts received by the PVH and MCR Foundation, this contribution is a generous endorsement of the work of our staff and caregivers throughout our hospitals and clinics in northern Colorado.

Total granted for scholarship support  | $170,250 The PVH and MCR Foundation scholarship program fosters the professional development of UCHealth staff in its affiliated hospitals and clinics through matching donors’ passions for education with employees’ academic dreams. This year $170,250 was provided to support northern Colorado staff in pursuit of their goals.

I am a proud recipient of a $1,000 scholarship to finish my master's degree in organizational leadership. When I turned 54, I decided to pursue graduate school. As I have studied and completed my classes in this program, it has uniquely offered me a different level of appreciation for this amazing organization where I am employed. It has not been an easy journey to complete the master's program. With a full time job, and two sick parents, every spare moment was dedicated to this program. It is with the generous help of the foundation and our donors that have helped me ease the financial burden of tuition. Linda Fisher, scholarship recipient

Total given by employees  | $282,216 The PVH and MCR Foundation is fortunate to include employees of Poudre Valley Hospital, Medical Center of the Rockies, and their affiliated clinics among its generous donors. Nearly a third of the employees at UCHealth in the northern region gave via payroll deduction this year, with more signing up every day. These employee contributions demonstrate the culture of giving at UCHealth and have a significant impact on the programs and services that the foundation is able to support.

Total granted for Employee Emergency Assistance Fund (EEAF) grants  | $56,543 “I would like to express my appreciation for approval of assistance that was given to me recently. That was the most unselfish thing anyone has ever done for me. It makes me proud to be part of such an awesome group of people that are willing to take that extra step to help someone out in need. It does feel like family and I love being a part of that.” – Anonymous, EEAF grant recipient

Behind the Numbers Stories of Impact Investing in the future of comprehensive cancer care – closing in on the goal The long-anticipated UCHealth Cancer Center was unveiled to the public for the first time at a ribbon cutting on June 4. The celebration brought together donors, community leaders, residents, UCHealth staff and musical entertainment in advance of the first patients receiving services on June 9. Speaking on behalf of the UCHealth Cancer Center, W. Thomas Purcell, MD, Ph.D., Associate Director for Clinical Services of the University of Colorado Cancer Center and Executive Medical Director of Oncology Services at University of Colorado Hospital, and Matthew Sorensen, MD, medical oncologist at the Medical Center of the Rockies, talked about the impact the facility is expected to have on cancer care and well-being in northern Colorado. Other speakers at the event included Mike Dellenbach, PVH and MCR Foundation board chairman, JoAnn Lovins, service line director for oncology at UCHealth, Bill Neff, MD, then interim president and CEO of UCHealth, Kevin Unger, CEO of Poudre Valley Hospital and Medical Center of the Rockies, Vikki Wagner, chair of the Survivor Advisory Council and Karen Weitkunat, mayor of Fort Collins. More than 1,000 attendees were invited to tour the state-of-the-art facility and learn more about the care team and comprehensive services offered. The individuals, corporations and foundations who provided the financial support needed to bring the facility to life were also recognized. While this main treatment hub of UCHealth's cancer services has been open to patients since June, fundraising will continue through December 31, 2014 to complete the wellness and survivorship center and to reach the comprehensive $6.1 million fundraising goal for the cancer center project.

Eight years in coming, my husband, Lee Curtis’, vision came true. As he spoke, ‘to build a comprehensive cancer center locally so patients don’t have to leave home for world-class treatment’. For the many trips out of state, multiple sites, various doctors and logistical planning, the fragmented journey for our community’s cancer patients has now ceased. When a patient has cancer, the whole family ‘has cancer’. Caregivers also need help to enhance quality of life and here, everyone receives an open arms greeting by a world-class facility providing comprehensive wellness for the patient, caregivers and families in addition to cancer treatment and research. This new cancer center is ‘the village’ to strengthen our community.

– Cathy Nelson Curtis, Survivor Advisory Council Member

A WISH come true...nearly $34,000 awarded to health programs in Larimer County A group of ambitious women came together last year with the goal of creating a healthier community. Working with the PVH and MCR Foundation, they started WISH – Women Investing in Strategies for Health – a women’s giving circle supporting public health needs through University of Colorado Health programs in Larimer County. In March, the group announced the inaugural WISH award recipients – granting from $1,500 to $5,000 to nine different local programs from its pooled resources of nearly $34,000. 2014 WISH award recipients include: BstrongBfit, Cardiovascular Family Intervention, Car Seat Education and Distribution Program, Continuing Therapy Outreach Pilot Program for Music Therapy, Digital Video Colposcope for Pediatric SANE Exams, Electric Breast Pumps, Healthy Hearts Club Expansion, Prenatal Substance Abuse Prevention Campaign and Women on a Roll.

Women on a Roll has greatly benefited from WISH dollars, as it has given the Vida Sana Promotoras (Community Health Workers) an opportunity to be ambassadors/trainers for increasing bicycling among underrepresented communities, specifically women and the Latina community.

– Edgar Dominguez, Health Equity Coordinator, Coalition for Activity and Nutrition to Defeat Obesity (CanDo), Vida Sana

Moving the needle - Innovative Research tackles preventive care and health care technology $55,000 supporting local studies Money raised at the August 2013 PVH and MCR Foundation Golf Classic is being used by the University of Colorado Health research department in northern Colorado to fund a number of projects that will ultimately enhance patient care and health services in our community: • A retrospective data analysis of current treatments for heart attacks. • An innovative study to explore the health effects of supplementing fourth grader’s diets with a higher intake of beans and rice bran powder in the form of muffins and smoothies. • Research related to the alarming increase of bike accidents will be published to support a community education program on street safety for bicyclists and motorists alike. • A new system called the Lifeboard for streamlining communications during a trauma situation. The Lifeboard, a tablet activated at the site of a trauma, enables health care providers to streamline communications between the remote health care team and the team at the hospital. Providers will be able to easily track and share information with regard to administered treatments, timing and other details related to delivery of care.

Pumping up knowledge about cardiovascular health $154,320 grant from AstraZeneca HealthCare Foundation for Healthy Hearts Healthy Hearts for Healthy Families is yet another example of a program that facilitates long-lasting positive changes to health and wellbeing of members of our community. The Healthy Hearts for Healthy Families program was made possible by a grant of $154,320 awarded to the PVH and MCR Foundation from the AstraZeneca HealthCare Foundation’s Connections for Cardiovascular Health℠ program. The PVH and MCR Foundation’s proposal was one of 19 selected out of 500 submitted nationally to the program in 2013. One of the key innovative components the program offers is making the participants agents of change. The curriculum is designed with the knowledge that parents are more motivated to modify behaviors if they know their child is at risk for heart disease. Healthy Hearts seeks to interrupt the patterns of cardiovascular disease in families through increased comprehension of risk factors; heart-healthy lifestyle behaviors; physical activity; and improved clinical measures such as body mass index, blood pressure and cholesterol levels. Children and their parents attend class together in this intergenerational initiative to reduce risks and improve heart health.

A grateful patient pays it forward to PVH and MCR staff Titus Estate gift for employee scholarships - $522,507 I have been longing to finish my RN to BSN degree, but with $30,000 in school loans already it was hard to justify. I saw the information about the opportunity to apply to the Richard G. Titus Scholarship and could not pass it up. I am elated that I have landed this opportunity, because without the generosity of the foundation and the Titus family, I would not be able to further my education and career. I am grateful beyond words. For Richard G. Titus, the kindness and care he received from nurses at PVH and MCR left a long-lasting impression. So much so that he left a bequest to the PVH and MCR Foundation to support employee education. A portion of the gift is held in an endowment to support scholarships in perpetuity. The remainder established the Richard G. Titus Scholarship for Nurse Leaders. Awards are granted on a competitive basis to nursing staff employed at UCHealth in the system’s northern region that are seeking bachelor of science in nursing (BSN). The fund is directed to honor and assist those engaged in the nursing profession, and to further his lifelong passionate commitment to education. To date, a total of 18 employees have been awarded generous scholarships from this remarkable gift. Planned gifts such as the Titus Scholarship allow the PVH and MCR Foundation to provide ongoing support for our hospitals’ most essential resource: our people.

Circle of Caring The PVH and MCR Foundation has formed a legacy society, the Circle of Caring, to honor those who have made provisions for our local hospitals in their estate plans, such as a will, trust, life insurance policy, retirement plan, or through a life income plan, such as a charitable remainder trust or a charitable lead trust. Circle of Caring donors leave their imprint through thoughtful, planned gifts from their estate.
