OSHA Fines Mill After Fatality

Joe N. Miles & Sons has been fined $145,350 by OSHA following a fatal accident at its Silver Creek. Ms.. mill.

A mill worker was killed May 7 when he fell 17 feet from the steel beams of a lumber sorter to the concrete floor below. The employee was trying to rearrange a misaligned wood board: he wore no fall protection.

Most of the fine came from two willful citations, lack of guardrails and personal fall protection for employees walking, first, on the lumber sorter's platforms and steel beams and, second, on a ll-ft. high conveyor belt used to access a chipper mill.

According to OSHA, management had been warned of fall hazards by its safety committee in January and reminded monthly thereafter, yet "made no effort to abate the dangerous conditions."

OSHA also assessed a $32,850 penalty for nine serious violations, including absence of guardrails on ele- vated platforms and walkways, lack of training on the use of fall arrest equipment, unguarded machinery, and no specific lockout procedures to render machinery inoperable during maintenance and repair.
