AN EXCELLENT REPUTATION FOR CARE
Situated in a residential estate, Brookholme Croft provides accommodation for 46 nursing & residential clients. We provide a high level of personalised care by professional and dedicated trained staff, also specialising in end of life care. We are able to provide both long and short term care.
Excellent facilities are provided in our purpose built accommodation, appointed in beautiful surroundings. Tastefully decorated single rooms, many of which are en-suite, create a homely environment. Three different lounges and a large garden with various seating aspects is one of the many ways in which client choice is promoted. A reminiscence room onsite allows our clients to experience and find comfort in nostalgia, surrounded by familiar furnishings and décor helping evoke memories.
A dedicated co-ordinator provides organised events and activities, in both group settings and one-to-one. Clients have the freedom to choose which activities they wish to join in with, from a full and varied schedule. Clients can attend the on-site hair salon with weekly visits from our hairdresser, and we also have frequent visits from the chiropodist. We have our own private physiotherapist who visits the home once a week. Family and friends are welcome to visit anytime, however the home does promote protected mealtimes for our clients. Fresh, home cooked food is available throughout the day with clients being involved in designing the seasonal menu. All special diets and needs are catered for.
In addition to respecting privacy & dignity, the home also promotes individual’s independence as far as possible.

Starting out – where to find help and support
There are lots of useful services and organisations to help you remain independent and well. Our adult social care online information finder is a one stop shop for all the support and advice you might need.
The online tool provides free, impartial information and guidance for anyone who might need a bit of extra support to remain living independently at home.
By answering a few questions, anyone over 18 can get tailor-made information, advice and guidance on a range of services, activities and resources you can access yourself to make your life easier.
If you think you may need extra support to remain independent, there is a self-assessment tool. Your answers will be reviewed by an adult social care professional to see if you have eligible care needs.
As well as our support, the information finder also promotes a range of services available through the private, community and voluntary sector including:
• Befriending services for anyone feeling isolated and lonely.
• Community groups available across Derbyshire.
• Derbyshire Connect and Active Travel which provide weekly transport to local shops.

Welfare rights
Did you know that our welfare rights service can check if you are maximising your income and are getting all the benefits that you are entitled to? The team can also give you benefits advice and support and help with appeals and representation at Tribunals.
Derbyshire Discretionary Fund (DDF)
If you are in serious financial difficulty, our Derbyshire Discretionary Fund could help.
This service can provide emergency financial assistance in two forms of grant payment – emergency cash payments and exceptional pressure grants. If eligible, you will be supported to resolve your immediate financial difficulties and will be put in touch
Carers
Are you a carer?
Do you care for someone who couldn’t cope without your help? If so, we have a range of carer support services to help you with your caring role. A carer is anyone including children and adults who provide unpaid support with day-to-day living tasks or personal care to a family member, partner, or friend.
The person they care for could be ill, frail, suffer from a physical or mental health condition or impairment, autistic, living with dementia, or drug or alcohol dependent. Our carers’ support service, delivered by Derbyshire Carers Association, includes:
• Information, advice, and guidance.
• Individual support, including face to face, home visits and telephone support.
• Carers’ assessments and support planning, which may include a carers’ Personal Budget.
• Support with emergency planning.
• Training and learning.
• Carers’ support groups and events around the county.

Carers’ assessments
Carers’ assessments can lead to personalised support which may include:
• Information and access to local carers’ peer support groups.
• Information and access to training and learning opportunities.
• Information on how your GP might support you.
• Advice on balancing life alongside caring.
• Services to enable you to take a break from your caring role.
• Help to make a plan in case of an emergency or crisis.
Carers’ emergency card
This is a free service which can offer peace of mind for carers who are concerned about what would happen if a crisis or emergency prevented them from providing care to someone who depends on them.
The card is linked to an emergency plan, which details nominated emergency contacts (family and friends who may be able to provide care in an emergency), the support required by the person depending on care and anything else that is important for the service to be aware of.

Support for young carers
A young carer is someone aged under 17 and a half years who helps to look after a family member who is ill, has a physical or learning disability, poor mental health or is dependent on drugs or alcohol.
Most young carers help look after one of their parents, a sibling or a grandparent.
Derbyshire’s young carers service is provided by Derbyshire Carers Association who can offer short-term support, including:
• Advice on how to access services such as housing, employment and education.

Lisa Nicholls, 51, from Ripley, is a carer for her husband Peter, 67, who has anxiety and agoraphobia – which prevents him from going outside without support.
A community support worker from the county council told Lisa about Derbyshire Carers Association (DCA). ‘The lines become very blurred when you’re someone’s spouse but you’re looking after them at the same time,’ explained Lisa. ‘I’m a confident and capable person but when you’re used to being the one giving the help, it’s hard to ask for it for yourself.

We know quitting isn’t easy but our friendly stop smoking advisers will support you over a 12-week period, with tips, information and advice on the range of stop smoking medications available to you.

Falls prevention
Falls are one of the largest causes of emergency hospital admission for older adults and in Derbyshire, around 70,100 people aged over 65 fall every year. Falls can lead to pain, distress, injury, loss of confidence and even death. But many falls can be prevented.

Mental health and wellbeing
Mental health is an important part of everyone’s life and good mental health and wellbeing is directly linked to good physical health. Positive mental health and wellbeing is also linked to increased life expectancy and improved quality of life.
If you need urgent help

Derbyshire Volunteer Passport
The Volunteer Passport is for anyone of any age who volunteers or is interested in volunteering in the future with organisations in Derbyshire.
You could be an experienced volunteer who already gives several hours a week, or you could be thinking about volunteering for the first time.
To gain a Volunteer Passport, you need to complete a short training course exploring:
• Equality, diversity and inclusion.
• Keeping yourself and others safe.
Time Swap Derbyshire
This is an initiative that encourages local communities to help each other by sharing time and talents. The idea is simple – as a Time Swap member you can earn ‘time’ by giving help and support to other members in your community. This could be things you enjoy or are good at.

Types of service available
Day opportunities
Day opportunities aim to support you with maintaining independence, inclusion in local community life and personal development. Day opportunities also support people with different needs, from low-level support through to more intensive personal care. They offer the chance to socialise and meet new people. For example, voluntary organisations provide support groups for people with Parkinson’s or there are activity sessions run by MIND for people with a mental health condition.
Your local Community Volunteer Organisation is a good place to start to find activities you may be interested in.
Some people, such as older adults and people with learning disabilities and autism, mental health conditions or physical or sensory disabilities, may wish to access more specialist day opportunities. We have a directory which lists opportunities to consider. Attendance can be self-funded or in some cases, people who have an eligible care need may receive a budget from adult social care that can be used to fund attendance.
Health and Wellbeing Team
This team supports people over 16 years with disabilities, mental ill health, older people and their families and carers. The team aims to help people to live fulfilling lives, stay safe and be well connected with their local communities.
Health and Wellbeing Coaches and Officers (HAWCs and HAWOs) can help you access services, information and other support you need. They can also help you to become more independent and part of your local community if you are at risk of becoming dependent on services or reaching crisis point.

Your HAWC or HAWO will then help you to find your own ways to get the life you want. This way of working is different because it asks what you want, rather than trying to fit you into a service that already exists, which you might not actually want.
Your HAWC or HAWO will be a named person who you can contact and see locally.
They should:
• Take time to get to know you and build a trusting relationship.
• Listen to your vision of a good life – what would make your life better and the steps needed to achieve this.
• Support you, your family and carers to get the information and advice you need.
• Help you to identify your strengths, skills and abilities.
• Help you become more resilient, connected and involved in your community.
• Support you to meet new people or reconnect with others you may have lost touch with.
• Help you to get your voice heard, for example, to speak to organisations and professionals.
Community Connectors
This service helps Derbyshire residents aged from 14 years old with learning disabilities or autism to develop new skills, get involved with activities, use facilities in their local community, build relationships and experience new things.

Assistive technology
Assistive technology can provide peace of mind for you and your carers whenever it’s needed, 24 hours a day. It can also provide much needed reassurance if you are worried about you or someone else feeling safe at home, falling, or becoming forgetful, for example.
There are two types of assistive technology which can be used either independently or as combined assistance. They are community alarms and telecare.
A community alarm is a pendant or wristband you can wear, which connects to a base unit in your home. If you need help, you simply press the button on the pendant or wristband and the base unit will quickly connect to a monitoring centre (or your family member or chosen contact), who you

Where it connects to a monitoring centre, the trained staff will assess your needs to determine the best way to support you. They may contact a family member, carer or neighbour and if required, contact emergency services.
Though some devices still summon help, they act more like a pager, so rather than connecting to a monitoring centre when the button is pressed, they contact someone else in the same property.
Telecare describes additional devices that can be used in conjunction with your basic community alarm unit to provide further, more specific support. When these additional sensors are triggered, they
summon help in the same way but do not rely on a button being pressed.
Examples of these include:
• Sensors placed throughout your home to detect falls, seizures, floods, gas leaks or fires to help prevent the chance of accidents, or to alert a family member or carer if you have left home without anyone knowing.
• Equipment that assists you with managing medication by providing a prompt.
• Sensors that are used to temporarily monitor activities of daily living, to support care staff in delivering the right level of support.
The choice of equipment, monitoring service and response to alerts is tailored to your needs. The monitoring element is subject to a charge which is generally paid for by you, or an organisation. These costs vary depending on the provider in your local area.
If you have assistive technology as part of a wider package of care, the monitoring charge will be assessed as part of your Personal Budget
and included in your contribution towards your care costs. If you have care and support needs that mean you are Care Act eligible, you will not need to pay for the equipment itself. You can also purchase the equipment privately via any assistive technology provider.

We
Service which is a council social care and NHS funded service that aims to support the residents of Derbyshire to live independently, in the comfort of their own homes.
What do we do?
Following an assessment by a health or social care professional we provide the prescribed equipment on loan. The service includes delivery, installation, servicing/repair, collection and recycling. Equipment is on loan to you for as long as you need it and are living in Derbyshire.

Making life easier at home
If you’re having difficulties with everyday tasks, these simple solutions could make life easier and enable you to retain your independence. These are a starting point; other solutions are available, which might better suit you.
Finding it difficult to get in and out of chairs? Try putting a piece of hard board under the seat base. Alternatively, buy chair raisers, a higher chair or an electric riser chair. Also try taking regular gentle exercise to improve your mobility.
If you can’t reach your windows, could you move furniture out of the way? Ask someone to help if you need to move heavy furniture. There are also tools for opening and closing windows.
Struggling to keep warm/cool? Consider a fan or heater. Is your house insulated? Are there any draughts? You may also be eligible for the winter fuel payment from the Government. Visit www.gov.uk/winter-fuel-payment
If you have trouble using light switches, think about replacing your switches for ones that are easier to use. Consider handi-plugs or light switch toggles, or there’s even technology available that turns your lights on and off using your speech.
Use subtitles if you can’t hear the TV or buy wireless headphones. Do you need a hearing aid? Request an assessment from your council.
Do you forget to take your tablets? Make a note of when you’ve taken them or buy an automatic pill dispenser or pill box. If you struggle to open your medicine, ask your pharmacist for advice on alternative packaging that could make it easier for you.
Can you reach everything in your cupboards? If not, try a handi-reacher or rearrange your kitchen so the things you use most are within easy reach.
If you are having problems with preparing food, consider buying ready-chopped options or try a chopping board with spikes. There are also longhandled pans, teapot tippers and lid grippers that could help. Palm-held vegetable peelers or a food processor might be a solution and meal delivery services are also available.
Is eating and drinking becoming difficult? Large-handled cutlery could help, as could non-slip mats for the table. Lightweight cups and mugs with two handles could also be a solution.
Having tap turners fitted can make using taps easier. You could also consider changing to leverstyle taps, which might be easier for you to use.

If moving whilst in bed is a problem, have you thought about using an over-bed pole? You might also want to buy a pillow raiser or change your bedding so it’s lighter.
Is it becoming difficult to get dressed? If so, specially adapted clothing is available, or you could buy a long-handled shoe horn, a dressing stick or a button hook. If you are having a lot of difficulty, consider home support – see page 20.
Clocks are available with large numbers or lights if you can’t read the time in bed. You can also buy clocks that speak the time.
If you are finding it harder to read in bed, consider an e-reader that allows you to change the font size. Some also have integrated lights. Look for bedside lamps with a step-on or button switch if yours are difficult to use.
Do you struggle to get in and out of bed? You could learn new ways of moving around, purchase a leg lifter or a hoist, or install grab rails for support. Seek advice about these options. If the bed is the issue, you could buy an electric adjustable bed or raise the bed to the right height.
If it’s hard to hold your toothbrush, try a toothbrush gripper. You might also benefit from having an electric toothbrush or sitting on a stool while brushing your teeth.
You might like to buy a raised toilet seat or a seat with a built-in support frame if it’s hard to use your toilet. Flush-lever extensions are also available.
Has it become more difficult to wash? Items are available, like long-handled sponges and flannel straps. You could also consider a slip-resistant bath mat, grab rails, a half step to help you get in and out of the bath, or a bath or shower seat. Tap turners can also be used in the bathroom.

Shopping and meals, housework and laundry
If you need help with day-to-day tasks like preparing meals, shopping, cleaning and laundry there are services that can help you, or the person you care for. Services could be provided by voluntary or charitable
Services to support you leaving hospital
Supporting people to leave hospital safely and quickly is a key priority for us. If you are likely to need adult social care support following your discharge, a referral will be made to our service.
An adult social care worker will liaise closely with you, your family and health staff to work out what is best for you. They will consider all the different services that are available to help you recover your health and independence and return home as soon as is safely possible. Derbyshire Home Not Hospital Service
If you’re leaving hospital and need some practical support when you return home, the Derbyshire Home Not Hospital Service could help.
This service is designed to help people with limited support from family and friends with practical tasks such as shopping, collecting prescriptions and making sure the home environment is safe

Services offered could include support with washing and dressing, showering, meal preparation, eating and moving safely around the home, either on a long- or short-term basis.
Our in-house home care service can offer a shortterm service designed to help you regain lost skills. This service supports you to do things for yourself so that you regain as much independence as possible.
To be considered for home care you will need an identified eligible need following an assessment. At the end of a period of short-term care you will be reassessed to see if you have any remaining longer-term eligible needs. If you do, we may commission one of our partner organisations in the private, voluntary or independent sector to provide you with a service.
Some people with high care needs can use their Personal Budget to employ a Personal Assistant (PA).
Live-in care
Live-in care is a viable alternative to residential care, where a fully trained care worker lives and works in your own home to support your individual needs and help you maintain your independence. The carer must be allowed breaks and have somewhere to sleep at night.
Support available from live-in carers can range from personal care and mobility assistance to more complex health requirements, as necessary.
PAs can help you with personal care and getting out and about and are not tied to agencies as you employ them directly.
A list of home care agencies in Derbyshire begins on page 25 and a useful checklist can be found on page 23. All home care agencies that provide personal care must be registered with the CQC, which is responsible for monitoring care services to make sure they meet the required essential quality standards. The CQC reports on the services it inspects and awards quality ratings.

The cost of live-in care varies depending on your circumstances, including the level, type and duration of care that you require.
Any organisation that employs live-in carers is required to register with the CQC and ensure that its employees follow the essential standards of quality and safety in their practices. For more information about the CQC, see page 39.
Specialist care services
There are several services provided in Derbyshire to meet the needs of specific client groups, such as people with a learning disability, physical disability, or mental health condition.
If you need a service, we will look in detail at the support needs that you have and will work with you to identify the options for meeting these.
If you are eligible for funded adult social care assistance, a support plan will be drawn up to
identify the outcomes you wish to achieve, and a Personal Budget will be calculated to meet these.
You may decide to buy your own support from us or from an independent sector provider. See the information starting on page 33 for more on assessments, eligibility and Personal Budgets.

Housing choices for independent living
Several age-exclusive housing options are available for older people to maintain independent living in their local community. These housing options can
provide a good alternative to residential care and allow you to keep living in a home with your own front door, maximising your independence.
Sheltered housing or age-exclusive housing schemes
This type of housing is available to rent or buy and is generally available to people aged 55 and over. One bed flats, small flats, apartments or bungalows clustered together with other similar properties are available through this housing option and may help you downsize from a larger property.
Often there are communal facilities nearby, such as an activity room where local services can pop in to provide services near to where you live. If you need help or support, there is a community alarm in the property that allows you to request assistance. Some schemes also offer access to an Independent Living Officer who can help support you to live well
Extra Care housing
Extra Care housing is the name given to purpose-built accommodation with care and support for older and vulnerable people.
Extra Care housing is designed to offer people the opportunity for more independence and to have their own front door while having the reassurance of staff on site 24 hours a day, seven days per week. Additional, regular support can be arranged for those who need it.
Extra Care housing provides:
• Your own attractive, self-contained independent accommodation suitable for couples and individuals.
• Apartments available for sale, rent or shared ownership, designed to ensure you feel safe and secure.
• Access to a range of support, personalised care and activities available to meet your assessed needs and support your independence.

Sheltered or age-exclusive housing for rent is provided by either local district/borough councils or a housing association. If you are considering renting a sheltered or age-exclusive property, you need to register with your local district/borough council, who will have information about all the social housing vacancies in your area. There are also several housing schemes available privately for purchase.

• A vibrant community life – you can enjoy your own privacy or choose from a range of on-site facilities, activities and services to support you, including a restaurant, hairdressers and shopping.
• Accommodation that is suitable for wheelchair users and offers level access showers. It is designed to adapt to your changing needs and is supported by assistive technology.
Extra Care housing is designed to be affordable to everyone who needs it.
The amount you will pay will vary depending on your individual situation and your income. You may have to pay all costs, or you may be entitled to benefits to cover some or all charges.
The main cost elements of Extra Care housing are:
• The cost of buying or renting your home.
• A service charge associated with your home for maintenance, management of the scheme and any communal facilities.
• Your care and support costs.
• Wellbeing and support service costs.
• Your everyday living expenses, such as utilities and food.
Housing options for younger people
There are also several independent living options for adults of working-age with a long-term health condition or disability. This can be very specialist housing that is adapted to meet specific needs, or it could be a range of supported living options, such as a small Extra Care scheme or adapted housing. If this early conversation has not resolved your need and identifies that you may be eligible for statutory support from us, you will be referred for a more detailed assessment from a social work practitioner based in your local area.
This practitioner will complete the assessment with you and, depending on the outcome, talk you through any next steps. This may include telephone or video contact, and/or face-to-face contact in your home or in another community setting. We aim to complete this within 28 days of your initial contact with us.
The assessment provides an opportunity for you (and, if required, a family member/s or nominated representative) to engage in a more in-depth discussion with one of our skilled and trained workers.
This will look at your particular circumstances, how you manage your day-to-day life activities, what strengths, skills and resources you can call upon and what might assist you to stay independent and in control of your life.
Personalised support
If you are eligible following an assessment, we will determine with you whether some or all of your outcomes can be met through universally available support (see page 6) or whether they require funded adult social care support. You and your carers will be involved in this process.
For any eligible outcomes that cannot be met without adult social care funding, you’ll be allocated an indicative Personal Budget to help develop your care and support plan. Once a viable care and support plan has been completed, we will finalise with you the money required to deliver this which
Any information you provide will be in confidence, subject to certain limitations which we will discuss with you. For example, if you have health, social care or housing needs, or if there are safeguarding concerns, we may need to share your information with other agencies.
We will always ask you first unless we need to act quickly to protect you from harm. As part of this assessment, we will provide further information and advice if we feel this may prevent, reduce, or delay your need for ongoing care and support and/ or improve your wellbeing. We will seek to agree the outcome of any assessment with you.
Following your assessment, we will provide you with:
• A copy of the assessment.
• Information and advice about what options are available to meet your needs, including whether you are eligible to receive funded support through adult care.
• For those eligible for funded support under the Care Act 2014, what the next steps are including producing a care and support plan.
• For those not eligible, a rationale as to why you are not eligible for ongoing funded support.

Types of care home
Care homes (personal care only)
Care homes provide accommodation with full board and personal care. Normally you would have your own bedroom, which may or may not be ensuite, and staff would be available on site 24 hours a day. Some homes can provide specialist services for people with a learning disability, dementia, mental health condition or for younger disabled people.
Care homes providing nursing care
This is a home which provides nursing care with trained nursing staff on duty 24 hours a day.
The homes in this guide are either owned by us, private sector proprietors or voluntary sector organisations. All care homes are regulated by the CQC and inspected to ensure they reach an approved standard.

Care homes for people with dementia
When considering a care home for someone with dementia, it is particularly important that it offers a service that can be tailored to meet individual needs and provides support to maintain and improve quality of life. Some homes can offer an environment which has been designed specifically for people with dementia.
People with dementia may also benefit from a range of group or individual therapies involving social interaction and mental stimulation. Activity co-ordinators and occupational therapists can offer a variety of therapies, including reminiscence, music therapy and other creative therapies.
Many care homes can offer places to someone with dementia. You should discuss your requirements with individual care home managers and ask about specific dementia training for staff. More than a care home
Inspecting and regulating care services
Health and social care services must be registered to show that they meet a set of standards.
The Care Quality Commission (CQC) is the independent regulator of health and social care in England. It registers care providers and inspects and rates services. When things go wrong, the CQC can also take action to protect people who use services.
After an inspection of a care home or home care agency, the CQC publishes a report which looks at how well the service meets the CQC’s five key questions: Is the service safe? Effective? Caring? Responsive to people’s needs? Well led?
Each care home and home care agency will get an overall rating of outstanding, good, requires improvement or inadequate. It will also get ratings for each key question. The ratings mean you can easily see where a service is performing well, and where it needs to improve.
It’s always a good idea to check inspection reports and ratings when choosing a care service. You can find reports and ratings on the CQC’s website www.cqc.org.uk/ Care providers must also display their latest rating at their premises and on their website.

A new CQC assurance process for adult social care functions is being launched. The CQC will visit local
authorities to assess their processes. Visits to local authorities in England began in April 2023 to assess how they are making a difference to people’s lives.

Paying for care
Charging for community care
Most people who receive adult social care support in their community will need to pay something towards that care.
We have a policy which sets out charges that apply to people receiving adult social care support to remain living independently at home. There is a separate policy covering anyone living in a residential or nursing home.
Under this policy, we charge for adult social care support as authorised under the Care Act 2014 and carry out a financial assessment to determine how much, if anything, people should contribute towards their care and support in the community.
Charging for residential care
If you require a residential placement, we will complete a financial assessment to calculate how much you need to contribute towards the cost of your support. During the assessment, we will consider your capital assets, savings and income.
The value of your home is also included in the assessment as part of your capital if you are moving into residential care, unless your spouse, partner or a qualifying dependent remains living in the property after you have moved into care. The value of any additional property is included in the assessment.
If you choose not to have a financial assessment, you will be responsible for paying the full cost of your care and support. For more information on paying for home care, see above.
Benefits and support
Anyone who has a disability or long-term physical or mental condition may be entitled to specific state benefits to support them in their day-to-day lives. These benefits are not means tested so it doesn’t matter what other money you get.
To work out what you will be asked to contribute we need to know what income and capital you have along with any other costs such as council tax, rent or mortgage payments that are not already met by benefits.

If you are eligible for support and a financial contribution from us and you are moving into a care home or care home with nursing, we will give you a choice of homes that charge the amount we would usually pay for someone with your needs.
If the home you choose charges more than we would expect to pay to meet your needs, you will have to find someone to meet the additional amount, known as a third party payment (see page 45).
Access our online financial assessment calculator at www.derbyshire.gov.uk/betteroff which will provide you with an estimate of the charges you would have to pay. It also allows you to submit your information, including supporting documentation, for review by our Finance Team.
The extra money is to help with living costs or if you need help with daily living tasks. In some circumstances your entitlement may continue even if you are living in a residential or nursing home, for example, if you self-fund your care and support.
Remember to seek benefits advice and consider claiming:
• Universal Credit (if you are aged 18 or over but under State Pension age).
• Pension Credit (if you are State Pension age or over).
• Attendance Allowance (if you are State Pension age or over).
• Personal Independence Payment (if you are aged 16 or over but under State Pension age).
Attendance Allowance (AA), Disability Living Allowance (DLA) and Personal Independence Payments (PIP)
These are non-means-tested, non-taxable benefits from the Department for Work and Pensions. There are different rates depending on the level of your needs.
AA is paid to people who are State Pension age or over and PIP to people aged 16 or over but under State Pension age. Check your State Pension age at www.gov.uk/state-pension-age
Everyone who needs care should consider claiming these benefits. However, they will not be paid if
NHS-funded Nursing Care Contribution
Whether you are a temporary or permanent resident, if you live in a care home that provides nursing care, you may be entitled to a non-means-tested NHS-funded Nursing Care
NHS Continuing Healthcare (CHC)
This is a package of ongoing care that is arranged and funded by the NHS. To establish whether you are entitled to have your care paid for by NHS CHC, there is national framework to determine whether you have a primary health need and are therefore eligible to receive NHS funding.
Care is provided to people aged 18 and over to meet health and associated social care needs that have arisen as a result of disability, accident or illness and
you are, or become, a permanent resident in a care home unless you are meeting the whole cost of your care. If the council is contributing towards the cost of your care, these benefit payments will usually stop after you have been in permanent residential care for more than four weeks.
If you are entitled to the mobility component of DLA or PIP, this payment will not stop once you are in a care home and is not included in your financial assessment. You can no longer make new claims for DLA, which is being replaced by PIP.
Contribution (FNC) of £219.71 per week (subject to change every April) towards the cost of your nursing care. This is paid directly to the home by the NHS and is used as part payment towards the overall fee.

CHC is free, unlike support provided by local authorities, and it is not linked to savings or income levels. If you are eligible for (CHC), you can receive the services you need in any setting, including your own home or in a care home. CHC only lasts for as long as someone has complex care needs that result from their having a primary health need.
Local authority contribution towards care in private sector care homes
We appreciate that the financial climate in the care sector is challenging. It is recognised that considerable pressure exists, both for those who purchase and those who provide care home placements.
Where we can assist with funding support, the fee rates that we might expect to pay for certain types of care are listed on page 46. Complex or variable needs are assessed individually and can attract additional funding.
Fees charged by some homes may be higher than we can pay for reasons other than complex or variable needs. It is important to appreciate that independent sector care homes can decide how much they charge for accommodation and care.
Third party payments
If we are contributing towards the cost of your care and the home you choose costs more than we will pay to meet your assessed needs, you will need to ask someone else to pay the difference between what we will pay and the amount the home charges.
This additional payment is known as a top-up or third-party payment and can be made by a family member, friend or charity.
Before anyone agrees to make additional payments on your behalf, they should be aware that the amount could increase, and they need to be confident that they can maintain the payments for as long as they are required.
Running out of money
If you are paying for your own care and your capital is likely to reduce to less than the amount set by the Government (£23,250 at date of publication), you must let us know well in advance. We must undertake an assessment of your circumstances before deciding if we will contribute towards the cost of your care.
If you have been self-funding your care, and the home you are living in charges more than we might expect to pay to meet your needs, you must find someone to help pay the difference. This is known as

Whilst we can offer help in negotiating a fee on your behalf, the care home can decide to change its charges from time to time, such as if you decide to move rooms or if your financial circumstances change.
If you are concerned about any potential future changes in the amount you may have to pay, particularly in relation to top-up fees, you should discuss this with your adult social care worker at the earliest opportunity.
The person paying the top-up will be asked to sign an agreement to formalise the arrangement. If the payments stop being paid for any reason, you must seek help and advice from us as soon as possible.
The law says that you are not allowed to make this additional payment yourself, except in very limited circumstances.
If you are eligible for a Deferred Payment Agreement against the value of your property and there is sufficient equity, then you may also be able to defer your top-up until the sale of your house is completed. Support for full-cost clients
12-week property disregard
If your former home is included in your financial assessment but your other capital and savings are less than £23,250 (as at date of publication), and your income is not enough to meet your care home fees, we will contribute towards the cost during the first 12 weeks of permanent care, provided we agree that residential care is needed.

Deferred Payment Agreements
Subject to eligibility criteria, after the 12-week property disregard period, you may be offered a Deferred Payment Agreement. This agreement allows you to delay selling your former home during your lifetime.

charged against the value of your home and must be repaid once the house is sold or from your estate. Interest is payable throughout the period of the loan and there is also a one-off fee to join the scheme. This fee covers all legal and administrative costs for the lifetime of the loan.
We may limit the amount of the loan, depending on the equity in your property, but if there is enough equity, you may also be able to defer the payment of any top-up as well. If you qualify for a Deferred Payment Agreement, you may retain an amount of money from your income, up to £144 per week.
Under the terms of a Deferred Payment Agreement, you can also choose to rent out your home to generate income, which will help to reduce the amount you need to borrow and subsequently pay back to the council. You are advised to check fee levels with the provider of any home you are considering at the time of your enquiry, including any homes provided directly by us.
Seeking financial advice
Planning for your future care and support needs can be complicated and funding this can be expensive. Taking professional advice may be helpful in enabling you and your family to identify the most suitable and cost-effective solution. There are several organisations that will provide free general advice about the funding of care and support. These are a good place to start if you are looking for information and want to see what sort of options are available.
There is an increasing number of useful websites that provide information to help you make the best financial decisions in your circumstances. Some provide resources such as care cost calculators and give you the opportunity to ask specific questions.
Buying independent financial advice
In addition to the free sources of information, we work with Trading Standards to ensure that there are various financial advisers available in Derbyshire that are members of our Trusted Trader scheme. Although these advisers will have met the requirements to become a Trusted Trader, you should still carry out your own checks to ensure they can meet your requirements.
Advisers can help you look at financial options and work out what would best suit your circumstances. Some advisers are also independent in terms of the products they endorse whilst others may have links with companies selling particular financial products.

Be scam aware
Scams are frauds and tricks designed to cheat people out of their money. Scammers approach people in a number of ways, including at home, by phone, by post or online.
Scams can include:
• Fake competitions, prize draws and lotteries.
• Rogue traders.
• False investment opportunities or pension schemes.
• Clairvoyant or dating scams.
• Requests for donations to fake charities or requests for personal loans.
Scams fall into four main types:
Safeguarding adults
In Derbyshire, we aim to do whatever we can to protect people from abuse and neglect. We work together to enable people in Derbyshire to make choices to stay safe and to live a life free from fear, harm, and abuse.
We work with local people and organisations, including care providers to ensure they alert abuse and neglect and refer to Derbyshire County Council if you or someone you know is experiencing or is felt to be at risk of harm or abuse.
Local authorities have a legal duty set out in the Care Act 2014 to assist adults over 18 who:
• Have needs for care and support (whether or not the local authority is meeting any of those needs).
• Are experiencing, or are at risk of, abuse or neglect as a result of those care and support needs, or are unable to protect themselves from abuse or neglect.

Derbyshire Safe Place Scheme
A Safe Place is somewhere you can go if you feel scared, are lost or need help when you are out and about.
Safe Places are public buildings in towns and communities across Derbyshire, such as libraries, shops, pharmacies and GP surgeries. They have stickers in their windows so you can see that they are part of the scheme and know you can go in and ask for help if you need to.

The LGSCO looks at complaints about councils and some other authorities and organisations, including adult social care providers (such as care homes and home care providers). It is a free service, and its job is to investigate complaints in a fair and independent way.
What happens when you send a compliment
It’s always nice to hear positive feedback and we appreciate anyone taking the time to provide us with this information.
We’ll always ensure that the compliment is sent on to the person or service it relates to and their senior manager. This will then be recorded, and we’ll look to ensure this good practice is shared more widely with the department.
What happens when you send a comment
A comment is a remark or observation that may require immediate action. However, this does not warrant a formal investigation.
How solicitors can help
A solicitor can give you impartial advice about wills, making gifts, estate planning and powers of attorney. Some can also offer guidance on immediate and long-term care plans or ensuring the NHS has made the correct contribution to your fees.
Powers of attorney are legal documents through which people can authorise others to do things on their behalf. There is a special type of power of attorney called a lasting power of attorney, which is designed to give people authority to act on your behalf or someone else’s once that person loses mental capacity to make their own decisions. Lasting powers of attorney can be made for health and welfare decisions or for property and finance decisions. Many people ask solicitors to assist them with creating powers of attorney.

If you are self-funding your care and have a contract with your care provider, you may find it helpful to speak to a solicitor with experience in this field. Any contract should explain the cost of care, what services are included in the price and how often this is reviewed; what complaints procedure is in place; what your rights are and the obligations of the care provider under the contract; the procedure for cancelling a contract; and the administering of medication.

Derbyshire Care Providers Association (DCPA)
This publication provides you with a lot of information regarding care homes and their services within the county.
The Derbyshire Care Providers Association (DCPA) represents the independent sector and supports care providers to enable them to improve the quality of life for residents in care homes. The DCPA managing committee meets regularly throughout the year and also meets with local authorities, councillors, MPs, NHS and the regulators, the Care Quality Commission, to lobby and consult on behalf of care providers and their residents.
The vast majority of care providers provide an excellent caring service carrying out their work quietly in an effective way promoting choice, dignity, privacy and independence in an individualised manner. Bad news is, howeve,r nearly always reported by the media.
The Derbyshire Home Care Association
The Derbyshire Home Care Association (DHCA) is the official body for all independent providers of ‘care at home’ in Derbyshire. Membership is open to all home care providers who are registered with the CQC and who are on Derbyshire County Council’s approved list.
The association meets regularly, and its committee also meets with the director and senior managers of the county council’s Adult Care Services, to discuss policies, procedures and fees.
The objectives of the association are to:
• Promote the quality and standards of care in the home care sector in Derbyshire through shared knowledge and awareness.
• Provide a forum for exchange of information on topics of common interest.
• Advise members of changes in policy or of a political nature which will impact on domiciliary care.
• Consult and liaise with Adult Care Services regarding matters which concern the membership.
Good news and good practice are rarely mentioned even though they are the norm.
It is common for people moving into a care home to rediscover what life has to offer, make new friends and start enjoying life again. The help that a person receives in a care home will be planned to meet their specific needs in such a way that privacy, dignity, independence and individuality are recognised and maintained at the highest level.
As well as having rights within the home, there is also a legal right to choose which care home you wish to live in. If you receive funding support from the local authority to pay for care home fees, you may need a relative or friend to pay a ‘top-up’ charge to the care home on top of what the local authority contributes.

Because of the importance of the role that the DHCA plays, the association feels that it is important that all agencies share its voice, and it is keen to encourage membership from all home care agencies in Derbyshire.

Ivonbrook Care Home features a Spa Bathroom, wet room and two regular bathrooms. There is also a dedicated hairdressing salon, on-site laundry room, library areas, sensory/allotment garden, and small kitchen areas to promote independent living.
The homes are split into three living spaces: Cromford Close Care, Wensley Square & Pinewood Lounge.
The residents enjoy regular visits from the peacock, birds and the wild forest deer that roam the grounds and can be seen grazing in our wheelchair friendly grounds.
This Derbyshire care home is set back in its own wheelchair accessible grounds with extensive views and hoards of wildlife. Rooms have either a wash basin or an en-suite bathroom and share fantastic views of the surrounding hillsides.

People who have sensory impairment and/or limited physical disabilities and / or early signs of dementia can also be accommodated. Most placements are for planned longterm care, but respite care can sometimes be offered. Darley Hall has been tailored to suit the needs of elderly people with cognitive or physical impairments, allowing them to maintain an excellent quality of life as well as maintain their independence as much as possible in a safe environment.
The home includes a visitor’s car park as well as many shared social spaces ideal for family visits come rain or shine. The wildlife surrounding Darley Hall also makes it ideal for those fond of the outdoors or nature. The Care Home features a duck pond, home to our domesticated ducks which provide enriching activities through caring for them and their ducklings in the spring.
Darley Hall is home to the more independent residents where they have weekly choir sessions, glass fusion and art therapy and an array of daily external entertainment.

What makes Richmond different to other care homes?
Richmond is a real home from home for our residents and those who work here. There’s a genuine feeling of friendship throughout our home, both between staff and residents, and amongst the residents themselves. All our staff have a natural warmth - it’s one of the qualities we look for when recruiting. We’re often told how welcoming and approachable our home is, and how it gives peace of mind to families. Richmond is a vibrant and upbeat home.
What facilities are available at Richmond Care Home?
We want residents to lead a comfortable and varied life. We have homely lounges where residents join in with activities, watch TV, listen to music, socialise with others or enjoy some privacy We have a wide range of games and books, for all tastes. And the home has a hair salon, accessible garden and café area where residents and their visitors can have a quiet chat and cup of tea.
What opportunites are there for social interaction?
There are lots of activities for those who want to socialise. From baking and sing-a-alongs to games days and social outings. Our activities co-ordinator organises endless activities including exercise classes, bingo sessions, and arts and crafts. We often meet up within the home for refreshments and a chat. And we have regular visits from the church, our hairdresser, pet therapy groups and playgroups.
How do you ensure the highest quality of care?
All of our training complies with Care Quality Commission regulations (CQC). Our training programme starts when a member of staff joins our team and it’s ongoing. Training covers health and
adults. Senior staff undertake training in the safe handling and administration of medication as well as team leadership. Dementia awareness training, end-of-life care and person-centred care planning helps our staff to give the highest standards of care.
