Bournemouth & Poole’s premier dementia care homes We follow the Dr. Gemma Jones model of care giving which concentrates on four different behavioural stages in dementia.

We are able to provide support, prompting and reassurance in the earliest stage, followed by increasing supervision, social stimulation and help with personal care as the stages progress. We can also meet the general needs and disabilities of old age on a twenty four hour basis as the home is well equipped with a range of equipment, such as hoists, assisted baths and walk-in showers, pressure relieving mattresses and cushions etc, to support older people. We aim to meet each resident’s specific needs, including health, social, cultural, religious and dietary needs. To achieve this, we will ensure that all residents have a care plan detailing each of their individual care needs and how these will be met. Our friendly and caring staff are committed to providing every resident with individual care and attention in a safe, homely and warm environment, with consistent high standards of service and care. Our rooms have en-suite facilities, underfloor heating, television, wireless call systems and telephone facilities.

Who can help? People finding it hard to manage on their own, and their families, need to know where they can go for advice and help on the many adult social care services available locally. This Directory is an important source of that information. There are many appropriate services provided by different organisations, including councils, voluntary organisations, private care agencies and the NHS. In this

Directory, which covers a large area, three councils are responsible for community care services: • Dorset County Council (Adult and Community Services), • Borough of Poole (Adult Social Care), and • Bournemouth Borough Council (Adult Social Care). Throughout the text, for ease of use, the services are described as coming from ‘Adult Social Care’.

Where do I start? Adult Social Care (ASC) assesses need and provides information, advice and services to the people of Dorset, Poole and Bournemouth, 24 hours a day, 365 days a year. ASC supports adults of working age and older people who have physical disabilities, learning disabilities, mental health issues, a sensory loss or general frailty. The aim is to actively promote independence and choice and help people to be self-reliant and live in their own homes for as long as possible. When this is no longer possible they help people to choose good quality supported housing or care homes.

If you need any form of advice about help to remain independent, or about your care options, you should phone your relevant Adult Social Care department to request an assessment. Contact details can be found below. Anyone can ask for an assessment, regardless of their financial situation. The assessment and advice is free. You may be asked to make a financial contribution towards services arranged for you, which could be the full cost of the service for people who can afford to pay.

Help and advice ASC will always provide support and advice, even if they cannot pay for the help you need. They will do their best to help get you back to living independently. One in ten people in the UK care for friends or family members. ASC supports unpaid carers in various ways, for example by providing training, links to support groups and assisting carers to get a break from their caring responsibilities. This Directory contains information about the services available to people in Dorset, Poole and Bournemouth. These include: • the protection of vulnerable adults; • assessment of needs; • advice about social care, health, housing and money; • short term help at home to recover your independence after an illness or a crisis; • equipment to support independence; • adaptations to the home environment; • benefits advice; • meals at home; • social activity; • support for carers and providing breaks; • personal care at home; • Personal Budgets; and • care homes and care homes with nursing.

You can use Source to find out about the choices available to help you keep healthy and active and to learn about care and support options. The Source website is complemented by telephone helplines and face to face interaction. Your assessment Before they can arrange care services for you, Adult Social Care must assess your needs. Social workers, housing staff and health professionals may work with you and your carer to identify what your needs are and how they might best be met. The assessment is based on your circumstances and will be different for each person. A social worker/care manager will look at ways to best meet your agreed outcomes and goals. The best way of meeting your goals will then be discussed with you, sometimes with an occupational therapist’s help. If you already have a relative or friend who is a carer, they can also be assessed if they wish. Once your needs have been assessed and your desired outcomes established, Adult Social Care will decide if you are eligible for their help. To do this, they will use a framework set by the Department of Health. Your ‘risks to independence’ and needs will be assessed in four eligibility bands ranging from ‘low’ to ‘critical’. A financial assessment will also take place, to determine how much you can afford to contribute towards the cost of your care. For details of financial assessments and contributions, see your Council’s separate factsheets. ‘Paying for Services in a Care Home’ is available if you live in Dorset. If you live in Bournemouth the leaflets ‘Paying for Care’ and ‘Choosing and Paying for Care in a Residential or Nursing Home’ will tell you about charges. Further details are in Poole’s ‘Charging for Residential Care’ leaflet ‘Contributions towards Personal Budgets (non-residential social care services)’ and ‘Paying for a Care Home Placement’ booklet. Am I eligible for an assessment? To be eligible for an assessment, you may have one or more of the following: • sight or hearing difficulties; • mental health needs; • frailty because of old age; • ongoing care needs when you leave hospital; • learning disabilities; • permanent, substantial physical disabilities; • a long term illness; or • problems related to substance misuse.

• a social worker or health professional such as a doctor or health visitor; or • any other agency. If you wish to request an assessment or need more information, telephone your local Adult Social Care office (contact details are on page 4). They will arrange for an interpreter if you do not speak English or if you use sign language. What does the process involve? The assessment of your needs will involve your social worker talking with you, your carer (if you wish) and other people who know you well, about different aspects of your life. These discussions may include your physical health, your emotional health, how you manage to look after yourself, what outcomes you are looking for from the care and support provided and your current family and support networks. Services may be offered to meet needs that pose a ‘critical’ or ‘substantial’ risk to your independence, in line with the Government’s ‘Fair Access to Care Services’ guidelines. Methods of best meeting your needs and achieving your outcomes will be discussed and agreed with you.

Alternatively, you may provide regular unpaid care for someone and subsequently be eligible for a carer’s assessment.

Do I have a say in what services I get? After your needs have been assessed, Adult Social Care will advise you about services you may be eligible for and/ or a Personal Budget (explained on page 8) to help you meet your care needs. They will discuss with you: • the type of help you think you need to meet your outcomes; • when you think it is needed; • who will be providing the assistance; • how long they will be with you; • how it will be paid for; and • your religious and cultural needs.

Who can refer you for an assessment? You can be referred by: • yourself or a relative, friend or neighbour;

Support plans are reviewed regularly and will include all references to any Personal Budget you may be eligible for, to help pay for your care needs.

Personalising Adult Social Care In the traditional system of social care, professionals made decisions about the support people could have and who would provide it. As a result, people often had little or no choice about the support they received. The central government directive ‘Putting People First’ has changed

the way communities, organisations and individuals work to support people. It recognises that people want better quality services that are personal to them, and more control over decisions that affect them.

Personal Budgets A Personal Budget is an amount of money to pay for your social care support if you are recognised as needing longterm support from Adult Social Care after an assessment of your situation. It allows you to have more choice and control over your life because it lets you make decisions about the support that you need. It asks how you want to live your life, what you want to achieve and the type of support you need to do this. There’s also a financial assessment to work out how much you need to pay towards your Personal Budget. Adult Social Care also checks that you are getting the benefits you are entitled to. A Personal Budget, for example, can be used to pay for

a personal assistant if this is needed to support you. The amount should be enough to meet your assessed needs. The assessment will look at things like how much help is needed to carry out practical tasks for daily living. This money can be managed by you or Adult Social Care and is not classed as income - it’s paid to meet your needs, and having a Personal Budget will not affect your benefits. A Personal Budget can also be managed by someone you nominate to act on your behalf or you can ask the council to look after your budget and use it to buy and arrange support. You can also choose to use a mixture of these methods.

Direct Payments Direct Payments are financial payments made to you, if you have been assessed as eligible to receive support from Adult Social Care. Instead of councils arranging or providing services directly, you can use this money to purchase support that you consider most appropriate to meet your assessed social care needs. Direct Payments give you more flexibility and choice, by enabling you to buy support for yourself that better suits your individual needs. If you receive a Direct Payment, you are in control you can decide how your support needs will be met, by whom and at what time. You can employ your own support staff, which means they will report directly to you. Or, you can have a contract with a local agency, in which case the agency will be accountable to you, not to Adult Social Care.

Jurassic Coast

8

If you are a carer you may also be eligible to receive Direct Payments to help you in your caring role or to have some time for yourself. A Direct Payment can be used for all or part of your care or support package. Who can have Direct Payments? Adult Social Care can make a Direct Payment to most people who need support, including: • older people who have been assessed as needing adult social care services; • disabled people (including people with mental health needs) aged 16 and over, and those with short as well as longer term needs; • carers; and • parents of disabled children, for children’s services. If you are assessed as being eligible to receive support and you want to consider having a Direct Payment, you must be: • over 16; • able to legally consent to receiving the payment from Adult Social Care; and • able to manage the money to meet your needs. This may be on your own or with someone’s help, as long as you are in control of how the money is spent.

7KLV 'LUHFWRU\¡V KHOSOLQH ² LQIRUPDWLRQ RQ FKRRVLQJ DQG SD\LQJ IRU FDUH

Direct Payments continued What can Direct Payments be used for? The money is for you to arrange support to meet your eligible needs. You may choose to employ people directly, to buy services from an independent or voluntary sector provider or purchase equipment to help you live more independently. Direct Payments enable you to choose and plan your support in more flexible ways that fit in with your life. It is important that your plans meet your assessed needs in the best way for you. As long as the money is used lawfully and your eligible needs are met, there can be a lot of flexibility about how you spend the money. For this reason there is not a list of things the money can be used for. Most Direct Payments are made to meet regular ongoing support needs. However they can also be made as a single payment, for example, to purchase equipment or a short respite break. If you prefer, Adult Social Care can arrange services to meet some of your needs directly, they can offer you a combination of a Direct Payment and services that they provide or buy from another provider on your behalf.

What do I do next? If you are interested in Direct Payments and are currently receiving social care support from Adult Social Care, please contact your care co-ordinator (who may also be called your care manager or social worker) for more information. Alternatively, to find out more information about Personal Budgets and Direct Payments, please contact your relevant Council directly: Dorset County Council Adult Access Team Tel: 01305 221016 Email: adultaccess@dorsetcc.gov.uk Web: www.dorsetforyou.com/socialcare Borough of Poole Adult Social Care Help Desk Tel: 01202 633902 Email: sshelpdesk@poole.gov.uk Web: www.poole.gov.uk/health-and-social-care Bournemouth Care Direct Bournemouth Borough Council Tel: 01202 454979 Email: caredirect@bournemouth.gov.uk Web: www.bournemouth.gov.uk/adultsocialcare

Adult Social Care ‘Adult Social Care’ is the name given to services which aim to help people live at home independently with a care support package. Services might include: • Reablement - help to build up strength, re-learn skills and get ready to cope with day-to-day living at home after an illness, accident or stay in hospital. See page 10. • Personal Budgets - a flexible way of providing social care services which enables people eligible for social care to have money to choose and buy services, that suit their lifestyle - see page 8 for further information. • Direct Payments - this scheme allows you to buy in the care you need for daily independent living - more information is on page 8. • Community meals - usually supplied frozen to reheat yourself. • Day services - available in care homes or community centres. For more information, see page 29. • Housing adaptations - Adult Social Care can provide information about equipment and adaptations to make it easier for you to continue to live in your own home – see page 14 for more details.

• Home care - help with personal care such as washing and dressing. See page 18 for more information. • Supported housing or extra supported housing provided by your district or borough council. See page 32. • Respite care - residential care or care provided in your own home to give you or your carer a break. Further details are on page 29. To decide what sort of help you need, and what kind of outcomes you are looking for, Adult Social Care will work with you to assess your needs. They will then need to agree a support plan for you. This sets out how you can meet your needs and what help can be provided to help you manage at home. It is likely that Adult Social Care will ask you to make a financial contribution towards the services you receive, based on your ability to pay. You should be given clear information about what you can expect to pay. Support plans may change if your needs change, leading to another assessment, which is often called a ‘review’. Assessments are explained on page 7.

Leaving hospital and staying independent Adult Social Care and the NHS work very closely together to ensure that hospital patients are discharged safely and are able to return to home life as quickly as possible. Social care staff are available to help with information and advice when you are ready to leave hospital. No matter which hospital you are in, you can request an assessment of your needs to help you stay independent and confident when you return home. The assessment involves asking you about your needs and what is required to help you live safely and independently at home. There will also be an explanation of any charges that may be made. A relative, carer, or a member of the hospital staff may also approach the hospital social care team and ask them to carry out an assessment, but this will not go ahead unless you agree. If you already have a social care service in place Adult Social Care will make an assessment of any other needs you may have after your stay in hospital. Carers should be offered an assessment and may also need advice and support. The hospital social care team is available to discuss any issues with them and also give them details of carers’ support groups. Adult Social Care can also introduce you to local voluntary organisations who can help with a variety of services such as laundry and domestic cleaning, or sitting services. Borough of Poole also works with people who are being discharged from hospital who are not eligible for financial assistance from Adult Social Care services, sometimes known as self-funding clients. A social worker will visit these clients and undertake a

full assessment of the person’s needs. The social worker will help people with information, advice and support about appropriate community services which can include information about care homes, home care, benefits information and day opportunities. They will also signpost people to other services who may be able to help. Bournemouth residents who are being discharged from Royal Bournemouth or Christchurch hospitals following a stroke are supported by the Stroke Coordination Team. They offer practical support to help people return to living as independently as possible. This could involve signposting to services for people who need ongoing care, helping people to join clubs and groups or assistance with finding work or volunteering. You can contact them on 01202 705440 or email: stroketeam@ bournemouth.gov.uk. Poole residents being discharged from Poole Hospital following a stroke are also supported by a Stroke Coordination service. This service provides information, support and signposting to ensure that people have the information they need to make choices which enable them to live as independently as possible after a stroke. The Stroke Coordination service can also refer clients to the care management team and psychological support services if additional care support is required, based on identified need. Dorset residents being discharged from Dorset County Hospital (Dorchester), Royal Bournemouth, Christchurch or Poole hospitals are also supported through a Stroke Coordination service. This service provides advice and information, support and signposting to help people to become as independent as possible.

Reablement and rehabilitation Reablement is often described as ‘helping people to do for themselves rather than doing it to or for them’. It is short term, intensive support that enables people to optimise their independence. Dorset County Council’s reablement service provides practical and emotional assistance to people who are at risk of not being able to remain independently in their own home because of frailty, disability or illness. The service is provided by the Community Support Service. The aim of reablement is to maximise independence,

10

choice and quality of life, and reduce the need for support in the future. Borough of Poole is looking to commence a new reablement service in the near future. This will be a short-term service designed to help people who have had a crisis (like a fall or spell in hospital) to regain the confidence and skills to live independently, so they can help themselves and continue to stay in their own home. If you live in Bournemouth, are aged 65 or over, and qualify for adult social care, reablement may be one of the

Reablement and rehabilitation continued services you are offered. Reablement can support you to become independent after a fall or illness, or if you get to the stage when you feel you cannot manage without support. Staff work with you for a short time to regain skills and confidence to achieve manageable goals. They offer advice and information, make sure you have the right equipment, suggest daily living aids that might help you, and let you know about other sources of support. The aim of rehabilitation is to maintain or increase your independence and allow you to live in your own home for as long as possible. You will be supported with the provision of short-term care support and/or therapeutic intervention for example physiotherapy or occupational therapy. The assessment is carried out by multi-disciplinary teams of health and in some areas, social care staff. (These are joint teams with reablement services and social care staff.) The intermediate care services will promote your health and well-being and can help to prevent falls at home. The service also aims to reduce admission to hospital by providing a rapid response to you at home. In addition, the service can enable you to be discharged home from hospital earlier by providing care support and intensive rehabilitation in your own home. The service

helps prevent unnecessary or premature admission to long-term residential and nursing care. A discharge or a care plan for any future care needs will be discussed with you and you will be given details on how you can receive help to set up on-going care arrangements. The service may last for a number of weeks and during this assessment period, intermediate care is free of charge. When this is completed a care plan for your future care needs will be discussed and you will be given details on how you can receive help to set up on going care arrangements.

NHS continuing healthcare NHS continuing healthcare is provided solely by the NHS if you have extensive healthcare needs. This is free of charge, wherever it is delivered and can be provided in any setting. In your own home the NHS funds all care required to meet your assessed needs. In a care home, the NHS makes a contract with the home and pays the full fees for your accommodation as well as all your care. You can ask for an assessment by health and social care

professionals who will visit you to identify and prioritise your needs. The assessment will be used to decide what services will be provided and where, and who funds them. If your needs change, regular reviews will be carried out and your care and/or funding arrangements will change to meet them. Contact the Dorset Clinical Commissioning Group (CCG) for more information (see page 51 for contact details of the relevant office for your region).

Occupational therapy Occupational therapists can help you lead a more active and independent life using a variety of methods, including the use of adaptive equipment. They can assess your home for hazards and point out things that might contribute to falls. They also advise carers on techniques for moving and handling the person they care for. Adult Social Care may be able to help with specialists. Visiting you at home, an occupational therapist will be able to suggest adaptations that will improve your mobility. Perhaps handrails would be useful? Would a ramp up to your front door help if you are in a wheelchair?

Major alterations such as fitting a downstairs bathroom or widening doorways can be paid for with a Disabled Facilities Grant (DFG). An occupational therapist will talk you through any new equipment you may be given, as well as help you apply for funding for major adaptations, if you need them. An assessment of your needs will determine whether an occupational therapist will be the best person to help and what charges (if any) you can expect to pay. Continued on page 12

Telecare and equipment Assistive technology and community alarms Assistive technology or telecare services help vulnerable people, particularly older people, to live in their own homes with greater safety and independence. They use a range of technologies that raise alarms to get help for someone 24 hours a day and so provide help, security and peace of mind. Special equipment can be linked to your telephone system so you can get help quickly if you need it, even if you are unable to speak. For some of the services you do not even have to press a button to call for help as they work on sensors that detect movements like falls, gas or carbon monoxide escapes, or floods. There are different types of telecare service depending on your level of need. Services can be provided if you

have been assessed as eligible for help by Adult Social Care, or equipment can be purchased privately. Dorset Those assessed as eligible for telecare equipment in Dorset will be provided with the equipment free of charge. Equipment provided can range from a Lifeline unit and pendant only, through to a Lifeline unit and a whole range of additional sensors. An individual’s ability to pay the monitoring charge (£4.15 per week) will be based on a financial assessment. Call the Adult Access Team on 01305 221016 if you would like to find out more.

Independent home care providers In order to enable people to live in their own home for as long as possible, a range of home care services are available. Help is on offer from day-to-day tasks such as cleaning, shopping and food preparation/cooking, through to personal care that helps with tasks like dressing, bathing, toileting and prompting the taking of medication. Carers need to be properly trained, particularly in moving and handling and the use of hoists for some tasks. Before you make any decisions regarding the provision of care, be sure to contact several providers and ask for a ‘Service User’s Guide’ and their charges and a draft contract between you and the care provider. Daily care/domestic support Carers can call in on a daily basis to assist with any of the tasks described above. Depending on the level of help required their visits can be just half an hour or up to several hours. Generally, visits are available from 7am until 10pm. Some people will need multiple visits per day. Night services can also be provided. The hourly rate for these types of services depend on the services required,

the time of day and the location. Rural areas may present particular difficulties and a travel charge will probably be made in addition to the normal hourly rate. Live-in care 24 hour live-in care can accommodate people with a very high dependency on a permanent basis. It can also provide respite breaks for regular unpaid carers and shortterm support following hospital discharge. In some cases it’s preferable and more economic to have a carer actually living in the home. This can be for a short period e.g. a week, or on an ongoing basis. Typical charges for this service depend on the amount of care and the particular skills required. Live-in care is also available to people with permanent physical or mental disabilities who require long-term ongoing care. All home care providers are regulated and inspected by the Care Quality Commission who publish inspection reports on its website www.cqc.org.uk. When considering a service it’s always a good idea to check their report. See page 46 for further details.

Home care agency checklist

© 2014 Care Choices Ltd

These questions may be useful when considering using the services of a home care (domiciliary) provider to help you build up a picture of how your care needs will be met in your own home. People arranging and funding their own care either privately or through a Direct Payment or their Personal Budget are able to choose their own worker or care agency. Lists of local home care providers begin on page 20.

Agencies • What experience does the agency have in your particular field of need? Can they supply references for you to check?

• How can you contact the agency in an emergency or outside office hours?

• Home care agencies providing personal care, must be registered and inspected by the Care Quality Commission. Ask to see a copy of their registration certificate.

• How hard or easy would it be to make a complaint and how are things then put right?

• How long has the agency been operating?

• How many carers would the agency assign to care for you and would you see the same one every day? If not, how does the staff rota operate and what happens if your carer goes on holiday or is sick, will you be notified in advance that a different carer will be attending?

• If this is a private contract ask for a copy of the agency’s contract terms and read these carefully with someone you trust. Get as much help as you can going over it and ask any questions you may have before signing anything. The Citizens Advice Bureau or Age UK could help with this.

Paid carers • You should expect your personal preferences, dignity and privacy to be respected. Discuss this with the agency and ask how the most suitable carers for your particular care are chosen. Can you talk to them before deciding?

• Carers should be fully trained or be in ongoing training. Ask the agency about their policies on this. • You will have a care plan drawn up by the agency which the carers will work to. Ask how often this plan will be reviewed by the agency. • Carers must be checked with the Disclosure and Barring Service (DBS) and have a criminal records disclosure – make sure this is the case.

Paying • If your care needs do not meet Adult Social Care’s eligibility criteria and have been arranged privately you should check carefully the fee rates charged and exactly what the payment you make covers.

Further local help Handy Van Repair Service East Boro Housing Trust’s Handy Van Repair Service provides a friendly, professional repair service, providing practical, useful help around the home. If you are aged 60 or over, own or rent your home in Bournemouth or Poole and receive benefit support, eg pension credit or attendance allowance you are eligible for the service.

Contact the service on 01202 883503 and a member of staff will be happy to help. There is no charge for labour, no membership fees and no hidden costs. All you pay for is the cost of materials needed for your repair as the Handy Van Repair Service is funded by the Borough of Poole and Bournemouth Borough Council.

Day services Day services are provided in a variety of locations, ranging from residential and nursing homes to community and specialist centres. The services aim to help people live as independently as possible. If you have a learning or physical disability or if you are an older person, Adult Social Care will help to support you in finding activities which will help promote independence.

For further information on day care services in your area visit:

for care or support? For independent, impartial information on your care options, call this Directory’s helpline on freephone 0800 389 2077

To be eligible for day services Adult Social Care will have to assess your needs. This means they will have a discussion with you to agree with you the best way of organising support. They will consider your needs and decide with you what services might be suitable for you. This is explained further on page 7.

Respite care and short stays Unpaid carers sometimes need a break and this can be provided in several ways: you may wish to go into a care home yourself for a week or two; or arrange home-based respite care where another carer moves into your home; or family-based respite care where you move into another family’s home for a period. The best option for you will depend on your needs, the urgency of the situation and

personal choice and cost. If you are thinking about long-term care, but have had no experience of living in a care home, you may wish to book in for a short stay to see what it’s like. This will help you make an informed decision. If you have nursing needs, ask for a nursing determination as a contribution to your fees - see ‘Care homes with nursing’ on page 39.

Supporting unpaid carers If you look after, help or support a person who is ill, frail or disabled who cannot manage without support and do not receive pay to do this then you are a carer. The person might be a child with a disability, an adult member of your family, or a friend. You do not need to be living with the person. You do not have to be giving physical care.

The care you provide might be: • help with washing and dressing; • feeding or cooking; • getting about outside; or • emotional support and company. Continued on page 30

Supporting unpaid carers continued Whilst looking after another person is mostly very satisfying, it can also leave you isolated, tired, resentful or angry. Caring can affect your personal life, your health, leisure time and finances. As a carer you need to think about things like: • the type of help that you need to continue caring; • whether you are willing or able to carry on caring; • planning for the future; and • how to meet your own needs. Dorset Whether you care for someone for a few hours a week or 24 hours a day, you can access local support and services that can assist you in your role as a carer. You can keep up-to-date with the latest news and national developments, find out about carers’ rights and benefits and search for events and activities in your local area. The Carers Hub has been developed by the Dorset Carers Partnership (DCP) that works to give local carers a voice. The Carers Hub also includes information on short-term and respite breaks for carers. Visit: www.dorsetforyou.com/carers. Poole and Bournemouth The Carers Information Service for people caring for someone living in Bournemouth and Poole keeps families and friends informed of helpful news and events. Regular events enable carers to meet up and enjoy an activity, some of these with the person they support. Carers will receive a free welcome pack when they join, copies of the

local Caring Matters magazine, regular newsletters and a friendly team to call when they need help. Contact 01202 458204 or visit: www.bournemouth. gov.uk/carersservices or www.boroughofpoole. com/health-and-social-care/carers/carersservices-and-support/ to find out more. There are different ways carers can access a break. Throughout the year there are beach huts available and a luxury holiday lodge that carers can apply to stay in. There is a short break sitting service in both Poole and Bournemouth that runs in a similar way to the service in Dorset and is available to unpaid carers of cared for people who are over 18. It is a flexible service that provides a care worker from a local approved agency to stay with the cared for person. The service in both areas is subsidised by the local authorities, but they are all run slightly differently.

Sherborne Castle

‘Home from Home’ day respite ‘Home from Home’ offers a day sitting/respite service in the ‘Home from Home’ host’s own home. Each host undertakes training, has an enhanced Disclosure and Barring Scheme police check and their home has been inspected to make sure it reaches appropriate health and safety standards. The service is managed by East Boro Housing Trust. As well as needing a break, carers worry about what would happen to the person they support if the carer is unwell, or has an accident. Carers in Crisis is an emergency back-up scheme, available to all carers of adults over 18, that has been set up to give carers peace of mind by alerting people to keep the person being cared for safe. In Dorset call the Adult Access Team 01305 221016 or email adultaccess@dorsetcc.gov.uk.

To find out about the support for carers, and to join the Carers in Crisis scheme or Carers Information Service, in Poole call the Adult Social Care Helpdesk on 01202 633902, or email: carersservices@poole.gov.uk. In Bournemouth call the Carers Team on 01202 458204 or email: carersteam@bournemouth.gov.uk. Help for carers in Dorset Dorset County Council carers’ caseworkers provide assistance to family carers to support them in their caring role. Carers may require help in finding out information on how to find local support groups and carers’ caseworkers can assist them with this. Carers’ caseworkers may be able to support family carers to access local services which may give them a much needed break. The carers’

Housing with care Sheltered (or retirement) housing This is usually provided by councils and housing associations across the region. Sheltered (or retirement) housing is a practical step if you would like more security and companionship but don’t need to move into a care home. In a sheltered (or retirement) housing scheme you would live independently in a flat or bungalow built especially for older people within a larger complex, sometimes around a garden or communal facility and linked to a central control centre for security. Some schemes have access to a resident or non-resident manager who provides support, advice and a link to services for older people. Dorset East Dorset Housing Association has a range of housing specially designed to meet the needs of older people consisting of flats, a communal lounge, a laundry and gardens. Schemes are especially suitable for very frail older people with flats and bungalows built with wheelchair access and level deck showers. All schemes have a visiting sheltered housing officer or scheme manager. There are

schemes in Bournemouth, Wimborne, Ferndown, Corfe Mullen, West Moors and Verwood. Visit: www.dorsetforyou.com/apply-for-housing for further information. Poole Poole Housing Partnership has a dedicated team of housing officers, who provide support to residents at the signing-up stage and throughout the duration of their tenancy. They spend a lot of time having direct contact with residents, visiting them at home and dealing with enquiries. You can contact the housing officers by calling 01202 264444. Bournemouth Adult Social Care can help you decide if sheltered housing can meet your needs and liaises with Bournemouth Borough Council’s Housing Services which manages sheltered housing schemes. Contact Bournemouth Care Direct on 01202 454979 for more details.

Extra Care housing Extra Care housing is a form of housing, similar to sheltered schemes, with self-contained accommodation together with some communal facilities. In an Extra Care scheme, instead of low level support traditionally provided by wardens in sheltered schemes, higher levels of care and support are provided on site for people with assessed needs by a designated team of care workers, who can be available 24-hours a day. Extra Care allows people to retain their own tenancy and have care services delivered ensuring that they are able to remain safely in their own self-contained accommodation. Dorset A new Extra Care housing scheme has been developed in Christchurch that will help older and disabled people to live independently. The scheme at Bure House, has been devised by East Dorset Housing Association in partnership with Christchurch Borough and Dorset County councils. There are 41 one and two-bedroom flats as well as three specially adapted flats for disabled people available

32

to rent. The scheme is designed to allow people who need physical help to live independently while having 24hour care support. Every resident has their own self-contained flat with their own front door, care and support when needed and the means to install safety devices - for example, to detect if a person has a fall. All flats have been designed to wheelchair standards and include fitted kitchens, walk in showers, central heating, double glazing and smoke alarms. Anyone interested in living at Bure House should contact Christchurch Borough Council on 01202 495000 to see if they are eligible. If so, they will be invited to make an application. Part of the application process will include an assessment of their needs. Poole Most of the schemes available to rent in Poole are provided by Borough of Poole or local housing associations. Allocation of these properties is usually through the Council’s Housing Register. You will need to speak to Housing and Community Services on

Extra Care housing continued 01202 633805 to establish whether you are eligible for consideration for this type of housing. People are expected to show a degree of housing need, this can be due to unsuitability of their present home for medical or social reasons. In addition, Adult Social Care will need to consider your ability to secure alternative accommodation by taking into account your savings and equity, if you are a homeowner. Some housing associations will take an application from you directly and may help you even when Adult Social Care may not. A list of local housing associations is available from Housing and Community Services. Properties are also available to buy. The price and types of property available to buy can vary considerably. To find out more about schemes available, locally and throughout the country, you should contact Age UK on telephone: 01202 523300. In early 2011 an Extra Care housing facility opened in Poole - Delphis Court has been developed as part of the new Poole Quarter developments and provides a purpose-built facility for people who need a higher level of care and support and want to maintain their

independence in their own home. Delphis Court offers 45 flats in a mixture of one and two bedrooms, every resident will have their own selfcontained flat with their own front door. Responsive support is available 24 hours a day and where appropriate, safety devices can be installed within flats to support people to live independently. Anyone interested in living at Delphis Court should contact Housing and Community Services within the Borough of Poole on 01202 633805. Bournemouth Self-contained apartments with extra care and support on site are located in several developments in the town. High standards of accommodation complement communal facilities where you can relax and meet other residents. Specialist care staff are available to provide services to meet assessed needs. The latest technology is also available to ensure your safety and security. Bournemouth Borough Council works in partnership with housing associations, the NHS, local doctors and pharmacies to support your wellbeing. To find out more contact Bournemouth Care Direct on 01202 454979.

Housing and support for younger adults There are a range of care homes catering for younger adults with learning difficulties, physical or sensory impairments or mental health issues. There is also a range of housing options you could explore. If you are thinking about a care home placement, seek advice from social and health care professionals about your needs. Does the home provide opportunities to learn and maintain skills, and opportunities to move on into living in the community? What are the home’s links with specialist staff in the community services? Use the care homes checklist on page 41 when visiting potential homes which includes helpful questions to ask. Are you happy living at home? Would you like a little more independence with help when you need it? What are your housing options? You could: • Continue living with your family – If your home is specially adapted and you receive the practical and emotional support needed, you may prefer to stay at home. It’s also convenient if your college or job is nearby. • Move into supported living accommodation – This offers a level of independence and the chance to live alone or meet other people in similar situations. Help from a visiting care assistant is available and usually nursing care is provided nearby.

• Rent a place – You can apply for council housing which is usually cheaper than renting privately. There could be a waiting list and you must fully explain your housing needs. Local housing associations may also be able to help. Ask your council housing department for a list. If you want to rent privately, look in your local newspaper for ‘To Let’ advertisements. Housing Benefit, if you’re entitled to it, might reduce your rent. Direct Payments can also help, ask your local authority for details and see page 4. • Buy your own home – Can you afford it? Are you able to live on your own? You could think about ‘part buying’ and ‘part renting’ a place from a housing association. • Move into a care home – If you want to leave home but are not ready to live alone, care homes can meet your personal needs and offer appropriate support. Adult Social Care and Health will contribute if you are eligible and if they agree the care home meets your needs. If your chosen home costs more than usual, you may get some help but you must pay the rest. Continued on page 34

Housing and support for younger adults continued When living independently you may require some adaptations around the house. You can choose housing that’s specially adapted. There are organisations that specialise in providing this sort of accommodation including Livability (formerly John Grooms) and the Accessible Housing Register. Livability Tel: 0207 452 2000 for general enquiries Email: info@livability.org.uk Web: www.livability.org.uk Accessible Housing Register Tel: 0774 911 9385 Email: conradh@accessible-property.org.uk Web: http://accessible-property.org.uk

New homes that have been built since 1999 should be fully accessible to disabled people. However, if you choose an older home, there are a number of adaptations that could make it easier for you to use. These include wider doorways; a downstairs toilet; bathroom equipment including handrails, a walk-in bath, electronic bath seat; kitchen adaptations including lower working surfaces; lower level light switches; easy-to-use controls for heating; a stairlift; security intercom at the front door and ramp access to the garden. You should also think about the general access to your home.

What choices of home do you have and is this restricted by the amount of care you need? Is the provision in accordance with Care Quality Commission’s standards? Does the staff team in the home have good links with the local Physical Disability Care Management Team?

Adult Social Care provides care where necessary and gives support to enable and encourage service users to reach their full potential in all areas of their lives. Access to

services is via an assessment, explained further on page 7. People with learning disabilities can also use a Personal Budget (explained on page 8), which puts people at the centre of how they choose to pay for their support. This may be in their own flat or house or a supported living scheme.

Mental health Your local Community Mental Health Team (CMHT) should be your first point of contact if you have mental health issues. You can ask your GP to refer you or ask someone who knows you to contact the CMHT on your behalf. The team will discuss your housing choices with you. If you have mental health needs, residential care can help improve the quality of your life - the structured environment and activities provided can build up your confidence. The care providers should assess you carefully to make sure

the placement is right for you. A qualified member of the residential care staff may visit you at home, talk to you and look at your assessments of health/social care need. To find out more about mental health care in your region, organise visits to prospective care homes. Ask the residents about the care they receive. Are psychiatric nurses available if needed? Do the activities provided assist rehabilitation? What are the attitudes to medication and other therapies?

Dementia care Usually, the initial point of contact to access care services is either your GP (for health care solutions) or your local Adult Social Care office (for most other services). Your GP, as well as discussing your concerns, will be able to carry out a series of tests which will establish the nature of your condition and advise whether treatment is possible. Some forms of dementia are not permanent and can be caused by a range of emotional, physiological and physical ailments, which bring on dementia-like symptoms. It is important that any diagnosis eliminates the possibility of a reversible dementia before a medical prognosis is made. Following a diagnosis of dementia, your GP may refer you to a specialist like a consultant in old age psychiatry, a community psychiatric nurse or an occupational therapist with a special interest in dementia care. Support in the community As it can be best for the person with dementia to stay in their own home for as long as possible, a range of adult social care services is available to facilitate this. For example, they may provide access to: home care (discussed on page 18); meals in the home; sitting services (which provide a break for any carer); advice on equipment and adaptations (covered on page 12); day care centres (see page 29) and respite care (explained on page 29). These services can be provided directly by registered providers to those who are not eligible for Adult Social Care

financial help, however Adult Social Care will still be willing to offer an assessment of need. You may have a choice of which agency you receive your care from whether or not Adult Social Care assists with the funding of your service. People are entitled to receive an assessment of their care needs and if eligible Adult Social Care may fund services to meet these care needs or provide a Direct Payment so that people can arrange their own care services in the way that best suits them. The Department of Health has published a five year Dementia Strategy which aims to transform the lives of millions of people with dementia and their carers. Health

and Adult Social Care organisations are working together across Bournemouth, Poole and Dorset to implement the requirements of the Dementia Strategy and to ensure that people living with dementia have access to good quality care services. The Dementia Strategy identifies how crucial it is for people who may be experiencing the early symptoms of dementia to be able to access good quality advice and support and receive a diagnosis as to whether in fact they have a dementia illness. Alternatively people may want to initially access one of the memory clinics or Dementia Cafés that are run by the Alzheimer’s Society, details of these can be obtained from the Alzheimer’s Society national helpline on 0300 222 1122 or the local office on 01202 716393. Information about services provided by the Alzheimer’s Society is also available on its website www.alzheimers.org.uk.

Things to look for in care homes specialising in dementia. Please use in conjunction with the care homes checklist on page 41. Residents The best indication of a good home is that the residents appear happy and responsive. • Are there rummage boxes around the home to stimulate residents? Access If the person with dementia needs or is likely to need equipment or adaptations you may want to check: • There is adequate signage and cues for different parts of the home such as dining room and bathroom. • The home’s policy about locking external doors. Bedrooms

Memory cafés Memory cafés are, first and foremost, a setting in which people with memory loss can share fun and laughter with their care partners and friends in a setting free from awkwardness and stigma. They are also a setting where accurate information about resources is available and relationships with persons in similar circumstances are formed. They are a vehicle for bringing dementia ‘out of the closet’ and into the heart of the community.

You may want to find out whether the person with dementia can have a single room and whether: • Residents are encouraged to bring in some of their own furniture and possessions to increase familiarity. Activities Residents should be stimulated without feeling stressed. • Are reminiscence activities available? • Are residents able to compile memory boxes? • Do staff compile life story books including photographs and mementoes?

• Will the person with dementia have a member of staff particularly responsible for their care?

Manager/head of home A manager who is caring as well as efficient can make all the difference to a home. • Does the manager have knowledge of dementia and can they deal with difficulties that may arise in an understanding way? • Is there a full assessment at home before a resident is admitted?

Questions to ask of any home registered for someone with dementia would include: who is the senior clinician who can act as a single point of contact for people to

approach for information and advice? What training programmes do staff participate in? Does the home offer a person-centred approach to their care planning? Is the home designed with the needs of its clients in mind, especially safety and accessibility? Is there plenty of natural light? Is moving around helped by easilyunderstood signage? How is technology employed to help alleviate falls and help orientation? Please also see the care homes checklist on page 41 and the residential dementia care checklist on page 37. Differentiating one home from another can be difficult, but the quality and quantity of staff training is especially important, along with the turnover of staff, as continuity of relationships in this type of home is critical. The use of medication is a highly emotive subject and any antipsychotic medication must be very carefully prescribed. A fine balance exists between ‘acting in the best interests of the individual’ by prescribing medicine and the benefits of ‘normalisation’ where time and energy are spent maintaining familiar, comfortable surroundings which help reduce stress and possibly avoid the need for certain medication.

Care homes All care providers in the country must be registered with the Care Quality Commission (CQC). All services are inspected by the CQC, who report on their findings. These

inspection reports are available from the care service or the CQC (visit: www.cqc.org.uk). Further information about the CQC can be found on page 46.

Types of care homes There are two types of care home: Care homes offering personal care only If you are reasonably active, but would like greater security and care, subject to a needs assessment, a care home offering only personal care may be the best option. Personal care includes bathing, feeding, dressing and help with moving and must be paid for in full if your capital/ savings exceed £23,250. If you have less than this amount, Adult Social Care may be able to assist with funding if they agree living in a care home is the best way of meeting your needs. Care homes with nursing If you think you may need nursing care in a home, you will

need to be visited by a social worker or a care manager to work out what care you will need. This visit might be in your own home, or in hospital if you’ve been ill, or in a care home. You will be fully involved in planning your care needs. If a care home providing nursing care is the best solution for you, the social worker will give you information to help you find a home which meets your care requirements. The cost of the nursing care part of your fees is paid by the NHS to the home directly: the current amount is £109.79 per week. You can contact the Nurse Care Management Teams for your area at your local Clinical Commissioning Group listed on page 51. The figures mentioned here may change after April 2014, check with Adult Social Care after this date. See page 42 for more information on paying for your care.

choose a care home in another area. The home you choose must be suitable for your assessed needs and comply with the terms and conditions set by the authority. The fees that your local authority will pay may vary. They may offer you the fees you would receive if you remained where you are, or they may offer you the fees that the local authority would pay in your chosen area. You must seek further advice before making your decision. If you’re self-funded, obviously you have freedom of choice, however it is important that you have considered the fees the home charges and whether the home will accept the local authority rates if or when your savings fall below the capital limit. If the home is unable to meet the local authority rate the possibility of moving to an alternative home increases substantially.

This Directory’s helpline This Directory’s free helpline provides an independent information and help service encompassing care, accommodation, funding and rights. A personalised report can be generated for callers providing details of all care homes or housing with care schemes that meet their criteria with supplementary information about

choosing and funding. One call to the freephone number 0800 389 2077 will enable the service to build a profile of exactly what type of care you’re looking for, while taking into account your personal needs and interests.

Paying for care Paying for your own care Even if you know you will have to pay for it yourself, if you need care in your own home or are considering moving into residential care, you still qualify for a social care assessment by Adult Social Care. The assessment will make sure that your needs will be met. Also, if you expect your capital to fall below £23,250 as a result of paying for care, Adult Social Care may then help you with the cost. If you are moving into residential care, you may be entitled to some of the following financial assistance and support: Twelve-week property disregard This applies if: • your former home is included in your financial assessment, • your other capital is less than £23,250, and

42

• your income is not enough to meet your care home fees. Adult Social Care will help with the cost during the first twelve weeks of permanent residential care, provided the assessment has shown that this is the kind of care you need. This is called the ‘property disregard’ period. Deferred payment agreements After the twelve-week property disregard period, Adult Social Care may be able to help you while your property is being sold. In these circumstances they will charge any financial help they give against the value of your home. This means they will take back the amount they have paid once your property has been sold. However, they may limit how much they will pay.

Paying for your own care continued It may affect your entitlement to Pension Credit if your property is not seen to be on the market and becomes treated as capital by the Department for Work and Pensions (DWP). What if you run out of money? If your capital is likely to reduce to £23,250 as a result of paying for care, you must tell Adult Social Care well in advance that this is going to happen. They will then be able to help with your care fees (provided your care needs assessment has shown that you need residential care). They will carry out a financial assessment to confirm the date from which your funds fell below £23,250, and the amount they, and you, will pay towards your care. If the home you have chosen charges more than the local authority will pay, you must find someone to pay the difference – a ‘top-up’ payment. Whoever does this, whether a family member or a charity, they should understand that the cost could increase, and that they will have to pay the top-up for as long as necessary. The local authority will not meet the additional cost even if you have lived in the home for a number of years. If you cannot find someone to help you meet the extra cost, you may have to

move to a home within the local authority’s funding levels. Understanding your rights before moving into care is essential. There are a number of financial products and specialist companies who may be able to help. It is important to seek financial and legal advice about the various options you may have before committing yourself. Attendance Allowance This is a non-means-tested, non-taxable benefit from the DWP. It’s paid at the lower rate of £53.00 a week if you need care by day or night and at the higher rate of £79.15 a week if you need care during the day and night. Everyone who needs care can, and should, claim Attendance Allowance. If you are paying the full cost of residential care, with or without nursing care, you will be entitled to the higher rate. NHS Nursing Care Contribution Whether you are a temporary or permanent resident, if you live in a care home that provides nursing care you may be entitled to NHS Nursing Care Contribution towards the cost. It is not means-tested and is currently £109.79 a week. This is paid directly to the home.

What will you have to pay for your care? Do you have more than £23,250 in capital and savings? The value of your home is included in the assessment of your capital unless one of the following still lives there: • Your partner (that is, your husband, wife or civil partner, or someone you live with as if they were your husband, wife or civil partner) or • A relative who is over 60 or disabled or • A child under 16 who you, or a former partner, are responsible for. NO Adult Social Care will contribute financially towards your care costs after they have carried out an assessment of your care needs. How much? You will need to ask Adult Social Care for an assessment. This will define your care needs and how they will be

best met. Any care homes you consider must be able to meet these needs. Adult Social Care will also make an assessment of your financial situation. This determines how much they will pay and how much you will have to pay. Your choice of care home will be limited to those that accept your local authority’s funding level. If you want to choose a more expensive home you would have to arrange for a third party - such as a family member or charity - to ‘top-up’ the difference. You are not allowed to do this yourself if your capital is below £23,250. If you have capital between £14,250 and £23,250 you will pay £1 a week for every £250 you have above £14,250. If your capital is less than £14,250, and the home you choose charges fees that are within your local authority’s funding rate, your contribution will be assessed only on your income. YES Ask your local Adult Social Care department for an assessment. See contact details on page 45. Continued on page 44

What will you have to pay for your care? continued Any care homes you consider must be able to meet your assessed care needs. If, apart from your property, your savings are less than £23,250, Adult Social Care can help with your care costs for the first twelve weeks. After this time you can apply for a loan to help you whilst your property is being sold. WHATEVER YOUR CIRCUMSTANCES: Remember: • It is just your own financial circumstances that are assessed, not your partner’s. • Your assessment will be made up of two elements; a care needs assessment and a financial one. • A care home with nursing will generally be more expensive than a care home.

Consider claiming: • Income support • Pension Credit • Savings Credit (if you are over 65) Definitely claim: • Attendance Allowance, worth either £53.00 or £79.15 a week depending on your care needs. Moving into a nursing home? • You will be eligible for the NHS Nursing Care Contribution – currently £109.79 a week. Third party payments for care home fees Some care homes and care homes with nursing charge fees that are higher than the maximum amount Adult Social Care can contribute. If they are contributing towards your care home fees and you choose to move into a home which charges a higher fee, the difference between the two amounts is paid by a third party, usually a relative. Third party payments are sometimes referred to as ‘top-up’ payments. Before you sign any contract, the home should give you written details of all the charges it intends to make in its ‘Statement of Purpose’ document. If there is anything that is not clear or which you do not understand, you should ask for advice. As the name suggests, this payment must be made by someone other than yourself or Adult Social Care. It can be a relative or friend, but it cannot be a ‘liable relative’ i.e. someone whose financial situation has been taken into account when your financial assessment was carried out by Adult Social Care. The general rule is that you cannot use your own money to fund a third party payment. However in certain specific circumstances you may make a third party payment from your own funds. These are:

considering these options. If you need further advice about how a top up to your care home fees might be arranged please discuss this with your care manager. If you move to a care home where a third party payment is required, the person who will make the payment on your behalf must sign a contract with Adult Social Care, before the contract with the home is signed. In doing so they must confirm that they have the financial means to make the third party payments (including any future increases) for the whole time you will live at the care home. If you are already a resident in a care home, and no third party agreement was required at the time you became a resident, the home may seek to introduce a third party payment at a later date. Also if a change to your arrangements is made at your request or with your agreement, for example you move to a superior room, then an additional third party payment can become due.

• when you are eligible for the twelve week property disregard (see page 42) or • where Adult Social Care have agreed to a deferred payment until your home is sold (see page 42). Adult Social Care recommends that you seek independent financial and legal advice if you are

Care fees can increase It is usual for the fees charged by care homes to increase each year. If you are not paying your own fees, the amount that Adult Social Care can contribute may not increase at the same rate as the home fees increase. Any shortfall in fees must be met by a third party (top-up) payment – discussed on page 45. The third party element of the fee can only be increased once in any financial year and as a guide, the amount should never increase by more than 25% a year. You should receive at least one

month’s notice that the third party payment is to increase. The person who makes the payment for you must sign a new contract with Adult Social Care if the amount of the third party payment increases. If you are already living in a care home, plan ahead and find out what help you will get from Adult Social Care when your savings fall below the £23,250 limit, explained on page 43.

Essential information Inspection and registration of care services All care providers must be registered under a system introduced by the Health and Social Care Act 2008, which brings adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety for the first time. The Care Quality Commission (CQC) is the independent regulator of health and social care in England. They regulate care provided through a range of methods including: • registering providers to ensure they are meeting the essential standards of quality and safety; • monitoring how providers comply with the standards; • promoting improvement in services;

Following an inspection, each service is given a report of how it rates against national essential standards of quality and safety. Each service’s report can be seen on the CQC website: www.cqc.org.uk For any enquiries or to register a concern or a complaint, contact CQC by telephone: 03000 616161 or by email: enquiries@cqc.org.uk The Care Quality Commission Citygate Gallowgate Newcastle upon Tyne NE1 4PA

• using enforcement powers where necessary; • publishing up to date information about the compliance of adult social care providers with essential standards of safety and quality.

When considering care services, it’s always a good idea to check a service’s inspection report on the Care Quality Commission’s comprehensive website: www.cqc.org.uk

Comments, compliments and complaints All organisations need to know how they are performing. They are happy to receive your feedback on their service whether they are compliments or complaints. Feel free to tell them what you think and your comments can be used constructively to improve the service. If you do need to make a complaint you should feel able to complain about any aspect of your care that affects your happiness or comfort. This could be about the way you are treated by a staff member or the quality of the food you are served. You can also make comments and suggestions about possible improvements to your surroundings and the services provided. Making a complaint should not be difficult for you.

46

Providers are required under essential standards of quality and safety to have a simple and easy to use complaints procedure that they will be happy to give you. If you are concerned about the care that you, a friend or a relative is receiving, it makes sense to speak to the manager before you take any further action. The problem may be resolved quite easily once they are made aware of it. However, if you need to make a formal complaint, you should initially contact the registered owners of the service. They have a duty to respond to any complaints made. If Adult Social Care have arranged and funded your care, another option is to complain to your social worker/care manager or the Complaints Manager in your local council:

Have your say about local services Healthwatch is the new independent consumer champion created to gather and represent the views of the public. Healthwatch plays a role at both national and local level and will make sure that the views of the public and people who use services are taken into account Three Dorset organisations - Help & Care, Citizens Advice in Dorset and Dorset Race Equality Council - have joined forces and will work to ensure the health and well-being of Dorset’s residents is championed across the county Local Healthwatch has taken on the work of the Local Involvement Networks (LINks) and also represents the views of people who use services, carers and the public on the Health and Wellbeing boards set up by local authorities, provides information and signposting about local health and care services and reports concerns about the quality of healthcare to Healthwatch England, which can then recommend that the Care Quality Commission take action.

Protecting vulnerable adults Report abuse of a vulnerable adult A vulnerable adult is any person aged 18 years or over • who is or may be in need of community services because of mental or other illness, disability or age, and • who is, or may be, unable to take care of themselves against significant harm or serious exploitation. Abuse is a violation of an individual’s human and civil rights and may consist of a single act or repeated actions. It may be physical, sexual, financial, psychological or institutional, or may relate to neglect or discrimination. If you have concerns that a vulnerable adult is being abused:

• Call your local Adult Social Care department (including out of hours) on the relevant numbers on page 48. • If there is a danger to life, a risk of injury or a crime is taking place, call the Police by dialling 999. • If the vulnerable adult is living in a registered care home or receiving domiciliary (home) care services, you can contact the Care Quality Commission (CQC) on 03000 616 161. • You can also share your concerns with a public service professional, such as a social worker, community nurse, GP, probation officer or district nurse who will be able to offer advice and support. The Registered Nursing Homes Association Good nursing homes continue to provide a safe, comfortable, caring environment for anyone requiring nursing – whether long-term or for short-stay respite, convalescence, or rehabilitation. The nursing home of today is a far cry from the outdated perception of latter years. Today’s nursing home combines an ambience of the client’s personal home, with quality furnishings and surroundings, together with a warm caring atmosphere backed up by qualified, professional nursing staff. A total package of care is provided, designed exclusively for the individual, which encompasses social, medical and welfare needs. It may be that you are making the choice, or more likely nowadays, a family member or close friend is looking on your behalf. There is one level of comfort that can go a long way to removing some of the concerns of placement – choosing a ‘blue cross’ home. What should you expect from a quality ‘blue cross’ home? • 24 hour nursing services provided by a team led by a qualified nurse.

48

• Accommodation furnished and equipped to provide comfortable ‘homely’ surroundings. Most homes have lifts, lounges, dining rooms and gardens/conservatories. • General welfare, social and occupational therapy activities designed to enhance your quality of life. The most important consideration from your perspective is that the nursing home provides a holistic approach to care – all aspects of health are taken into consideration and the use of NHS or GP resources ensures prompt action. If you are frail or elderly, detecting health problems, and responding early, stops you suffering unnecessary pain or discomfort and prevents avoidable traumatic admission to hospital. LEGAL HELP AND ADVICE It would be nice to think that your retirement and old age will be worry free. It can be. Advance thought and preparation can pay dividends for you and your loved ones at a time when you may be less able to cope by yourself. Some people are lucky enough never to need help with their ďŹ nancial or personal arrangements. For many, though, health issues or general slowing down in old age mean that decisions and action on day to day matters (which seem routine when you were younger and ďŹ tter) need to be taken by someone else on your behalf. If you take no steps to make arrangements yourself then the only course of action may be the appointment of a Deputy through the Court of Protection. The Court will oversee the actions of your Deputy and complicated work is necessary to comply with the Court’s requirements. A level of formality is needed which can seem harsh and intrusive, not to mention expensive.

The alternative is for you to make decisions yourself ahead of the difficult times which you may face in the future in the form of a Lasting Power of Attorney (LPA) for either your ďŹ nancial or welfare affairs or both. An LPA enables you to appoint people of your choice to handle your affairs and gives your personal authority to them to act with greater ease on your behalf. It is important to note that if you have already made an Enduring Power of Attorney it will probably still be effective, but it would be sensible to take advice on whether a welfare LPA would suit your future needs. Dickinson Manser LLP has a large established team, ready to give you the beneďŹ t of their experience, understanding and professionalism. To arrange an initial discussion, please contact Jane Freeman or Diane Vincent on 01202 673071.

Quality care and companionship in the comfort of your own home

Deciding to ask people to come into your home to care for you or a loved-one can seem daunting. However, there are a few straightforward questions you can ask to help decide on a care provider. Ask how carers are recruited and trained. They should be subject to a Disclosure and Barring Service Check (formerly known as a CRB check). Carers should also have been trained in moving and handling, to protect both their clients and themselves. If carers do not hold a QCF (Qualifications and Credit Framework – previously the NVQ) award in Health and Social Care, then a good provider will have enrolled them on a course. Professional development opportunities grow a motivated, quality team of carers. You should check how the calls will be scheduled. Will you get care at a time convenient for the company’s existing schedules or will it be given when you need it?

01202 280 855 or 0800 298 1139

So, if you want to go to bed after watching TV in the evening, that is when the carer should come, not in the afternoon. In just over a year, Right at Home has recruited thirty incredible carers. That such a relatively new company has enough work to employ so many staff is testament to the reputation they have established. Please give us a call if you want to chat about the options available to you. Photo: Paul Sumner of Barrett & Coe
