Welcome from Gloucestershire County Council Gloucestershire County Council, working in partnership with Gloucestershire Care Services, provides social care to vulnerable adults, those with disabilities and older people. We recognise that for many people, health and social care needs overlap, so we have developed an integrated care model with health colleagues. This will support people to live independent lives and help people get back home with help and support, rather than going into residential care before they are ready to. We are also working closely with our colleagues in health to make sure things run smoothly as you get support from different organisations.

Care Providers Association (GCPA) and care providers to ensure quality standards are consistently maintained in the care we provide. Regulations for registered residential, nursing and domiciliary care services focus on improved outcomes around involvement and information, personalised care and treatment, safety and safeguarding. This Guide provides information and advice on a range of options and signposts you to local organisations and services. I hope you find it useful in helping you and your family to make informed decisions about the choices available to you in Gloucestershire.

Introduction from the Gloucestershire Care Providers Association The Gloucestershire Care Providers Association (GCPA) is a membership body open to all independent providers of social care (private, not-forprofit and charitable organisations) delivering care and support to vulnerable people across a variety of settings within Gloucestershire. Members work with older people, people with learning difficulties, people with mental health issues and people with disabilities. Membership of the GCPA is a sign of a progressive organisation, committed to delivering the highest standards of quality care and keen to play an active part in the county’s wider social care community. Representatives of the GCPA attend regular strategic and liaison meetings with County Council staff, to review and influence contracts, policy and procedures. The GCPA also works closely with the Acute Trust and Clinical Commissioning Group in the county. The GCPA works with its members together on a variety of initiatives aimed at both tackling immediate issues facing vulnerable people and designing effective, quality ways of working for the future. In addition, the GCPA has strong links with the partners and regularly liaises with senior county councillors, local MPs, and other decision makers, influencers and interested organisations.

The GCPA’s main aims are to: • represent the interests of independent care/ support providers and promote membership within the county; • work in partnership with Gloucestershire County Council, Gloucestershire Clinical Commissioning Group, Acute Trust and other statutory and regulatory bodies in Gloucestershire to help improve quality social care delivery; • encourage the sharing of best practice amongst members; • negotiate with Gloucestershire County Council staff the annual fee reviews and contractual revisions within the county; • arrange workshops and training opportunities in partnership with various organisations; and • support managers through peer-to-peer network meetings. Finding information Accessing the internet There is lots of information available on the internet. If you have never used the internet but would like to learn how, help is available. Libraries can provide courses for people who have never used the internet before. Library staff and computer buddies help over 800 people a week make better use of the internet. Sign up and a course can be organised in your local library. To book a place, contact Gloucestershire Libraries on 0845 230 5420.

Connect for Carers Run by Gloucestershire Rural Community Council (GRCC), this project offers free help to carers in Gloucestershire to get started and feel more

confident online, learning computer basics on their own terms. GRCC provides help and advice with computers and the internet for those who are either new and nervous users or those who want to further their knowledge. Support can be given through one-toone training and support by volunteers in your own home or group sessions around the county. Get health advice ASAP The NHS in Gloucestershire has launched its ASAP (Choose Well) public information campaign. Adults and parents of young children can find advice on what to do if they are ill or injured and are unsure where to turn.

The ASAP website and app allow users to ‘Search by Service’ or ‘Search by Condition’ – providing a stepby-step guide through symptoms, self-care and signposting to the appropriate NHS service/s.

As well as online options, you can call NHS 111 for advice, 24-hours a day, seven days a week and can visit your pharmacy for advice on whether or not you may need to use other NHS services.

It includes information about how the social care system works and how to use it when you need it.

The service is committed to collaborative working across agencies and seeks community solutions wherever possible, reconnecting people and places to help older people remain living independently.

Formal social care support Adult social care services have changed across England to give service users, their families and carers much greater choice and control over the support that they receive and who delivers it; this is called self-directed support. It’s a move away from a traditional system where care professionals make all the decisions. Your Circle is the name of Gloucestershire County Council’s approach to this more flexible and personal method of support. Instead of individuals fitting into existing services, the changes reflect a more personcentred approach, ensuring that: • people have access to short-term support to help them regain their independence and reduce reliance on longer term support; • people’s changing needs are met, according to their own individual circumstances; • people feel in control of decisions about their own support; • support and services are flexible and reflect people’s day-to-day requirements; and • everyone has easy access to information, advice and advocacy.

If you or your family feel that you need extra support, you should first call the Adult Social Care Helpdesk on 01452 426868. The person you speak to will help you to talk about any problems you are facing and will give you information about what help may be available locally. Alternatively, visit www.yourcircle.org.uk

Assessing your needs What is the assessment process? An assessment is how the Council decides whether a person needs care and support to help them live their day-to-day life or, in the case of a carer, their needs for support. The aim is to get a full picture of the person and what needs and goals they may have. If the person being assessed has a relative, friend or carer then they may want to involve them in the assessment too. If someone is likely to have substantial difficulty in being involved in their assessment, and if they do not have anyone else appropriate to support and represent them, then the Council will arrange an independent advocate for them. The assessment must be carried out by an

appropriately trained assessor, for instance a social worker, who will consider a number of factors, such as: • the person’s needs and how they impact on their wellbeing – for instance, a need for help with getting dressed or support to get to work; • the outcomes that matter to the person – for example, whether they are lonely and want to make new friends; and • the person’s other circumstances – for example, whether they live alone or whether someone supports them. After carrying out the assessment, the Council will consider whether any of the needs identified are eligible for support. Because not all care needs can be met by the Council, it uses a national eligibility

Assessing your needs continued framework to decide which needs are eligible to be met by public care and support. Whether your needs are eligible or not, the Council

will be able to offer guidance and information about sources of support that you can access or provide for yourself. The Council views carers as equal partners in your care and they are entitled to have a separate assessment of their needs in their own right as a carer, to see if there are services that might help them. See page 44 for more information on support for carers and carers’ assessments.

Advocacy – help to be heard Advocacy in all its forms seeks to ensure that people are able to: • have their voice heard on issues that are important to them; • defend and safeguard their rights; • have their views and wishes genuinely considered when decisions are being made about their lives; and • understand the information that they need to use to make choices about their care and support arrangements.

Keeping active and staying healthy To ensure people age well and later years are accompanied by good health, it’s important to remain active, engage in regular exercise, maintain

your social contacts and adopt a healthy lifestyle. Chronic disease, disability and poor health don’t automatically come with old age.

Keeping active As people get older, keeping active can help them remain independent for as long as possible. Improving strength and balance can prevent falls and keep people from slowing down, so they can keep living the life they want. Some of the benefits of keeping active include: • a reduced risk of developing a life-threatening disease; • a greater likelihood of maintaining or reaching a healthy weight; • a greater sense of wellbeing; and • improved sleep and increased vitality. There are many activities that can help reduce falls, including: • community exercise groups specifically developed for increasing strength and balance; • tai chi, a form of exercise that has many health benefits and can improve balance; • swimming, which improves muscle tone and strength while the body is supported;

• walking in a group, which can benefit fitness as well as being a social activity; and • dancing, which can improve fitness and balance. If you are concerned about falling, talk to your GP about being referred to a suitable exercise group through the ‘exercise on prescription’ scheme or to your local district council who can advise on suitable activities in your area. Remember, before beginning a new exercise regime, it’s a good idea to talk to your GP.

Social activities Social activity plays an important part in keeping you fit and healthy. This could be meeting a friend for lunch or a cup of tea, going to the theatre, or attending a day centre regularly. The In Touch Project The aim of Gloucestershire Rural Community Council’s In Touch Project is to enable older people to access health-related and social activities in their communities, through the provision of information, advice and signposting, resulting in greater

independence and physical/mental wellbeing. It does so by: • providing older people with information, advice and access to health-related and social activities in Gloucestershire (existing and new); • liaising with and further developing relationships with health and social care professionals providing information and learning relating to the work of the project; • continuing to hold events relating to health, the

Social activities continued transformation of social care and emerging needs of older people; and • remaining visible and engaging in strategic activity to ensure the needs of older people living in the county are considered in the planning process. Hubs are established in a variety of locations depending on what is available within each

community. The Hubs offer a resource to the community that helps people to remain independent, maintain their wellbeing and stay connected with other people. The diagram below includes just a selection of the broad range of activities now being offered at hubs across the county.

Travel and transport Am I still safe to drive? Safer Driving with Age (SAGE) is a programme providing older drivers with support, guidance and coaching to enable them to drive for as long as it is safe to do so. Go to www.roadsafety-gloucestershire.org.uk for more information.

Concessionary Bus Passes You are eligible for an age-related concessionary bus pass when you reach the pensionable age applicable to a woman, irrespective of whether you are male or female, that will entitle you to free travel on most local bus services, throughout England, between 9.30am and 11pm on Mondays to Fridays and all day at weekends. Certain exceptions apply, for example bus passes cannot be used on night buses. If you have not yet reached the pensionable age and you have a disability, you may still be eligible for a concessionary bus pass. Dial-a-rides are wheelchair accessible and are generally for anyone who has difficulty with public transport. Accessibility services Shopmobility schemes hire out or lend manual wheelchairs, powered wheelchairs and powered scooters to anyone who needs help with mobility to get around. In Gloucestershire, there are Shopmobility centres in Cheltenham and Gloucester. Pre-booking is advised in Cheltenham by calling 01242 255333 and Gloucester by calling 01452 302871. There are around 8,000 locked toilets in Gloucestershire specifically for disabled people. You can get a key from most tourist offices.

Established in 2001, Celtic Care Services is a family owned and run domiciliary care provider. We are the domiciliary care provider of choice in Chippenham, Corsham, Calne and surrounding towns and villages. Staying healthy Illness or injury? Not sure where to turn? If you need medical help and advice but it is not an emergency, contact NHS 111, available 24-hours a day, 365 days a year. Calls are free from landlines and mobiles. The service is staffed by a team of fullytrained advisers with clinical support, who will ask questions to assess your symptoms. They will then give you the healthcare advice you need or direct you straight away to the local service that is best placed to help you.

Alcohol If you are concerned about your drinking, contact your GP. They may offer you advice or support in the practice, put you in touch with Independence Trust community health trainers or refer you to another professional, such as a hospital consultant.

Any agreement for work made with the trader present at your home for more than £42 must include the right to cancel by law and you must be given written information about your right to cancel up to 14 days from the agreement, even if the work has started. Occupational therapy If you are experiencing difficulties with day-to-day living due to a physical disability, sensory impairment or old age, you can request an occupational therapy assessment. Occupational therapy promotes health and wellbeing through occupation. If you are eligible, an occupational therapist will look at all the activities that make up your day, including self-care, leisure pursuits and productivity. They will work with you to help you live as independently and safely as possible. Occupational therapists will look at a range of ways to enable your independence, including: • teaching and practising different techniques; • providing equipment on loan; and • referring on for minor and major adaptations.

The occupational therapy service will work with you to identify what issues are important to you. It may: • help you to achieve a balance between your selfcare, work and leisure activities; • advise you on suitable housing and ways of adapting your home to meet your needs; • help you to remain at home for as long as you are able to do so; and • offer advice on benefits you may be entitled to. Equipment and adaptations Mobility equipment There are many types of equipment that can help you to live independently at home. Something as simple as fitting a grab rail in the right place to help you get up and down steps safely and easily, or equipment to help you get out of bed, could be all you need. The service also offers everything from kitchen and bathroom adaptations to a handyperson service. You may be charged for some services. You can contact the Mears Safe at Home service yourself on 0800 032 3139, or, if you are being assessed by a health or social care worker, they may refer you for minor adaptations, for example, access ramps or lever taps. You can also contact the Adult Social Care Helpdesk on 01452 426868, which may be able to organise items costing less than £250 to be fitted quickly and free of charge without an assessment by a health or social care worker. See below for more information on home adaptations. Before you buy any costly equipment, it is important to get the right advice to ensure your money is well spent. The service is for anyone who is at risk of losing their home or is in need of practical support to develop skills around tenancy management, maintaining their homes or maximising their income. It is also for anyone who needs support to develop practical skills around access to work and volunteering. Support is provided in the community through a series of drop-in sessions, one-to-one sessions and group work. The service is open to any age group

Getting back on your feet Reablement Reablement helps people to get back their independence after a period of ill health, a hospital or care home stay or a fall or accident. Reablement provides support to help you manage your ongoing health and wellbeing and can be provided in your own home or in a care home. The reablement team will work with you by offering an assessment in order to identify outcomes and

agree a plan of reablement. You will be allocated a key worker who will review progress with you regularly. Throughout the period of reablement, you will be encouraged and supported to rebuild your independence to a level which is safe and meaningful for your personal lifestyle. The main focus is to support you to remain in

Getting back on your feet continued your own home. For example, you may need opportunities to: • rebuild your confidence to carry out daily living tasks essential for your health and wellbeing; • increase your strength, stamina and mobility; • learn new ways of carrying out activities to increase independence and reduce risks, organise your day and prioritise tasks differently; • look at ways to manage your worries or concerns; or • try out equipment and ideas designed to support your independence. Eligible people may have access to reablement support for up to six weeks. There will be no charge for the service. However, should you need ongoing care and support, and this can be decided at any time during your period of reablement, it would be means-tested and possibly charged for. Community health and social care teams working together will provide this service. The team may include physiotherapists, occupational therapists, social workers, community mental health nurses, community nurses and reablement support workers.

Short stay residential rehabilitation Short stay residential rehabilitation may suit people who can benefit from a programme of rehabilitation but who, in the short-term, may not be safe staying at home on their own. There are several residential units across the county providing this service. Short stay residential rehabilitation gives you the opportunity to recover, rebuild your confidence and to practise daily living skills, such as washing, dressing and preparing simple meals. If ongoing support is required to maintain your independence, the Council might undertake an assessment and may arrange a suitable long-term service provider. You could also buy your own support to suit your lifestyle and give you greater control over how your needs are met. If your needs are assessed as eligible, you will be offered a financial assessment to establish whether the Council will contribute towards the cost of your services. Community meals Community meals or ‘meals on wheels’ are cooked hot meals which are provided to people in their own homes, to assist them to live as independently as possible.

Home care Home care is care and support that is provided in people’s own homes. This is sometimes referred to as ‘domiciliary care’. Not all agencies provide the same services and charges vary between agencies. For care packages which have been arranged by Gloucestershire County Council, contracts and support plans will have been completed by your social worker or support planner and the agencies used will be appropriately registered with the Care Quality Commission, see page 66 for more information. If you are not eligible for support from the Council and/or would like to organise your own care at home, you need to think about it carefully. You could

use the listings of home care providers on page 33 to find an agency. However, before you make any decisions, ask for a copy of the agency’s contract terms and read these with someone you trust, or seek the support of an advocate. Get as much help as you can going over it and ask any questions you may have before signing anything. The Citizens Advice Bureau or Age UK could help with this, contact details are on page 71. For more information about independent advocacy services, see page 9 or, for a referral to advocacy services, call the Advocacy Service on 0800 644 6448. You should expect the care agency and the individual care workers to respect your personal preferences, dignity and privacy and promote your wellbeing. The checklist on page 31 gives you useful questions to ask any prospective home care agencies. Carers Who is a carer? A carer is somebody who provides support or who looks after a family member, partner or friend who needs help because of their age, physical or mental illness, or disability. This would not usually include someone paid or employed to carry out that role, or someone who is a volunteer.

Three in five, or more, of the adult population may become carers at some point in their lives and there are many organisations that can give them information, support and help.

Support for carers Carers Gloucestershire Carers Gloucestershire is an independent, countywide, carer-led organisation which provides information, advice and guidance on carers’ issues, emotional support (including counselling) and a helpline which operates from 8am to 8pm weekdays. Carers Gloucestershire also gives carers the chance to have their say in a variety of ways, including attending carers’ forums or taking part in Gloucestershire Carers Alliance, the Parent Carer network or Adult Carers United.

Carers’ assessments A carer’s assessment is an opportunity for carers to express their feelings and needs as a carer. The aim is to find out what impact, now and in the future, caring responsibilities may have on their everyday life; to look at finding ways of supporting the carer to maintain their own health, and balance caring with their life, work and family commitments. Carers will also be asked if they are able and willing to carry out the tasks involved in caring. The assessment is not about judging the carer’s ability and the way in which they carry out their caring role. The assessment may result in information, advice or locally-available support which may help to make things easier for the carer. It may also result in services being provided to the person that they care for.

Who can have a carer’s assessment? If it appears that a carer has needs arising as a result of the care they are providing, they have a right to a carer’s assessment. They can have an assessment whether or not the person they care for has had an assessment of their needs and whether or not the person they care for is eligible for support from the Council. If someone cares for a child with a disability, the carer’s needs will be considered as part of an assessment for their child and family. Carers can also have a combined assessment where their needs will be assessed at the same time as the needs of the person they care for. Short breaks for carers Short breaks give carers a break by providing care for the person being cared-for. The break may be within a residential home, a respite care service or within the person’s own home. Housing with care Shared Lives Gloucestershire Shared Lives is a county-wide scheme enabling vulnerable people to live with an approved carer or family in the Shared Lives carer’s home. It offers outreach/day support, short breaks, respite and longer-term care to vulnerable adults living as part of the family. Short breaks can be just for a few hours a week, a weekend, or overnight care from one night to a few weeks. Longer-term care on a more permanent basis is also an option. Carers are

specially recruited and must go through an approval process. The scheme can be used by any eligible adult with a learning disability, mental health need, or a physical disability, and older people who may need special care and attention, for example, following a hospital discharge. A needs assessment will be carried out to determine whether you are eligible, see page 8. Sheltered housing Sheltered housing for rent is owned and managed by councils and housing associations across the region. Sheltered housing is a practical step if you would like more security and companionship or wish to downsize from a larger home that has become difficult to manage. Private providers have also developed retirement housing for sale within the county. In a sheltered, or retirement, housing scheme

you can live independently in a flat or bungalow built especially for disabled or older people within a larger complex, sometimes around a garden or communal facility. Please visit www.housingcare.org for further information.

Extra care housing New forms of sheltered housing have been pioneered to cater for older people who are becoming less able to do things for themselves; these are known as extra care housing schemes. These schemes provide self-contained, wheelchairfriendly accommodation that meets the requirements of people who need care and support. For example, the bathrooms are fitted with walkin showers to make it easier for older people to

maintain their independence. Schemes usually have their own care staff on-site and some schemes provide one or more meals each day. Extra care housing provides a real alternative to residential care. Depending on the scheme, you can either rent or purchase the properties. Care villages A care village will generally be a development of 50 units or more offering a variety of accommodation, including one and two bedroom apartments and bungalows available to buy and to rent. Care and support is provided to tenants, dependent on their need, with the added benefit of knowing that 24hour emergency support is available.

The economies of scale of a care village mean that there are greater opportunities to provide health and wellbeing facilities and services that encourage mental and physical stimulation.

The concept of a care village is based around supporting tenants and residents throughout the ageing process, creating a rich and diverse community within the village in tailor-made accommodation. Units are usually available to those aged 55 and over who move in as part of their lifeplanning process. This gives the reassurance that, as the needs and circumstances of people change, they do not have to go through the upheaval of moving from their home to receive a higher level of care and/ or support.

There are currently three care villages in Gloucestershire – St Oswald’s Village in Gloucester, run by the Extra Care Charitable Trust, Richmond Village in Painswick, run by Richmond Care Villages Ltd, Crystal Fountain Village in Nailsworth, known as Woodchester Valley, which is owned by and run by the residents.

Residential care Going into a care home is a major commitment for your future – it involves changing where you live and potentially committing to paying considerable amounts of money for your ongoing accommodation and care needs. Any council help with the cost of residential care is means-tested. You can make your own arrangements if you have the means to cover the long-term cost. However, consider asking the council for an assessment, because you might be eligible for support with some or all of your care costs. During the financial assessment, the Council can only take into account income and assets you own. The Council cannot ask members of your family to pay for the basic cost of your care. See page 63 for more information on paying for care.

Listings of care homes and care homes with nursing in Gloucestershire begin on page 73. Before you decide to move to a care home, consider other options, including home care, equipment to help you live independently at home or a Shared Lives scheme, see the information earlier in this guide. You should also assess whether you need the amount of care on offer at a care home, and look at alternatives such as extra care housing schemes or warden-controlled sheltered accommodation. These options offer independence with an increased level of care and support. All care providers in the country must be registered with the Care Quality Commission (CQC). All services are inspected by the CQC, which reports on its findings. These inspection reports, along with quality ratings, are available from the care service or the CQC at www.cqc.org.uk/ Further information about the CQC is on page 66. Gloucestershire County Council contracts regularly with some providers in the county; contact the Council for more information. Care homes and care homes with nursing listings begin on page 73, and are divided into the areas as shown in the map on page 4.

If you are eligible for financial support, and you choose a care home that costs more than the Council usually expects to pay for a person with your needs, you may still be able to live in that care home if a relative or friend is willing and able to pay the difference between what the Council pays and the amount the care home charges. This is called a ‘top-up’ payment. However, if your friend or relative’s situation changes and they are no longer able to pay the top-up, the Council may have no obligation to continue to fund the more expensive care home place and you may have to move. Consider this potentially difficult situation when deciding on care home options. If you have eligible needs, you have the right to ask the Council to contribute towards the cost of your care while you try to sell your home. This is known as a ‘deferred payment agreement’. See page 64 for more information.

If you’re looking for a care home, there is a variety of options available. There are care homes for older people, homes for younger adults with physical disabilities, learning disabilities, brain injury resulting from an accident, or mental health conditions. Care homes can care for adults with more than one condition and some homes will have expertise in providing care for adults with alcohol or drug dependency. These care homes may offer permanent residence or provide care for a temporary period. Care homes may be privately owned or run by charities or councils. Some will be small care homes based in home-like domestic dwellings, while others will be based in large communal centres. One of the first options you have to consider when choosing residential care is whether you need the care home to provide nursing care or personal care.

Types of care home Care homes (personal care only) If you need support 24-hours a day, but don’t need nursing care, a care home offering only personal care may be the best option. Personal care includes bathing, feeding, dressing and help with moving. Care homes with nursing Care homes with nursing offer the same personal care as care homes but with nursing staff on duty 24-hours a day. If you need a home with nursing care, you will be assessed by a nurse, who will determine the level of nursing care that the NHS will pay for. This is paid directly to the home. It is important that this nursing assessment is carried out before you go into a care home with nursing.

Any support from the Council with paying towards the cost of a care home will be means-tested. For information on paying for care, see page 61.

Out-of-county care You can choose a care home outside your home county. You may want to be closer to friends and family members for example, or you may want to relocate to another part of the country. If you choose to move to a new local authority area and, before you move, you are eligible for support from Gloucestershire Council to pay for your care, it may contribute to the costs of your care home placement in the other area. The Council will usually only pay care home fees that are the same as the fees that the local authority where you are moving to would pay. If the care home that you would like to move to is more expensive than this, the Council would offer you an alternative home at a fee that it would agree to pay. If the home you

choose charges a fee in excess of the local authority’s funding limit, you will need to find someone else to pay the additional amount. This is often referred to as a ‘third party payment’ or ‘top up’. For more information about paying for care, see page 61. If you are paying for your care yourself, it would be advisable to contact the local authority in the area that you are moving to, so that they can support you with advice and information. It will be important to find out about the support that your new local authority could give you if, in the future, you needed help to pay the care home fees. If you paid for your own care at the time that you moved, any help that you needed in future in relation to paying for your care would need to come from the local authority in the area you moved to. End of life care ‘How we care for the dying is an indicator of how we care for all sick and vulnerable people. It is a measure of society as a whole and it is a litmus test for health and social care services.’1 Although the subject of dying is often painful to contemplate, it is essential that you and your relatives discuss your wishes as you near the end of your life. Good planning for all your needs should mean that you can (if you choose) remain in your home or care home in your last days and not have to be admitted to hospital. When choosing a service you should consider its end of life care philosophy and policies. Enabling people to die in comfort and with dignity is a core part of providing care and the way care professionals approach the process will be incredibly important for you, your family and carers. The service should have an open approach to end of life care. It should initiate conversation with you and your family and work with your GP to ensure your needs and wishes are discussed and planned for. An end of life care plan should also be drawn up, as it is vital to establish your preferences and choices. You could use the Preferred Priorities for Care (PPC) document to help with outlining your wishes. The PPC document is designed to help you prepare for the future and gives you an opportunity to think about, talk about and write down your preferences and priorities for care at the end of life. It is also important to know that all health and social care staff must be trained in communication

regarding end of life care and will be trained in assessing the needs of individuals and carers. It is worth asking care providers whether they are working towards The Gold Standards Framework. This can be used in various settings, for example hospitals, primary care and care homes, to improve the co-ordination and communication between different organisations involved in providing care for someone near the end of their life. ‘End of Life Care Strategy - promoting high quality care for all adults at the end of life’ April 2008. Care and support for people with specialist needs Hearing loss It is easy to underestimate the personal and practical problems that can result from a hearing impairment, but a lot can be done to help, and it doesn’t always need to involve a specialist service. Signs of hearing loss can include:

Visual impairments Visit your optician or GP if you’re concerned about any aspect of your eyes or vision. Some general health conditions, like diabetes, show themselves in the eye and an optometrist is trained to detect these. They will refer patients to other healthcare professionals as appropriate. If appropriate, you may be referred to a consultant

ophthalmologist who will assess whether you qualify for registration as either sight impaired (partially sighted) or severely sight impaired (blind). If they think you do qualify, the ophthalmologist will complete a certificate of vision impairment and send it to the Sensory Services Team at Gloucestershire County Council.

Drop In Centres Drop In Centres are for people with disabilities who are over the age of 18. They provide a safe informal environment where you can simply pop in and find out about other services that are available in your area. Drop In Centres provide advice and support on a wide range of issues and can offer access to independent advice and advocates who can work with individuals to talk about and resolve concerns and problems independently. Drop In Centres also organise community activities (football, swimming, healthy walks etc.) and a variety of educational talks and groups. You can also just pop in for a coffee and a chat. Learning disability If you are considering moving out of your family home, there are a number of options that could be explored. There is the option of supported living, where you can live independently or share a property with other people, with support provided. The property is owned by a company other than the support provider, which gives security for tenants. It means the care provider can be changed, without you losing your tenancy.

Support can include learning independent living skills like cooking, cleaning and money management, as well as help with health needs. Mental health Mental ill health affects one in four people in England. There is a wide range of specialist mental health support available to people in Gloucestershire – from support in people’s own homes or in the community to specialist inpatient support. If you have concerns about your mental health, your first stop should be your GP, where you can find out which therapies and treatments are available. The Wellbeing Project is delivered by The Independence Trust, and provides a wellbeing service for people 18+ years of age with a mental health condition. People can refer themselves to Independence Trust, but they can also be referred via a health provider such as their GP, their housing provider, or by other support organisations. The services provided are: Peer Support, Bridge Building, Safe Space, online Client Forum and an Activity Programme. 2gether NHS Foundation Trust provides specialist mental health and learning disability services to the people of Gloucestershire. Paying for care in your home If you or your family feel you need extra support, you may be eligible for support from Gloucestershire County Council. If your needs are eligible for support from the council to help you live at home, you may be

offered a personal budget, discussed on page 62. Some people will not have to pay towards the cost of meeting their needs but others may be asked to contribute towards, or pay the full cost of care following a financial assessment. See page 63 for more information on financial assessments.

Non-means-tested support There is a range of benefits, tax credits and other financial support that you may be able to receive. Benefit Entitlement The Government website, www.gov.uk has links to useful benefits calculators to help you find out what is available. The Citizens Advice Bureau also has online advice, contact your local office – see page 71 for details. Age UK can also help older people to access benefits. Call their national advice line on 0800 169 2081 or Age UK Gloucestershire on 01452 422660, email enquiries@ageukgloucestershire.org.uk or visit www.ageuk.org.uk/gloucestershire Personal Independence Payment (PIP) PIP is a tax-free, non-means tested benefit that helps towards some of the extra costs of living with a longterm health condition or disability. It is based on how a person’s condition affects them, not the condition they have. Attendance Allowance This is a non-means tested, non-taxable benefit from the Department for Work and Pensions (DWP) paid at the lower rate of £55.10 per week for those needing care by day or night, and at the higher rate of £82.30 per week for those needing care both during the day and night. You can get Attendance Allowance even if no-one is actually giving you the care you need – even if you live alone. Attendance Allowance is not usually affected by any savings or income you may have. Everyone can, and should, claim this benefit. Please note that entitlement to this benefit does change upon admission to care so it is important that you contact the Attendance Allowance Helpline at

the DWP on 0345 605 6055 with any change in circumstance to avoid any overpayments in benefit being made. Special Rules If you have a progressive disease and are not reasonably expected to live for more than another six months, there are special rules to help you get Attendance Allowance quicker and easier. You can get the higher rate immediately, whatever your care needs are. You can make a claim for someone under the special rules without them knowing or without their permission. If they satisfy the relevant conditions, they will get a letter saying that they have been awarded Attendance Allowance, but special rules will not be mentioned. For further advice, contact the Attendance Allowance Helpline on 0345 605 6055 or your local Citizens Advice Bureau. NHS Continuing Healthcare NHS Continuing Healthcare is fully-funded care and support, provided and paid for by the NHS. To be eligible, your needs must be primarily health-related and are likely to be severe. If you are eligible for NHS Continuing Healthcare, you can receive the services in any setting, including your own home or in a care home. The NHS will pay if you need healthcare from a community nurse or a therapist as well as personal care to help you at home.

Personal budgets Everyone’s needs for care and support are different, and needs can be met in many different ways. If someone is assessed as having eligible needs, see page 8, the Council can help them plan their support. The care and support planning process considers a number of different things, such as what needs the person has, what they want to achieve, what they can do by themselves or with the support they already have, and what types of care and support might be available to help them in the local area. The planning process takes place with the person, any carer they have and any other person they ask the Council to involve. The process will also determine the amount of your personal budget. This is the amount of money that the local authority has worked out it is likely to cost to arrange the necessary care and support for you. If you are eligible for financial assistance from the

Council, see page 63, it might contribute towards your personal budget. You can then decide how much control you want to have over arranging your care and support and managing the money. Personal budget by direct payment This option provides the greatest flexibility. Any Council contribution is paid directly to you. You can use this money to organise, buy and manage your agreed support. A thorough guide to help you manage your money – The Direct Payments Guide – is available online at www.yourcircle.org.uk or from the Adult Social Care Helpdesk, call 01452 426868. Managed personal budget The Council can organise, buy and manage your agreed support for you, although this does reduce the level of flexibility available to you. Combination personal budget You can choose a combination of the above two options. This will be discussed with you.

Personal Budgets continued Direct payments If you receive a direct payment, you are responsible for how the money is spent. The Council will need to see that you have used the money in the right way, so keep a careful record of how you use the money and always keep your receipts. You aren’t allowed to use your direct payment to pay for permanent residential accommodation – but you may be able to use it to pay for occasional short stays, if the Council agrees that is what you need. Most direct payments are made to meet regular

ongoing support needs, however they can also be made as a single payment, for example, to purchase equipment or a short respite break. Direct payments are paid directly into your bank, building society, Post Office or National Savings account. Additional information Various leaflets about direct payments are available. Paying for residential care When a local authority provides or arranges services for you, you may be asked to contribute towards the

cost of care at home or in a care home, depending on your personal circumstances.

Financial assessment To determine whether or not you will need to contribute towards the cost of your support, the Council will conduct a financial assessment. This will look at your capital, savings and income. Most people are likely to have to contribute something towards the cost of their care. The value of your home is included as part of your capital in this assessment if you are moving into a care home, except in limited circumstances. It is not included if you are receiving care in your own home. If you choose not to have a financial assessment, you will be responsible for paying the full cost of your care and support. If you have capital and savings of less than £14,250, you will be asked to contribute part of your income towards the costs and the local authority may meet the shortfall. If you have capital and savings between £14,250 and £23,250, you are likely to have to contribute towards the cost of your care. You may need to contribute

part of your income, and a sliding scale operates whereby you contribute £1 for each £250 you have in capital and savings above the lower figure. If your capital and savings amount to more than £23,250, you will have to pay the full cost of your care yourself. However, you are still entitled to an assessment of your needs by the Council and you may be able to get some support with funding your care, see page 64. The Council will discuss this with you in more detail if you choose to have a financial assessment. These figures may change during the lifetime of this Directory. If you are eligible for support from the Council and you are moving into a care home or care home with nursing, you will be given a choice of homes that accept the amount the Council would usually pay for someone with your needs. If the home you choose charges a fee in excess of the local authority’s funding limit, you will need to find someone else to pay the additional amount. This is often referred to as a ‘third party payment’ or ‘top-up’ (explained further on page 64).

Running out of money If your capital and savings are likely to reduce to £23,250 through payment of care home fees, you must let the Council know well in advance. It must undertake an assessment of your circumstances before deciding if it will make a contribution. If you become eligible for the Council’s support with funding your care, and the home you are living in charges more than the Council’s fee levels, you must find someone to help pay the difference. This is known as a ‘top-up’ or ‘third party payment’, see

below for more information. If funds for a top-up are not available, you will need to find accommodation which can meet your needs at the local authority’s fee level. Understanding your rights before moving into care is essential. There are a number of financial products and specialist companies that may be able to help. It is important to seek independent financial and legal advice before committing yourself to anything. See page 65 for more information.

Self-funding your care Paying for care can be an expensive and long-term commitment, so the Council strongly recommends that you seek specialist information and advice before entering into any arrangements. It is important that you seek specialist advice from appropriately qualified and regulated financial advisers, see page 65 for suggested contacts.

during your lifetime. Any fees paid by the Council will be charged against the value of your home, and must be repaid once the house is sold or from your estate. Interest is payable throughout the period of the loan and there is also a one-off fee to join the scheme which covers all legal and administrative costs for the lifetime of the loan.

Twelve-week property disregard If your former home is included in your financial assessment but your other capital and savings are less than £23,250, and your income is not enough to meet your care home fees, the Council may share the cost of the first twelve weeks of permanent care, provided it agrees that care is needed.

The Council may limit the amount of the loan, depending on the equity in your property. Speak to the Council for more information on this.

Deferred Payment Agreements After the twelve-week property disregard period, you may be offered a Deferred Payment Agreement. This will allow you to delay selling your former home

NHS Nursing Care Contribution Whether you are a temporary or permanent resident, if you live in a care home that provides nursing care, you may be entitled to a non-means tested Registered Nursing Care Contribution (sometimes referred to as Funded Nursing Care) towards the cost of your nursing care. This is paid directly to the home.

Third party payments If you are eligible for the Council’s support, you will be offered a choice of homes that accept the local authority’s funding rates. If you decide to live in a more expensive home and someone is able to make an additional payment for you, they will have to pay the local authority the difference between its rate and the amount the home charges. This additional payment is called a ‘top-up’ or ‘third party payment’.

You are not allowed to make this additional payment yourself, except in limited circumstances, so the responsibility usually falls to a family member, friend or charity. Before anyone agrees to pay your top-up, they should be aware that the amount may increase, usually once a year, and they need to be confident that they can sustain the payments for as long as

Third party payments continued they are required. They will need to sign a contract with the Council to confirm that they are able to do this. If the additional payments stop being paid for any reason, then you should seek help and advice from your Council. You may have to move to a home that accepts the local authority’s funding levels. If you are already resident in a care home, and no top-up was required at the time you became a

resident, the home may seek to introduce one at a later date, which would need to be agreed first with the local authority. This may happen if a change to your arrangements is made at your request or with your agreement, for example you move to a different room. It is important to note that councils have a duty to offer you a place at a home that accepts their funding rates. If no such place is available, the Council should not charge a top-up.

Ensuring quality of care Inspecting and regulating care services All care providers must be registered under a system which brings adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety. The Care Quality Commission (CQC) registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others. Inspectors carry out a mixture of announced and unannounced inspections looking at the quality and safety of the care provided. They look at whether the service is: Safe, Effective, Caring, Responsive to people’s needs, and Well-led. Inspection teams are led by an experienced CQC manager and often include experts in their field. The team may also include professional or clinical staff;

Experts by Experience; people who use services; and carers. Following an inspection, each care home and home care agency is given a report of how it rates against national essential standards of quality and safety. The CQC has also started rating services as: Outstanding, Good, Requires Improvement and Inadequate. When considering a care service, it’s always a good idea to check its inspection report and rating on the CQC’s comprehensive website www.cqc.org.uk or ask the care provider you are considering for a copy of their latest inspection report. Complaints about health and social care services If you have a complaint about a service, the first thing you should do is tell the service. This applies whether you pay for your care or the Council funds it. By law, every care/support service and social care service must have an efficient procedure for dealing with complaints. Ask for a copy of the service’s complaints procedure so that you understand what you must do and how they will deal with your complaint. Care funded by your local council If you are not happy with the reply you receive from a care service when you complain, you can take your complaint to your local social services department. You will need to follow their complaints procedure –

your social worker, care manager or the department itself will explain what to do. Protecting vulnerable adults Far too many vulnerable adults suffer abuse, neglect and worse, often at the hands of relatives and carers, those who you would least expect to treat them in that way. A vulnerable adult is a person aged 18 years or over who may be unable to take care of themselves, or protect themselves from harm or from being exploited. This may be because they are old and frail or have: • a mental health condition; • a disability; • a sensory impairment; or • some form of illness. What is adult abuse? Abuse is mistreatment by any other person that violates a person’s human and civil rights. The abuse can vary from treating someone with disrespect to causing actual physical suffering. The Council has a statutory duty of enquiry where there is reasonable cause to suspect that an adult, aged 18 or over, in its area: • has needs for care and support (whether or not the

authority is meeting any of those needs); and • is experiencing, or is at risk of, abuse or neglect; and • as a result of those needs is unable to protect himself or herself against the abuse or neglect or the risk of it. Abuse can happen anywhere – in a care home, a hospital, in the workplace, at a day centre, at home or in the street. Abuse could be physical, sexual, psychological, emotional, financial, discriminatory, modern slavery, organisational abuse, neglect or selfneglect. If you think someone is being abused call the Adult Social Care Helpdesk on 01452 426868. Your concerns will be taken seriously and will receive prompt attention. If the abuse is also a crime such as assault, racial harassment, rape or theft, you should involve the police to prevent someone else from being abused. If the police are involved, the adult social care team will work with them and with you to support you. If you feel someone is in immediate danger, call the police on 999.

Legal matters A solicitor can give you impartial advice about wills, making gifts, estate planning and Lasting Powers of Attorney. Some can also offer guidance on immediate and long-term care plans, ensuring (if applicable) the NHS and/or Council has made the correct contribution to your fees. Lasting Powers of Attorney (LPAs) allow you to appoint someone you trust to make decisions about your personal welfare, including healthcare and consent to medical treatment, and/or your property and financial affairs. A LPA is only valid once registered with the Office of the Public Guardian. It allows for a person of your choice to make decisions on your behalf at a time when you may be unable to do so. The Court of Protection can issue Orders directing the management of a person’s property and financial

affairs if they are incapable of managing their own affairs should they not have a LPA. An ‘advance directive’ allows you to communicate your wishes in respect of future medical treatment but it is not legally binding. You may instead wish to make a living will, properly known as an ‘advance decision’ setting out treatment that you do not want to receive in specified circumstances, which would legally have to be followed, even if you die as a result. Any proposed gift out of your estate needs careful consideration of the benefits, risks and implications, particularly on any future liability for care costs or tax liability. Making a will A will sets out who will benefit from your property and possessions (your estate) after your death.

Legal matters continued Although you do not have to make one by law, it is the best way to make sure your estate is passed on to family and friends exactly as you wish. If you die without a will, your assets may be distributed according to the law rather than your wishes. There are many good reasons to make a will: • you can decide how your assets are shared – if you don’t have a will, the law says who gets what; • if you’re an unmarried couple (whether or not it’s a same-sex relationship), you can make sure your partner is provided for; and • you can make sure you don’t pay more Inheritance Tax than necessary. You can draft a will yourself or use a solicitor. If you choose to write your own will you can buy a form on

the high street for example, from WH Smith.
