Norfolk County Council’s Community Services Department is committed to helping people maintain their independence and live fulfilling lives. Adult Care is striving to achieve a more personalised service, to put you at the centre of your care choices and to enable you to be in control of those decisions. You will find information on Self-Directed Support and Personal Budgets on page 37. We know that many people come to Adult Care at times of need and in worrying circumstances. Whether you will fund your own care or not, we can help you to walk through our services and provide information and advice along the way. The NHS is changing Changes are happening within the NHS but there is no change in how we as patients are cared for or how we can access NHS care. Patients should still speak to their GP or community pharmacist for less serious issues if they have health problems. Hospitals, community and mental health services are there for patients in the same way as before. However from April 2013 the responsibility for health commissioning transfers from NHS Norfolk
and Waveney to Clinical Commissioning Groups (known as CCGs). CCGs are led by family doctors and nurses. The role of CCGs in commissioning is to plan which services are required locally, then use public funds to ‘buy’ those services from hospitals, other NHS organisations or independent providers as appropriate. To do this, CCGs will work with patients, healthcare professionals and other stakeholders such as Norfolk County Council.

Arranging your care Your choices Some people who are eligible for services from adult care will choose to make their own arrangements with a person or an organisation to provide them with the help and support they need. You can do this without first having had an assessment if you wish. This is known as a ‘private arrangement’. Those people who have had an assessment from adult care and are found to be eligible for services can have the choice about how the service(s) is arranged for them.

How do I access services and get help and support? continued >> telephone, or in more complex cases, we will arrange to do this face-to-face. In either situation you will be asked your personal circumstances and current situation. You will be asked to describe what your care needs are and what you believe will be of help to you to meet these needs. It will be explained to you that Norfolk County Council has an eligibility criteria as our priority has to be the most vulnerable people. However, we can give you and your carer information and advice, whatever your needs are. We can also put you in touch with other organisations that may be able to help you. We will discuss with you the options available to meet your needs. It may be that other voluntary agencies may be able to help and we can provide this information. If you are eligible for services they may be provided under a Personal Budget, except for in emergencies. This will allow you to either have the services directly arranged by us, or you will be allocated an amount of money to enable you to purchase your own care directly. The Care Assessor will also advise you about Norfolk County Council’s charging policy as you may have to pay for some or all of the social care you need. After your Personal Budget has been arranged, the
Care Assessor will contact you again to ensure you are satisfied with the service you are receiving. We want to provide services that enable you to live as independently as possible. We have to take into account any risks to you or to others and we have to be fair to everyone in Norfolk who needs our help. The eligibility criteria describes how serious a risk to your independence there is if your needs are not addressed immediately or in the near future. The levels of risk are in four categories, these are: • • • •
‘Critical’; ‘Substantial’; ‘Moderate’; and ‘Low’.
These definitions are taken from guidance issued by the Department of Health. The full guidance can be found on the Department of Health website www.dh.gov.uk. If our assessment is that the risk is ‘critical’ or ‘substantial’, then it is likely that you will be eligible for our social care services. You may still need to pay towards the cost of your services. You can find out more about charging here: www.norfolk.gov.uk. For those who have a ‘moderate’ or ‘low’ level of risk, we will provide advice and information about the help that may be available for you. Staying in your own home Home care Also known as domiciliary care or home support, home care provides practical help for people to stay independent at home, in Housing with Care and Supported Living. Examples of the sorts of services that home care can include are: • Personal care – such as help with getting up or going to bed, bathing, showering and going to the toilet. • Food and drink – such as help with eating and with special diets, help to manage different types of meal services and menu planning, help to prepare and cook food.

• Simple treatments – such as help with prescribed medication, reminding you when medication needs to be taken, applying creams and lotions, simple dressings and eye drops. • Personal assistance – such as help with dressing, surgical appliances and manual aids, help to get up and go to bed and getting around your home using a hoist. • Domestic tasks – such as washing up, lighting fire/ helping you stay warm, washing clothes or soiled linen. • Night Care or Night Sitting Services – which means a

Home care continued home care assistant in your home during all or part of the night to make sure you are safe and provide personal care services listed above. You can either arrange these services yourself or contact Community Services. To decide what sort of help you need, Community Services will carry out an assessment. If you are eligible for services, you will receive services through a Personal Budget, except for in emergencies. This will allow you to either have the services directly arranged by Community Services or you will be allocated an amount of money to enable you to purchase your own care directly, or a mixture of both. Community Services also provide advice and information which enables you to live safely and independently in your own home. An emergency respite line is available for carers, called ‘In My Place’. The service allows carers to log vital information about the needs of their cared for person and who to contact in an emergency. By completing a simple form, carers can create an emergency plan which can be activated quickly by health or Community Services. To find out more and to request a pack call 0344 800 8020. There is an emergency telephone service for outside normal office hours on 0344 800 8020. It is for people who are in danger or are unable to remain at home without immediate help. If you call outside normal office hours and can reasonably wait until the next working day, we will ask you to call back.

Many people can be involved in arranging a support plan, but it is important that you and anyone who is caring for you take part in the discussions. Your views and wishes should always be taken into account. It is likely that we will make a charge for services provided, based on your financial circumstances. You should be given clear information about what you can expect to pay.
Older People’s Community Outreach Service A new service aimed at supporting older people living in their own homes to maintain their independence has launched in Norfolk. The community outreach service for older people launched in 2012 and supports vulnerable people who live in their own homes but might need some extra help to live full and active lives in their communities. The support service, which is free to use for up to two years, has been created and funded by
Norfolk County Council and aims to reach more than 1,000 people in its first year. It will see trained staff or volunteers visiting older people’s homes and providing them with help and advice on areas which can become challenging in older age. These could include personal safety and security, managing finances, using health or legal services, social or community activities, making adaptations to their home and measures to reduce the risk of falls at home.

It can include help with understanding complicated official forms, support with claiming benefits and grants and help to contact other services. There is a free countywide service, ‘Stonham Homestay’, which provides this help: telephone 0845 155 0390. Home care charges Adult Care may be able to help you with the cost of some of your home care, ask for an assessment of your care needs. Any financial assistance will depend on your assets and income and can come in the form of a Direct Payment. The help that you are provided with should be reviewed at least annually. This will provide you with a regular check on how well your arrangements
are meeting your needs. If at any time you feel your circumstances have changed or that costs have increased, you should contact Adult Care to request another assessment. For those care packages which have been arranged by Adult Care, contracts and care plans will have been completed by a care manager and the agencies used will have been accredited to work for the Council. People arranging and funding their own care either privately or through a Direct Payment (see page 38) can choose their own worker or care agency. Live-in care 24 hour live-in can accommodate people with a very high dependency on a permanent basis. It can also provide respite breaks for regular carers and shortterm support following hospital discharge. In some cases it’s preferable and more economic to have a carer actually living in the home. This can be for a short period eg a week, or on an ongoing basis. Typical charges for this service depend on the amount of care and the particular skills required. Live-in care is also available to people with permanent physical or mental disabilities who require long-term ongoing care. All home care providers are regulated and inspected by the Care Quality Commission who publish inspection reports on their website www.cqc.org.uk. When considering a service it’s always a good idea to check their report. Before you make any decisions regarding the provision of care, be sure to contact several providers and ask for a ‘Service User’s Guide’ and their charges and a draft contract between you and the care provider.

Norfolk County Council Home Support service commissions the independent sector to support the people of Norfolk to lead independent and dignified lives at home. The service helps with a range of tasks, dependent
on an assessment and eligibility, which includes personal care, assistance with food and drink and assistance with medication. The service is delivered by a team of trained, experienced and competent staff. The quality of the service will be regularly reviewed by the registered manager of the service and is regulated and inspected by the Care Quality Commission.

What can I do if I need urgent help?

Norfolk Swift Response is a service that provides help and support to people who have an urgent and unplanned need. Available 24 hours a day, it provides assistance or reassurance during a period of urgent need to people in their homes who don’t need help from the emergency services. They can assist with the likes of: • Supporting somebody with getting up, washing and dressing if their partner has suddenly been admitted to hospital. • Assisting someone who has fallen, but no significant injury has occurred.
These questions may be useful when considering using the services of a home care (domiciliary) provider to help you build up a picture of how your care needs will be met in your own home.
Agencies What experience does the agency have in your particular field of need? Can they supply references for you to check? Home care agencies providing personal care, must be registered and inspected by the Care Quality Commission. Ask to see a copy of their registration certificate. How long has the agency been operating?
There is no charge for this service, and a referral can be made if longer term care is needed. Any resident who finds themselves in need in Norfolk is eligible for help.
How many carers would the agency assign to care for you and would you see the same one every day? If not, how does the staff rota operate and what happens if your carer goes on holiday or is sick, will you be notified in advance that a different carer will be attending?
How can I contact Norfolk Swift Response?
How can you contact the agency in an emergency or outside office hours?
Residents in the Great Yarmouth area can call 01493 843097 (24 hours). All other residents in Norfolk should call 01603 442008 between 8am and 10pm, and 01603 481409 between 10am and 8pm. You can expect the Swift Response team to: • Show their identity cards when they visit somebody for the first time, and always be dressed in a Norfolk Swift Response uniform. • Behave professionally and politely at all time, respecting customers’ privacy and dignity. • Not smoke in a person’s home. We also ask that you do not smoke while a member of Norfolk Swift Response is in your home. • With the person’s agreement, make ongoing referrals for them.
How hard or easy would it be to make a complaint and how are things then put right? If this is a private contract ask for a copy of the agency’s contract terms and read these carefully with someone you trust. Get as much help as you can going over it and ask any questions you may have before signing anything. The Citizens Advice Bureau or Age Concern (now called Age UK in some areas) could help with this.
Carers You should expect your personal preferences, dignity and privacy to be respected. Discuss this with the agency and ask how the most suitable carers for your particular care are chosen. Can you talk to them before deciding? Carers should be fully trained or be in ongoing training. Ask the agency about their policies on this. You will have a care plan drawn up by the agency which the carers will work to. Ask how often this plan will be reviewed by the agency. Carers must be checked with the Criminal Records Bureau (CRB) and have a criminal records disclosure – make sure this is the case.

SAXON HOUSE, LODDON Saxon House is a Very Sheltered Housing Scheme, providing Independent living with a care element. The scheme is located in Loddon, which is a small village off the A146 in Norfolk. The rented accommodation is mainly single bedroom flats with a few double flats and all are en-suite. We have a team of care staff who provide 24-hour personal care and support on an individually assessed basis. On the premises there is a catering service which provides fresh cooked lunches and light teas six days per week. The building has a large patio and a well kept garden area. Saxon House is ideal to enable older people to stay in the area and to retain independence, choice and control over their lifestyle in a safe relaxed environment.

Keeping independent Norfolk Home Shield Norfolk Home Shield is a cross-agency referral service for vulnerable and older people and their carers. The service is supported and coordinated through a group of partner agencies to help with:
• Ensuring you are able to secure your house against unwelcome visitors.
• Health and well-being.
• Ensuring you have a working smoke alarm.
• Helping you live independently in your own home.
• Checking for any fire or trip hazards in your home.
• Identifying problems before they become acute. • Referring people with unmet needs to the right support services. • Checking that you are receiving the income and benefits to which you are entitled. • Ensuring you get regular hot meals and help with grocery shopping. • Advising on how to keep your house warm and have access to hot water and toilet facilities. • Helping you to have regular contact with other people.
• Helping you protect yourself from scams or other forms of ‘rogue’ trading.
If a front line officer, in the course of their ordinary business, finds that you have a need which he/ she cannot address but which could be fulfilled by another organisation, the officer will refer it, via Home Shield. An officer will briefly explain Home Shield to you and complete the referral form. You will then be requested to sign the form in order to show that you have agreed to your details being referred to partner agencies.
Day opportunities Day services are provided in a variety of locations, ranging from care homes and care homes with nursing to community and specialist centres. The services aim to help people live as independently as possible. If you have a learning or physical disability or if you are an older person, Community Services will help to support you in finding activities which will help promote independence. To be eligible for day services Adult Care will have to assess your needs. This means we will have a discussion with you to agree with you the best way of organising support. We will consider your needs and decide together what services might be
suitable for you. If we conclude that you do have social care needs, you will be allocated a sum of money - known as a Personal Budget – to spend on getting your needs met. You will be offered the chance to take this as a cash sum – known as a Direct Payment – or you can ask Adult Care to arrange the services on your behalf, or a mixture of both (see page 38 for further information). Whilst traditional day centres provide an excellent service, you can use your Personal Budget to pay for any arrangement that meets your social care need for a day activity.
Norfolk Trusted Trader Scheme This is a free, Norfolk-wide service run by Norfolk County Council. It is aimed at helping older and vulnerable people remain safe and independent in their own homes. It is a directory of reliable providers – businesses and voluntary agencies – who meet the scheme’s standards. They provide a wide range of services such as domestic help, window cleaning, help with shopping, bathing and gardening,

handyperson and painting and decorating. All the traders in the directory have gone through a process of accreditation requiring background checks to be carried out by Trading Standards, references must be provided and a minimum of £2 million of public liability insurance must be in place. They are also required to sign up to the core standards of the Norfolk Trusted Trader

Assistive technology and community alarms Assistive technology or Telecare services help vulnerable people, particularly older people, to live in their own homes with greater safety and independence. They use a range of technologies that raise alarms to get help for someone 24 hours a day and so provide help, security and peace of mind. Special equipment can be linked to your telephone system so you can get help quickly if you need it, even if you are unable to speak. For some of the services you do not even have to press a button to call for help as sensors will automatically detect things such as floods, carbon monoxide escapes or if you fall. There are different types of assistive technology
depending on your level of need, from simple battery operated items for use within the home to more complex telecare equipment which links to one of the community alarm service centres. Services can be provided if we have assessed you as eligible for help or equipment can also be purchased privately. There are also four ‘Smart’ homes in Norfolk where you can have the equipment demonstrated. Telehealth If you suffer from a long term condition, such as heart or lung disease or diabetes, you may be suitable for telehealth monitoring. Telehealth enables regular assessment of your condition without the need for you to visit the GP surgery, or clinic. A small touch-screen device is installed in your home so your GP, community or specialist nurse, can check readings of your blood pressure, pulse, weight, temperature, the amount of oxygen in your blood and even your blood sugar levels. These results are transmitted to a secure computer where they can be accessed at the clinic or surgery. Using the information collected through the
telehealth monitor, it is possible to detect changes in your condition. These help your healthcare professional respond earlier and make changes to your treatment or medication which may reduce or prevent symptoms from getting worse or lead to a hospital admission. By using telehealth, you and your carer can become more actively involved in your care, and you should experience some improvements in your quality of life and overall symptom management. It could also reduce the number of visits you have to make to the GP surgery or outpatient clinic, and help you to maintain your independence. Equipment to aid daily living Before beginning to look around for equipment, it is helpful to be clear about exactly which areas of daily living are becoming difficult and then for each area of difficulty try to pinpoint exactly why things are difficult. This may seem obvious; however, not all difficulties are best helped by equipment and certain items of equipment can be more helpful in certain situations than in others. Taking stock of the situation before you begin to buy any equipment should help you when you talk to a supplier.

Carers and families If you look after, help or support a person who suffers from a long-term illness, is frail or disabled and do not receive payment to do this, then you are a carer. The person you care for might be a child with a disability, an adult member of your family, or a friend. You do not need to be living with the person in order to care for them. Carer’s Assessments A Carer’s Assessment is free and is a supportive measure for you in your role as a carer. It is a chance for you to talk about your needs so you can get the right sort of help to assist you in supporting the person you care for. You may not feel that you need help now but would like to plan for the future. A Carer’s Assessment gives you the opportunity to: • Speak confidentially to a member of Adult Care staff • Describe the help you are giving • Talk about how caring affects your life, for
example, your health, your relationships with others, finances, employment, training, education and leisure time. • Discuss any difficulties you have • Talk about the needs of the person you care for and the services they have or you feel they should have; for example, equipment and adaptations, home care, day services. • Be given information, advice and other help, for example, what would happen in an emergency, how to get a break and other services such as Direct Payments.

Short breaks for carers If you are a carer it is natural that from time to time you will need a break from your daily duties. ‘Short Breaks at Short Notice’ offers carers a break from anything from two hours to 48 hours, enabling them to take important time out. A trained and experienced care worker can take over a carer’s responsibilities for a period of time.
It is understandable that a carer will be worried about leaving the person they care for, so we can arrange for your care worker to come along before the break to get to know you and the person you care for. We ask for a minimum of two days’ notice to arrange a short break, with the scheme free to all carers in Norfolk.
Young carers Young carers are children and young people up to the age of 18 whose lives are affected by caring. They might be caring for a parent, sibling or other family member who may have a long term illness or disability, mental health problems, learning disability or misuse alcohol and/or drugs. Many young carers are not recognised, or are ‘hidden’. Children and young people will almost always choose and wish to take a role in caring for their loved-ones, but it is important that a child or young person’s caring responsibilities do not become excessive or inappropriate. Understandably, carrying out caring responsibilities can result in young carers becoming physically or emotionally tired. This can have a knock-on effect on their school or college work, their relationships with friends or their ability to engage in hobbies and leisure activities. Young carers can feel isolated from other children and find it difficult to balance their own
needs and those of the person they’re caring for. A range of services and support exists to help families in Norfolk. Parents are encouraged to share information about the caring responsibilities carried out by children and young people. This will enable workers to look at identifying additional support for the adult which might reduce the need for high levels of caring by their children, or to provide additional care to ensure that the needs of the young carers can be met. Leaving hospital First steps When leaving hospital, you may need the help of Adult Care. With your consent, hospital staff will contact the Hospital Social Work Department, a team of Nurses, Doctors, therapists and Social Care Professionals. The members of the team will discuss your needs and options with you, involving your family, carer and representative if you wish. The assessment will consider how best to help you: • Return home to live independently, with support services such as home care or specialist equipment if necessary
move to alternative accommodation while you wait – usually a nursing or residential home. If it is the case that you need time to adjust and recover after an illness, a temporary stay in accommodation with 24 hour care can be arranged until you are able to return home or you make a decision on what care you need. Any charges for care will be explained to you by your Social Worker. However, you will not be charged more than you would be paying for the service you are waiting for.
• Return home after short-term rehabilitation or care at another hospital or appropriate accommodation, which we will try to make as close to your home as possible.
Norfolk First Support This is a reablement service which aims to maintain or increase your independence and the service may last up to six weeks. Contact 0344 800 8020 for more information.
We aim for your stay in hospital to only be as long as is clinically necessary. A plan will be put in place as soon as possible to avoid you staying in once you are well enough to return home or to other accommodation. It may take some time for your care, equipment or adaptation of your home to be arranged. It will not always be possible to wait in hospital until these services have been arranged, so you may have to
NHS Continuing HealthCare NHS Continuing healthcare is provided solely by the NHS if you have extensive healthcare needs. This is free of charge, wherever it is delivered and can be provided in any setting. This could mean in your own home or a care home. Moving to a care or nursing home While we always strive to help people return home, this is not always possible. If long term decisions have to be made about moving to a care home, we will ensure that we talk to you, your family, and your Carer or representative. We will make sure that: • Clear advice and guidance is offered to help you make the right decision about the home of your choice. • That if there is no vacancy in your preferred
home, we will find a suitable home for you until a place becomes available (an extended stay in hospital is not possible as other patients need urgent medical care). • If you are placed in an interim home, your care needs will be reviewed and you will be given more time to consider your future. • A social worker will help you in moving to your first choice home as soon as possible.
Moving from an Acute Hospital to a Community Hospital When leaving an Acute Hospital – such as Queen Elizabeth, James Paget or Norfolk and Norwich University Hospitals – you may need a period of rehabilitation in a community hospital or NHS funded nursing home bed.
In this case, you will be given the first available bed in an appropriate unit, although this will not necessarily be closest to your home. When you are fit to leave, we will help you return home with support to live safely and independently.

Elderly patients can speak to Age UK Norfolk, which provides free, confidential information, advice and advocacy on a number of issues. Housing with care Local schemes Housing with care is designed for dependent people who can no longer live at home. They are welcoming environments with a series of flats within a bigger complex with a range of communal facilities. People must have significant personal care and housing needs to live in housing with care. The schemes are all run in partnerships with district councils and housing associations. Some have units for older people with dementia. These are selfcontained with their own secure gardens. Each housing with care complex has around 30 to 40 flats, all under one roof. Some are designed for couples. Every flat has a living room, kitchen, bedroom, level access shower, toilet and wash basin. Each scheme is different, but they all have lots of communal space like lounges, a restaurant, shop, hairdresser, library and fully accessible gardens. Tenants are encouraged to get involved in activities and all aspects of life within their housing with care scheme, as are their families. But people can choose how much time they want to spend in their flat and which activities they join in. Tenants have access to health services such as a GP, district nurse and chiropodist. Some schemes have day centres. Schemes have restaurants which can provide three
meals a day but the emphasis is firmly on choice. People decide what they want to eat and where they have their meals – in their flat, a friend’s flat or in the restaurant. There is always a responsible member of staff on duty 24 hours a day, along with fully trained care and domestic staff. They help tenants with every aspect of daily living as and when they are needed. This could mean cleaning the flat, shopping, socialising, helping people get up and go to bed, administering medication and having a bath. Every tenant has a care plan, written with the tenant, and if they wish, their family or other relevant person. The care plan explains what the tenant can do and where they need help and support. Sheltered housing Sheltered housing is adapted accommodation for older people which is generally linked into an alarm system. Low level help and support is provided. Units are generally unfurnished and there are over 6000 sheltered housing units to rent throughout Norfolk.

Housing and support for younger adults There is a range of care homes catering for younger adults with learning difficulties, physical or sensory impairments or mental health issues. There is also a range of housing options you could explore (see below). If you are thinking about a care home placement, seek advice from social and health care professionals about your needs. Does the home provide opportunities to learn and maintain skills, and opportunities to move on into living in the community? What are the home’s links with specialist staff in the community services? Are you happy living at home? Would you like a little more independence with help when you need it? What are your housing options? You could: • Continue living with your family – If your home is specially adapted and you receive the practical and emotional support needed, you may prefer to stay at home. It’s also convenient if your college or job is nearby. • Get support to improve or adapt your home to better suit your needs – Home Improvement Agencies assist older people and other people with disabilities to live as independently as possible with support to make the home environment warm, safe
and secure. Services may include advice, assistance with grants and help with the process of adapting, repairing and improving your home. There are four services covering Norfolk. • Move into supported living accommodation – This offers a level of independence and the chance to meet other people in similar situations. Help from a visiting care assistant is available and usually nursing care is provided nearby. • Rent a place – You can apply for council housing which is usually cheaper than renting privately. There could be a waiting list and you must fully explain your housing needs. Local housing associations may also be able to help. Ask your council housing department for a list. If you want to rent privately, look in your local newspaper for ‘To Let’ advertisements. Housing Benefit, if you’re entitled to it, might reduce your rent. Direct Payments can also help, ask Community Services for details and see page 40 for more information. • Buy your own home – Can you afford it? Are you able to live on your own? You could think about ‘part buying’ and ‘part renting’ a place from a housing association.

Housing and support for younger adults continued • Move into a care home – If you want to leave home but are not ready to live alone, care homes can meet your personal needs and offer appropriate support. Community Services will contribute if you are eligible and if they agree the care home meets your needs. If your chosen home costs more than usual, you may get some help but you must pay the rest. When living independently you may require some adaptations around the house. You can choose housing that’s specially adapted. There are organisations that specialise in providing this sort of accommodation including Livability (formerly John
Grooms) and the Accessible Housing Register. New homes that have been built since 1999 should be fully accessible to disabled people. However, if you choose an older home, there are a number of adaptations that could make it easier for you to use. These include wider doorways; a downstairs toilet; bathroom equipment including handrails, a walkin bath, electronic bath seat; kitchen adaptations including lower working surfaces; lower level light switches; easy-to-use controls for heating; a stairlift; security intercom at the front door and ramp access to the garden. You should also think about the general access to your home.
Care homes Most people can be helped to return home but for some this may not be possible. We understand that a hospital setting is not the best place to make longterm decisions about moving to a care home. But if this needs to be considered, we will talk to you, your family, carer or representative and will make sure that: • Advice and guidance is offered to help you decide the home of your choice. • If there is no immediate vacancy in your first choice
of home, we will find another suitable home as an interim arrangement. It is not possible to wait in hospital as there are other people waiting for urgent medical care in hospital. • If you are in an interim home, your care needs will be reviewed and you can have more time to consider your future. • A social worker will be available to help you move to your first choice of home as soon as possible.
Residential care There are two types of residential home: Care homes (personal care only) If you are finding it increasingly difficult to manage at home or in the community and would like greater security and care, a care home offering only personal care may be the best option. Personal care includes bathing, feeding, dressing and help with moving but if your capital/savings are more than £23,250 you will need to pay the home directly. Care homes with nursing If you think you have particular health needs which can only be met by a nurse, then you may need nursing care in a home. You will need to be visited by a social worker or a registered nurse to determine what care you will need. This visit might be in your own home, or in hospital if you’ve been ill, or in a care home. You will be fully involved in planning your care needs. If a care home providing nursing care is the best solution for you, your social worker will give you

information to help you find a home which meets your care requirements. If appropriate, a specially trained nurse will assess your nursing needs to determine what level of nursing care the NHS will fund. The cost of the nursing care part of your fees is paid by the NHS to the home. It is important that you get your nursing determination done before you go into the care home. Although the NHS may fund the nursing element, you will still need to make a contribution towards the other care costs. If your savings and capital are more than £23,250, you will need to pay the home directly for the full cost of the care less the part funded by the NHS. Specialist care Learning disability The Norfolk Learning Disability Service is a partnership between Norfolk Community Services and NHS Norfolk and Waveney. We provide help and advice for people with a diagnosed learning disability or who have a learning difficulty in Norfolk. Our philosophy is based around the belief that people with a learning disability should have the same opportunities as anyone else to lead full and normal lives. We assess the social care needs of individuals aged 18 years (to death) in line with Fair Access to Care criteria. We also provide information about the support, facilities and opportunities they can use in the community in line with Valuing People Now. Access to health aspects of the service should be the same as other NHS services. Valuing People Now is a three-year strategy, published in 2009, for people with learning disabilities which aims to give individuals ‘more choice, more control and more independent living’.
The strategy aims to: • ensure people with learning disabilities receive the healthcare they need and the support they want to live healthy lives • support more people, including those with more complex needs, into paid work • ensure people have the choice to form relationships, become parents and continue to be parents • give people opportunities to study and enjoy leisure and social activities. A significant change affecting people with learning disabilities is the growth of Self-Directed Support and the use of Personal Budgets. This puts them and their carers at the centre of how they choose their support. Personal Budgets are explained further on page 37.
Dementia care The initial point of contact to access services is either your GP (for health care solutions) or by calling Norfolk Care Connect on 0344 800 8020 (for social care services). However, there are also many voluntary sector organisations providing information and support for concerned about dementia, and those diagnosed with dementia and their carers. More information is available from www.heron.nhs.uk. Your GP, as well as discussing your concerns, will be able to carry out a series of tests which will establish the nature of your condition and advise whether
treatment is possible. Some dementia-like symptoms (cognitive impairment) can be caused by a range of emotional, physiological and physical ailments. It is important that reversible dementia-like symptoms are investigated before seeking a diagnosis of dementia. To obtain a diagnosis of dementia, your GP may refer you to a specialist, such as a Consultant in Old Age Psychiatry, a Community Psychiatric Nurse or an occupational therapist with a special interest in dementia care.
Dementia care: support in the community As it can be best for the person with dementia to stay in their own home for as long as possible, a range of community care services is available to facilitate this. Adult Care for example, can provide access to: home care (discussed on page 8); meals in the home; sitting services (which provide a break for any carer); advice on equipment and adaptations; day care centres and respite care.

These services can be provided directly by registered providers to those who do not require financial help; however Adult Care will still be willing to offer an assessment of need. You may have a choice of which agency you receive your services from whether or not Adult Care assist with the funding of your service.

Dementia care: care homes If the time comes when remaining at home is no longer viable, the benefits of residential or nursing care need considering. Questions to ask of any home registered for someone with dementia would include: who is the senior member of staff who can act as a single point of contact for people to approach for information and advice? What training programmes do staff participate in? Does the home offer a personcentered approach to their care planning? Is the home designed with the needs of its residents in mind, especially safety and accessibility? Is there plenty of natural light? Is moving around helped by easily-understood signage? How is technology employed to help alleviate falls and help
orientation? Further questions are listed on page 36. Differentiating one home from another can be difficult but the quality and quantity of staff training is especially important, along with the turnover of staff as continuity of relationships in this type of home is critical. The use of medication is a highly emotive subject and any psychotropic medication must be very carefully prescribed. A fine balance exists between ‘acting in the best interests of the individual’ by prescribing medicine and the benefits of ‘normalisation’ where time and energy is spent maintaining familiar, comfortable surroundings which help reduce stress and possibly avoid the need for certain medication.

Primary Care: mental health Mental health problems affect one in four of the UK population at some point in their lives. There are many activities and interventions that can help with these problems which can be provided in a community setting in addition to pharmaceutical interventions. Services such as Improving Access to Psychological Therapies (IAPT) is a community based Primary Care Mental Health Service that is designed to improve the mental wellbeing of the population of Norfolk. To access this service you will need to initially speak to your GP who will then refer into the service. Since 2011 the service has included a self-referral function. Further information can be found at: www.norfolk.nhs.uk/yourhealth. Secondary Care: specialist mental health Serious mental illness affects one in six of the UK population at any one time. Your GP should be your first point of contact if you have mental health problems. He or she will consider whether they can provide the help and treatment you need from staff who work in the primary care setting, or whether you should be referred to the secondary mental health services. The local NHS mental health service provider is the Norfolk and Suffolk Foundation Trust. They provide an integrated health and social care service. Your needs will be assessed under the Care Programme Approach and a care plan will be devised to meet your needs. This will include the support you need to live your life in the community such as, personal care and one to one support, access to work and community services. Things to look for in care homes specialising in dementia. Please use in conjunction with the care homes checklist on page 33. Residents The best indication of a good home is that the residents appear happy and responsive. Are there rummage boxes around the home to stimulate residents? Access If the person with dementia needs or is likely to need equipment or adaptations you may want to check: There is adequate signage and cues for different parts of the home such as dining room and bathroom. The home’s policy about locking external doors. Bedrooms You may want to find out whether the person with dementia can have a single room and whether: Residents are encouraged to bring in some of their own furniture and possessions to increase familiarity. Activities Residents should be stimulated without feeling stressed. Are reminiscence activities available? Are residents able to compile memory boxes? Do staff compile life story books including photographs and mementoes? Meals Do meal times appear to be a relaxed and enjoyable experience? Do staff approach support at meal times in a flexible manner? Is there a variety of food available throughout the day (eg finger food)? Staff It is important to note whether staff seem friendly and caring towards residents and whether they treat residents with respect. Do they have any training and experience in dementia care? Will the person with dementia have a member of staff particularly responsible for their care? Is there a member of staff you can talk to about your own worries about the person with dementia? Manager/head of home A manager who is caring as well as efficient can make all the difference to a home. Does the manager have a knowledge of dementia and can they deal with difficulties that may arise in an understanding way? Is there a full assessment at home before a resident is admitted?

Paying for care Personalisation An enormous change is taking place in the way social and health care services are paid for throughout the country: ‘Personalisation’. The Government has given an unequivocal commitment to making this ‘the cornerstone of modernised public services’. In future everyone
who receives social or health care support, regardless of their level of need, in any setting, whether from statutory services, the third and community or private sector, or by funding it themselves, will have choice and control over how that support is delivered.
Self-Directed Support Self-Directed Support is a process by which you are offered choice and control over the support you need to go about your daily life. Government, councils and health departments across the country have worked with people and their carers to create this transformation. This is based on the understanding that people and their carers should have choice and control in their lives. They should be directly involved in thinking about how the money being spent on social and health care could be used in better ways. This will be partly about meeting basic support needs like help in the home. It could also be about people and their carers thinking about how some of the money could be spent in a way that recognises their individual circumstances (eg in terms of family relationships and interests). This approach also takes account of the support needs of those people who provide assistance to disabled people as unpaid carers. Everyone who is eligible for social care will have a Personal Budget. There is no obligation for people to manage their budgets themselves, and a range of options is available to help plan and arrange suitable care for each individual. In some cases individuals may continue to require Community Services to arrange services to meet their needs or a keyworker to arrange services to meet their health needs. Your social worker will be able to tell you what this will mean for you personally, which will depend on where you live and when you need to look at your social care needs. How does it work? First, your needs are assessed. This establishes your Personal Budget. You then draw up a support plan outlining the care that will meet your assessed and eligible social care needs. You can ask anyone you
feel comfortable with to help with this, for example a family member, a friend, your key professional, or support broker. The support plan encourages you to think about what would make a difference in your life and what you would like to achieve. It enables you to describe the changes or improvements that could help address the difficulties you face or describe what you need to stay healthy, safe and well. It will say who will provide the support you need, how much it will cost, and how it will be arranged and managed. Finally it sets out the practical steps that need to be taken to make it happen. What’s different? The important difference is that the Personal Budget can be used for care solutions you would benefit from which may currently not be within reach. It offers more flexibility than the current care system allows. As long as the care solutions meet your eligible social care needs, the use of the Personal Budget is legal and would not bring the council into disrepute, is not considered day to day living (eg food, rent) and is not provided by a health professional, you should be free to spend your Personal Budget in ways that makes best sense to you. Each support plan will be carefully considered and approved to ensure it meets your needs.

Direct Payments Direct Payments can be made to: • People aged 16 or over who are in need of community care services. • People with parental responsibility for disabled children. Carers aged 16 or over may have a Direct Payment for a specific carer’s service following a carer’s assessment even if the person for whom they care declines an assessment or service. What is the point of Direct Payments? The aim of a Direct Payment is to give more flexibility in how services are provided. People have greater choice and control over their lives and are able to make their own decisions about how their care is delivered. Following a Community Care Assessment, or reassessment, you can now choose whether to have services provided directly by Adult Care or have Direct Payments. What am I allowed to spend the money on? You can use the money to employ a personal assistant or you can use an agency to provide staff for you. Or you can ‘mix and match’ and have some of your support provided by Community Services
and take some as a Direct Payment. You must use the money as agreed in your support plan to meet your social care assessed needs. What else do I need to know? You will need to open a separate bank account to have the money paid into and send Norfolk County Council a monthly bank statement from this account. The Direct Payment Support Agency will give you all the help you need to operate Direct Payments. Direct Payments are not a social security benefit, will not affect any means testing benefits you are entitled to and are not taxable. What happens if my circumstances change? Your social worker should review your assessment at least annually. This will provide you with a regular check on how well your arrangements are meeting you needs. If at any time, you feel your circumstances have changed or that costs have increased, you should contact your social worker to request another assessment. If you wish to apply for this service or ask for more information, contact Community Services.
Self-funding advice Paying the full cost of care yourself – being a ‘selffunder’ Even if you know you will have to pay for it yourself, if you need care in your own home or are considering moving into residential care, you still qualify for a Social Care Assessment by Adult Care. The assessment will identify your needs including those that may be eligible for support from the local authority. Also, if you expect your capital to fall below £23,250 as a result of paying for care, Norfolk County Council may then help you with the cost. (See page 40.) If you are moving into residential care, you may be entitled to some of the following financial assistance and support: Twelve-week property disregard This applies if: • your former home is included in your financial assessment; • your other capital is less than £23,250; and • your income is not enough to meet your care home fees.

Adult Care will help with the cost during the first twelve weeks of permanent residential care, provided their assessment has shown that this is the kind of care you need. This is called the ‘property disregard’ period. Deferred payment agreements After the twelve-week property disregard period, Norfolk County Council may be able to help you whilst your property is being sold. They will charge any financial help they give against the value of your home. This means they will take back the amount they have paid once your property has been sold. Until your property is sold, you will be expected to make a part-payment every four weeks based on your weekly income. Once the property is sold a review will take place to determine whether or not it is appropriate for you to make private arrangements to pay the home direct. If this is the case, you will be advised of the date you should start to pay the home from. It may affect your entitlement to Pension Credit

Self-funding advice continued if your property is not seen to be on the market and becomes treated as capital by the Department for Work and Pensions (DWP). Attendance Allowance This is a non-means-tested, non-taxable benefit from the DWP. It’s paid at the lower rate of £51.85 a week if you need care by day or night, and at the higher rate of £77.45 a week if you need care during the day and night. Everyone who needs care can, and should, claim Attendance Allowance. If you are paying the full cost of residential care, with or without nursing care, you will be entitled to the higher rate. NHS Nursing Care Contribution Whether you are a temporary or permanent resident, if you live in a care home that provides nursing care you may be entitled, following an assessment, to NHS Nursing Care Contribution towards the cost. It is not means-tested and is currently £108.70 a week for the standard rate and is paid directly to the home. Third party payments for care home fees Some care homes and care homes with nursing charge fees that are higher than the maximum amount Adult Care can contribute. If Norfolk County Council are contributing towards your care home fees and you choose to move into a home which charges a higher fee, the difference between the two amounts must be paid for by a third party, usually a relative or a charity. Third party payments are sometimes referred to as ‘top-up’ payments. Please note, this amount will be charged in addition to your assessed contribution. Before you sign any contract the home should give you written details of all the charges it intends to make in its ‘Statement of Purpose’ document. If there is anything that is not clear or which you do not understand, you should ask for advice. As the name suggests, this payment must be made by someone other than yourself or Adult Care. The general rule is that you cannot use your own money to fund a third party payment. However in certain specific circumstances you may make a third party payment from your own funds. These are: • When you are eligible for the 12 week property disregard (see page 38) or • Where Adult Care has agreed to a deferred payment until your home is sold.
We recommend that you seek independent financial and legal advice if you are considering these options. If you need further advice about how a top up to your care home fees might be arranged please discuss this with your care manager. If you move to a care home where a third party payment is required, the person who will make the payment on your behalf must sign a contract with Adult Care, before the contract with the home is signed. They must not pay the home directly. In signing the contract with Adult Care, they must also confirm that they have the financial means to make the third party payments (including any future increases) for the whole time you will live at the care home. If they are unable to maintain the payments and there is nobody else able to meet this cost, you may have to move to a cheaper home within the local authority’s funding levels. If a change to your arrangements is made at your request or with your agreement, for example you move to a superior room, then a third party payment can become due. Running out of money whilst in care If your capital is likely to reduce to £23,250 as a result of paying for care, you must tell Adult Care well in advance that this is going to happen. They will then be able to help with your care fees (provided your care needs assessment has shown that you need residential care). They will carry out a financial assessment to confirm the date from which your funds fell below £23,250, and the amount they, and you, will pay towards your care. If you cannot find someone to help you meet the extra cost, you may have to move to a home within Adult Care’s funding levels. Understanding your rights before moving into care is essential. There are a number of financial products and specialist companies who may be able to help. It is important to seek advice about the various options you may have before committing yourself. What will you have to pay for your care? Do you have more than £23,250 in capital and savings? What is ‘capital’? The value of your home is included in the assessment of your capital unless one of the following still lives there: • Your partner (that is, your husband, wife or civil partner, or someone you live with as if they were your husband, wife or civil partner) or; • A relative who is over 60 or disabled or; • A child under 16 who you, or a former partner, are responsible for. If the answer is no: Adult Care will contribute financially towards your care costs after they have carried out an assessment of your care needs. How much? You will need to ask Adult Care for a Social Care Assessment. This will define your care needs and how they will be best met. Any care homes you consider must be able to meet these needs. Adult Care will also make an assessment of your financial situation. This determines how much they will pay and how much you will have to pay. Your choice of care home will be limited to those that accept Adult Care’s funding level. If you want to choose a more expensive home you would have to arrange for a third party - such as a family member or charity - to ‘top-up’ the difference. Please refer to the section ‘Running out of money whilst in care’ above. If you have capital between £14,250 and £23,250 you will pay £1 a week for every £250 you have above £14,250. This will be included in your financial assessment and will be in addition to any income you are receiving eg State Benefits, Occupational Pensions, etc. If the answer is yes: Ask Adult Care for an assessment – telephone 0344 800 8014. Any care homes you consider must be able to meet your assessed care needs. If, apart from your property, your savings are less than £23,250, Adult Care can help with your care costs

for the first twelve weeks. After this time you may be eligible to enter into a Deferred Payments Agreement until your property is sold. Please refer to the information above about Deferred Payment Agreements or for further information please refer to the booklet ’Thinking about Residential and Nursing Home Care Money Matters’. WHATEVER YOUR CIRCUMSTANCES: Remember: • It is just your own financial circumstances that are assessed, not your partner’s. • Your assessment will be made up of two elements; a care needs assessment and a financial one. • A nursing home will generally be more expensive than a residential home. Consider claiming: • Income support (if you are currently receiving Severe Disablement Allowance (SDA) and/or Incapacity Benefit) • Employment and Support Allowance (ESA) • Pension Credit • Savings Credit (if you are over 65) Definitely claim: • Attendance Allowance, worth either £51.85 or £77.45 a week depending on your care needs (as long as your capital is more than £23,250 and you are paying the full cost of your care). • Disability Living Allowance (Care), worth £20.55 or £51.85 or £77.45. This is paid if you are aged under 65 and your capital is more than £23,250 and you are paying the full cost of your care. • Disability Living Allowance (Mobility), worth £20.55 or £54.05 – if you are aged under 65 and regardless of your level of capital. Please note this allowance is totally disregarded in your financial assessment. Moving into a nursing home? • You may be eligible for the NHS Nursing Care Contribution of £108.70 a week depending on your assessed care needs.

What will you have to pay for your care? continued • If eligible for Continuing Healthcare and your primary health needs require nursing care then you will be offered a nursing home place and the fees will be met in full by Continuing Health Care. Care Funding Information for Older People In 2012 Norfolk County Council launched a new Care Funding Information Service. We are working with CareAware, a non-profit making public information and advisory service specialising in issues relating to planning long term care funding for older people. CareAware’s service has no cost, charge or obligation for the customer or their family. In instances where people need a greater level of financial planning support, CareAware will ask the customer if they wish to be directed to Care Asset Management (CAM), a provider of specialist
financial planning products. In such instances there may be some charges for this service depending on the customer’s requirements. Essential information Inspection and registration of care services The Care Quality Commission (CQC) registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others. CQC will hold and publish up to date information about the compliance of adult social care providers with essential standards of safety and quality. Following an inspection, each care home and home care agency is given a report of how it rates against national essential standards of quality and safety. Each service’s report can be seen on the CQC website: www.cqc.org.uk. The focus of an inspection is on the standards of care that people who use the service receive and whether they are happy with their care. Virtually all inspections are unannounced.

Comments, compliments and complaints All organisations need to know how they are performing. They are happy to receive your feedback on their service whether they are compliments or complaints. Feel free to tell them what you think and your comments can be used constructively to improve the service. If you do need to make a complaint you should feel able to complain about any aspect of your care that affects your happiness or comfort. This could be about the way you are treated by a staff member or the quality of the food you are served. You can also make comments and suggestions about possible improvements to your surroundings and the services provided. Making a complaint should not be difficult for you. Providers are required under the essential standards of quality and safety to have a simple and easy to use complaints procedure that they will be happy to give you. If you are concerned about the care that you, a friend or a relative is receiving, it makes sense to speak to the manager before you take any further action. The problem may be resolved quite easily once they are made aware of it.

End of Life Care Norfolk’s joint End of Life Care Strategy, which is based on the National End of Life Care Strategy, is a five year plan that aims to improve care for someone nearing the end of their life. The overall vision is for people to be able to have: ‘The care they want, where they want and when they want, during their life and after death for themselves and their family/carer(s), delivered by competent, confident professionals”. Services will be changed and developed to ensure there is support for carers, that end of life care needs are identified and assessed, that everyone has the opportunity to complete advanced care plans, and
those near the end of their life are able to choose where they would like to be cared for. To help us make these improvements, we work with other organisations and use the following guidance: Gold Standards Framework (GSF) This can be used in various settings, for example hospitals, primary care and care homes, to improve the co-ordination and communication between different organisations involved in providing care for someone near the end of their life. Liverpool Care Pathway (LCP) This is best practice guidance to medical staff on how to ensure quality of care in the last few hours/days of someone’s life. This can be used in hospitals, care homes, hospices or individuals own homes. Preferred Priorities for Care (PPC) This document is designed to help people prepare for the future and gives them an opportunity to think about, talk about and write down their preferences and priorities for care at the end of life.

End of Life Care continued A programme of training for care homes is taking place in Norfolk, so staff can improve their skills and knowledge in how to deliver high quality end of life care. This should also help to avoid unnecessary admissions to hospital. Some people who are nearing the end of their life
will be eligible for continuing health care and there is a fast-track assessment process to provide a quick response to assessing needs. The initial point of contact for services and support is either through your GP or by calling Norfolk Care Connect on 0344 800 8014.
Out of county care options Did you know you can choose a care home outside your home county? You may want to be closer to friends, family members or you may want to relocate to another part of the country. If your care home place is state-funded, your home county local authority is still responsible for your care fees subject to your financial assessment, even if you choose a care home in another region. The home you choose must be suitable for your assessed needs and comply with the terms and conditions set by the authority. The fees that your local authority will pay may vary. They may offer you the fees you would receive if you remained within the county, or they may offer you the fees
that the local authority would pay in your chosen region. You must seek further advice before making your decision.

Safeguarding adults Most Adults in Norfolk with physical or mental illness, learning disability or other special needs, live their lives comfortably and safely. They do this either independently or with assistance from professional carers, relatives, family and friends or volunteers. Some people may find it more difficult to protect themselves from harm (abuse). The abuser is often someone they know and it can happen anywhere. There are many types of abuse, including physical, sexual or emotional abuse; financial or domestic abuse or neglect or acts of omission. You can find out more in the Safeguarding Adults leaflet. Community Services Adult Care in Norfolk has a
lead role in protecting vulnerable adults from abuse and work closely with the Police and other partners to identify and investigate all allegations of abuse. If you feel you are being abused, or know a person you believe is at risk then it is important to tell someone. You can contact Adult Community Services on: 0344 800 8020. They will take your concerns seriously and will work with you and others to make sure everyone is safe.
