The Keep Safe Scheme A card system designed to make the holder more aware of their personal safety, to encourage reporting of crime and to support them to seek help if needed. Brain injury Brain injury (BI) is caused by a trauma to the head (head injury) or as a result of a physical illness. There are many possible causes, including road traffic accidents, assaults, falls and accidents at home or at

work, as well as brain haemorrhage and cardiac arrest (causing lack of oxygen to the brain). Many brain injuries are minor, causing temporary

• Active/Complex Rehabilitation

Individuals may present with a range of physical, cognitive, psychological, behavioural or communicative support needs

• Community Rehabilitation

For individuals who have received a period of active rehabilitation and demonstrated the ability to gain further independence and functionality in a community setting.

• Progressive Neurological Conditions

Expert disability management for individuals with neurological conditions with a progressive deterioration of functioning, which include amongst others, Huntington’s Disease, dementia, multiple sclerosis and Parkinson’s Disease

• High Dependency & Specialist Nursing Care

For individuals who require more intensive observation and intervention or specialist equipment resources including the management of tracheostomy care, percutaneous endoscopic gastrostomy (PEG) or dysphagia feeding regimes.

The Oakleaf Group support men with complex healthcare needs, including acquired brain injury (stroke, trauma, tumour, encephalitis, aneurysm, haemorrhage). Services are delivered at the main site in Hartwell, Northampton and step down services in other parts of Northampton such as Duston and Weston Favell.
