Support and care – where do I start? This Guide will give you a good starting point to help you think about options and provide you with information so that you can find solutions yourself to help you to remain independent and well.
If you have access to the internet, visit Oxfordshire County Council’s Adult Social Care pages at www.oxfordshire.gov.uk which have further information and advice to support you.
The Care Act The Care Act outlines how care and support should be provided in England. It came into effect on 1st April 2015 and replaced all existing adult social care legislation. The law focuses on promoting wellbeing and health with emphasis on personal dignity, physical and mental health, protection from abuse and neglect and control over day-to-day life. This has been Oxfordshire’s approach for a number of years.
The Care Act introduced a nationally agreed level of care and support needs that all councils consider when assessing the support they can give to individuals. It also introduced the right to an assessment for carers (see page 17) and independent support from an advocate if you have difficulty with participating in assessments or planning for your needs. Visit www.oxfordshire.gov.uk to find out more.
Assessments If you have difficulty with day-to-day activities such as getting washed and dressed or getting yourself something to eat, you may be eligible for support from Oxfordshire County Council. Being ‘eligible’ means that, because of illness or disability, you need support and care to manage some of the basics of everyday life, such as eating, using the toilet, maintaining relationships or keeping yourself safe at home. There are national criteria for the Council to use to work out if you are eligible. To find out if you are eligible you can have an assessment. This is an opportunity to understand your situation, the needs you may have and your strengths and capabilities, and will involve talking to you, your partner, carer, relatives or friends as appropriate to consider the factors that affect your health, wellbeing and independence. The assessment will include the outcomes that you want to achieve in your day-to-day life as well as consideration of any needs you may have for care and support. The assessment will also consider how the provision of care and support could help you achieve those outcomes and promote your wellbeing. You have the right to have an assessment regardless of your ability to pay for the support you may need. The assessment may be over the telephone or face-toface, and in the future you will also be able to choose to complete the assessment online. You can arrange for an assessment either for yourself or for someone else: • via the Oxfordshire County Council website at
16
www.oxfordshire.gov.uk; • by phone on 0845 050 7666; or • by email socialandhealthcare@oxfordshire.gov.uk If the assessment finds that you do have needs for care and support which are eligible to be met by the Council, you will be helped to make a support plan. This will include the Council telling you how much meeting your needs is likely to cost – known as a Personal Budget. Having a Personal Budget helps you to have control over your life, identify your needs and the things you want to do and, if you wish, to manage your own support. You can choose to have your Personal Budget paid to you as a Direct Payment, which means you can plan exactly how to use it to meet your needs. A Direct Payment is money paid into a person’s bank account – you have to spend it on meeting your care needs. Contact Oxfordshire County Council’s Social and Health Care team on 0845 050 7666 or visit www.oxfordshire.gov.uk/directpayments to find out more. One of the ways in which you could meet your needs could be to engage a Personal Assistant. Personal Assistants can help with everyday tasks like going to the shops, as well as personal care such as washing and getting dressed. Oxfordshire County Council has set up the ‘Support with Confidence’ scheme which approves people offering Personal Assistant services. Visit www.oxfordshire.gov.uk to find out more.

Looking after someone Caring can be very stressful but you are not alone. At any one time in Oxfordshire over 60,000 people are providing unpaid care for a family member, friend or neighbour. It is very important that you take time to look after yourself while you are caring for someone else. You may not see yourself as a ‘carer’ - you see yourself as a son, daughter, partner, friend or neighbour. But if you regularly support and care for someone who could not manage without you then you are a carer and there is help available to support you to carry on with this vital work. If you provide unpaid support to someone who could not manage without your support, the law says you have a right to have your own needs assessed, even if the person you care for has refused support services or an assessment of their own needs. This assessment, called a ‘Carer’s Assessment’, gives you the chance to say what you need as a carer, and to find out what support might be available to help you.
The assessment can also give you information about services available, and other ideas for supporting you in your caring role. Your Carer’s Assessment will show if you qualify for support services from the Council. Many carers will be entitled to receive a support plan and Personal Budget in their own right, for services that enable them to continue in their caring role while still being able to pursue their own interests or lifestyle. Carers will not be charged a contribution to services arranged through the Council.
Carers Oxfordshire Carers Oxfordshire is an organisation set up for carers which provides free information and advice, specialist oneto-one support, a quarterly newsletter, the carer’s discount card, access to support groups, training opportunities including ‘Confidence to Care’, moving and handling, and first aid plus support to help you balance work and caring responsibilities. South and Vale Carers Centre The South and Vale Carers Centre provides free and confidential face-to-face information, advice and support to unpaid carers of all ages living in South Oxfordshire and Vale of the White Horse. It gives unpaid carers who are struggling financially, practically and emotionally access to the support they need so their quality of life improves and so they never feel alone in their caring role. The support they offer includes: • help with benefit checks and forms; • ongoing emotional support; • trips and activities for adult carers; • practical support to unpaid carers with issues such as discharge from hospital, accessing disability aids and equipment, and entitlement to social care;
• representation of unpaid carers at disability benefit appeals and tribunals • support to children and young people aged eight to 17, who live with a parent or sibling who has a physical illness or disability, a mental health problem or is dependent on drugs or alcohol; • emotional support to young people and children; • free trips and activities for children and young people; and • outreach with local employers so that unpaid carers who are trying to juggle their caring responsibilities at home with paid employment do not miss out on support. To find out more call 01235 510212 or email carers@svcarers.org.uk

Respite at home Home-based respite services give carers a break from their caring responsibilities. A care worker comes in to look after the person you care for, so you can have some time to yourself. Respite at home is considered to be a service for
the person you care for. If they have a Personal Budget for their own needs (see page 16) they could use that money to pay for it. A financial assessment of the person being cared for will confirm whether they need to pay towards the cost of this service.
Benefits you may be entitled to as a carer Carer’s Allowance If you care for someone for at least 35 hours a week and you are over 16 you may be entitled to Carer’s Allowance. However, you cannot claim it if you are in full-time education or employment and it can affect other benefits or pension you receive. Carer’s Premium If you are over 65 you may qualify for Carer’s Premium. This can entitle you to an increased amount of incomerelated benefits (such as Pension Credit, Council Tax Benefit and Housing Benefit). Carer’s Credit Carer’s Credit is a National Insurance credit which will let carers build up qualifying years for the basic State Pension and additional State Pension. This means that there will be no gaps in your National Insurance record if you have to take on caring responsibilities. Contact Carers Oxfordshire on 0845 050 7666 for more information about carers’ benefits and carer organisations in your area. Help in an emergency Emergency Carers’ Support Service is a support service for adult carers of adults in Oxfordshire. Experienced staff look after the cared-for person when the carer
has an emergency and is unable to do so. Care can be provided from one to 48 hours and, in exceptional circumstances, up to 72 hours. Carers must register with the service. It is simple and quick to do. You can complete a form online at www.carersoxfordshire.org.uk call Carers Oxfordshire on 0845 050 7666 or email carersoxfordshire@oxfordshire.gov.uk Once you are registered you can access emergency help quickly and efficiently when you need it and can call the 24-hour hotline on 01865 849562. Equipment, aids and gadgets Assistive Technology - devices to help you stay safe and independent Assistive Technology refers to devices that can be used to increase your independence and safety and help you with day-to-day tasks. It can also give you, your family, carers or professionals the peace of mind that help is available when you need it and can help determine how you are managing at home if you have memory problems. Assistive Technology can help you to: • do something, such as remember to have something to eat or drink, take medication or go to an appointment; • keep safe at home, through smoke, flood, or fall detectors; • request immediate help; • keep safe when you’re out and about; and • reassure family, friends and carers that you are safe when they are not with you. Assistive Technology ranges from very simple gadgets, such as medication aids (boxes with compartments for particular days of the week and times of day), to Telecare (devices that can automatically and remotely monitor emergencies and call for help). Telecare Telecare is a term used to describe devices (for example, movement, flood, gas, and smoke or fall detectors) that automatically send a signal when a problem is detected and calls for assistance. Personal pendants are also available as part of Telecare; when pressed, a fully trained operator assesses the nature of the emergency and reacts accordingly. Simple gadgets A range of gadgets exists to remind or enable you to do
something, or alert a family member or carer living with you that you need help. These gadgets are sometimes referred to as standalone Assistive Technology. Here are some examples of gadgets that Oxfordshire County Council can help to provide: Memory prompts These are used to assist people with memory difficulties by providing daily memory prompts using personally recorded voice messages at specific times of the day to do specific activities, for example, ‘take medicine’ or ‘eat lunch’. Movement activated memory prompts A movement activated message player allows a message to be recorded and played back when a person walks past the sensor, for example, ‘take your keys with you and lock the door’ or ‘make sure you have turned the cooker off’. Location devices Location devices are designed for people who may be at risk of getting lost when they go out on their own and can help them to return home safely. They can be used to locate a person when needed, allow the wearer to call for help and alert friends and family if a person goes outside of areas they know and feel safe in, has a fall or uses transport. People can also have two way conversations using these devices. Will I have to pay for my Assistive Technology? If you arrange this through Oxfordshire County Council they will undertake a financial assessment to work out what, if any, contributions you would need to make to the cost of Telecare. Gadgets (standalone Assistive Technology) are provided to you based on your need for the equipment and do not require a financial assessment. How can I find out more and get an assessment of my needs? Oxfordshire County Council can tell you about the full range of Telecare and simple gadget options available to you, simply visit www.oxfordshire.gov.uk/ assistivetechnology - you will also be able to fill out an online form at this address to request an assessment. If you are unable to go on the internet, call the Council’s Social and Health Care team on 0845 050 7666 and let them know that you’re interested in Assistive Technology.

Occupational therapy Oxfordshire County Council’s Social and Health Care team takes referrals for occupational therapy, which can help identify if you would benefit from any pieces of equipment, from simple solutions such as a walking aid or a second stair rail, to major adaptations such as walk-in showers. If you have difficulty with day-to-day activities such as getting washed and dressed or getting yourself
something to eat, an assessment by a social worker can help you find out if you are entitled to a Personal Budget to pay for the support you need. If you would like to find out more, or would like to make a referral for yourself or someone you know, please visit Oxfordshire County Council’s website www.oxfordshire.gov.uk or call 0845 050 7666.

Support and care at home All home care providers are regulated and inspected by the Care Quality Commission which publishes inspection reports and new quality ratings on its website www.cqc.org.uk/ When considering a service it’s always a good idea to check their report. Before you make any decisions regarding the provision of care, be sure to contact several providers and ask for a ‘Service User’s Guide’ and their charges and a draft contract between you and the care provider. Daily support and care A carer can call in daily to assist with day-to-day tasks.
Visits are generally available from 7am to 10pm. Some people will need more than one visit per day. Night services can also be provided. The hourly rate depends on the services required, the time of day and the location. To find out more contact Oxfordshire County Council’s Social and Health Care team on 0845 050 7666. Live-in care 24-hour live-in care can help people who have ongoing complex needs. It can also provide respite for regular carers and short-term support following a hospital stay.
The standards you can expect If you receive support at home from a care agency, these standards will set out what you can expect. If you are a care agency, these are the standards that are expected from your staff and the standards that Oxfordshire County Council will use to monitor the quality of your services. Home care staff will: • introduce themselves when they arrive; • know you and your support plan; • be trained to deliver the support you need; and • always deliver support to a good standard. When your home care worker visits you, they will: • focus their attention on you; • be pleasant and treat you with dignity and respect; • do their very best to arrive on time and let you know by telephone if they are going to be late; • tell you when they are leaving; • check to see how they can best support you at the start of each visit; • not rush you - they will help you at a pace that suits you;
• carry out all agreed tasks; • make sure that you are comfortable at all times; • communicate and discuss topics that interest you; • check that you are happy with the support you are given and encourage you to tell us how they can improve; • show you your care record if you ask for it; and • be aware that their visit may affect other household members. These standards have been jointly written by people who receive care in their home and home care support agencies. If you have any compliments, comments and/or complaints about the home support you receive please contact your care agency; usually talking to someone in the service can put it right. If not, please contact Oxfordshire County Council by calling 01865 815906, emailing complaints@oxfordshire.gov.uk or writing to Complaints Team, Oxfordshire County Council, County Hall, New Road, Oxford OX1 1ND.

I think she is a lucky find!
I would be lost without her
we can help you and your family Looking for care and/or assistance within your own home? Are you self funding? From 10 hours to 24 hour live in care. Many people use domiciliary care agencies to receive care at home, however this is not the only option. You could also consider employing your own carer or personal assistant through My Care My Home.
What is My Care My Home? My Care My Home work with councils across England and Wales to provide advice and support to anyone who has to pay for part or all of their care themselves. There are many advantages to employing your own carer/ personal assistant. Consistency You have the same carer(s) consistently who you have chosen yourself and like their manner and personality, and are confident in their competence. Loyalty Their loyalty is to you personally and they become committed to you and your welfare at all times. Calibre The pay they receive from you can be higher than that received from a Care Agency. This can attract carers with more experience, knowledge and/or higher qualifications. Flexibility You can agree with them just exactly when and how you want your care delivered, what tasks and duties you want them to do, and how you would like them to deliver it.
How can My Care My Home help you? If you want to employ your own staff directly My Care My Home can support you by handling all employment matters such as: • • • • •
Job description and advertisement. Shortlisting applicants. Arranging and conducting interviews with you in your own home. Obtaining necessary documentation such as references, disclosure and barring service, copies of qualifications, etc. Managing the payroll and ensuring that tax, NI, and other deductions are made correctly and the carer is paid on the agreed dates. Providing monthly visits to monitor the performance of your carer(s) to ensure you are happy with the service you are receiving.
For more information about this service, or any of the other services that My Care My Home provide, then please speak to one of our care advisors or visit our website at www.mycaremyhome.co.uk

Housing options Sheltered or retirement housing Sheltered and retirement housing is provided by housing associations and some District Councils. Sheltered housing, which is mostly available for rent
only, is a practical step if you would like more security and companionship and, for some, additional help and support.
Extra Care housing Extra Care allows people to have their own home to rent, purchase, or part-purchase under a shared ownership scheme, and have flexible care services delivered to them by the on-site team if they need it. This ensures that residents have peace of mind knowing they are able to remain safely in their own home. For many people Extra Care housing is an alternative to a residential care home. To get Extra Care housing, you normally have an assessment of your care needs from a social worker
and an assessment of your housing needs by a housing officer. You can also contact the scheme provider directly. If you wish to refer yourself or someone else for assessment then you can arrange an assessment online or telephone the Social and Health Care team on 0845 050 7666. To find out more about Extra Care housing you can watch a short film about the benefits. You can find this on www.oxfordshire.gov.uk

Residential care Care homes If you are reasonably active, but would like greater security and care, a care home offering only personal care may be the best option. Care homes with nursing If you think you may need nursing care in a care home, you will need to be visited by a social worker to identify the care you will need. This visit might be in your own home, or in hospital if you’ve been ill, or in a care home. You will be fully involved in planning your care needs.
Paying for residential care If you want to find out more about paying for residential care, call Oxfordshire County Council’s Social and Health Care team on 0845 050 7666. All care providers in the country must be registered with the Care Quality Commission. All services are inspected by the Commission, which reports on its findings and has started to award quality ratings. These inspection reports are available at www.cqc.org.uk

Care Choices’ helpline Care Choices provides a free helpline giving information and help about care services and accommodation. A personalised report can be generated for customers providing details of all home care or care homes that meet their criteria.
One call to the freephone number 0800 389 2077 will enable the service to build a profile of exactly what type of care you’re looking for, while taking into account your personal needs and interests.
Money and paying for support and care Care funded by the Council Do I qualify for local authority financial assistance? If you have been assessed as needing a care home place and your capital is below £23,250 (correct at 1st April 2015 but may change) you may be entitled to financial support from your local authority. To find out more, contact Oxfordshire County Council’s Social and Health Care team on 0845 050 7666.
What if a home costs more than the local authority is prepared to pay? The local authority will allow a third party to top-up your fees as long as they are able to do so for the long term. You are not allowed to top-up the fees yourself from capital below £23,250 (correct at 1st April 2015 but may change).
Do I have a choice of care home if the Council is paying? Yes and it can even be in a different county. The home you choose must be suitable for your assessed needs, comply with any terms and conditions set by your local authority and not cost any more than they would usually pay for someone with your needs.
My partner needs care, how does this affect me financially? The Council will do a joint financial assessment for you and your partner, and will also carry out individual assessments on whoever needs care. The contribution you are asked for will be whatever the lower amount is from these assessments. If you receive joint benefits you will only be offered a joint assessment.
Paying for your own residential care If you are funding your own care because you are not eligible for local authority funding there are other forms of financial assistance you may be entitled to. The important thing is not to try to do it alone, always seek professional advice. My Care My Home My Care My Home offers a free telephone and face-toface advice service through home visits aimed at helping people find a suitable home care provider, or care in sheltered or Extra Care schemes and care homes. They are also able to visit people who are currently in hospital and require a package of care before they can be discharged. Protecting vulnerable adults Far too many adults suffer abuse and neglect, often at the hands of relatives or people in trusted positions. A vulnerable adult is a person aged 18 years or over who may be unable to take care of themselves, or protect themselves from harm or from being exploited. organisation where this is not addressed by the management. If you are worried about contacting the police, you can contact Oxfordshire County Council’s Social and Health Care team to talk things over first on 0845 050 7666. Further information is also available on the Oxfordshire Safe From Harm website at www.oxonsafeguardingadults.org.uk If someone is in immediate danger, call the police on 999.

Having your say How to make a complaint or comment If you are unhappy or worried about any adult social care services, it is important to let someone know. Usually talking to someone in the service can put it right. You can talk to the people who provide your care or their managers. They will have a process in place for managing complaints and listening to concerns that are raised. Wherever you raise a concern it should be listened to and taken seriously. The good and bad things people raise can help improve services for everyone. You will not be treated unfairly because of anything you raise. It is never wrong to give the service a call. The Comments and Complaints team will help you
decide what to do - you do not have to make a formal complaint to talk things through. You can contact them on 01865 323589, Email commentsandcomplaints@oxfordshire.gov.uk Write to Comments and Complaints Service Freepost RRYR-XTBE-GBTZ, County Hall, New Road, Oxford OX1 1ND For complaints concerning the NHS, contact NHS Complaints on 0300 343 5718 or email oxfordshire@seap.org.uk

Advocacy Sometimes we all need help to get our views across and to understand the information we are given. But if you have substantial difficulty in understanding, retaining or weighing up information, or in communicating your wishes, you might have a family member, friend or neighbour you can ask to help you. If there is no-one appropriate you can ask, Oxfordshire County Council can arrange for you to have an independent advocate to help you. An independent advocate is a person who supports you to understand information, express your needs and wishes, secure your rights, represent your interests and enables you to be fully involved in planning your care and support. To find out more about how you can get
independent advocacy, please contact Oxfordshire County Council’s Social and Health Care team by emailing socialandhealthcare@oxfordshire.gov.uk or by phone on 0845 050 7666.
