Welcome from Shropshire Council Welcome to the 2015/16 edition of the Shropshire Support Finder, produced annually to provide information on support and care for adults available in the county. Shropshire Council is committed to helping people in Shropshire by supporting and promoting stronger, more resilient communities so that people can live their lives as successfully, independently and safely as possible. We know that people, regardless of age or ability, are best placed to determine what help they need to lead successful lives. Our aim is to help some of the county’s most vulnerable people and their carers to get help sooner. We do this by working differently – such as doing basic assessments over the phone straightaway, and then supporting people according to their needs. It’s working really well and is something we pride ourselves on. Working in this way has also ensured a smooth introduction of the Care Act earlier this year – the biggest change to English adult social care law in over 60 years, reforming the law relating to care and support for adults and their carers. Shropshire Support Finder is full of support services, voluntary organisations and community-based solutions that are available across Shropshire to help you or a loved one lead the lives you want as well as maintain or regain

independence. It is designed to give you help and advice so that you can find your own way to meet any needs for support and care. You might want to know how to find out about support and care to help you live at home, and how much you might pay for it, or how to find the right place for you to live. Getting out and about Community transport Community transport groups are independent charities or voluntary organisations, working to support local people. Community transport is for those who may not be able to use public transport or do not have access to public transport. This may be people who can’t walk to a bus stop or may need assistance to travel or it could be someone living in a rural area where there are no buses.

North Salop Wheelers A community transport minibus service for members in North Shropshire. The minibus is wheelchair-accessible and is available for young people and the elderly. The ring and ride service runs from the Hanmer area to Wem each Thursday and the Wem area to Whitchurch each Friday, when there is a town service. The minibus is also available by arrangement for hospital and doctor appointments, and excursions at other times. There are currently additional services connecting the following villages to a market town: Cheswardine, Colemere, Hinstock, Lyneal, Norton, Myddle, Pipegate, Tetchill, Welshampton and Woore. If you live in or near these places the service may be able to help you. Buses are available for clubs, societies and youth groups. Call 01948 880037 for more information.

Transport concessions Blue Badges Scheme for people with severe walking difficulties and people who are registered as severely sight impaired (blind) who travel either as passengers or drivers. You can check to see if you are likely to be eligible for a blue badge by going to the blue badge eligibility tool at www.gov.uk You can download the appropriate application from the website below or contact the Customer Service Centre on 0345 678 9014 to request an application pack. Health, recovery and wellbeing Befriending schemes Age UK Shropshire Telford and Wrekin runs a number of befriending schemes which support volunteers to visit people’s homes providing friendship and a little bit of extra support for lonely and less mobile older people. The volunteers all have a criminal records check, provide two references and are supported by a dedicated co-ordinator in the Age UK office. The Volunteer Visiting scheme This scheme is available in Telford and Wrekin. Volunteers visit older people in their own homes, providing friendship and support. Visits take place at times to suit both the volunteer and the older person and can vary from once a week to once a month. Volunteers can bring shopping, play a game of cards or just chat, whatever the older person wishes to do. For more details, contact the Telford office on 01952 201803. Telephone Buddy This is a free service. Telephone buddy volunteers come into Age UK’s office at Mardol Gardens on a Monday morning to ring older people for a friendly weekly chat. Volunteers provide two references and are supported by a dedicated co-ordinator in the Age UK office. Looking after yourself Energize Shropshire, Telford and Wrekin This is the county sports partnership for Shropshire and a source of information and communication for everybody involved, or looking to get involved in sport and physical activity – whatever level of interest or ability. Tel: 01743 453495 Email: info@energizestw.org.uk Help2Change Help2Change provides free support delivered by specialist advisers in a range of venues, including GP surgeries, pharmacies, community venues, hospitals and supermarkets in Shropshire. For further information on Help2Change services, telephone 0345 678 9025 or visit www.healthyshropshire.co.uk Help2Quit Help2Quit, the local Stop Smoking Service in Shropshire, offers free support delivered by stop smoking specialists (including practice nurses, midwives, pharmacists and school nurses) that really works. The service is available in almost all GP surgeries and is also provided in many pharmacies, community venues, hospitals and selected supermarkets. If you use a combination of stop smoking medicine, along with specialist help and support from Help2Quit, you’re up to four times more likely to successfully go smoke free than if you try to stop on your own. Around 1 in 20 adults now uses electronic cigarettes. Whilst they are not risk free they are considered to be less harmful than smoking and smokers who want to use e-cigarettes to quit smoking are encouraged to contact Help2Quit. Advisers support you to make small sustainable changes that fit with you and your lifestyle. NHS Health Check The NHS Health Check programme has been developed to prevent heart disease, diabetes, stroke and kidney disease. If you are aged 40 to 74 years and are not already diagnosed with one of these conditions you will be invited by your GP once every five years to have a check to assess your risk of developing one of these diseases. If necessary, you will be given advice to help reduce or manage your risk of disease.

Slips, trips and falls If you have fallen in the past, speak to your GP and mention any impact on your health and wellbeing. Your GP can carry out simple balance tests to see if you are at an increased risk of falling in the future. They can also refer you to useful services near you. Regular exercise to improve your strength and balance can help reduce your risk of having a fall. This includes simple activities, from walking and dancing to more specialist training programmes. Tips for preventing falls in the home:

• avoid walking on slippery floors in socks or tights; • avoid wearing loose-fitting, trailing clothes that might trip you up; • wear well-fitting shoes that are in good condition and support your ankles; and • take care of your feet by trimming toenails regularly, using moisturiser and seeing your GP or chiropodist about any foot problems.

Community opportunities Gusto Part of Shropshire Rural Community Council, Gusto is a network of active people who meet regularly and enjoy participating in activities together. These include regular social events, like theatre trips, cycling, dog walking, going to the cinema, meeting for lunch and enjoying

Books on Prescription Books on Prescription (BOP) is a scheme offered by Shropshire libraries in partnership with Shropshire Clinical Commissioning Group where GPs ‘prescribe’ self-help books to their patients. The books cover a wide range of common topics including anxiety, anger, depression, self-esteem, stress and worry. Talk to your GP if you would like to know more.

Community links Community Enablement Team Shropshire Council’s Community Enablement Team is a community-enabling service. The team has a diverse range of skills, knowledge and experience to deliver cost-effective grass roots solutions that make the most of community resources, and meet community priorities at a local level. Living independently Adapting your home To remain in your own home for as long as you would like you should ensure that your home is safe, secure and warm. Some forward planning to make sure that the garden doesn’t become a burden is also sensible. If you are not planning to move, think about adaptations that would make life easier now and later on. Of course, these things can be daunting, especially if it’s a case of having to bring in someone to do jobs you used to happily take on yourself, but there are reliable tradespeople to be found. If you are eligible for council support, following an assessment, it may be able to help with minor adaptations. However if your home needs a lot of work, you may be able to apply for a Disabled Facilities Grant. For more information on these visit www.gov.uk/ disabled-facilities-grants There are also non-profit organisations known as Home Improvement Agencies that work specifically for older or disabled people and can tackle, or help you tackle, most kinds of home maintenance, repairs and adaptations. Disabled Facilities Grant If you need adaptations to your house so that you can gain better access to, and move more freely around your

Home Improvement Agencies Home improvement and handyperson service providers are local organisations dedicated to helping older people, people with disabilities, and vulnerable people to live in safety and with dignity in their own homes. Services are focused on ensuring that existing housing is fit-for-purpose and that vulnerable people, predominantly homeowners, are able to continue living independently for as long as possible. Assistive technology In April 2015, Shropshire Council appointed an ‘Assistive Technology Co-ordinator’. The purpose of the role is to promote and support the use of assistive technology across the county, to enable individuals to remain at home, living independently and safely for as long as possible, whilst maintaining their dignity and allowing them to make choices about their own level of support. Assistive technology includes a wide variety of equipment, from simple low-tech items such as small plugs that can be placed in a kitchen sink, washbasin or bath that can sense if taps have been left on or if water temperature is too high. These plugs, known as Magi Plugs, automatically release the water thus alleviating the risk of flooding or scalding. Other items range from calendar clocks to more high-tech items, such as sensors that can detect accidents or emergencies in the home that can be monitored by a community alarm service, known as telecare. Shropshire Council offers a great range of assistive technology, from standalone items, such as the Magi

Plugs, to its telecare service. The Council is partnered with a company called Tunstall to make it easier for local people to access the latest technology and to help themselves or their family independently, or as part of a wider package of care. Alternatively, you can obtain further information on assistive technology that might be suitable for you by using the AskSara website (www.asksara.dlf.org.uk) where you will also have the option of having some items on loan to try out.

Case study Mrs M has dementia. Symptoms include hallucinations, a decline in her memory and increase of short-term memory loss. She gets confused about time and she cannot comprehend the difference between day and night. As a result she began telephoning her family at all times of day and night, causing them anxiety and concerns. Mrs M and her family were initially supported by practitioners from the Community Mental Health Teams, including the Memory Service, and a variety of services were investigated and instigated. As Mrs M’s anxieties increased, she required further support with her personal care at home and her family became increasingly worried about her ability to remain living at home independently. Further support was given through Adult Services with a care package and arrangements for Mrs M to attend a day centre on a regular basis. Telecare solutions were also introduced, initially with a period of assessment and monitoring via a Just Checking assessment tool, which enabled both the social worker and family to evaluate how safe Mrs M was in her home and where concerns and issues might arise. These concerns were met with the installation of Lifeline Hub, to which a variety of sensors were attached so that, should Mrs M suffer a fall whilst at home, contact could automatically be made with a call centre which could alert the family and emergency services if required. A heat detector was placed in the kitchen, which again could alert family should Mrs M leave the cooker on. These pieces of equipment allayed Mrs M’s anxieties because she knew support was immediately available if it was required. Other pieces of equipment were also installed, enabling family communications to remind her about the time of day and alert her to any forthcoming appointments or visitors. The installation of the assistive technology has also given Mrs M’s family peace of mind, knowing that they can be contacted in an emergency and that Mrs M is able to continue to live independently, as she desires. Without the installation of the equipment Mrs M’s safety would have been at risk. Her anxiety and confusion would also worsen, which could lead to issues regarding medication and the need for care calls, possibly resulting in a longterm placement, which Mrs M did not want. Her family is now reassured about her safety in the home, they are no longer exhausted due to the high level of night time calls, and the quality of their relationship with Mrs M has improved. Technology has allowed independence, both for Mrs M and her family, so that the quality of care they are able to provide has improved and they are able to spend more time with the whole family. It has prevented the need for an increased care package and the possibility of a permanent placement.

Telehealth The use of technology can also ensure that family members and carers have reassurance and peace of mind that their loved one will receive a quick response in the event of an emergency. Telehealth monitoring is the remote exchange of physiological data between a patient at home and medical staff at hospital to assist in diagnosis and monitoring. This could include support for people with lung function problems, diabetes, etc. It includes (amongst other things) a home unit to measure and monitor temperature, blood pressure and other vital signs for clinical review at a remote location (for example, a hospital site) using phone lines or wireless technology. Examples of what telehealth can monitor remotely: • blood glucose levels; • cardiac arrhythmia; • asthma; and • medication reminder systems.

Keeping safe Safeguarding adults who are at risk of abuse or neglect Living a life that is free from harm and abuse is everyone’s fundamental human right. We all need to act as good neighbours and citizens in looking out for one another and seeking to prevent the isolation that can lead to abusive situations and put adults at risk of harm. One of the fundamental principles of communities is that we are all fair, compassionate and caring to each other. Please do your part to prevent and stop abuse from occurring to others. If you find out about something you cannot manage on your own, please speak to someone who will be able to help, like the Police, the person’s doctor, or someone who is supporting the adult affected. Shropshire Council will want to know if the adult at risk of abuse or neglect has care and support needs. This means someone who:

• has a long-term illness/condition; • misuses substances or alcohol; or • is a carer such as a family member/friend who provides personal assistance and care to adults and is subject to harm. Looking after someone Young carers Shropshire Young Carers service is part of the British Red Cross which is a registered charity and works with children and young people who have a caring responsibility for a family member. The service’s vision is to provide support for all young carers in Shropshire who need it through a network of peer support groups across the county and by working in partnership with schools. Referrals can be taken from all agencies, parents and young carers themselves who live in the Shropshire area and must come with parental, or guardian, consent.

Adult carers If you are a carer, you are entitled to an assessment of your own needs. This is your opportunity to tell the Council about the things that could make caring easier for you. Carers’ training and benefits Shropshire Council can offer carers a Carer Aware online training course, which takes under an hour to complete, on the website www.shropshire.gov.uk/carer-awarecourse The course helps carers, people who work with carers, or anyone who is interested in learning more, to find out who a carer is, what their rights are, what support is available and where to find further information. If you are 16 or over and spend at least 35 hours a week caring for someone with substantial needs, you may be eligible for Carer’s Allowance, worth £62.10 a week. You may also be able to claim other benefits, such as an

Carers Direct A national, one-stop information and advice service for carers who look after ill, frail or disabled friends and relatives. and emotional wellbeing and their physical and mental health; • access to health and social care for the person being cared for; and • how to access information about, and support from, health and social care services for the person being cared for. Care at home Care in your own home is often referred to as either ‘domiciliary care’ or ‘home care’. Given the choice, many people prefer to stay in their own homes, surrounded by their possessions and near to their family and friends. This, of course, applies to everyone, whether they are elderly and frail, convalescing, have a physical or learning disability, or have dementia. The range of care available is varied and each client, working with the care provider, should agree a care plan, which states their needs and the expected outcomes. Unless you arrange your care privately, you will require an assessment of your needs with the Council (see page 33) and, if you are eligible, you can decide whether you would prefer to arrange the services yourself. However, if you do not meet the eligibility criteria dictated by the Care Act – a new national legislative framework – the Council can still help and advise you about other ways to meet your needs. Listings of home care providers start on page 25. See page 37 for advice on self-funding your care. Domiciliary care will normally involve personal care (assistance with washing, dressing etc.) and/or one or more care workers to assist with day-to-day tasks, such as moving around the home. You may also wish to have assistive technology installed in your home to provide security and reassurance. Live-in care is another option. This is where a care worker lives in your home, is allowed time off each day and must have a night’s sleep. Live-in care can be arranged as a short respite for your main carer or a permanent arrangement to suit your needs. 24-hour care is required when you need assistance or monitoring through the day and night; it differs from livein care in that care workers work on a roster to ensure that someone is on duty and awake both day and night.

Some home care agencies are also registered as nurses agencies, meaning they can provide registered nurses if needed. These would be arranged by the Clinical Commissioning Group (CCG) if you have health care needs. All service providers must be registered and inspected by the Care Quality Commission (CQC). It is important to be assured that all care workers have been vetted by the Disclosure and Barring Service, have received necessary training, are properly referenced, and are eligible to work in the UK prior to them visiting your home. Ask the service provider to show evidence that the care workers they introduce are indeed compliant with the legislation, and check the provider’s latest CQC report at www.cqc.org.uk/ More information on the CQC can be found on page 34. Paying for home care Domiciliary care for social needs may be funded in part or whole by the local authority, if your eligible, or alternatively private arrangements can be made with service providers. Council-funded care is provided by independent companies. Fees for services vary; the service provider will be able to give you a detailed cost breakdown prior to the commencement of any service and you should ensure that you check this includes any weekend, bank holiday and mileage costs. Most service providers will accept clients who pay using Direct Payments (discussed further on page 36).

Specialist care Learning and physical disabilities For care and support provided by the Council, everyone goes through the same assessment process (explained on page 33), regardless of whether you have a physical or learning disability, mental health conditions, or are an older person who requires additional care and support. Services are always designed around people and reflect the needs of those who use the service, their families and carers. Self-funders are also entitled to an assessment to determine their needs. The Council can arrange supported living and day opportunities as well as work experience, training, and employment opportunities for people with learning or physical disabilities. Through various means of community support, Shropshire Council promotes independence and living in your own home. For people with particularly complex needs that can’t be met at home, it can also arrange residential care. The Council aims to ensure people with disabilities

have access to the same community facilities as everyone else. It is working closely with other areas of the community to strengthen community links. A care manager will carry out the assessment (as described on page 33) with you, and your carer (if appropriate). You will then find out what your indicative Personal Budget will be, and then proceed to develop a Support Plan to meet your identified needs and outcomes. Help will be available from the voluntary sector to complete your Support Plan. Your Support Plan could include considering future accommodation needs, employment, respite and help to support you to remain in your own home. Carers are offered an assessment in their own right and support services can also be provided to help meet their identified needs as a carer, see page 19. Dementia care If you are worried about your memory you should, in the first instance, arrange an appointment to see your GP, who will carry out some tests for further investigation. Your GP may refer you to the Memory Service, which will provide specialist assessment and offer a formal diagnosis. The Memory Service may advise commencing medication where appropriate and the team can also provide specialist support and information following your diagnosis. If you have been formally diagnosed with dementia, or you have a family member who has been diagnosed, your initial point of contact to access services for health care solutions should be your GP or, for other services, the First Point of Contact Team on 0345 678 9044. Support in the community It is important that people with dementia and their carers are supported to live well and independently in their own homes for as long as possible. There is a range of services available within communities across Shropshire to facilitate this. The local Alzheimer’s Society offers support workers, dementia cafes, peer support groups, Singing for the Brain sessions and carers’ support groups. All of these are places for people with dementia and their carers to go to get practical information and support. Some also offer

activities and signposting to other services in the area. For further details contact the Shrewsbury Alzheimer’s Society office on 01743 341800. Age UK Shropshire Telford and Wrekin runs two fortnightly Diamond Drop In Centres, in Monkmoor and Copthorne, in Shrewsbury and one in Oswestry to support local people living with dementia and their families, friends and carers. The Drop In Centres provide a welcoming and friendly environment to give emotional support and information and a chance to take part in interesting activities. They also provide benefit advice,

Support and care Call the First Point of Contact team to get going If you wish to talk to somebody about staying independent or anything at all regarding your personal wellbeing, please call the friendly and dedicated team of advisers at the First Point of Contact Team on 0345 678 9044. You can also call the Council on behalf of any friends, relatives, or neighbours that might benefit from support or care. The advisers will ask you a selection of questions in order to understand your situation. They will discuss what works well for you at the moment and what might not be working so well. They will then try their best to sort out your query over the phone – by telling you about organisations in your local community. Hopefully, you will then be in a position to get the right support both easily and quickly. If the adviser thinks that you would benefit from further talks face-to-face with one of the Council’s social workers, they will arrange a Let’s Talk Local session at a date and a time which is convenient for you. This is an opportunity for you to set out your circumstances and to have your needs assessed. Do not worry if you cannot travel or if you are unable to leave your home. If this is the case, the Council can arrange

for a social worker to visit you. Just like every other local authority, Shropshire Council has strict guidelines to follow which are set out in the Care Act 2014 (see shropshire.gov.uk/adult-social-care/ the-care-act). The person who you meet with locally will use these guidelines when deciding how best to advise and help you. An assessment of your needs will allow the worker to decide what sort of support you require – you may have needs that change, of course, and they will always try hard to plan for this. Finally, you should also mention if you have support from a friend or family member. If you do, the Council will happily look at what needs your carer might have as a result of providing you with necessary care – this means that they help you with things that you cannot manage yourself, such as personal care and hygiene.

Types of support Shropshire has a reputation for enabling residents to live independently, in their community, with dignity. This is, in part, made possible by exciting new technologies. Home care is a great option if you require help with tasks, such as bathing, eating, or dressing. However, if your needs mean that you might require care in a care home, the following information will help

you to decide the most appropriate option for you. It’s always best to have an assessment carried out by the Council to make sure that you are choosing the best option to meet your needs. Personal care only A residential home might be an option if you have needs

Types of support continued which cannot be met in the community but you do not require nursing care. It is important to make a balanced decision based on both your present needs and also what your future needs might be. Equally, you should think carefully about your finances. Take along the care homes checklist on page 35 to prompt any questions you will have for the manager. You might also want to consider whether domiciliary care can meet your needs. Care home with nursing This kind of home is suitable for those needing nursing care, where it will be delivered by qualified nursing staff alongside care workers. Nurses are on duty around the clock. Fees are generally higher than residential care homes. Some of the cost of your nursing care will be met by the NHS, who will pay the home directly. At the time of publication, the rate for NHS Funded Nursing Care is £112.00 per week. This is paid directly to the home, though the figure may change in April 2016. NHS Continuing Healthcare Continuing care is care provided over an extended period of time, to a person aged 18 or over, to meet physical or mental health needs that have arisen as a result of disability, accident or illness. A person who needs continuing care may receive services from the Council or from the NHS. NHS Continuing Healthcare is a package of continuing

care that is arranged and funded solely by the NHS. This is free of charge, wherever it is delivered. NHS Continuing Healthcare support may be provided in a nursing or residential care home or in your own home. People eligible for NHS Continuing Healthcare will have been assessed by health and social care workers to have a ‘primary health need’ and are likely to have complex medical needs and substantial or intense ongoing care needs. National guidance says that the assessment for NHS Continuing Healthcare should be ‘person-centred’. This means that the person being assessed should be fully involved in the assessment process. They should be kept informed, and have their views about their own needs and support taken into account. A decision about eligibility should usually be made within 28 days of a referral for assessment being made to the NHS Clinical Commissioning Group. If you are already receiving support from the Council, your social worker will be able to give you more information about NHS Continuing Healthcare and complete with you an NHS Continuing Healthcare screening checklist, from which a referral for assessment might be made. Your GP or community nursing service should be able to give you advice or make a referral with you if you are not currently receiving support from the Council.

Inspecting and regulating care services All care providers must be registered under a system which brings adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety. The Care Quality Commission (CQC) registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others. Inspectors carry out a mixture of announced and unannounced inspections looking at the quality and safety of the care provided. They look at whether the service is: Safe; Effective; Caring; Responsive to people’s needs; and Well-led. Inspection teams are led by an experienced CQC manager and often include experts in their field. The team may also include professional or clinical staff; Experts by Experience; people who use services and carers. Following an inspection, each care home and home care agency is given a report of how it rates against

national essential standards of quality and safety. CQC has also started rating services as ‘Outstanding’, ‘Good’, ‘Requires Improvement’ and ‘Inadequate’. By March 2016, CQC expects to have rated every adult social care service in England. When considering care services, it’s always a good idea to check a service’s inspection report and rating on the Care Quality Commission’s comprehensive website: www.cqc.org.uk or ask the care provider you are considering for a copy of their latest inspection report. Paying for care Personal Budgets If you are eligible for social care funding, a Personal Budget could be made available to meet your needs. This means that you can influence how a budget could be used in a way that works for you and in accordance with your approved Support Plan. You will be asked for, and supported to develop, a Support Plan that sets out how you wish the funding to be used and how you intend to manage the money. This will need to be agreed by your care manager.

Ways in which a Personal Budget can be managed in Shropshire It is important to remember that more than one of the options illustrated above can be chosen. Your Support Plan needs to state clearly how much of the budget will be managed in a certain way.

Direct Payments A Direct Payment is money from a council’s Adult Social Care department to cover a specific assessed need as detailed in a Support Plan. People who are eligible are given an agreed amount and can use it to employ a personal assistant themselves or through an agency to help support them. You aren’t allowed to use Direct Payments to pay for permanent residential accommodation – but you may be able to use them to pay for occasional short periods in residential accommodation, if the Council agrees that is what you need. If you are applying for services for the first time,

your social care practitioner should discuss the Direct Payments option with you when they assess your care needs. Direct Payments are made directly into a nominated bank, building society, Post Office or National Savings account. When in receipt of a Direct Payment the Council will expect you to provide documentation to demonstrate how you have utilised your Direct Payment. Such documentation will include bank statements and invoices. A Direct Payment must be spent in accordance with your approved Support Plan.

Individual Service Funds You can choose a service provider that you wish to provide your care or support. (For existing clients this may be their current provider if they are happy with them.) Some or all of the Personal Budget is then paid by the Council directly to the service provider as an ‘Individual Service Fund’. In practice, this is not very different to what happens currently but the important difference is that you (and/

or those closest to you) are making a choice about that organisation. The Council will enter into a contract with that service provider and you and the provider must agree how the money will be used. The provider is accountable to you and the Council to deliver the service they have agreed. The provider should provide regular details, as requested.

Self-funding advice If you have capital or savings of more than £23,250 you will have to pay for any care services you require but you are still entitled to an assessment of your needs. If you will be moving into care, you may be entitled to some of the following financial assistance and support, subject to an assessment of need. Twelve-week property disregard Adult Social Care Services will help with the cost during the first twelve weeks of permanent care in a home, provided your assessment has shown that this is the kind of care you need. This is called the ‘property disregard’ period. This applies if: • your former home is included in your financial assessment; • your other capital is less than £23,250; and • your income is not enough to meet your care home fees. Deferred payment agreements After the twelve-week property disregard period, Adult Social Care Services may be able to help you while your property is being sold. They will charge any financial help they give against the value of your home. This means they will take back the amount they have paid

once your property has been sold. However, they may limit how much they will pay. Also, Shropshire Council will charge administrative costs and interest at variable rates. It may affect your entitlement to Pension Credit if your property is not seen to be on the market and becomes treated as capital by the Department for Work and Pensions (DWP). Attendance Allowance This is a non-means tested, non-taxable benefit from the Department of Work and Pensions paid at the lower rate of £55.10 per week for those needing care by day or night, and at the higher rate of £82.30 per week for those needing care both during the day and night. Everyone is entitled to claim this benefit. A benefits check is part of your initial assessment with the Council. NHS Funded Nursing Care Whether you are a temporary or permanent resident, if you live in a care home that provides nursing care you may be entitled to a non-means-tested NHS Funded Nursing Care Contribution (currently £112.00 per week for the standard rate) towards the cost of your nursing care. This is paid directly to the home by the Clinical Commissioning Group. >>

Self-funding advice continued >> Running out of money If your capital is likely to reduce to £23,250 (correct at October 2015), you must let the Council know well in advance as it may step in to help with your care fees. It must conduct an assessment of your situation and may make a contribution. If the home you have chosen charges more than the amount the Council will contribute, you must find someone to help pay the difference – a ‘top-up’ payment. Any top-up would be separate and additional to your assessed weekly contribution. Whoever does this, whether family or a benevolent charity, they should realise that they may have to pay this for some time. Alternatively, you would have to find a home with lower fees. Shropshire has a reputation for meeting the needs of citizens in their communities, which means that more and more local people get the support that they need to lead successful lives in their own homes. The Council

advises anybody considering residential care to call the First Point of Contact team on 0345 678 9044 to discuss all of the options available. Understanding your rights before moving into care is essential. There are a number of financial products and specialist companies who may be able to help. It is important to seek advice before committing yourself. Figures may change over the life of this Directory.

Having your say Comments, compliments or complaints All care providers need to know how well they are doing. Your feedback is very important to them. They want to know whether their services meet your needs and are delivered effectively. They welcome your comments, compliments and complaints so that they can serve you better. If you are concerned about the care that you, a friend or a relative is receiving, you may want to talk first to the service’s manager. The problem may be resolved quite easily once the manager is aware of it. You should feel able to complain about any aspect of your care whether in your own home or in a care home. You may want to complain about the way you are treated by a staff member or the quality of the food you are served. Sometimes you may just want to make comments and suggestions about how things are done. Making a complaint should be easy to do. Care services must have an easy-to-use complaints procedure that they can share with you.

If you do not wish to contact the manager, or talking to the manager may not resolve your concerns, you can contact the registered owners of the service. They have a duty to respond to your complaints. If the Council has arranged, funded or contributed to the funding of your care, you can also complain to the Adult Social Care Department. Local authority advisory groups Shropshire Council wants better communication with local individuals to support the improvement of Adult Social Care services. Across the county, the Council has established a Making it Real Board and is developing advisory groups to support conversations between those in receipt of services and the agencies delivering the services, including People2People, Healthwatch and others. At advisory group meetings, you can expect: a venue with easy access; encouragement and support to take a full part in the meeting; an agenda previously agreed by the participants; a way of working that is structured yet informal; all personal information to be dealt with confidentially; to receive notes and any supporting paperwork either by email or post; and your representative to take the views of the group to the Board.

In return, you should be prepared to: work together to improve services; respect others in the meetings and treat any personal information confidentially; take personal responsibility for the level of detail shared in meetings; avoid discussion about specific cases and named individuals; comment based on experience but from a general viewpoint; raise personal concerns or complaints in an appropriate way; commit to regular involvement if you. How solicitors can help A solicitor can give you impartial advice about wills, making gifts, estate planning and Powers of Attorney. Some can also offer guidance on immediate and longterm care plans, ensuring (if applicable) the NHS has made the correct contribution to your fees. Lasting Powers of Attorney (LPA) allow you to appoint someone you trust to make decisions about your personal welfare, including healthcare and consent to medical treatment, and/or your property and financial affairs. A LPA is only valid once registered with the Office of the Public Guardian. It allows for a person of your choice to make decisions on your behalf at a time when you may be unable to. The Court of Protection can issue Orders directing the management of a person’s property and financial affairs if they are incapable of managing their own affairs should they not have a LPA. The Court procedure is presently very slow and the fees are quite expensive so preparing a LPA is always advisable, providing you have somebody sufficiently trustworthy to appoint as your attorney. An ‘advance directive’ allows you to communicate your wishes in respect of future medical treatment but it is not legally binding. You may instead wish to make a living will, properly known as an ‘advance decision’ setting out treatment that you do not want to receive in specified circumstances, which would legally have to be followed, even if you die as a result. Any proposed gift out of your estate needs careful

consideration of the benefits, risks and implications, particularly on any future liability for care costs or tax liability. Long-term care: whether you remain in your own home or move into sheltered or residential care, you may qualify for financial assistance in the form of social care and NHSfunded care and welfare benefits. If you don’t have your own solicitor, ask family or friends for their recommendations. Contact several firms, explain your situation and ask for an estimate of cost and an idea of timescales involved. Many firms will make home visits if necessary and will adapt their communications to meet your needs. It’s important to find a solicitor who specialises in this area of the law. The Citizens Advice Bureau offers an advice service and will be able to recommend solicitors in your area.
