Welcome Welcome to the 2013/14 Wiltshire Care Services Directory which provides information about the range of social care services available in Wiltshire. At Wiltshire Council we’re committed to supporting vulnerable adults and older people. This is why we’re investing in care services which help you live independently in your own homes and communities for as long as possible. Supporting you to live independently in your own homes underpins our Help to Live at Home (HTLAH) programme. Since it was introduced in 2011, it has resulted in a number of new services which have been developed by local people, care providers and front-line staff. This has ensured the services we provide are focused on what you need and want. We understand that for some of you living in your own homes is simply not an option and living in supported accommodation or a care home is your preferred choice. Therefore, this Directory contains useful information about care homes which will help you choose the right home for your needs. It also provides details of services that support those of you who care for others. If you need any more information, please do not hesitate to contact us. Our Customer Advisors are dedicated to supporting you whether you are a customer, a carer, or someone who is looking for more information about adult care services in Wiltshire and can be contacted on 0300 456 0111.

The Council has appointed/identified a number of partners to deliver a range of services to you in your home. These companies are: Wiltshire Medical Services who provide a response service linked to telecare (assistive technology); Telecare provided by Aster Living; Community equipment and practical help in the home provided by Medequip, independent support planning and brokerage services provided by the Wiltshire Centre for Independent Living and specialist financial advice provided by Care Fees Investment and Ashcourt Rowan. Details of these organisations and the services they provide can be found in the Directory. In addition four companies have been chosen by the council and NHS to provide care and support services to older people and others who need some extra assistance to ensure they can continue to live the lives they want as independently as possible. Established in 2001, Celtic Care Services is a family owned and run domiciliary care provider. We are the domiciliary care provider of choice in Chippenham, Corsham, Calne and surrounding towns and villages. This is borne out by our customer base predominantly being made up of individuals who fund their own care or are in receipt of Direct Payments. We have also recently expanded the areas we cover to include Wootton Bassett, Swindon, Marlborough and surrounding areas with the opening of our new branch in Chiseldon. Domiciliary care is a real alternative to a residential or nursing home. We offer a wide range of flexible services including all aspects of personal care including help getting up in the morning or accessing a bath or shower, social care including assisting you to go shopping or accompanying you to groups or social activities, and also complex care for people with more individual health care needs. These services are available to children, adults and the more elderly members of our community and can be long term or for short periods; to help during holidays or just to enable you to regain your independence. First steps Wiltshire Council’s aim is to support people so they can live independently for as long as possible. We will work with customers, carers and care providers to make this happen. This guide outlines what support the council will put in place to help you. We also have staff on hand

to discuss any long-term support you may require. This will help ensure you have all the information so you can make the right decisions for your care needs. For information on care or support options your first step is to contact our customer advisors on 0300 456 0111.

Assessment If you want to take the next step and need to access services you can ask for a community care assessment. Everyone is entitled to receive an assessment regardless of their financial situation. You can be referred for an assessment by: • yourself or a relative, friend or neighbour • a social worker or health professional such as a doctor or health visitor • any other agency. You can have a carer, friend, relative or advocate

with you to help put your views and wishes forward. Your doctor, district nurse or other health professional who knows you well can also be involved if you wish. If you do not speak English, or if you use British Sign Language, we can arrange for an interpreter to be present. We will meet with you so that you can tell us what your needs are. Our assessments focus on what you want to achieve. We will encourage and assist you to contribute as much as possible to your assessment, which means we will talk through with you how best to meet your needs and choices.

Care and support planning Following your individual assessment, we will look at whether you are eligible for services paid for by the council (see below). Everyone who is eligible for support will be offered a choice of staff arranging care, or the transfer of funds which they can use to purchase their own care. This is called a direct payment. You will be offered support and assistance throughout this process either from the

council or from the Centre for Independent Living – see advertisement on page 23. We can help those who are not eligible for financial support from the council with information and advice and with assistance in finding services. Alternatively, you may wish to contact one of the local agencies listed on pages 15–23.

People in greatest need are our priority Regardless of your financial situation, you are entitled to information and advice from us about any care and support needs you may have. We follow central government guidance set out in a document called ‘Prioritising of Needs Guidance’, which has been issued to local councils across the country. The aim is to make sure all decisions on who receives financial help with care and support are made fairly, openly and consistently. This means each council has to set criteria to make sure support is allocated fairly and to those in greatest need. Eligibility criteria is divided into four bands representing the seriousness of the risk to your

independence: ‘critical’, ‘substantial’, ‘moderate’ and ‘low’. Currently in Wiltshire you are eligible for funded care and support services if your needs are assessed as ‘critical’ or ‘substantial’. This ensures that people in the greatest need will get most help. There are many council services which are not means tested, including bus passes and Blue Badges. Community living Care in your own home We know that lots of people want to stay in their own homes and we want to help you to do this. However, if you are finding it difficult to manage, you could consider using the services of a care and support or domiciliary care provider. These companies employ staff who can help you with bathing, washing and dressing, as well as practical support with shopping, laundry and cooking meals. Staff will be trained in personal care and safety procedures, moving and handling, hygiene and

infection control and first aid. If you need nursing care at home, many of these care providers employ registered nurses. These companies can also help after a stay in hospital with shopping, cleaning and preparing your house for your return and so on. Agencies providing care at home are monitored by the Care Quality Commission (CQC) which regularly inspects agencies and produces reports on their findings which can be found on the CQC’s website: www.cqc.org.uk

Equipment for independent living If you are struggling to manage everyday tasks such as bathing, climbing the stairs, or getting up from your chair, it may be that an item of equipment could make the task easier. If you would like an assessment, contact our customer advisors on 0300 456 0111, or you can contact Medequip on 01249 815052. Medequip has assessors available to advise you and

provide you with the equipment you need. Your request may be dealt with immediately by the person you talk to on the telephone, but if not they will arrange for someone to visit you and assess how best to resolve your difficulties. They may be able to suggest alternative ways of doing something which could mean there’s no need for any equipment.

Telecare and response service – keeping you safe Telecare is sometimes called ‘assistive technology’. It aims to promote your independence, improve your safety at home and provide reassurance for you and your carers. It usually consists of a base unit linked to a telephone and a personal pendant which is worn around the neck, on the wrist or attached to a belt. Sensors automatically monitor the home environment. These are linked to the base unit without the need for wiring and can detect potential dangers such as gas, smoke or overflowing water. When the alarm is activated, trained staff at a monitoring centre will respond and, if necessary, arrange for someone to visit you at any time of the

day or night. They may also contact the emergency services if necessary. This call centre monitoring and response service is provided to us through Wiltshire Medical Services. If you are interested in Telecare, we can discuss the options with you so that you can decide which sensors will assist you. Both Telecare and the response service can be purchased for a small weekly charge.

Adapting your home If you have mobility problems, you might benefit from adaptations to your home. This is another service we can provide. A case co-ordinator will be allocated to you to make an initial assessment of what you need. Major

changes will involve occupational therapists and referral for a means-tested Disabled Facilities Grant to help with some or all of the costs. If you are on a low income, you may qualify for a full grant.

Care at home agency checklist These questions may be useful when considering using the services of a domiciliary (care at home) provider to help you build up a picture of how your care needs will be met in your own home. For care packages which have been arranged by us, contracts and care plans will have been completed by your care manager and the agencies used will be accredited by the Care Quality Commission. People arranging and funding their own care, either privately or through a direct payment, can

choose their own worker or care agency. It is important that people have choice and control about how they are supported. Before deciding on which agency will support you, ask them to discuss and agree a support plan with you. This should cover the aspects to your care that are important to you. There are a wide range of activities or resources in your community and your agency should be able to discuss how you can access these so you can meet people with common interests.

Domiciliary care agencies – some things to consider: • What experience does the agency have in your particular field of need? Can they supply references for you to check? • What is the agency charging per hour? What is included in that price? • How long has the agency been operating? • How can you contact the agency in an emergency or outside office hours?

• How many carers would the agency assign to care for you and would you see the same one every day? If not, how does the staff rota operate and what happens if your carer goes on holiday or is sick? Will you be notified in advance that a different carer will be attending? • I f you’re not happy, how easy is it to make a complaint? • Home care agencies providing personal care, must be registered and inspected by the Care Quality Commission. Ask to see a copy of their registration certificate. • If this is a private contract, ask for a copy of the agency’s contract terms and read these carefully with someone you trust. Get as much help as you can going over it and ask any questions you may have before signing anything. The Citizens Advice Bureau or Age UK could help with this. Home improvement agency This agency will work with you if you are receiving a Disabled Facilities Grant (DFG) for a major adaptation to your home such as installing a stair lift or a level access shower. However, should you wish to carry out adaptations privately or are not eligible for a grant but would like

Handyperson service As finding someone to do small jobs can be difficult, we commission a handyperson service. The service can help with small jobs such as replacing a tap washer, tacking down loose carpeting to prevent trips, installing a grab rail to assist you up a step. You can access this service whether you live in your own home or in rented accommodation. There is a charge for this service, but if you receive low income benefits you may be entitled to the labour costs free of charge up to a certain amount, but, there is always a charge for materials.

Occupational therapy Occupational therapists can help you lead a more productive, active and independent life using a variety of methods, including the use of adaptive equipment. They can assess your home for hazards and point out things that contribute to falls. The reason you need occupational therapy can determine how you access the service. Shortterm conditions, for example if you need support following an operation, will mean you will access services through the NHS. Speak to one of the healthcare professionals treating you. They will discuss your needs with you and decide if you would benefit from occupational therapy. They can arrange an assessment with an Occupational Therapist (OT) as part of your care. For long-term conditions, such as a permanent physical disability, occupational therapy is usually accessed through your local council. You should contact us to arrange an assessment with an OT, or you can be referred for an assessment by: • your GP or consultant (specialist doctor) • a nurse • another healthcare professional; • a social care professional.

If you do not want to access occupational therapy through the NHS or Wiltshire Council, you could contact an occupational therapist directly. If you decide to see a private occupational therapist, make sure they are fully qualified and are a member of a recognised body, such as the British Association of Occupational Therapists (visit: www.cot.co.uk). Only healthcare professionals who are registered with the Health Professions Council (HPC) are allowed to use the title of Occupational Therapist. You can check that your OT is registered by checking the Health Professions Council online register at: www.hpcheck.org. An OT can carry out a health and social assessment to identify what areas of your everyday life are causing problems. They will discuss your needs with you and explain what help is available. Community resources There are many different resources in the various communities in Wiltshire ranging from day centres providing a hot meal, some company and activities to keep fit, to walking groups, reading groups and so on. Further information and details on what activities

Other help that is available Support for carers A carer is someone who provides unpaid support to a family member, partner, friend or neighbour. This could be because they are ill, frail, or disabled, or have mental health or substance misuse problems. Anyone can become a carer. Carers can be any age and may or may not live with you. If there is someone who helps and cares for you on a regular basis, they will be a carer. We understand that the caring role can be both satisfying and exhausting. Carers are entitled to an assessment and may be able a range of services. Respite care and short stays Carers sometimes need a break and this can be provided in several ways. 1. You can go into a care home yourself for a week or two to give the person who looks after you a break 2. Home-based respite care can be arranged when a care worker moves into your home

Support from the NHS NHS Continuing Healthcare is ongoing care arranged and funded by the NHS, when it has been assessed that an individual’s primary need is a health need. It can be provided in any setting. In a person’s own home, it means that the NHS funds all the care that is required to meet the assessed health needs. In care homes, it means that the NHS also makes a contract with the care home and pays the full fees for the person’s accommodation as well as their care. A national policy has been drawn up by central government that sets out the types of conditions that will meet the eligibility criteria for continuing care. Currently these are: • NHS Continuing Healthcare which is provided if you have extensive healthcare needs. This is free of charge. • Continuing Care is a package of care that involves services and funding from both the NHS and ourselves.

3. Family-based respite care may be arranged when you move into another family’s home for a period. The best option for you and your carer will depend on your needs, the urgency of the situation and personal choice and cost. People with mental health problems There are two locality based specialist mental health social work teams in adult care which provide support for adults with mental health social work needs. The team is responsible for completing assessments and reviews and for arranging specialist placements or services including shortterm supported housing or vocational support. The initial point of contact to access mental health services is usually your GP and assessments are carried out in conjunction with your GP and the Avon & Wiltshire Mental Health Partnership NHS Trust, the NHS mental health provider in Wiltshire. The mental health social work team is also responsible for co-ordinating assessments under the Mental Health Act 1983. For information on care or support options your first step is to contact our customer advisors on 0300 456 0111. The Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) provides specialist mental health services based on individual need which help people to find the best way to recover and stay well. The initial point of contact to access mental health services is your GP. They will discuss with you whether help and treatment is best provided from primary health services, or whether a referral to AWP for specialist services is needed. AWP offers a Patient Advice and Liaison Service (PALS) which is a free, confidential and impartial service available to everyone who uses AWP services, their families, carers or anyone who needs advice, information, support or guidance with a particular issue. PALS can help if you: • are worried and not sure what to do or where to go • need clarity about a situation • want someone to listen to your experience of services • need information and help. This offers a list of things to look for in care homes specialising in dementia. Please use it in conjunction with the care homes checklist on page 35. Residents The best indication of a good home is that the residents appear happy and responsive.  Are there rummage boxes around the home to stimulate residents?  Are residents taking part in activities?  Are residents talking to each other, staff and visitors?  Do residents look well dressed and groomed?  Do residents get to choose their clothes every day?  Are people supported to stay independent and do things for themselves e.g. getting dressed, making food etc.? Access If the person with dementia needs, or is likely to need, equipment or adaptations you may want to check that:  there are adequate signs and cues for different parts of the home such as dining room and bathroom  the home has a policy about locking external doors. Bedrooms You may want to find out whether the person with dementia can have a single room and whether residents are encouraged to bring in some of their own furniture and possessions to increase familiarity. Activities Residents should be stimulated without feeling stressed.  Are reminiscence activities available?  Are residents able to compile memory boxes?  Do staff compile life story books, including photographs and mementoes? Staff It is important to note whether staff seem friendly and caring towards residents and whether they treat residents with respect.  Do they have any training and experience in dementia care?  Will the person with dementia have a member of staff particularly responsible for their care?  Is there a member of staff you can talk to about your own worries about the person with dementia? Manager/head of home A manager who is caring as well as efficient can make all the difference to a home.  Does the manager have a knowledge of dementia and can they deal with difficulties that may arise in an understanding way?  Is there a full assessment at home before a resident is admitted?

Dementia care If you are worried that you or someone you know may have dementia, it is important that you talk to your GP as soon as possible. They will be able to rule out any other illnesses or conditions that may have similar symptoms to dementia, including depression and infections. Once your GP has checked for any easily treatable causes, then they will be able to talk to you about having an assessment. They may refer you to the memory service, which is for people who are concerned about their memory and think it is affecting their day to day life. Here an assessor will talk with you about your memory difficulties, complete some further checks and discuss the impact of any problems or issues. Sometimes getting a diagnosis can take time, but this should not put you off. If you do not feel confident about talking to your GP, there are local charities you can talk to. Alzheimer’s Support and Alzheimer’s Society also have a dementia adviser service that offers personalised information and signposting for people with dementia and their carers – see page 44 for contact details.

Support in the community As it can be best for the person with dementia to stay in their own home for as long as possible, a range of community care services is available to facilitate this. We can help you to access care in your home; sitting services, which provide a break for any carer you may have; advice on equipment and adaptations; day care centres and respite care. These services can be provided directly by registered providers to those who do not require financial help. However, we will still be willing to offer an assessment of need. You may have a choice of which agency you receive your services from, whether or not we assist with the funding of your service. Accommodation Wiltshire Council’s Adult Placement Service This service provides long-term accommodation, short-term stays, respite and day activities to: • people with a learning disability • people with a mental health difficulty • older people • people with physical disabilities, head injury or sensory impairment • people requiring ‘Home from Hospital’ services. Our service is provided in the households of approved ‘adult placement carers’ who offer support or care as required by the person placed with them. Once a person has been referred to us, usually by their care management team, we match them with an

adult placement carer who has a vacancy. A series of introductions are arranged to help them get to know each other before deciding whether or not to set up an adult placement. Our team then monitors each placement to help ensure that the arrangement is an enjoyable and fulfilling experience for all concerned. Sheltered housing continued (landlords). The landlord is responsible for the upkeep and maintenance of the building and grounds and housing services such as communal lighting and heating. You pay rent to your landlord for your home and service charges to your landlord for utilities and maintenance of the scheme in which you live. Your service charge includes the provision of an alarm service to access help 24 hours a day, seven days a week. Sheltered housing can be rented or owner-

occupied accommodation. To apply for sheltered housing, you can download the application form from the Homes 4 Wiltshire website and return it to the Homes 4 Wiltshire team. Extra care housing Extra care housing is a growing part of the housing market. It can be owned, rented, part owned/ rented (shared ownership) or leasehold. Extra care housing can broadly be described as housing into which a range of care and support services can be delivered on-site 24 hours a day. Extra care is gaining a reputation for offering accommodation and care to people who would otherwise be frequent users of acute services, largely because they cannot look after themselves in unsuitable housing. Models of extra care are various, because it is a concept rather than a

housing type. Generally, an extra care scheme is a development of flats and/or bungalows with a manager and the availability of care and support for tenants/ leaseholders 24 hours a day, usually supported by a lifeline and other technological assistance. There is normally a café or meal provision available and an additional range of services and activities which are open to tenants/leaseholders and others living in the surrounding area. Residential care including care homes that provide nursing What is a registered home? There are two types of residential home: • Care homes (personal care only) If you are reasonably active but would like greater security and care, a care home offering only personal care may be the best option. Personal care includes bathing, feeding, dressing and help with moving but it must be paid for if your capital/ savings exceed £23,250. See page 38 for more information. • Care homes with nursing If you think you may need nursing care in a home, you will need to be visited by a social worker or a care manager to determine what care you will need. This might be in your own home, or in hospital, or in a care home. You will be fully involved in planning your care needs. If a care home providing nursing care is the best solution for you, your care manager will give you information to help you find a home that meets your care requirements. The cost of the nursing care part

of your fees is paid by the NHS to the home. The current amount is £109.79 per week. It is important that you get your nursing determination done before you go into the care home. Personal care is not paid for by the NHS and must be paid for if your capital/savings exceed £23,250. See page 38 for further information. My Home Life ‘My Home Life’ is a UK-wide initiative, sponsored by Age UK, City University and Dementia UK, aimed at promoting quality of life for those who are living, dying, visiting or working in care homes for older people. Its focus is on “what makes life good in care homes now, what could make them better and how might we get there”. ‘My Home Life’ has a website which gives information and examples of good practice. www.myhomelife.org.uk Wiltshire Council as part of its commitment to working with care homes to improve quality of life has invested in the My Home Life programme. Over the past four years the council has supported more than 30 care home managers to participate in the Leadership Support Programme which gives the skills to integrate the best practice themes detailed below into the dayto-day culture of their care home. ‘My Home Life’ identifies 8 key themes that focus on what best practice in care homes for older people looks like in the 21st century. 1. Maintaining Identity – by staff learning more about residents’ lives it enables them to adopt a more person-centred approach to tailor care and support to each individual’s needs.

2. Sharing decision-making – everyone’s contribution being valued to help the care home function as a true community. 3. Creating Community – through the relationship between residents, their family, friends, staff and the wider community. 4. Managing Transitions – moving to a care home is a major transition in life which may involve considerable losses, but with appropriate planning and support can bring benefits and improved quality of life for older people and their families. 5. Improving Health and Healthcare – recognition that health is fundamental to quality of life. 6. Supporting Good End of Life – an emphasis on personal need and dignity through which the resident, their relatives, friends and staff are supported. 7. Keeping Workforce ‘Fit for Purpose’ – using research and sharing good practice to identify and meet the education and training needs of the care home workforce. 8. Promoting a Positive Culture – promoting a positive perception of life in care homes for all those involved and highlighting good practice. These eight themes provide a good starting point for conversations with care homes when making decisions.

Inspecting care services and checking quality The Care Quality Commission (CQC) registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others. Since October 2010, all care providers have been registered under a system introduced by the Health and Social Care Act 2008, which brings adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety for the first time. Following an inspection, each care home and home care agency is given a report of how it rates against national essential standards of quality and safety. Each service’s report can be seen on the CQC website: www.cqc.org.uk

Paying for care The information here has been developed by a group of public organisations and charities to help people make better and more informed decisions

about paying for care and to encourage people to get information and advice about the use of their money in this regard.

Personal budgets Personal budgets are an amount of money councils can allocate to help people get the support they want due to disability, frailty or vulnerability. Everyone who is eligible for funded social care will be allocated a personal budget once their needs are agreed.

There is no obligation for people to manage their budgets themselves, and different options are available to help plan and arrange suitable care for each individual. People will be able to use their budget through a cash payment, services arranged by us, or a mixture of both.

Direct payments If you have been assessed as needing community care services and are capable of managing a direct payment, with assistance, if necessary, you can receive a payment and organise and buy services yourself. Direct payments can be used to arrange support designed to meet your individual needs. We will agree these needs with you and write them in your care plan. A direct payment is sometimes given so that you can arrange a short break (respite) or buy any equipment that you need to stay independent. They can also be used to pay for help with: • personal tasks such as getting up, washing, dressing, going to bed etc • practical tasks such as food preparation, cooking, shopping and other related tasks to help you to live in your own home. You can choose to buy care from an agency or employ your own personal assistant. If you are a carer, you can get a direct payment to pay for support or services which will help you

to carry on caring, although you cannot use the payment to support the person you care for. Direct payments cannot be used for: • health services • permanent care in a care home or care home with nursing • services which are managed by us – such as our in-house home care service. The amount of money you receive depends on the level of your needs and the support you require to live in your own home. We take into account what it would cost if we provided the services instead. Unless you are a carer, you will usually have to pay a contribution to the cost of your services. If you are interested in learning more about having a direct payment or wish to access support with an existing direct payment, we have commissioned the Wiltshire Centre for Independent Living to provide this service. Care can involve help in your own home as well as care homes or care homes with nursing. If you, or a member of your family, need care and support now or in the future there are a number of important things you need to consider.

Seeking financial advice and help Later-life advisors Advisors specialise in the financial needs of older people. Those advisors who have taken steps to become independently accredited can offer added reassurance that they offer the practical help and guidance needed to make the right decisions at the right time. The later life advisor accreditation scheme has been developed by the standard-setting body, the Financial Services Skills Council (FSSC) in collaboration with the specialist financial services consultancy. There are three main ways of paying for advice. • Fees – by paying the advisor a fee, either at an hourly rate or a set fee. • Commission – by paying indirectly through commission, which is deducted by the product provider from the product(s) you invest in. As well as the initial commission charged when the product is taken out, there may be annual commissions, known as ‘trail commissions’. • Fees and commission – by paying a combination of fees and commission. Not all advisors offer all three payment options. Make sure the advisor offers the payment option that you want. Ask the advisor to explain what you’re paying for, how much it will cost and how you will pay for it. If you want to pay by commission, ask your advisor how this will be calculated and whether this has any effect on the advice you will be given. Make sure you understand and are comfortable with the payment arrangements before proceeding. If you’re getting investment advice, ask your advisor if the costs include a review of your investments from time to time or if you must pay for that service separately. If you don’t you may make poor decisions about either the care you organise, or about how you pay for it, or both. If you need ‘nursing care’ you may be entitled to funding from the NHS for all or part of the cost; you should ask us, your hospital or GP whether you would be entitled to this. There are different ways to pay for long-term care. If you need care and support immediately or in the near future first check with us about any support we give. We assess your care needs and those of your carers.

Seeking financial advice and help continued If your circumstances meet the conditions for support and you wish us to pay towards the cost of services, we will then assess your income and capital such as savings. If your capital and income are below certain levels (locally set if you continue living in your own home, but nationally set if you decide to move into residential care) then we will pay some or all of your long-term care costs. We will also be able to help you with information about the choices you have for meeting your care needs and how to arrange care. Many people wrongly assume that they will need residential care, whereas there may be options available to stay at home. We, your GP or other NHS services can also offer some people help to try to regain their ability to manage more independently through the use of equipment or adaptations to your home, or through programmes of rehabilitation. You should ask about this. Also check what benefits you are entitled to. Not

all benefits are means tested and you may qualify for Disability Living Allowance (DLA) if you are under 65 or Attendance Allowance (AA) if you are over 65. If you don’t qualify for financial help from us, you will normally have to pay the cost of care out of your own income and savings. If you move into residential care, the value of your home, if you own one, will be taken into account and if you seek a loan from us a charge may be placed against the property pending its sale, athough this doesn’t necessarily have to take place immediately if you don’t wish it. If you have capital or savings of more than £23,250 you will have to pay for any care services you require, but you are still entitled to discuss your needs with someone at Wiltshire Council who is trained to advise you. If you have capital or savings of less than £23,250 you may be entitled to funding from us. A financial assessment will be undertaken to determine what we consider you can afford to pay towards your care.

If you need to move into a care home, you may be entitled to some of the following financial assistance and support: Twelve-week property disregard If your former home is included in your financial assessment, but your other capital/savings is less than £23,250, and your income is not enough to meet your care home fees, we may assist with the cost during the first twelve weeks of permanent care if we agree that care is needed. Deferred payment agreements After the 12-week property disregard period, any financial help from us will be in the form of a loan and a charge will be placed against the value of your home and recovered once your house has been sold. However, we may limit how much we will pay and it may affect your entitlement to pension credit or income support if your property is not seen to be on the market as it will be treated as capital by the Department of Work and Pensions (DWP). Attendance Allowance/Disability Living Allowance (care component) These are a non-means tested, non-taxable benefits from the Department of Work and Pensions paid at

How Wiltshire Council can help continued the lower rate of £53 per week for those needing care by day or night, and at the higher rate of £79.15 per week for those needing care both during the day and night. Everyone in these circumstances can, and should, claim this benefit. This benefit will not be paid if your care home is funded by us, but can be if the financial help provided by us is a ‘loan’ pending the sale of a property. Running out of money If your capital/savings are likely to reduce to £23,250, you must let us know at least two months in advance as we will step in to help with your care fees. We must conduct an assessment of your situation and will normally make a contribution towards your care

costs. If the home you have chosen charges more than we consider is appropriate, you will need to find someone to help pay the difference – a ‘top-up’ payment. Whoever does this, whether family or a benevolent charity, they should realise that they may have to pay this for some time. Alternatively, you would have to find a cheaper home, although before this happens we will undertake a risk assessment to confirm that you are fit enough to move. Understanding your rights before moving into care is essential. There are a number of financial products and specialist companies who may be able to help. It is important to seek advice before committing yourself. Citizens Advice or Age UK can advise you further.

Essential information Keeping people safe Wiltshire Council and its partner agencies expect health and social care services to work in a way that respects people’s dignity, independence, choice and safety and to have a zero tolerance of abuse. The council and its partner agencies recognise that people can be abused physically, psychologically, financially, sexually, be discriminated against or neglected. This abuse can take place where a person lives, within a care home or day service, in hospital, in custodial situations, in a place where support services are being provided, or in a public place. It can be carried out by partners, relatives, friends, volunteers or someone employed to provide a health, housing or social care service. All organisations providing health, housing and social care services in Wiltshire should follow the Policy and Procedures for Safeguarding Vulnerable Adults in Swindon and Wiltshire (2007) and report any allegations or suspicions that a vulnerable person is being abused. Complaints, compliments and feedback If you receive care at home or in a care home you should feel able to complain about any aspect of your life there which affects your happiness or comfort. This might be anything from the way you are treated by the staff to the quality of the food you are served. You should also feel free to make comments and suggestions about possible improvements to your surroundings and the services provided and compliment staff when things go well. Making a complaint should not be made difficult for you and should not affect the standard of care that you receive. Care homes are required under government regulations to have a simple and easy to use complaints procedure. If you are concerned about the care that you, a friend or a relative are receiving in a residential home, it makes sense to speak to the manager of the home about your concerns before you take any further action. The problem may be resolved quite easily once they are made aware of it. However, if you need to make a formal complaint, you should initially contact the registered owners of the home. They have a duty to respond to any complaints made. If your complaint is about a breach of regulations, contact the Care Quality Commission:

Out of county care Did you know you can choose a care home outside your home county? You may want to be closer to friends and family members, or you may want to relocate to another part of the country. If your care home place is state-funded, your home county local authority is still responsible for your care fees, even if you choose a care home in another region. The home you choose must be suitable for your assessed needs and comply with the terms and conditions set by the authority. The fees that your local authority will pay may vary. They may offer you the fees

you would receive if you remained within the county, or they may offer you the fees that the local authority would pay in your chosen region. You must seek further advice before making your decision. If you’re self-funded, obviously you have freedom of choice. How solicitors can help A solicitor can give you impartial advice about wills, making gifts, estate planning and Powers of Attorney (PoA). Some can also offer guidance on immediate and long-term care plans, ensuring (if applicable) the NHS has made the correct contribution to your fees. Lasting Powers of Attorney (LPoA) allow you to appoint someone you trust to make decisions about your personal welfare, including healthcare and consent to medical treatment, and/ or your property and financial affairs. An LPoA is only valid once registered with the Office of the Public Guardian. It allows for a person of your choice to make decisions on your behalf at a time when you may be unable. The Court of Protection can issue Orders directing the management of a person’s property and financial affairs if they are incapable of managing their own affairs should they not have a LPoA. The Court procedure is presently very slow and the fees are quite expensive so preparing a LPoA is always advisable, providing you have somebody sufficiently trustworthy to appoint as your attorney. An ‘advance directive’ allows you to communicate your wishes in respect of future medical treatment, but it is not legally binding. You may instead wish to make a living will, properly known as an ‘advance

advertorial decision’ setting out treatment that you do not want to receive in specified circumstances, which would legally have to be followed, even if you die as a result. Any proposed ‘Gift’ out of your estate needs careful consideration of the benefits, risks and implications, particularly on any future liability for care costs or tax liability. Long-term care: whether you remain in your own home or move into sheltered or residential care, you may qualify for financial assistance in the form of social care and NHS-funded care and welfare benefits. If you don’t have your own solicitor, ask family or friends for their recommendations. Contact several firms, explain your situation and ask for an estimate of cost and an idea of time scales involved. Many firms will make home visits if necessary and will adapt their communications to meet your needs. It’s important to find a solicitor who specialises in this area of the law. Care Associations in Wiltshire The Wiltshire Care Partnership

The Wiltshire Care Partnership is an umbrella group that represents the interests of independent providers of residential and nursing care and works to drive up the quality and safety of services available for older people in the county. It is a new independent, member-led organisation that has been established through the combined efforts of independent providers of care for older people in Wiltshire and the Council. Almost 50% of independent providers have joined WCP so far making it the largest single representative organisation of its kind ever in Wiltshire. WCP represents a coming-together of commissioners and providers of care and means that there can be much better sharing of information and greater understanding of what

kind of care is needed now and in the future. We will be better able to identify best practice and make sure that we use what we learn to improve services for everyone. WCP is developing services that will assist and strengthen independent care businesses as well as offering mentoring and guidance to them. The Wiltshire Care Association The Wiltshire Care Association (WCA) was formed in 1984 by care home owners who were determined to set and maintain standards of care and accommodation in their homes, well above the standards set by legislation in force at that time. Its members are now drawn from all sectors that provide all levels of care mainly for elderly persons from Wiltshire and other counties. WCA is the only care association specific to Wiltshire and gives active support to its members, advising them on statutory matters such as government regulations, Criminal Records Bureau (CRB) and Vetting and Barring Scheme (VBS) checks and assisting in staff training and other activities to promote the provision of quality services. Association members through their elected committee nominees liaise, meet and discuss care related matters with the CQC, social services care managers, Local Authority officers, Wiltshire Council, Primary Care Trust officers, vulnerable adults units and other caring agencies within the county, talking to them about their first-hand experience

of the frailties of some procedures and legislative interpretations. Committee members regularly sit on committees and meetings that plan, monitor, contract and pay for care. WCA is affiliated to the National Care Association (NCA) which enables members to have their opinions and experiences discussed and brought to the notice of central governmental agencies and committees; it also keeps all members fully up-to-date on a wide variety of care topics through six weekly newsletters. The last few years have brought an incredible number of changes to our routines, legislation, training requirements, standards and trading conditions and the administration of our homes.
