Welcome from Worcestershire County Council Welcome to the 2014/2015 Worcestershire Care Services Directory which provides information for adults about the range of social care services that are available across the county. Some are provided directly by Worcestershire County Council but many are provided by voluntary, charitable and private organisations who support thousands of adults with social care needs. Some of these people are funded by the Council and many others pay for their own care. Nationally, local government is having to respond to the new Care Act, which comes into effect from April 2015, and to make substantial savings. This means that the way that services are organised and arranged by the Council will need to change. The Care Act is the most significant piece of legislation in adult social care for 75 years, and the Directorate of Adult Services and Health will need to make savings of at least £32 million by 2016/17. We are addressing this challenge proactively with the ‘Future Lives: Pathways to Independence’ programme, which will review all aspects of adult social care. It will ensure that we meet requirements of the Care Act, and other national legislation, and that we can continue to meet the needs of service users and carers with the money we will have available in the future. We will work with service users, carers, the voluntary sector, our partners and the wider public to understand how our resources can best be used and to redesign services.

‘Future Lives’ aims to: • Keep people healthy and independent for as long as possible and reduce the need for adult social care. • Give service users greater choice over the services they use and greater control over their lives. • Make sure that services are safe and effective. • Achieve the savings the Council needs to make. • Make sure the Council complies with its legal duties.

This last project means that we expect existing web and information resources to evolve over the next 12 months into an e-marketplace, which will include details of all of the voluntary, charitable and private social care services, along with information about quality and cost, feedback from service users and carers, and over time the ability to buy services online.

‘Future Lives’ is part of an overarching programme, ‘Well Connected’, which is being run with the three Worcestershire Clinical Commissioning Groups (CCGs), Worcestershire Health and Care Trust and Worcestershire Acute Hospitals Trust. Together the partners are aiming to further integrate adult social care and health services and improve the experience of service users and patients whilst addressing the growing financial challenge across both the Council and the NHS. The vision for service users and patients is that: ‘I can plan my care with people who

‘Future Lives’ involves a number of projects and workstreams: • Keeping Well project will look to offer high quality information and advice on a new website, in a format that is easily accessible, and with support for people to use the internet where necessary. It will also look to promote volunteering to help vulnerable people, as well as re-commissioning a range of prevention and early help services to reduce the need for social care. • The Integrated Recovery project will develop new integrated intermediate care services across adult social care and health that help keep older people out of hospital, offer rehabilitation and re-ablement after a hospital stay and avoid or delay the need for long-term care. • The New Models of Care project will develop a new assessment and case management process, including the option of a supported self-assessment online. A key part of this will be to embed personalisation, and ensure that people entitled to funding from the Council have a personal budget, which they can use flexibly along with their own money, to control their care. As part of this we want to expand the range of social care services so that people have a greater choice.

Self-Directed Support Self-Directed Support is designed to help you take control of your own social care personal budget, manage your own support and choose the services that suit you best (using the money in different areas of need more flexibly). You are at the centre of the support planning process as you are best placed to understand your needs. As the name suggests, Self-Directed Support allows you to design and tailor your own support package to meet your unique requirements. In Adult Services and Health we are proud to be leading the way in providing

targeted and personalised services that will really help to improve quality of life for our citizens. Gone are the days of a ‘one size fits all’ approach, where standard packages were the norm, and you had to choose from a set number of available options. Self-Directed Support means, as the name suggests, that you can control your own care. Self-Directed Support recognises that people who use our services are the experts on their own needs and Self>> Directed Support will give more choice and control

When eligible for a Personal Budget (see below), you will be able to choose the support that best meets your outcomes as identified in your Personal Support Plan. Whereas previously you may have been limited to traditional options such as visits to a day care centre, or home help, Self-Directed Support offers a wider range of options. Perhaps you would like to keep your independence by funding transport to attend a social group every week or to visit a relative who you may have lost touch with otherwise? Or maybe you would have a better quality of life if a course at a local college could be attended to learn a new skill, or even have some help in getting out and about to keep active? Supported from the start People on Self-Directed Support can access support with every stage of the process - from filling out an assessment form, to completing a financial assessment, writing the support plan to meet their identified outcomes, finding

the services or activities and, if needed, help with the management of a Personal Budget. A Personal Assessment will identify if you meet the eligibility criteria for a Personal Budget. The Personal Support Plan will be written with you, confirming what the Personal Budget will be used for and how it will meet your identified social and care outcomes. A Personal Budget can be managed in three ways As a Direct Payment This is when you manage your own Personal Budget, receiving it as a Direct Payment, and choosing where, when and how to purchase your support and services as identified in your Support Plan. You will need to shop around and make comparisons between the different goods and services available, or you may ask a family member, friend or third party to do this for or with you, possibly using web information resources like Carewise (www.carewiseworcestershire.org.uk). Fully-Supported You ask that we purchase all your support and services on your behalf using your Personal Budget. Combined This is when you have a mixture of fully supported services and a Direct Payment. We and you agree and share the responsibility of purchasing the support and services that have been agreed using the Personal Budget that has been allocated to you for your support.

Eligibility for Social Care support - Fair Access to Care Services (FACS) The Government has clear guidelines for deciding who is eligible to receive social care support from their local authority. The eligibility criteria are based on Government guidance called ‘Fair Access to Care Services’ (FACS). The aim of this guidance is to make sure that local authorities make decisions about who gets social care support in a consistent way. The FACS guidance directs us to talk with you, your family, your carers and any other relevant health or social care professionals to help us determine your needs for social care support. We will assess your ability to manage your own personal care, including any risks to your health and well-being, taking into account the care you currently receive. If the risk to your independence is deemed ‘critical’ or ‘substantial’ under the Government’s FACS criteria you will be entitled to social care support if you need it.

Following this Personal Assessment, and agreed eligibility for social care support, we will do our best to work with you to use your Personal Budget/Direct Payment to devise a Personal Support Plan to meet your needs. This Support Plan will be a personalised care package that may involve a range of service-providers supporting you in flexible way that most meets your individual needs. Our aim is that you are in control of this Support Plan and you choose how, when and who delivers your care. The assessment process You can contact us for information and advice by calling the Worcestershire Health and Social Care Access Service (WHASCAS) on 0845 607 2000 (8:30am to 8:30pm seven days a week) all calls are charged at local rates. Alternatively you could write to WHASCAS, PO Box 585, Worcester WR4 4AD or visit any of the Worcestershire Hub Customer Service Centres. If you are unable to do this you can send emails to socialcare@worcestershire.gov.uk, a fax to 01905 768052 or a text message to 07939 572850. If you find this difficult you can ask a friend, relative or your doctor to do this on your behalf. Following a discussion with one of the Access Service team you may be given information and advice, or a referral made on your behalf to an appropriate team. If a referral is made then a social worker or health care worker will contact you, either at home or in hospital to discuss your situation and determine your care needs. This process should only happen once, as your personal details will (with your permission) be shared with health and housing services. There is no charge for this assessment. The assessment will consider personal circumstances such as:

• how you can best be supported to live as independently as possible; • your home and how you are coping in it; • your emotional well-being; • your diet; • any health and medical treatments you need; and • your financial status and any benefits you may be claiming. Once we have an understanding of your needs, the next stage is to determine what services you are eligible for and who will pay for them. If you have been admitted to hospital and either you, your family, your carers or the ward staff think you need care on discharge they may refer you to the discharge team for an assessment. The team is multi-professional, made up of social workers, carers’ officers, mental health workers and housing officers. Your assessment will take place on the ward and a plan of care developed with you and your family or carer to facilitate your discharge. Habito gives you greater choice and control during your day and means you can enjoy much more flexibility.

The assessment process continued >> to admission or provide the appropriate service for discharge. Sometimes interim care packages are set up to facilitate your discharge home and will be reassessed by the community social worker within four to six weeks. We do not normally arrange for people to go into residential care straight from hospital as this needs to be a considered option and arrangements planned, but if

you need a care home we will ask our brokerage team to identify appropriate homes for you and your family to view. If your home of choice has no vacancies then sometimes an interim placement may be appropriate until you can move to your permanent home. A financial assessment will be requested. Information on paying for care can be found on page 38.

What care is available? Care in your own home If you are frail, have been ill, or have just come out of hospital, the short-term support of a home care assistant could be welcome. Known as ‘domiciliary care’, it is available to those who are elderly as well as people of any age with a physical or learning disability or with a significant mental health issue. These services also give valuable help and support to parents of children with disabilities and carers should they need it. Domiciliary care services are provided by: • Worcestershire County Council; • independent agencies; • the voluntary and community sector; and • personal assistants (using Direct Payments). All domiciliary care organisations and agencies that provide personal care must be registered with and inspected by the Care Quality Commission (CQC) who inspect performance against essential standards of quality and safety and that staff are appropriately trained to do the job. Domiciliary care staff will work with you to help you regain and retain your independence. Some of the tasks they will do include: helping you get in and out of bed, dressing and toileting. For those who need it, they will help you regain confidence in tasks such as food preparation. All staff have received specific training in areas such as moving and handling, first aid and food hygiene. All staff also have a background check with the Disclosure and Barring Service. The amount of care you receive each week will be dependent on your assessed needs. We will work with you to find out how much support you need. This is called a ‘Personal Assessment’. Depending on the level of help required, care assistants’ visits can be as short as fifteen minutes or up to several hours throughout the day. Generally, visits are available

between the hours of 7am and 10pm. Night services can also be provided. The hourly rate for these types of services depends on the required tasks, whether the call is on a weekday or weekend, the time of day and the location of the visit. Additional information can be accessed on the Carewise website at www.carewiseworcestershire.org.uk Paying for domiciliary care You won’t have to pay for any assessment or any visits or advice from your social care worker. However any personal care that you receive will need to be paid for. The exact cost will depend on your personal finances. This will need a confidential discussion with a Care Contribution Assessment Officer who will look at your income and expenditure. Currently, a capital threshold of £23,250 applies (this may change after April 2015). If you have less than this, a detailed calculation will determine what you may need to pay. If you have more than £23,250 you will have to pay the full costs. You may also be offered the choice of Direct Payments (also discussed on page 38), if you are considered eligible, as an alternative to receiving part or all of these services. If you think this service will help you to remain in your own home and keep your independence, then please telephone 0845 607 2000 and we will refer you to the appropriate team.

These questions may be useful when considering using the services of a home care (domiciliary) provider to help you build up a picture of how your care needs will be met in your own home.

If you are arranging and funding your own care either privately or through Direct Payments or a Personal Budget, you are able to choose your own worker or care agency.

For care packages which have been arranged by us, contracts and care plans will have been completed by your care manager and the agencies used will have been accredited to work for us.

Agencies • What experience does the agency have in your particular field of need? Can they supply references for you to check? • Home care agencies providing personal care, must be registered and inspected by theCare Quality Commission. Ask to see a copy of their registration certificate. • How long has the agency been operating? • How many carers would the agency assign to care for you and would you see the same one every day? If not, how does the staff rota operate and what happens if your carer goes on holiday or is sick, will you be notified in advance that a different carer will be attending?

• How can you contact the agency in an emergency or outside office hours? • How hard or easy would it be to make a complaint and how are things then put right? • If this is a private contract ask for a copy of the agency’s contract terms and read these carefully with someone you trust. Get as much help as you can going over it and ask any questions you may have before signing anything. The Citizens Advice Bureau or Age UK could help with this.

Carers • You should expect your personal preferences, dignity and privacy to be respected. Discuss this with the agency and ask how the most suitable carers for your particular care are chosen. Can you talk to them before deciding?

• Carers should be fully trained or be in ongoing training. Ask the agency about their policies on this. • You will have a care plan drawn up by the agency which the carers will work to. Ask how often this plan will be reviewed by the agency. • Carers must be checked with the Disclosure and Barring Service and have a criminal records disclosure – make sure this is the case.

Paying • If your care needs do not meet our eligibility criteria and have been arranged privately you should check carefully the fee rates charged and exactly what the payment you make covers.

Assistive Technology (Telecare) The range and diversity of equipment is growing and ‘Telecare’ no longer describes what is available to health and social care staff in supporting service users, carers and patients. Assistive Technology is the new term that covers a wide range of technical equipment that supports service users/patients/carers. You have probably heard of Telecare which is a generic name for products and services that can: • alert a monitoring centre that something has gone wrong and ensure appropriate action is taken – for instance, alerting a family member or the ambulance service – this is based on the call/pendant alarms and has been used traditionally to alert someone when the person wearing the pendant has fallen. However, there are now other things that be monitored with bed or chair leaving sensors;

to facilitate ‘safe walking’ and enable the carer to know where someone is. In Worcestershire, the range of equipment available would be subject to a Personal Assessment under the ‘Fair Access to Care Services’ (FACS) guidelines (discussed on page 6). If you do not meet the eligibility criteria for a Personal Budget for telecare equipment, or if you choose to fund the equipment yourself, many of the items can be purchased directly from a supplier. In the case of a Telecare alarm and additional items of equipment, these can be purchased directly from the Call Centres that operate through the county. Access to Telecare services in Worcestershire can be made directly through our Health and Access Centre on 0845 607 2000 or through a social worker.

Respite care and short stays We appreciate that caring for someone can be very rewarding but also at times quite stressful. It’s important to us that we support your well-being as a carer to ensure you fulfil your caring commitments without putting yourself under unnecessary pressure or stress. To do this we support carers to take short breaks (sometimes referred to as ‘respite care’). Short breaks can be provided in a number of different ways: • A few hours break on a regular basis through our Flexible Breaks Service. • The person you care for staying for a few days in a care home with the advantage of staff available day and night.

• Support in the home. You may prefer helpers to come to the home of the person you’re caring for. This will depend on how long you will be away and the needs of the person you care for. • With local families in their home offered by our Shared Lives Service. • A Direct Payment so that you can organise and purchase your own short break services tailored to the needs of the person you care for. How do I arrange a short break or respite? You need to contact the Health and Access Centre on 0845 607 2000 or visit the Health and Access Centre website and request a carer’s assessment for yourself and a personal assessment for the person you care for. These assessments will look at whether you and the person you care for are eligible for services and take into account the needs of both you and the person you care for. They will look at what support you both need. Looking at the need for short breaks in order to support you to continue to care will be one element of these assessments. Is there a charge for short breaks services? There is no charge for the flexible breaks service but some short break services may be subject to a charge. A Contribution Assessment Officer will visit to discuss with you and the person you care for, how much someone will need to contribute towards the cost of their care, and to ensure that all the benefits that both you and the person you support are entitled to, are being claimed.

Supporting carers Many people have an informal carer living with them, or perhaps living nearby, who provides care on a regular basis. Looking after someone whether they are family, friend or a neighbour can be very satisfying but it can also leave you feeling isolated, stressed, exhausted and in need of support. Information and advice needs to be available to all carers in Worcestershire. ‘Worcestershire Association of Carers’ provides a countywide information, advice and support service if you are a carer. They offer a telephone helpline where questions can be answered and information given on services and support available if you are a carer. Importantly, ‘Worcestershire Association of Carers’ also provides an independent voice for you. For further information and support please contact 0300 012 4272.

Intermediate care Intermediate care is provided by a range of services to promote faster recovery from illness, prevent unnecessary acute hospital admissions and support timely discharge from hospital. It is a programme of therapy, care and treatment, usually lasting no more than two weeks, designed to help you to become as independent as possible. The structured programme of support may mean that you do not have to go into hospital at all, or may enable you to get home faster following an admission to hospital. Intermediate care is designed to rehabilitate you and to help you manage by yourself. Community intermediate care provided by the NHS is free and is provided in your own home. The Directorate of Adult Services and Health and Worcestershire NHS also work together to support you to recover from illness at home rather than being admitted

into hospital. We can provide services in nursing/care homes when 24 hour care is necessary to be able you to be monitored effectively and usually lasts for no more than two weeks to allow for recovery and return home. The Urgent Promoting Independence Service is currently provided by Worcestershire County Council in partnership with Worcestershire Health and Care Trust.

Promoting Independence (PI) The Promoting Independence Service provides short-term assessment and reablement to enable you to live as safely and independently as possible within your own home and to reduce the need for ongoing care. The service is available between 7am and 10pm, seven days a week, covers all geographical areas within Worcestershire and is free of charge to service users. The team supports independence with activities such as washing, dressing, managing medication, preparing drinks and meals and getting around your home safely.

The PI Service may support you to return home from hospital or from a temporary residential/nursing home placement or, for example, after a key event such as a fall. The team consists of team leaders, occupational therapists, physiotherapists, coordinators and PI assistants who work closely together to help service users achieve their desired outcomes. If it is identified during your time with the PI service that you need some continuing support, the PI team will liaise with health and social care services on your behalf, as appropriate.

Urgent Response The Urgent Response service provides short-term homecare that is free of charge for up to 72 hours. The service supports people who are in crisis, who need support with daily activities such as washing, dressing, preparing drinks and meals and getting around your

home safely. The service also provides a ‘Carer’s Back-Me Up Service’ which supports people whose main carers are unable to support them because they have been admitted to hospital, for example. A health/social care worker will help identify your longer term needs.

Continuing care Continuing care is care provided over an extended period of time to a person aged 18 or over to meet physical or mental health needs which have arisen as the result of disability, accident or illness. If you need continuing care, your care needs will be complex, substantial and ongoing, caused by a disability or chronic illness, or following hospital treatment. Continuing care is also commonly known as ‘long-term care’. Continuing care is provided by: • healthcare services from the NHS, and/or • s ocial and community care services from your local authority. Health services are provided through NHS organisations such as Clinical Commissioning Groups, and the Health and Care Trust. Continuing care services may be provided by the NHS or Worcestershire County Council in various places, for example, in hospital, in a care home or care home with

nursing, in a hospice, or in your own home. What is NHS continuing healthcare? NHS continuing healthcare is the name given to a package of care which is arranged and funded solely by the NHS for individuals outside hospital who have ongoing health care needs. You can receive NHS continuing healthcare in any setting, including your own home or in a care home. NHS continuing healthcare is free, unlike support provided by local authorities for which a financial charge may be made depending on your income and savings. If you are found to be eligible for NHS continuing healthcare in your own home, this means that the NHS will pay for healthcare (e.g. services from a community nurse or specialist therapist) and associated social care needs (e.g. personal care and domestic tasks, help with bathing, dressing, food preparation and shopping). In a care home, the NHS also pays for your care home fees, including board and accommodation.

Continuing care continued Who is eligible for NHS continuing healthcare? Anyone over 18 years of age assessed as having a certain level of care needs may be entitled to NHS continuing healthcare. It is not dependent on a particular disease, diagnosis or condition, or on who provides the care or where that care is provided. If your overall assessment of care needs shows that you have a ‘primary health need’ you should be eligible for NHS continuing healthcare. Once eligible for NHS continuing healthcare, your care will be funded by the NHS, this is however, subject to regular reviews, and should your care needs change, the funding arrangements may also change. Whether someone has a ‘primary health need’ is

assessed by looking at all of their care needs and relating them to four key indicators. NHS-funded Nursing Care If you qualify for NHS-funded Nursing Care, the NHS is responsible for arranging and funding nursing from a registered nurse in a care home which is registered to provide nursing care. This nursing care cannot be provided by local authorities. The NHS funds this nursing care through a payment called the Funded Nursing Contribution (FNC). Sheltered (or retirement) housing This is a practical step if you would like more security and companionship but don’t need to move into a care home. In a sheltered (or retirement) housing scheme you would live independently in a flat or bungalow built especially for older people within a larger complex, sometimes around a garden or communal facility and linked to a central control

centre for security. Some schemes have access to a resident or nonresident manager who provides support, advice and a link to services for older people. After your needs have been assessed, you will still be able to receive care in the home if required.

Extra Care This type of housing offers older people who need a higher level of support the chance to continue living independently within their own self-contained properties. Unlike ordinary sheltered housing, Extra Care schemes offer 24-hour care and support from social and health care teams, provided in safe surroundings managed by professional staff. Each older person is assessed first to work out how much care they need. Extra Care schemes offer domestic support, personal care where necessary, meals and recreation facilities.

Types of care homes All care providers must be registered with the Care Quality Commission (CQC). Care home listings start on page 42 in this Directory. All services are inspected by the CQC, who report on their findings. These inspection reports are available from the service or from the CQC. There are two types of care home:

NHS will fund. The cost of the nursing care part of your fees is paid by the NHS to the home (see page 24 for further information). Figures mentioned here may change over the lifetime of this Directory.

Care homes (personal care only) If you are reasonably active but would like greater security and care, a care home offering only personal care may be the best option. Personal care includes bathing, help with eating, dressing and support with mobility, but it must be paid for if your capital/savings exceed £23,250. See pages 38 - 41 for more information on paying for care.

The Care Services Quality Team (CSQT) The CSQT has been set up to improve care home services in Worcestershire by providing support and guidance where required. The team work alongside managers and staff to identify areas which need improvement and to formulate action plans to make changes.

Care homes with nursing If you think you may need nursing care in a home, you will need to be visited by a social worker or a registered nurse to work out what care you will need. This visit might be in your own home, or in hospital if you’ve been ill, or in a home. You will be fully involved in planning your care needs. If a care home providing nursing care is the best solution for you, the social worker will give you information to help you find a home that meets your care requirements. A specially trained nurse will assess your nursing needs to determine what level of nursing care the

for state funding, you will need to contact the local authority in which your care home is located, as you will no longer be a resident of Worcestershire. Specialist care services If you have additional or special needs, support and care services offer a range of options aimed at maintaining your independence by living in your own home and community. Connect ‘Connect’ services provide short-term support to adults in the community who have a disability, or who are an older adult. The services, which are free of charge, aim to identify leisure, education and/or employment opportunities within the local community, and increase your confidence to attend these independently. ‘Connect’ also provides ‘travel development’ which aims to increase your ability to travel independently, and weekly drop-ins offering opportunities to meet with the others. Referrals can be taken from social and health care professionals, private and voluntary agencies and you can refer yourself via the Health and Social Care Access Service (telephone 0845 607 2000). Learning disability If you are considering moving from your family home, there are a number of options that could be explored before you consider residential care. You may like to consider living with other people in a supported living environment and a range of options are available, such as living with a small group or a larger group of people, with whom you are compatible, sharing elements of the property, but having your own bedroom and space. You will have a choice of support and care according to your needs. The Shared Lives Service also provides an alternative to residential care. These options can often give you greater independence and choice to live your preferred lifestyle, and can offer more flexibility than a residential care home. Many adults with learning disabilities can live in their own homes in their local communities with support but there may be times when an individual has specialist health care needs or behaviour that is particularly challenging when a care home may be more appropriate. Care homes for people with learning disabilities often

provide a specialist service e.g. homes for people with autism and challenging behaviour or dementia. A good home will provide support to ensure skills are maintained and developed, and provide opportunities to engage in enjoyable and stimulating activities. Information and support to enable you to make the right choice can be received from both social workers and nurses in the Community Integrated Learning Disability Teams in the Directorate of Adult Services and Health. Social Care support for people over 65 and people over 18 with a physical disability If you feel that you require support with your social care needs, you may request an assessment from one of our Social Work Teams. Following this Personal Assessment, and if you are eligible under the FACS criteria, there are many types of services available, including personal care, day opportunities, occupational therapy, adaptations to your home and/or a Carers’ Assessment for the person caring for you. See page 6 for further information about assessments. Support will usually come in the form of a weekly Personal Budget that you can take as a Direct Payment to purchase the support you need. This support can be personalised to your specific needs and can be provided by the private, independent and not-for-profit sectors as well as ourselves. There are a wide range of organisations, including domiciliary care agencies, in Worcestershire that provide support with personal care, day opportunities, equipment etc. See page 9 for more information. You will also have to have a Financial Assessment as you may have to make a contribution to the cost of your social care support, although the amount will vary depending on your income and savings. Adult mental health Your local Community Mental Health Team (CMHT) should be your first point of contact if you have mental health issues that require specialist secondary services. Specialist care services continued >> both health and social care interventions and support, focusing on the needs of people experiencing a severe and/or enduring mental illness. The services are recovery-focused and work alongside individuals (and their carers) to enable them to manage their mental health and assist in building a meaningful and satisfying life. There is an increased need for a range of community support for people with mental health issues. The Reablement team within the secondary service is able to help people achieve a wide range of goals and outcomes including extending social networks, building confidence and self-esteem and developing individual living skills You may require other forms of community support and if you are assessed as eligible for a funded social care service, this can be paid for via a Personal Budget and people can opt to take this as a Direct Payment as discussed on page 38. Dementia People with dementia can expect to have the quality of their lives improved by person-centred care provided in care homes and by home care agencies. Person-centred care ensures that you are recognised and treated as an individual, taking into account your own unique history and life story. Care should then be tailored to meet your needs with an emphasis on your interests, likes and choices.

Residential dementia care checklist

Care homes offering person-centred dementia care should also provide a range of activities which promote a sense of well-being and purpose for the residents on a day to day basis. The home should provide a homely and comfortable environment which allows residents freedom of movement within a safe garden. Residents and carers should be involved in the planning of care, with relatives still able to continue to be part of the care process as they wish. Residents should have a wide choice of food which promotes and maintains their continued good health. We have initiated a program of quality improvement and training to support homes in providing personcentred care. The Care Services Quality Team (CSQT) is supporting care homes to specifically improve the standard of person-centred dementia care, and has been involved with developing an observational framework for measuring the ‘lived experience’ in a care home. See page 25 for further information on CSQT. We have commissioned specialist dementia training which can be accessed by all care homes and the CSQT provide support for them to make any changes. Residents The best indication of a good home is that the residents appear happy and responsive. • Are there rummage boxes around the home to stimulate residents? Access If the person with dementia needs or is likely to need equipment or adaptations you may want to check:

• Are residents able to compile memory boxes? • Do staff compile life story books including photographs, mementos? Staff It is important to note whether staff seem friendly and caring towards residents and whether they treat residents with respect. • Do they have any training and experience in dementia care?

• There is adequate signage and cues for different parts of the home such as dining room and bathroom.

• Will the person with dementia have a member of staff particularly responsible for their care?

• The home’s policy about locking external doors?

• Is there a member of staff you can talk to about your own worries about the person with dementia?

Bedrooms You may want to find out whether the person with dementia can have a single room and whether: • Residents are encouraged to bring in some of their own furniture and possessions to increase familiarity. Activities Residents should be stimulated without feeling stressed. • Are reminiscence activities available?

Manager/head of home A manager who is caring as well as efficient can make all the difference to a home. • Does the manager have a knowledge of dementia and can they deal with difficulties that may arise in an understanding way? • I s there a full assessment at home before a resident is admitted?

Essential information Inspection and registration of care services All care providers must be registered under a system introduced by the Health and Social Care Act 2008, which brings adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety. The Care Quality Commission (CQC) is the independent regulator of health and social care in England. It regulates care provided through a range of methods including: • registering providers to ensure they are meeting the essential standards of quality and safety; • monitoring how providers comply with the standards; • promoting improvement in services; • using enforcement powers where necessary; and • publishing up to date information about the compliance of adult social care providers with essential standards of safety and quality.

Following an inspection, each service is given a report of how it rates against national essential standards of quality and safety. Each service’s report can be seen on the CQC website: www.cqc.org.uk. All homes will be inspected on an unannounced and yearly basis. Comments, compliments and complaints All organisations need to know how they are performing. They are happy to receive your feedback on their service whether they are compliments or complaints. Feel free to tell them what you think and your comments can be used constructively to improve the service. If you or your relative’s care is being funded or part funded by us then you can complain directly to us about aspects of the care provision that you are unhappy with. It is expected that you would initially have complained to the manager of the service first, however, if you do not feel that they have fully dealt with your concerns then you can contact our Consumer Relations Officer, Resources Directorate on 01905 766365, or write to Consumer Relations Unit, Resources Directorate, County Hall, Spetchley Road, Worcester WR5 2NP. If you do need to make a complaint you should feel able to complain about any aspect of your care which affects your happiness or comfort. This could be about

the way you are treated by a staff member or the quality of the food you are served. You can also make comments and suggestions about possible improvements to your surroundings and the services provided. Providers are required under essential standards of quality and safety to have a simple and easy to use complaints procedure which they will be happy to give you. If you are concerned about the care that you, a friend or a relative is receiving, it makes sense to speak to the manager before you take any further action. The problem may be resolved quite easily once they are made aware of it. However, if you need to make a formal complaint, you should initially contact the registered owners of the service. They have a duty to respond to any complaints made. If your complaint is about a breach of regulations, contact your local office of the Care Quality Commission, see above for contact details.

Safeguarding adults Safeguarding adults is the term used to describe all work to help adults with care and support needs stay safe from significant harm. When an alert is raised regarding concerns that an adult with care and support needs may be a victim of abuse or neglect, a process is followed that can include a strategy meeting or discussion, an

investigation, a case conference, a care/protection/safety plan and monitoring and review arrangements. This is a multi-agency process, led by Adult Services and Health. The adult with care and support needs will be involved in deciding how to respond in a safeguarding situation. Abuse or neglect can happen anywhere and by

How solicitors can help A solicitor can give you impartial advice about wills, making gifts, estate planning and Lasting Powers of Attorney. Some can also offer guidance on immediate and long-term care plans, ensuring (if applicable) the NHS has made the correct contribution to your fees. Lasting Power of Attorney (LPA) An LPA allows you to appoint someone you trust to make decisions about your personal welfare, including healthcare and consent to medical treatment, and/or your property and affairs. An LPA is only valid once registered with the Office of the Public Guardian. The Court of Protection The Court of Protection has powers to: • decide whether a person has capacity to make a particular decision for themselves; • make declarations, decisions or orders on financial or welfare matters affecting people who lack capacity to make such decisions; • appoint deputies to make decisions for people lacking capacity to make those decisions; • decide whether an Lasting Power of Attorney or Enduring Power of Attorney is valid; and

• remove deputies or attorneys who fail to carry out their duties. Long-term care Whether you remain in your own home or move into sheltered accommodation or a care home, you may qualify for financial assistance in the form of social care and NHS-funded care and welfare benefits. If you don’t have your own solicitor, ask family or friends for their recommendations. Contact several firms, explain your situation and ask for an estimate of cost and an idea of timescales involved. Many firms will make home visits if necessary and will adapt their communications to meet your needs. It’s important to find a solicitor who specialises in this area of the law. Paying for care Personal Budgets A Personal Budget is the weekly amount of funding following an assessment and identification of eligible social care needs. The amount of the Personal Budget depends on the eligible needs identified in the Personal Assessment. This is the way in which an adult social care service is offered. The assessment looks at what is working well and what isn’t working so well in your life and then allocates funding where more support is required to meet the eligible needs. A Personal Budget will only be finally agreed once we have worked with you to ensure you have achieved as much independence in your life as possible. Once the amount of the Personal Budget is agreed the aim is that you will have greater choice and control and flexibility in the way you receive services and are supported. There are three main ways in which the Personal Budget can be taken; you can choose to: • take a Direct Payment – take the money allocated and purchase services directly; • have the budget fully supported – ask us to manage the budget and purchase services; or • have a combination of both. More information is on page 39.

The advantages and disadvantages of each option will be discussed with you (or the person representing you). If you need support to manage the Personal Budget you can get this from: • family members; • a trust or financial services; • a Deputy or Appointee; • a suitable person; • a provider who can provide an individual service fund and hold the money, which can be drawn on when required; or • the local authority. You may be eligible for a Personal Budget if you have a learning or physical disability, mental health needs, are an older person, or a carer. The amount identified will depend on the results of the Personal Assessment or Carer Assessment.

Direct Payments If you choose to take the Personal Budget as a Direct Payment this means that you have the money paid directly into a separate bank account enabling you to arrange and pay for your own services that meet the outcomes identified through the assessment and support planning process. Direct Payments should be offered to everyone who is eligible to receive them, as the preferred way in which the Personal Budget can be taken. The Government is really

keen that Personal Budgets should be managed by the individual through a Direct Payment. Why? This is because they offer the most flexibility and independence to manage support needs. This means you can choose who provides support; when support is delivered and how the support is delivered. You may want to employ your own personal assistants or you can

Direct Payments continued go directly to a provider which means the contract is between you and them. This gives far more control over how support is delivered. When taking a Personal Budget as a Direct Payment, you will be given money to meet your outcomes, however services you buy should not cost more than they would if they were provided directly by us. A financial assessment will be required and you may be asked to contribute towards the cost of your services. Direct Payments are paid directly into a separate bank account specifically for this purpose. You will be asked to sign an agreement, a bit like signing to agree to the rules of having a Direct Payment. You will also be asked to keep records and receipts to show that the Direct Payments have been spent on care to meet your needs as agreed. The finances will be looked at as part of the annual review of the Personal

Support Plan. One of the key elements in encouraging people to use Direct Payments is to have good support and advice available. Direct Payment support organisations provide information and advice services to people thinking about or using Direct Payments. This includes advice on how to recruit and employ personal assistants and support workers. We are currently supported by Penderels Trust who can be contacted on 01299 253225. Third party payments If the care home you choose costs more that the rate we usually pay for someone with your needs, you will have the choice to ask someone else to make an additional payment. If there is no-one available to supply the extra payment, you can ask us to find you another placement at the rate we would normally expect to pay. If, however, you do decide to live in the more expensive home and you have someone who is able to make an additional payment for you, they will have to pay the difference between our rate and the amount the home charges. This additional payment is often referred to as a ‘top-up’ or ‘third party contribution’. The law says that you are not allowed to make this additional payment yourself, except in limited circumstances. Therefore, the additional payment has to be made by someone else, for example, a family member

or charity. Before anyone agrees to make additional payments on your behalf they should be aware that the amount could change, usually increase, once a year and they need to be confident that they can sustain the payments for as long as they are required. If the additional payments stop being paid for any reason, then you should seek help and advice via the Health and Social Care Access Centre on 0845 607 2000. Once it has been established that someone is willing and able to make these payments they will be asked to sign an agreement to formalise the arrangement. Because the financing of care is a complex area it is advisable to contact the social worker who is arranging your placement with you or the Health and Social Care Access Centre to ask for advice and individual guidance.

Care funded by the council Do I qualify for local authority financial assistance? If you have been assessed as needing home care or a care home place and your capital is below £23,250 you should be entitled to financial support from your local authority. If you have capital below £14,250 you will be entitled to maximum support contributing your income less £23.50 per week retained for personal expenses. If you have capital between £14,250 and £23,250 you must pay a capital tariff of £1 per week for each £250 between these two figures. Capital includes the value of your former

home unless it’s occupied by your partner, or a relative who is aged over 60 years or incapacitated, or a child under 16 years who you maintain or a separated partner who is a lone parent. Do I have a choice of care home if we are paying? Yes and it can even be in a different county (explained on page 28). The home you choose must be suitable for your assessed needs, comply with any terms and conditions set by your local authority and not cost any more than >>

We will allow a third party to top-up as long as they are able to do so over the long term. You are not allowed to top up the fees yourself from capital below £23,250. My partner needs care, how does this financially affect me? We will take into account 50% of any joint savings therefore, to accelerate financial help, it is better to have separate single accounts meeting care costs paid from the account of the person in the care home.

Paying for your own residential care Self-funding If you are self-funding your care because you are not eligible for local authority funding there are other forms of financial assistance you may be entitled to. Will the local authority pay my fees whilst I am selling my former home? If, apart from your property, your other capital is below £23,250 we will help with the costs during the first twelve weeks of permanent care. Beyond that period any financial help will be charged against the value of your former home and recovered from the eventual sale proceeds. Do I have to sell my property? No, we can lend you the money to pay for your care charged against your property value. However, we may limit how much we will pay and it could adversely affect your welfare benefit entitlements. Do I have to pay Council Tax on an empty property? If you move into a care home and your property is left empty then you should receive full exemption from Council Tax until it’s sold. Is there any financial help that is not means tested? Attendance Allowance is a non-means tested, nontaxable allowance paid at the lower rate of £54.45 for those needing care by day or night and, at a higher rate of £81.30 for those needing care by day and night. Whether your stay is temporary or permanent if you receive nursing care in a care home you may be entitled to a Funded Nursing Care Contribution (previously known as a Registered Nursing Care Contribution (RNCC))

towards the cost of your nursing care, paid directly to the home at £110.89 per week. If your needs are primarily health care needs, you may be entitled to full funding from your local PCT following an assessment under their continuing care eligibility criteria. This figure applies to England only. What happens if I move into a care home independently and run out of money? If your capital is likely to reduce to £23,250 you should seek our assistance. However, if the home costs more than we usually pay and won’t reduce its fees you could be in the difficult situation of either finding a source of top-up or seeking less expensive accommodation. If there is a likelihood of running out of money it’s important that you arrange a Personal Assessment with us to ensure we will step into help. Also check if the care home owner can continue to accommodate you at our funding rates or will require a third party top-up. See page 39 for information about top-up payments. What can I do to avoid this situation? There are ways of meeting care costs for as long as you need care whilst using up only part of your capital. For example, the use of Immediate Need Care Fee Payment Plans can contribute towards capping the cost and, as so many older people wish, enabling an inheritance to be left for the family. The important thing is not to try to do it alone, always seek professional advice. Figures mentioned here may change over the lifetime of this Directory.
