Dear Neighbors, If I were to sum up the past year in a single word, it would be “collaboration.” Whether it’s new space to foster greater interaction among specialists, technology tools that connect patients with care or a deepening relationship with Virginia Tech, we’re working together in more ways than ever. What we’ve been able to accomplish is truly inspirational. We began performing sophisticated procedures like deep brain stimulation and in vitro fertilization, and continued to improve quality and transform how you receive care. With an expanded role in community health, we made great strides in keeping people well, too. And when it comes to learning and discovery, the same collaborative approach led to the opening of a unique anatomy lab and important advances in our understanding of illness and injury. These and other accomplishments were the result of many individuals working as one toward a common goal and making us all stronger along the way. BUILDING A BETTER APPROACH Orthopaedics and neuroscience have traditionally been separate areas of focus. At Carilion, we recognize that patients are more than the sum of their parts, and that answers often lie where disciplines overlap. This is the thinking behind our Institute for Orthopaedics and Neurosciences which opened its Roanoke location in January 2016. Because when lively medical minds share a common space, patients benefit—with experts committed to healing mind and body in a whole different way.

Our new Institute of Orthopaedics and Neurosciences facility in Roanoke is the result of a multimillion dollar renovation. The first floor, which previously housed a grocery store, was completely remodeled, and a mezzanine level was expanded to create a second floor. 10 | CARILION CLINIC


GOING DEEP FOR RELIEF

There’s new hope for patients suffering from neurologic movement disorders such as dystonia and Parkinson’s disease: deep brain stimulation (DBS). Carilion is the first in western Virginia to offer DBS, meaning patients can experience amazing moments of recovery, right here.

New ways of working Plans are well underway for a multidisciplinary institute devoted to cardiovascular care. Like our new Institute for Orthopaedics and Neurosciences, it will represent the highest level of specialized care that’s enhanced by collaboration. Our vision moved closer to reality in 2015, with a defined leadership structure, teams devoted to quality and patient experience, and monthly grand rounds. It builds on our more than 30-year history as a leader in heart care. Our expertise in gastrointestinal care also is significant and includes the addition of nationally and internationally renowned doctors in inflammatory bowel disease, digestive cancers and liver care.

Putting cancer in the hot seat Gastroenterologists at Carilion are deploying a new weapon against abdominal cancer with hyperthermic intraperitoneal chemotherapy (HIPEC). After surgery, heated chemotherapy drugs are placed in the patient’s abdominal cavity. Then caregivers gently rock the patient back and forth to distribute the drugs and kill additional cancer cells.

Boosting our quality measures isn’t a glamorous, high-profile initiative. However, nothing can impact the success of our organization more, and the role quality plays in patient outcomes is profound. In fact, we’ve made quality priority one, by creating a board-level oversight committee and establishing the position of chief quality officer.

•• We’ve seen improvement in overall patient safety scores in both our hospitals and medical offices. In 2015, 85 percent of hospital employees rated patient safety “Excellent” or “Very Good,” up 11 percentage points from 2013. Our medical offices show a similar trend, with 70 percent of employees rating patient safety “Excellent” or “Very Good,” up 4 percentage points since 2013. •• In employee engagement, the percentage of those engaged was up nearly 4 percentage points over 2014, our largest year-over-year increase. Certified as the region’s only Level 1 trauma center since 1983, Carilion Roanoke Memorial Hospital is prepared to treat whatever comes our way. And with a more than 15,000-square-mile coverage area that includes two major interstates and thousands of acres of national forest, it’s a lot. Whether it’s a mountain biking accident or injuries sustained in a multi-car crash, our interdisciplinary team of full-time trauma surgeons, emergency medicine physicians, nurses, specialists, therapists and technicians is available 24 hours a day, seven days a week to provide immediate care. We work just as hard to prevent injuries, too, through education and outreach, helping to make the region safer for all of us.

PRIMARY CARE THAT’S CLOSE TO HOME When it comes to health care, sometimes the best care is none at all. And by pioneering the medical home model, we’ve made dramatic and measurable progress toward that goal. It began with a radical proposal by physician leaders aimed at addressing frequent patient readmissions. The idea: a proactive, team-based approach to care, centered on the patient and geared toward managing higher-risk patients.

IN NEW WAYS

Closing the distance for well-being The ability to deliver care without the constraints of time and distance—known as telehealth—is changing the dynamic of care delivery. At Carilion Clinic’s family medicine practice in Pearisburg, Robert Devereaux, M.D., (pictured above) and his team successfully piloted a telehealth initiative designed to improve access to mental health care. Using a secure link, Carilion psychiatrists in Roanoke were able to treat patients in real time, via desktop, laptop, tablet or mobile device at our Pearisburg office. Not only are our family doctors better equipped to treat more complex mental health issues, the arrangement improves access to specialty care for patients in more rural areas. That’s technology at its best.

PATIENTS The most significant revolution in medical technology is here. In fact, it’s probably already on your smartphone, tablet or home computer. Making care accessible wherever and whenever it’s needed, and making caregivers and patients true partners in health.

FIVE TOOLS THAT ARE TRANSFORMING CARE AT CARILION •• The MyChart app and website offer patients 24-7 secure access to medical records, prescriptions, lab results and more. •• With the tap of a button on a smartphone, the iTriage app lets VelocityCare urgent care patients send symptoms and other information so physicians and staff can be ready for their arrival. •• Initially offered to Carilion employees, e-visits will soon be available to a wider pool of Carilion patients, enabling remote diagnosis and prescriptive care from the comfort of their homes. •• Video visits began their rollout in 2015, and over time will offer greater access to care via a real-time online video link, connecting patients and providers virtually anywhere. •• The LiveChat feature on our website allows users to communicate real-time with clinical experts. Last year, we had 3,940 “chats” averaging just under seven minutes and achieved a 91 percent satisfaction rating. 24 | CARILION CLINIC


EARNING OUR REPUTATION Today, word of mouth travels fast and far. Sites such as RateMDs provide a space for patients to score their caregivers, while many others rate their care on Yelp, Google+ and other social platforms. These opinions matter to us, and by using social monitoring tools to aggregate and analyze what’s being said about us online, we can better focus on delivering the experience every patient desires and deserves.

That’s why we created a transgender clinic, dedicated to coordinating physical and psychosocial care for the transgender community. EMBRACING THE COMMUNITIES Blacksburg — In just one year, we opened a new VelocityCare location and added sports medicine, rehabilitation and orthopaedics services. Overall, we’ve invested more than $5 million in the New River Valley in the past two years. Lexington — We expanded access to family medicine with the addition of Laura Lanier, M.D.; Robert Pickral, M.D.; Thomas Peck, M.D.; and Beth Cullen, F.N.P. And with more options for general orthopaedics treatment, including foot and ankle, hand and upper extremity, and spine care, that adds up to healthier neighbors, head to toe. Bedford — Building on more than three decades of comprehensive adult and adolescent care in Bedford and the surrounding communities, Carilion opened a new family medicine location, helping ensure good health for generations to come. Roanoke — By purchasing property next to Carilion Roanoke Memorial Hospital, we’re ready to meet the growing need for clinical space. And across town, the landmark Shenandoah Life Building may see new life as a Carilion office. Salem — PCA Healthcare, with sites in Salem, Floyd and Daleville, joined Carilion, expanding our primary care services in the region and adding two VelocityCare urgent care sites.

WE SERVE WITH SOARING APPRECIATION In his nearly 40 years of service as a board member, George B. Cartledge Jr., oversaw tremendous progress for our organization. And it is in his honor—and reflecting his passion for our Life-Guard helicopter fleet— that Carilion’s patient transport center at 431 McClanahan Ave. in Roanoke is now known as the George B. Cartledge Jr. Center for Transportation.

CHIEF AMONG US We knew for some time that we wanted to be part of a system that was able to focus on primary care. So we took the next step by establishing the Carilion Children’s brand. More than a new name for Carilion Clinic Children’s Hospital, this new identity unifies our pediatric services, no matter where they’re offered. Community leaders and caregivers worked hand in hand to make it a reality—which is all the more reason our whole region can be proud of Carilion Children’s.

Working together—it’s proving to be the smartest way to battle our most serious illnesses. And through a pioneering collaboration right here in the Roanoke Valley, doctors, scientists and patients are making great inroads against one of the deadliest forms of cancer: glioblastoma. Because glioblastoma cancer cells have such strong defenses against chemotherapy drugs like temozolomide (TMZ), treatment can be futile. Working as partners, Carilion Clinic’s chief of neurosurgery, Gary Simonds, M.D., and Virginia Tech Carilion Research Institute (VTCRI) scientists professor Robert Gourdie, Ph.D., who is the Commonwealth Eminent Scholar in Heart and Regenerative Medicine, and assistant professors Samy Lamouille, Ph.D., and Zhi Sheng, Ph.D., have made a discovery that could yield much better outcomes for patients.

Glioblastoma tumors, which can be found anywhere in the brain or spinal cord, are highly malignant. Treatment involves surgical removal of the cancerous tissue, followed by chemotherapy.

With brain cancer tissue from willing Carilion patients collected by Dr. Simonds and his clinical team at the hospital and provided to the VTCRI scientists, the research team uses a peptide discovered by Dr. Gourdie and his team called ACT1 to restore glioblastoma cells’ sensitivity to TMZ. When treated with recalibrated dosages of TMZ in combination with ACT1, previously resistant glioblastoma cells begin to die off in response to the drug again. These are just first steps, but they are huge steps for our doctors, scientists and, most important, our patients.

Across the board, Carilion’s doctors and scientists at the Virginia Tech Carilion Research Institute (VTCRI) are building a brighter future in the world of patient care, treatment and rehabilitation. Among their many paradigm-shifting approaches is the partnership between Damon Kuehl, M.D., director of Carilion’s emergency medicine residency program, and Stephen LaConte, Ph.D., associate professor at the Virginia Tech Carilion Research Institute’s Human Neuroimaging Laboratory. The pair are performing innovative brain injury research with advanced functional magnetic resonance imagining (fMRI) in Dr. LaConte’s laboratory at VTCRI with patients who are seen through Carilion Roanoke Memorial’s Emergency Department. Dr. LaConte’s brain imaging research in collaboration with the detailed clinical evaluations of patients with brain injury by Dr. Kuehl are providing new information about the injured brain and its links to chronic pain, depression and afflictions where no acute cause can be found. Dr. LaConte uses an innovation for fMRI that he developed and called “temporally adaptive brain states,” or TABs. Using three-dimensional video images of the brain, he and his research team extract in real time what the brain is doing midthought—the opposite of the usual goal of neuroimaging, which is to map what’s going on in the brain while the person is doing something.

Partnering to advance research across the board Improving the health of the communities we serve. Serving as an academic model for other Carilion departments and our peers nationally. Facilitating progress, giving guidance and creating infrastructure so that busy clinicians can be more academically productive. These are the goals of the partnerships between the Carilion Clinic Orthopaedics–Musculoskeletal Education and Research Center (CCO–MERC) and a growing group of collaborative researchers. Recent collaborations have included work with: •• Virginia Tech Carilion Research Institute •• Virginia Tech-Wake Forest University School of Biomedical Engineering and Sciences •• Virginia Tech Department of Biomedical Engineering and Mechanics •• Thomas Jefferson University (Philadelphia) •• University of Virginia •• Wake Forest University •• University of Rochester As a result, VTC School of Medicine students and their Carilion Clinic Department of Orthopaedics mentors have published and presented their work at national and regional meetings on topics such as: •• Improving the care of patients with osteoporotic fractures •• Elevating spine fusion rates without using complex bone growth stimulants •• Performing biomechanical testing of new spine fusion techniques and fracture care concepts •• The development of revelatory new X-ray views in hip fracture surgery •• Teaching students the time management skills necessary to be academic orthopaedic surgeons

Learning from one another. Working together. No matter the profession, the benefits are innumerable. In the field of health care, sharing knowledge, skills and technological discoveries— working as one great team—is more crucial than ever. During the past five years, the Collaborating Across Borders (CAB) conference has become the leading venue for practitioners, scholars, educators, health system leaders and students to gather and advance this interprofessional approach. Over four days in 2015, more than 1,200 top medical and human resources professionals from six countries convened in Roanoke for the fifth annual CAB conference. Hosting this premier international conference was the result of a collaborative effort by the city of Roanoke, Virginia Tech, the Virginia Tech Carilion School of Medicine and Research Institute, and Jefferson College of Health Sciences. Sessions centered on educating and implementing more effective health care by: •• Nurturing students as thought leaders in their careers and communities •• Teaching providers how to work as part of effective health care teams •• Balancing the improvement of patient care and cost reduction •• Emphasizing the basic importance of giving individuals and families the gift of compassion, kindness and respect •• Including patients as members of the health care team

CONVENING FOR KNOWLEDGE Collaboration is nothing new to Roanoke, Virginia Tech or Carilion Clinic. MAKING HEALTH A

COMMON CAUSE

Staying healthy is a commitment. It means taking responsibility for personal choices. It also means whole communities pulling together, identifying challenges, and taking action to address them and grow stronger together. This is why we’re at our best when Carilion Clinic is engaged with the community, working to find solutions and rallying partners. From helping neighbors make smarter decisions about what they eat, to seeing our whole organization fulfilling a pledge to be better stewards of the environment, we all play a role in building a healthier, stronger region.

BUILDING HEALTHY COALITIONS By listening to the residents of our region, we’ve learned a lot about our area’s most pressing health care needs: Access to care, mental health and substance abuse services, and health and wellness literacy. Community Health Needs Assessments (CHNAs) in the Roanoke Valley and Rockbridge and Giles counties in 2015 brought the needs — and their contributing factors – into sharper focus. It isn’t enough to conduct CHNAs every three years, it’s also essential that the findings lead to solutions. In the Roanoke Valley, for example, Healthy Roanoke Valley — a broad-based, grassroots coalition of more than 50 community organizations including United Way – came together to address the needs identified in our earlier Roanoke Valley study. Their common goal: to educate and empower the people of the Roanoke Valley to lead healthier lives. 44 | CARILION CLINIC

GROWING A GREENER INSTITUTION To Carilion, conserving resources isn’t simply a virtuous habit; it’s a sound model for business. And the mission to move Carilion Clinic to a more sustainable, environmentally responsible organization isn’t an anomaly—it’s a mission shared by hospitals across the country participating the Healthy Hospitals Initiative (HHI). Developed by the 12 largest hospital systems in the country, the HHI serves as a guide for hospitals to reduce energy and waste, choose safer and less toxic products, and purchase and serve healthier foods. Carilion’s efforts are spearheaded by our volunteer Environmental Stewardship Council.

Familiar mission, new home. One of Roanoke’s most stunning historical houses is now home to the Carilion Clinic Foundation. Built in 1906 for D.W. Flickwir (at right) and later named in honor of Horace and Ann Fralin of Roanoke, this landmark on Jefferson Street previously housed the Fralin Center for Community Education. Now, as home to the Carilion Clinic Foundation, Fralin House will live on as a venerable community asset.

Few people are more beloved in our region than Bill Erwin, M.D. With five decades of service to the community as a physician leader and expert in the areas of internal medicine and pulmonary disease, Dr. Erwin has earned every bit of recognition he’s received for his recent retirement. Healthgrades analyzed several years of data from the Centers for Medicare and Medicaid Services (CMS) and determined the following: ••Carilion Roanoke Memorial Hospital performed as expected or better in nearly every category, and ranked in the top 50 hospitals for vascular care. ••Carilion New River Valley Medical Center and Carilion Stonewall Jackson Hospital performed as expected or better in all but one category. ••Carilion Giles Community Hospital performed as expected or better in every category. ••Carilion Tazewell Community Hospital performed as expected or better in every category but two.
