HOSPICE QUALITY MEASURES WE OFFER
a Wide Variety of Services
Carilion Clinic Hospice had a busy year preparing for upcoming quality initiatives. We are in the process of transitioning to the Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey for 2015. The hospice CAHPS tool will allow a more accurate reporting of caregiver satisfaction with services provided and will provide valuable insight to our hospice staff on areas that we can improve upon. It also aligns hospice quality measurements with those used in the hospital and ambulatory settings. We also implemented the mandated Hospice Health Information Set (HIS) data requirements. The HIS collects data on seven quality outcomes for our HO patients that will eventually be publicly reported. This includes the outcomes listed below, among others. All locations are currently meeting or exceeding the benchmark in all of the upcoming publicly reported outcomes, based upon our preliminary review.

MUSIC THERAPY Music therapy has proved to be an effective and
often requested way for patients and families to be comforted as the end of life approaches. It is currently available in all communities we serve.
CAMP TREEHOUSE Because we are concerned for families as well as patients, we make
bereavement education and support available for at least a year. One way we accomplish this is through Camp Treehouse, a program dedicated to children who have lost a loved one. This year marked the camp’s 16th year. Volunteers helped the program run smoothly for the 35 children who attended.
SHARING THE HEART The holiday season is a poignant time for
MIFI Devices Both Carilion Clinic Hospice and Home Care clinicians depend on access to McKesson’s electronic medical record to review important patient information, such as discharge summaries or clinical information related to a recent hospitalization or office visit. To enable our clinicians to utilize this information while on home visits, we continue to use MiFi mobile devices, a wireless connection allowing clinicians to connect to the McKesson server, the Epic server, or the Internet. This access allows field clinicians to view a more complete clinical picture. As we partner with inpatient settings to prevent avoidable hospital admissions, access to current and more complete information is invaluable.

WE HONOR VETERANS We Honor Veterans is a program created by the National Hospice and Palliative Care Organization to serve our veterans. We proudly participate by recognizing our veterans with gratitude for their service. We also have veteran volunteers who pin our veterans in recognition of their valor and sacrifice for our country.

BRIDGE TO CARING HANDS To help caregivers in our community, we began
offering the Bridge to Caring Hands program in the New River Valley. In a series of classes taught by our hospice team, caregivers learn about physical care and safety for both caregiver and patient, how to give medications properly, and receive training on medical equipment (oxygen tank, wheelchair, hospital bed, etc.), among other helpful tips.
FAMILY TREASURES Family Treasures is a program that allows our patients to tell their life story in their own words, and in their own way, as a gift for their loved ones. Our staff can guide patients in finding the best way to tell a complete life story, or just a memorable chapter, through a variety of options, such as video memoirs, oral history, audio memoirs, legacy books, photo collages, legacy letters, recipe collections, and more.
JOURNEY OF HOPE Losing a loved one can be a lonely and painful experience. To help ease the pain we offer Journey of Hope, a facilitated education and support group led by an experienced bereavement staff member and meets weekly for six to eight weeks.
