HOSPICE QUALITY MEASURES Carilion Clinic Hospice had a busy year implementing quality initiatives. We are in the process of transitioning to the Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey. The Hospice CAHPS tool will allow a more accurate reporting of caregiver satisfaction with services provided and will provide valuable insight to our hospice staff on areas on which we can improve. It also aligns hospice quality measurements with those used in the hospital and ambulatory settings.

Our Community
MUSIC THERAPY Music therapy is a therapeutic intervention aimed
at reducing patient stress and improving quality of life. As the music therapist shares a familiar genre of music, patients often join in the singing or visibly relax in response to the familiar sounds.
SUPPORT PROGRAM Because hospice care is a concern for families as well as patients,
we make bereavement education and support available for at least a year. One way we accomplish this is through ongoing outreach dedicated to children who have lost a loved one.
SHARING THE HEART The holiday season is a poignant time for
hospice patients and their families, and we help make it special through our Sharing the Heart of Christmas at Home program. Our caring physicians and staff adopted 300 patients and family members in 2015, delivering over 2,300 gifts to make the holidays a time of joy and gratitude.
WE HONOR VETERANS We Honor Veterans is a program created by the National Hospice and Palliative Care Organization to serve our veterans. We proudly participate by collecting supplies to distribute to patients and their families.
CARD PARTY The Hospice Card Party in Franklin County, now in its 17th
year, drew approximately 170 people in April and raised over $15,000 in funds for hospice services in Franklin County
BRIDGE TO CARING HANDS To help caregivers in our community, we offer the
Bridge to Caring Hands program in the New River Valley. In a series of classes taught by our hospice team, caregivers learn about physical care and safety for both caregiver and patient, learn how to give medications properly and receive training on medical equipment (oxygen tank, wheelchair, hospital bed, etc.), among other helpful tips.
FAMILY TREASURES Family Treasures is a program that allows our patients to tell their life story in their own words, and in their own way, as a gift for their loved ones. Our staff can guide patients in finding the best way to tell a complete life story, or just a memorable chapter, through a variety of options, such as video memoirs, oral history, audio memoirs, legacy books, photo collages, legacy letters, recipe collections and more.
JOURNEY OF HOPE Losing a loved one can be a lonely and painful experience. To help ease the pain, we offer Journey of Hope, a facilitated education and support group led by an experienced bereavement staff member that meets weekly for six to eight weeks.
MIFI Devices Both Carilion Clinic Hospice and Carilion Clinic Home Care clinicians depend on access to our electronic medical records to review important patient information such as discharge summaries or clinical information related to a recent hospitalization or office visit. To enable our clinicians to use this information while on home visits, we continue to use MiFi mobile devices, a wireless connection allowing clinicians to connect to the server or the Internet for educational purposes. This access allows field clinicians to view a more complete clinical picture. We partner with inpatient settings to prevent avoidable hospital admissions, so access to current and more complete information is invaluable.
Staff Education To ensure quality is delivered, a clinical educator assists with onboarding of new hires and develops education/ competencies for our current hospice and home care staff.
