UNLIKE GAUL, WHICH JULIUS CAESAR DESCRIBED AS BEING IN THREE PARTS, PLASTIC SURGERY HAS TWO: RECONSTRUCTIVE AND AESTHETIC. A FURTHER DISSIMILARITY IS THAT THESE PARTS OVERLAP. A BREAST RECONSTRUCTION, AS A PRIME EXAMPLE, WILL SOON BE JUDGED BY THE PATIENT AND THE SURGEON ACCORDING TO AESTHETIC STANDARDS. OF APPROXIMATELY 6,500 CERTIFIED PLASTIC SURGEONS IN THE UNITED STATES, MOST DO A COMBINATION OF AESTHETIC AND RECONSTRUCTIVE SURGERY, AS DID I THROUGHOUT FOUR DECADES OF PRACTICE. Remuneration from cosmetic surgery usually far exceeds that from restorative procedures, the obvious reason being that surgery of appearance falls outside the assignment fee system of third party payers. Not only, therefore, is the speciality of plastic surgery divided into cosmetic and

reconstructive, but so also is the professional life of the individual plastic surgeon as a result of his or her choices – and there are choices. The young plastic surgeon begins a career by covering emergency room and performing reconstructive operations but as the years pass, the aesthetic component grows and takes over. Aside from the fiscal rewards, the associated lifestyle has appeal because of its elective nature. An additional advantage is that the plastic surgeon may choose to do fewer major aesthetic operations but more minor office procedures, such as injecting Botox and fillers, dermabrasion, laser treatments, dispensing cosmetic products, advertising, and even opening a spa – all in the name of beauty, better health, and better income for the plastic surgeon. Cosmetic surgery would not exist without a societal demand for it. The availability of the plastic surgeon, or other surgeons and non-surgeons, who do aesthetic surgery, not only answers the need but reinforces it. Aesthetic surgery has special features, some so obvious that they are frequently overlooked. In contrast to reconstructive surgery, which is performed to improve function and to restore normalcy, aesthetic surgery is done to enhance the patient’s already normal appearance. All aesthetic surgical procedures are directed to areas where the eye can reach under certain conditions. The face, for example, is usually in view in most cultures; the breast, as another example, can be seen only to special observers under certain circumstances. Unlike almost every other surgical situation, many of which are emergencies, operating for aesthetic reason is completely elective, thus giving both patient and surgeon time to make a decision and permitting the

patient to seek more consultations. The patient has gone to the plastic surgeon because he or she wants an operation and may become disappointed and angry if refused. Aesthetic surgery generally involves only a brief physical examination focusing on the body part that concerns the patient. Patients, moreover, are generally healthy. They are willing to risk their physical wellbeing to be happier because of an improved self-image. Happiness, as we all know, is an elusive entity. Aesthetic patients commonly feel guilty and embarrassed because of what they term “vanity”. Adding to this feeling of guilt is the reality of spending money perhaps stretching the family resources in pursuit of something not essential to health. Another distinctive feature of aesthetic surgery is that at least 80 to 85 percent of patients are women. The reasons are not always anatomic but cultural. Our society as well as many others puts great pressure on the female to improve her appearance to the point of subjecting herself to an operation that can produce an unfavourable result or a frank complication. As Freedman observed: “Beauty counts for everyone, but more so for women. From the moment of birth, beauty is sought, perceived and projected onto girls… .” (Freedman, R. Beauty Bound. Lexington, Ma: Heath, 1986, p9) That most plastic surgeons in the world are men is another fact to consider. Male plastic surgeons would not think it unusual for a woman to have a cosmetic operation. The rise of women to greater prominence and powerful positions has not lessened their desire to seek improvement in their appearance. The fact that the surgery of appearance is routinely prepaid is another unique aspect of cosmetic surgery. Aesthetic surgery offers the patient a way of improving the quality of his or her life, the plastic surgeon an opportunity to help that individual and gain satisfaction in doing so but only if he or she acts as a physician committed to whatever is in the best interests of the patient, and not conducting himself or herself as a proprietor of a body shop.

THE QUEST FOR ENHANCING PHYSICAL BEAUTY IS AS OLD AS MANKIND ITSELF. PLASTIC SURGERY REACHES FURTHER BACK THAN MOST OF US REALISE AND THE FIRST PATIENTS WERE PREDOMINANTLY MALE. FEMALE PATIENTS STARTED OUTNUMBERING MALE PATIENTS TOWARDS THE END OF THE 19TH CENTURY. COSMETIC SURGERY WAS FIRST DEVELOPED IN THE LATE 1400s TO TREAT DISFIGURED VICTIMS OF THE SYPHILIS EPIDEMIC. Cosmetic surgery, as we know it today, was pioneered by German surgeons (some of the most notable names are Carl Ferdinand von Graefe, Eduard Zeis, Hermann Eduard Fritze, Friedrich Dieffenbach and Jacques Joseph) and further developed to deal with the ravages of World War I. When Hollywood introduced the camera close up concept, Louis B. Mayer, made Greta Garbo go for cosmetic dentistry. Marlene Dietrich had rhinoplasty in 1929. Rita Hayworth had her hairline moved. Marilyn Monroe’s dalliances with cosmetic surgery are well documented and obvious when comparing photographs. Hollywood, along with TV and print media, defines our perception of beauty today, although the vast majority of women and men in the public eye would never admit to having resorted to cosmetic surgery. Phyllis Diller was the first to admit to having had surgery and made headlines in 1971 by divulging

details of her operations. There exist on the Internet dozens of websites that delight in highlighting cosmetic surgery gone wrong and informing the public who has had what done. The best and most visited is www.awfulplasticsurgery.com, listing scores of celebrity before-and-after photographs for the delight and entertainment of mere mortals

FIRST DOCUMENTED NOSE JOB

The Bolognese surgeon Gaspare Tagliacozzi performed what is believed to be the first nose job. The operation lasted 20 agonising days as anaesthesia was not available at the time (having only been discovered in 1846).

1899

FIRST TUMMY TUCK

Baltimore surgeon Howard Kelly removed a piece 90cm long, 31cm wide and 7cm thick from the body of Mrs M. a 32-year-old Jewish woman.

1901

FIRST FACE LIFT

German surgeon Eugen Hollander performed the first known face lift on a Polish aristocrat who persuaded him to undertake the surgery by showing him a drawing. The drawing illustrated how, if facial skin were removed at the front of the ear, the corners of the mouth would be tightened.

1906

FIRST EYE LID SURGERY

Chicago surgeon Charles Conrad Miller is credited with performing the first aesthetic eye surgery (rhytidectomy), much copied by his contemporaries.

1912

FIRST SAGGING CHEEKS AND CHINS OPERATION

German surgeon Jacques Joseph developed the procedure for tightening sagging cheeks, leaving virtually no scarring. Jacques Joseph is also the god-father of the modern endo-nasal rhinoplasty. Raymond Passot developed the procedure for removing the double chin in 1919.

1920s

FIRST FAT INJECTIONS

Subcutaneous fat injections were developed to give a younger appearance without the scarring.

1920s

FIRST TRANSSEXUAL SURGERIES

Berlin surgeons Ludwig Ley-Lenz and Felix Abrahams were the first surgeons to transform male genitalia into simulacra of the external female genitalia.

1930s Sir Archibald McIndoe and Sir Harold Gillies as well as a lot of other notable surgeons, helped patch up veterans of World War I and then World War II, especially the badly burned Spitfire pilots. At the same time, they pursued a very lucrative career in cosmetic plastic surgery in Harley Street. Chemical peels were popular in Europe and then Florida and LA from the Thirties, often practiced by non-medically qualified individuals. Sylvester Stallone’s mother was one of the very early practitioners. Ivo Pitanguy is deservedly famous for popularising plastic surgery in the post-World War II era. Living in Brazil he practices in a country obsessed with looking good.

1950s

FIRST BREAST IMPLANTS

Silicone was first used in the form of subcutaneous injections until it proved to carry substantial infection risk. Thomas Cronin and Frank Gerow developed the first silicone gel breast implants in Houston, Texas in 1962. The women in Texas are some of the best groomed and most plastic surgery-loving in the universe.

1970s

FIRST BUTTOCK LIFT

Professor Ivo Pitanguy developed the first buttock lift, a procedure which was widely copied by other surgeons. Also in the 1970s Tord Skoog from Uppsala in Sweden, described the SMAS technique for the facelift and made it more than just a skin operation. Claude Lassus from Nice popularised the vertical breast reduction technique and Benelli from Paris the peri-areolar technique (both Frenchmen, obviously, and both passionate about reducing scars on petite French breasts).

1980s

DEVELOPMENT OF LIPOSUCTION

Dr. Yves Gerard-Illouz described, against much scepticism, the concept of blunt cannula liposuction using the wet technique to a crowded arena in Honolulu in 1982. Marco Gasparotti from Rome further refined the concept of liposculpture and in the 90s we had the advent of Power Assisted as well as Ultrasound Assisted Liposuction.

For the last 20-30 years various types of laser have removed hair, broken blood vessels and wrinkles and the whole concept of non-surgical surgery with Collagen, followed by synthetic fillers such as Hyaluronic Acid and Hydroxyapatite, has come of age. The most important product of all being Botulinum toxin, or Botox, since the late 1980s. Today elective surgery has become a safe option and the idea of having it done and going home the same day has made it seem even less invasive. Deft surgery with minimal blood loss and using skin glues has reduced bruising and pain and speeded up recovery. Multiple surgeries can be carried out at the same time. New developments are anti-obesity surgery for the fast-food classes and cosmetic surgery to fix the side effects of

THIS CHAPTER OUTLINES THE SURGERY PROCEDURES AVAILABLE TODAY, BUT EVEN AS IT IS BEING WRITTEN, NEW COSMETIC SURGERY TECHNIQUES ARE BEING DEVELOPED IN RESPONSE TO DEMAND FOR REDUCING RECOVERY TIME AND SCARRING. Procedures such as facelifts, which were previously performed with scalpel, will in the future be done entirely with lasers. Tissue engineering is no longer in the realm of science fiction. The creation of living tissues in a laboratory setting for use as a replacement for damaged or diseased body parts will soon be a reality, as will be the growing and reproduction of skin grafts, breast tissue, muscles, stem cells and even nerves. Many of the above

may still be in their infancy stage, but the future of cosmetic and reconstructive surgery will likely have tissue engineering at its core.

Facial surgery RHYTIDECTOMY (FACE LIFT) Types of facelifts available SMAS lift: Most popular face and neck lift. Relies primarily on lifting the cheek/neck contour. SMAS stands for Superficial Musculo-Aponeurotic System. The operation tightens the face’s subcutaneous layer of muscles connective and fatty tissue. Mini Lift: The most basic lifting operation. The same incisions as for SMAS lift are used but only the skin is tightened. Mid Face Lift: This lifts sagging cheeks below the lower eyelid and corrects the drooping corners of the mouth. Mask lift (or Deep Plain Lift): A special technique is used to dislodge the cheek from the inside of the mouth. Skin, tissue and muscles are detached from the bone to achieve a long lasting affect. Endoscopic Lift: This procedure results in minimum scaring, and the most natural postoperative look. Starting at the hairline, tiny incisions are made to feed the endoscope and other miniature instruments beneath the skin. Every movement is traced on the video monitor. Sagging tissue is pushed back up and the skin realigned. This procedure’s success depends entirely on the surgeon’s qualifications and skills. Endoscopic brow and eyebrow lift: Brow and eyebrow’s appearance can benefit greatly from this surgical procedure, which produces no visible scars. Tiny incisions are made for inserting special surgical instruments under the brow skin. The skin is lifted and some of the brow muscles are removed, in order to deemphasise frown lines. The eyebrows can be lifted during the same procedure. Lip lift: If the upper lip and the nose are too far apart, this can be corrected with an invisible cut at the edge of the upper lift or just underneath the nose. If the problem is a narrow upper lip, one incision is made along the contour of the lip and another several millimetres above it. This second incision defines the new, fuller shape of the lip, and hence needs to be made extremely carefully. The strip of skin between two incisions is removed, the red of the lip is infolded from the inside, and the lip is sown onto the new upper edge. If the problem is an oversized lip, the incision is made on the inside of the lip. Again, a flawless technique is decisive for the success of the operation. Eyelid correction/blepharoplasty: The lifting of the upper or lower eyelid, is one of the most popular medical procedures. It requires minimally invasive surgery and rejuvenates tired looking eyes. Corrections are mainly for ‘hoods’ or ‘bags’ around the eyes to give a more radiant, youthful look. Upper eyelid lift: A CO2 laser is used for this procedure in order to remove the excess skin as well as muscle and fatty tissue underneath it. Lower eyelid lift: Recent techniques have focused on redistributing the fatty tissue instead of removing it. This method is known as the SOOF (Sub-Orbicularis Oculi Fat) lift. This is the only effective procedure for treating skin laxity and fat or fluid deposits in the skin directly under the eye.

Nose: Rhinoplasty, the medical term for a Nose job. The nose can be reduced, augmented, narrowed, straightened, widened, and reshaped by correcting the cartilage and bones. Nose bumps can be reduced, nose tips corrected or saddle noses enlarged. A nose operation needs to fulfil aesthetic as well as functional criteria; it is arguably the surgical procedure that transforms the face the most. The majority of operations last for one to two hours and are performed under full sedation. It is imperative to select a surgeon who specialises in rhinoplasty for the best aesthetic result and also in order to avoid operative risks or post-operative complications. Genioplasty/chin corrections Chin Augmentation: A receding chin is frequently augmented by using a pre-shaped silicon implant. Bones from the pelvis and the skull cap and cartilage from the ears or the ribs can also be used. An implant is inserted through an incision 2-4 millimetres from the lower lip. The cartilage can be removed to augment the chin. Chin Reduction: The bone is shortened or filed down but if an external incision is used stitches are usually removed after a week. Osteotomy: This is a complex procedure in which the surgeon shifts the chin bone forward and it is set using metal plates and screws which are removed after 6-12 months. Ears Ear correction or otoplasty: This is a procedure in which disproportionately large or protruding ears are remodelled and/or pinned back. A surgical cut is made on the back of the ear or in the concha, the cartilage is shortened then remodelled. Then the cartilage is fastened. Correction of the zygomatic arch: This is a procedure to emphasise the cheek bones. To achieve a higher cheek bones effect the surgeon implants autologous material or a silicone pad. Cheek bone operations are mostly performed on Asian patients. For European patients it is more common to augment or reduce the rim of the eye socket.

Body contouring Breast enlargement or augmentation mammaplasty: One of the most common cosmetic enhancement procedures, this is always performed using implants. These can be filled with silicone gel, a physiological saline solution or hydra gel. The most common implant contains silicone gel. The operation is carried out under sedation and last two to three hours. An incision is made, either in the armpit, below the nipple or in the fold under the breast. If the incision is made at the nipple a deflated inlay is implanted and filled once in place. Breast reduction or reduction mammaplasty: This is a more complex procedure than breast enlargement lasting three hours under sedation. It reduces skin, sub-cutaneous fat and gland tissue, after which the breasts are reshaped.

pregnancies and breast feeding. In some cases the breast lift can be combined with implants. Breast reconstruction after cancer or reconstructive mammaplasty: There are two

Breast lift or mastopexy: This procedure aims to correct the effects of gravity, weight loss,

methods for reconstructing the breast. Post-mastectomy, by using silicon implants following

tissue expansion or by remodelling the breast from autologous material. The latter procedure has been developed by Professor Dr. Axel-Mario Feller. The breast is reconstructed with an autologous tissue flap harvested from the buttocks or lower abdomen (this has the added benefit of a tummy tuck). If no excess abdominal tissue is available this could also be harvested from the back muscles although the latter surgical technique is very complex and demanding.

Liposuction/body contouring/lipoaspiration Liposuction is one of the most popular surgical procedures today. It is used to improve the bodies’ contours and requires first class technical skill and aesthetic judgement on the part of the surgeon. It is not suitable for overly obese patients because a maximum of three litres of fat can be removed. Liposuction addresses problematic areas that are immune to low calorie food and fitness training.

Techniques Tumescent technique: Small incisions are made on the fatty tissue and saline solution is pumped through these holes. The fat cells absorb this liquid and start to swell up which makes it easier to dislodge them. The loosened fat cells are removed using a thin suction canular. This procedure can last several hours and requires a considerable physical effort on the part of the surgeon. Power-assisted liposuction (lipolysis by vibration): Developed by Dr. Gerhard Sattler, this technique involves the use of an electrically powered canular containing 24 suction holes. The fat cells are agitated up to four thousand times per minute and are shaken out of the connective tissue. Only local anaesthetic is used with this procedure. Ultrasonic liposuction a.k.a. Ultrasound-Assisted Lipoplasty: This technique projects ultrasound waves into fatty tissue which explodes and liquefies fat cells without damaging the connective tissue. Electrolipolysis: This technique is normally combined with a hormone therapy for cellulite. An acupuncture needle is inserted into the fatty tissue and an electrical current causes it to drain out. After eight to twelve sessions the orange peel skin on the thighs is gone.

Liposuction works with different degrees of success for different problem areas. Once fatty tissue is removed it stays away permanently from the areas treated. Unfortunately, during subsequent weight gain it is deposited on other, untreated areas.

Body contouring with the scalpel If a patient’s skin is not flexible enough to contract around reduced tissue after liposuction a further surgical intervention may be required to contour the body. The downside of this is large visible scars. Abdominoplasty: A surgeon removes excess skin where a flab hangs over the lower abdomen, usually from pregnancy or extreme weight loss. It is a major procedure which can last up to two hours and involves a large loss of blood. The skin is cut right up to the lowest

ribs, sparing out only the navel. The surgeon pulls the undamaged skin downward and removes the excess skin. Brachioplasty (Upper arm lift): This procedure is suitable for patients with sagging skin around the arms when it is lifted. The procedure leaves long scars on the inside of the upper arm. Thigh dermolipectomy or thighplasty (Thigh Lift): Where skin has become lax and wrinkled around the inner thighs, a thigh lift can be performed on the inner and outer thighs or on the buttocks. Incisions (and scarring) run along the right side of the pelvic bones, around the inner thighs to the buttocks. Calf augmentation: The calves can be enlarged with hard or soft silicone gel implants if the calves are overly narrow or one bigger than the other. The surgeon cuts from the thigh, two pockets and implants are inserted. Buttocks augmentation: In this procedure pre-shaped silicone implants are used to round out flat buttocks. The implants are placed between the gluteus muscles.

GENITAL SURGERY Perhaps the most controversial, but increasingly popular branch of cosmetic plastic surgery. Phalloplasty (penis enlargement): An extension of up to 5cm and a widening of up to 2cm can be achieved through surgery under full sedation. Both procedures are challenging and need to be considered seriously. Vaginoplasty: Surgical procedure for reducing and enlarged vagina after giving birth. Labioplasty: Reduction or/and reshaping of the labia Hymen restoration: Where virginity needs to be re-established for cultural or religious reasons.

Otoplasty

Otoplasty Correcting prominent ears (or a distorted ear) is one of the most rewarding procedures in plastic surgery, resulting in a tremendous change in the patient’s self-confidence and self-esteem. Dr. White believes that ear re-shaping, or otoplasty, should involve much more than simply “pinning the ears back.” With an intricate knowledge of ear anatomy and his artistic ability, he sculpts the ears into the desired shape and position. Body contouring Liposuction Liposuction Body sculpting with liposuction has continued to grow in popularity for both men and women. Dr. White believes that success in liposuction is determined by three factors: the amount of fat removed, the improvement in overall body shape and contour, and the smoothness of the skin after liposuction. He optimises each aspect using his advanced Circumferential Micro-Lipo Technique. His specially designed, custom-made cannulas (hollow instruments for fat suction) allow him to create a more natural look with less chance of unevenness. IF BEAUTY IS IN THE EYE OF THE BEHOLDER, is the beholder a male or female? Until very recently, the male beholder certainly defined our social version of beauty. But how has this perception arisen? A woman is an object of attention: men look at women and women are aware of it. When a woman walks into a room, she is observed and the observers are men. Consider how the female form has been the subject of so much figurative art. Women, often nude women, have been painted for men to admire. Similarly most pornography caters for male tastes because – to give it a positive spin – the female face and body have always been seen as something to admire. That is an historical truth, and that is why so much art is representational of women. The men pay for it and the women pose: Hugh Hefner and Larry Flint understood this instinctively and didn’t require a lengthy rationale. The reasons men chase good-looking women are rooted deep in our biology. We know attractiveness is a desirable asset, but what is it that defines the physically attractive? Does it have to be supermodel good looks or is it just being well presented? It’s a mixture of both, of course. Even supermodels have their physical imperfections and you can see, in how they dress, that most of them are observing the ‘What not to wear’ rules so as to maximise the good parts and minimise the bad. When men are attracted to women, it is not necessarily for the purpose of reproducing with them, which limits the credibility of the biological conditioning argument. Of course, men are more attracted to sex for its own pleasurable sake than women since, amongst other things, they don’t have the babies. And they are aroused by the obvious parts of the female body that signal sexual pleasure: their lips, their breasts, their buttocks. This was all determined by nature a long time before you and I arrived on the scene but even allowing for the fact that beauty is not all that matters, it does carry some biological weight.

Beauty may not be everything, but it is one of the strongest fixations of the human species.

Becca’s mom flew with her from Alabama to Los Angeles for the breast reduction procedure. “The moment we met Dr. Stevens, we knew everything would be fine,” Becca remembered. “Dr. Stevens reassured my mom that he would take care of me as if I were his own daughter. And he did.” Dr. Stevens explained his approach to breast reduction in great detail. He said he would separate skin from breast tissue and remove portions of each, but that he would preserve tissue under the nipple without severing the nerves and blood supply. That way, even with her breast size much reduced and her nipple in a new position, sensation would most likely be preserved and Becca would recommended I have his ‘Laser Bra’ – the extra internal support he could give me using a piece of my own tissue inside my breasts. Dr. Rubinstein on Beauty What defines beauty? Beauty is the subtle combination of soft lines, volume, and proportion. There is a quality of softness to any beautiful feature. Hard angles and sharp lines are generally not perceived as beautiful. Volume is another component of beauty. A youthful face has volume and youth is attractive in itself. With age, one begins to lose volume and acquire a hollow look. A breast would not be beautiful without appropriate volume. Plastic surgery can rectify the loss or absence of volume but never at the expense of proportion. Too much volume is equally unattractive. Furthermore, each feature (whether body or face) must be in balance with the rest. THE DEFIANCE OF AGEING ITSELF IS SUCH A RELATIVE TERM. WE CAN AGE OUR BODY BY WILD SWINGS IN WEIGHT AND LIFESTYLE AS WELL AS EXCESSIVE SUNBATHING, SMOKING, DRINKING, OR EVEN EXERCISE AND DIETING. IN GENERAL, EXERCISE AND DIET WILL HELP KEEP US IN SHAPE AND DEFY AGEING TO SOME EXTENT. COSMETIC SURGERY CAN ADDRESS ISSUES THAT EXERCISE AND DIET ALONE CANNOT REMEDY. As we get older we usually lose some of the subcutaneous fat on our face and look more skeletal. While it is trendy to look skeletal, this is not necessarily a good thing – the image of youth is to have a nice smooth look with a fuller face. This is derived from the layer of fat beneath the skin, which covers the underlying muscle. As one gets older, this layer is difficult to recreate – excessive weight loss merely compounds the difficulty. Cosmetic surgery cannot do everything… but we are getting there. There is a section of society that seeks to retain and perfect their youthful looks. Their obsessive drive towards perfection has created something approaching a new breed of people: the super-enhanced who, while growing older, appear not to age. A necessary condition of joining this breed is relatively unlimited funds, and the desire to belong – a kind of ‘celebrité oblige’– but what truly marks out the super-enhanced from the rest of us is the quality of eternal youth. Of course, when this is mentioned, the critics begin reaching for their Dorian Gray parallels. Unlike Dorian Gray, who aged completely when he saw his portrait, there need not be some sudden cataclysmic ageing event when you come due for your next facelift – providing you maintain the look regularly. The startling thing is that women in their sixties, ten years on from their first facelift in their fifties, still look ten years younger than their peers. Until now the give-away has been the hands and décolleté which reveal the true age of the canvas. Yet even these can be corrected. Remember: a continuing relationship with your plastic surgeon is not some Faustian pact with the devil. Agelessness is not bought by selling your soul but by paying your surgeon’s fees – although the ancient family heirlooms might have to be sold to achieve it.

Signals of youth and health, such as good teeth and a clear complexion, are obvious examples. Others are more subtle. The classic hour-glass figure, appreciated by so many men, is a case in point. Women with a waist-to-hip ratio of 0.7 are repeatedly judged to have the most attractive figures, and there is evidence that these measurements are also the healthiest. The preference for these proportions holds true for iconic beauties from the Venus de Milo to Marilyn Monroe: Size Zero may be a current obsession, but curves are never going out of fashion.

Whilst the elusive quality of “beauty” cannot entirely be reduced to a clinical formula, there are mathematical principles underlying the construction of the face and body, the perfection of which are often exemplified in the most attractive people. The Golden or Divine Ratio, a proportion of approximately 1 to 1.618, appears time and again in nature in both plants and animals and has, unconsciously and deliberately, been mirrored in art and architecture through the centuries. Leonardo da Vinci's drawing Vitruvian Man famously used it to emphasise the proportions of the ideal human body.

Spirals based on the Divine Ratio are found in patterns of seeds on flower heads, and the shells of snails, as well as the shape of the human ear. Faces of iconic beauties, from the Mona Lisa to almost every top model of the present day, turn out time and again to be governed by the divine ratio, with the size and spacing of eyes, nose, ears and mouth – even the widths of the teeth themselves – following this same precise mathematical formula.

Beauty, it seems, is neither skin deep nor fleeting.

Dr. Balshi was one of the head clinicians for the US Army prosthetic department at Fort Dix during the Vietnam War. He taught at the Temple University School of Dentistry for ten years before going on to set up the Pi treatment center. Dr. Balshi is a keen amateur/ “almost professional” photographer of nature. Another great hobby, historical restoration, has been put to use at the Balshis’ home, a 1699 house built by Welsh Quakers on property acquired from William Penn.

Publications and Awards Professor Vidal has published numerous scientific articles and two books. He is the Editor in Chief of the Chilean Plastic Surgery Journal and associated editor of the IberoLatinoAmerican Plastic Surgery Journal. Professor Vidal researched plastic surgery perceptions for a number of years and published a fully illustrated Manual of Plastic Surgery aimed at the general public, and with a foreword by Professor Ivo Pitanguy. Professor Vidal was awarded the National Plastic Surgery prize, as Plastic Surgeon of the year 2005, conferred by the Chilean Society of Plastic Surgeons.

Speciality Prof. Avelar is Brazil’s master of, and active in all areas of, aesthetic surgery including body contouring, such as reconstruction of the buttocks, and facial reconstruction such as the nose (rhinoplasty) and the facelift (rhytidoplasty) involving the volumetric sculpturing of the lower two thirds of the face and neck. His speciality lies in the reconstruction of the ear and the tummy tuck (abdominoplasty).

Credentials and Associations Prof. Avelar was twice president of the Brazilian Society for Plastic Surgery (1986-87 and 1990-91), a 3800-member group and one of the largest plastic surgeon associations in the world. He is a member and postgraduate professor of the International Society of Aesthetic Plastic Surgery, a member of the Pan Pacific Surgical Association, the American Association for Science Development and the New York Science Academy. Prof. Avelar is also a fellow of the International College of Surgeons, the Brazilian Society of Plastic Surgery, the Medicine Academy of São Paulo, and president of the Alumni Association of Prof. Ivo Pitanguy and the Professor Illouz Association. Prof. Avelar is also president of the student association of Ivo Pitanguy’s former students.

I’VE NEVER UNDERSTOOD WHY SOME PEOPLE GET SO PO-FACED WHEN THE MATTER OF PLASTIC SURGERY – OR COME TO THAT ANY OF THE NEW PROPS THAT ARE THERE TO MAKE THOSE OF US WHO ARE NOT SO YOUNG FEEL BETTER ABOUT OURSELVES – ARE UP FOR DISCUSSION. IT’S ALMOST AS IF IT WERE A MATTER OF DEEP MORAL IMPORT AND THAT THERE IS SOMETHING INHERENTLY SUPERIOR ABOUT ALLOWING THE WRINKLES TO WRINKLE AND THE FROWNS TO FURROW AND THE DROOPY BITS TO GET DROOPIER. Plastic surgery seems to me to be fundamentally a physical issue and providing you’re not proposing to pay for it by swiping the bread from your children’s mouths I cannot see that it is a moral issue at all. What good plastic surgery does is to cut away the droopy bits of skin. It removes bags from under the eyes, the droopiness on the top eye-lids (not a good look bloodhound eyelids), sagginess from the chin, and generally tightens up the skin. That’s it. Now where’s the problem with all of that? Of course if it’s badly done it’s a hideously expensive mistake but these days it seems to me surgeons are getting better and better at it – heaven knows they’re getting more practice – but the general notion that it would be better to put up with the brown spots, red veins, droops and sags when there is something that can be done about them seems to me just plain daft. Nobody who looks at the before and after pictures of such cosmetic success stories as Mary Archer, Julie Christie, Sharon Osbourne could surely still take the view that in their shoes they wouldn’t have done the same? All three – and there are thousands of others like them – look a million times better and happier now that the damage the years can do has been held back a little longer. Sharon Osbourne has spoken out loudly and frequently about how happy she is with the work that has been done and quite right, too – whatever it is (and I’m not privy to the details though I know it includes a face-lift and some body treatment) it was worth it every time. It speaks of vanity is the usual charge by those who get all sniffy at the very idea. Well, firstly I see nothing wrong with vanity – vanity makes us try harder, vanity makes us strive in many areas of our lives, vanity makes us look better which adds considerably to the gaiety of the nation. But there is more than vanity at stake. Quite often these days it is often a matter of job survival. As we all live longer and longer (hurrah!) we want to go on living full and busy lives and if we want to do that it is no use denying that looks count. Whether you’re behind the counter in a shop, working in a lawyer’s office, trying to hold your own in the world of fashion or running a small business of your own, how you are seen by the outside world will affect your prospects. Middle-aged women – think of Lady Thatcher in her hey-day, or Madeleine Albright to name but two extremely high-flyers – are these days doing extraordinary things.

Whereas their mothers and grandmothers were winding down (if they weren’t dead) many are just coming to the peak of their careers. They may have ten, twenty, thirty more productive years ahead. Of course they want to look their best. Until fairly recent times they only had the beauty companies, the fashion designers, the therapists and nutritionists to help them on their way. These days there is a bewildering array of often very technical options for them to choose from, ranging from the surgeons’ knife to lasers, light pulses, fillers, acid peels, and lots, lots more. Anything from sun-damaged skin to brown spots, red veins, dubious moles, too much hair and many other afflictions can now be treated. And if the treatments are there that will deal with the things that indubitably make women feel less confident about themselves I cannot for the life of me believe it is anything other than a good idea to have them attended to. “But it’s dishonest” is the cry one sometimes hears. It is only dishonest, it seems to me, if those who try to look their best try to pass themselves off as something that they are not. A foolish ploy it seems to me and not one that most of the women I know have in mind. Most simply want to look as good as they can and want the unattractive signs of ageing (the droops, the brown spots, the whole sorry lot) removed. Apart from anything else, as Joan Bakewell said so eloquently on the radio the other day, most people feel most comfortable with people who have largely lived through the same span of years - the music, the books, the theatres, the history that have been shared are a bond that is too precious to ignore. There are, of course, those who are frightened of the surgeons’ knife – and not without reason. Mistakes are sometimes made (it must be almost impossible to estimate the damage that Mrs. Wildenstein alone must have done to the world of cosmetic surgery) and the problem is that once made, they are hard to put right. And common observation does reveal that we do only have one face apiece (and only one body come to that). But the answer to that isn’t no cosmetic surgery – it’s better cosmetic surgery, more finely tuned lasers, more skilled technicians. The key has to be to take time and trouble to find surgeons with a good track record. As more and more women who have had “work” done begin to speak out and tell their friends the good news, it will become easier and easier for those who are nervous to hear at first-hand about who is good and who is not. As to those who claim that it is undignified and that they themselves would never do anything quite so drastic. Take a good look at them and they nearly always turn out to be twenty-something beauties who haven’t the faintest idea of what it feels like to look in the mirror day after day and see things gradually going to pot. Just wait, I long to say to them, and see if you’re quite so adamant when your own looks begin to fade. Read Nora Ephron’s rail against the trials and tribulations of growing older “I feel bad about my neck” and those

Gore Vidal once put it, “isn’t for cissies.” But praise be these days there are things - highly technical, very skilful, very successful - things, that can be done and for that I think we should all be duly grateful.
