40 Years for Conservation Foundation

Online: Browse the full activity guide online, register for activities and pay by credit card at csparks.org In person: Complete a registration form and pay by cash, check or credit card at Simkus Recreation Center. By Mail: Complete a registration form and mail the form and check to Simkus Recreation Center, 849 W Lies Rd, Carol Stream, IL 60188

Payment Full payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/ AMEX/DISC). Make checks payable to the Carol Stream Park District. There is a $20 fee for returned checks.

Program Capacity All programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits will be issued.

Waiting Lists Once the maximum enrollment is met for a class, a waiting list will be formed. We will create additional classes when feasible and you will be contacted. Refunds will be processed after the first class.

Insurance The Park District does not carry medical or accident insurance for program participants or park patrons. Please review your personal health insurance plan to be certain that you and your family have proper coverage.

Financial Assistance We offer financial assistance/scholarships for qualified Carol Stream Park District residents. Confidential applications are available at the Simkus Recreation Center.

Satisfaction Guaranteed! Our goal is your satisfaction! If you are not completely satisfied with any Carol Stream Park District program, class or event, please tell us! We will work to earn your satisfaction. If you are still not completely satisfied, we will transfer your registration to another program or session, or issue you a refund - that’s our guarantee!

Green Up Your Workout Running shoes are designed to last for 300-400 miles of use. So what do you do with worn out running shoes? Nike’s Reuse-A-Shoe program was created to solve the problem. Old shoes of any brand are collected and recycled to be used to build new tracks, basketball and tennis courts. Shoes can be dropped off at Nike stores or mailed to the recycling center. Dan Bird (2003-2015), John Jaszka (2011-2015), Jacqueline Jeffery (2011-2015), Tim Powers (2007-2015). The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by residents to represent the community’s best interests. The charge of the Board is to set policies that will guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait - a commitment to serving the park and recreational needs of our community.

Saturday, Oct 6, 7:15pm Get ready for a night of fun and movie-excitement at Splash Central. We will be showing a movie on our Big Screen that the whole family will enjoy. As dusk falls on Hampe Park, join us to learn some essential skills to prepare you for the zombie apocalypse! Learn how to zombie proof your house and make a zombie survival kit. Be trained on how to fight and/or outrun a zombie. After the tutorial, test your knowledge in our zombie trivia challenge. Wear your best zombie costume and we’ll provide the make-up to transform you into a zombie. Prizes awarded for best zombie costume, walk and moan.

Experience the excitement of opening a personalized letter from Santa Claus! Your child can drop off a letter to Santa that we will deliver to the North Pole. Look for the special mailbox inside Simkus Recreation Center from November 26 - December 9. Please make sure that your child’s full name and address are included in the letter so that Santa knows where to send his correspondence. You can also email him at santa@csparks.org. Don’t miss out on this special holiday tradition that is sure to put a smile on your child’s face!

Don’t be scared to attend our new family Halloween event! The Simkus Recreation Center will be transformed into a ‘spooktacular’ palace, oozing with Halloween activity. Wear your costume and visit the mad science lab, make spooky crafts, eat ghoulish snacks and hunt for candy in the haystack! Polar Express Sunday, Dec 9, 11am & 3pm All aboard! Come along on our annual train ride for a reading of the children’s classic story ‘Polar Express’. Bring a copy of the book and read along with us if you wish. We will ride from the Roselle train station to the ‘North Pole’ (Elgin/Big Timber). Santa will then join us on the train for the ride back to Roselle and sing holiday songs with us. Afterwards, Santa will have plenty of time to visit with each child and hear their holiday wishes. Refreshments and gift included in fee. A parent or guardian must accompany children on the train. Adults will also need to be pre-registered for this event. Please arrive 20 minutes prior to departure and parking is available. The deadline for registration is November 2. Santa Visits Tuesday, Dec 18 & Wednesday, Dec 19 Why wait in line at the shopping mall to sit on Santa’s lap when Santa can come see you? Surprise a child with a visit from Santa right to your house. Santa will arrive in his ‘custom’ RV and listen to your child’s holiday wish list inside the RV! Due to his heavy schedule, Santa can only stay for five minutes in front of each house and will only be able to visit with immediate family members. You will need to be available 5:30 to 8:00PM so Santa finds you at home. Santa visits are open only to residents of the Carol Stream Park District. Santa Visits are sponsored by Hometown RV, located right here in Carol Stream. They’ve generously agreed to donate the use of Santa’s RV. Visit them on the web at www.hometownrv.com. SWIM LESSONS Before registering for a swim lesson • Review previous lesson history and age limitations of class • Returning Bartlett Park District swimmers, register for lessons according to a previous swim lesson report card. • Registration will close three business days prior to the start of the class. • Refunds will not be given for classes missed or forgotten. • Class levels may be combined if deemed appropriate by aquatics staff. • Classes fill quickly; transferring from one level to another is not always possible. If you have questions, please call Bartlett Park District. • Due to the limited pool deck space, parents will be asked to sit in viewing area or bleachers. What to bring: Towel, swim suit already on child, report card from previous session. What not to bring: Swimming aids; lifejackets, water wings or flippers, snorkel masks that cover the nose. On the first day arrive 15 minutes early for check-in. Please stay for the entire swimmer’s class, in case of emergency where consent is required. Wait for your child in viewing area or bleachers. It is important to keep the pool deck clear so the instructor can have your swimmers full attention during the class time

Develop comfort in the water, water adjustment, entering and exiting the water, front and back kick, safety skills, swimming readiness, floating with assistance, putting face in water, and blowing bubbles. Swim instructors facilitate the lesson, parents instruct child.

Learn how to enter deep water with confidence, establish independence from instructor, front and back float, rotary breathing, front and back stroke fifteen feet without assistance, elementary backstroke fifteen feet without assistance, introduction to breaststroke, butterfly kicking, and safety skills.

Registration is by grade level. Players are to be registered for the grade the player will be in for the 2012-13 school year.

Preschoolers must be entering kindergarten for the 2013/2014 school year.

WDSRA provides a fun, safe environment for more than 4,500 children, teens, and adults to choose from over 1,500 programs, trips, and special events throughout the year. Programs everyone to participate at their own ability level. We offer a wide range of programs to reach a variety of interests, including: swim lessons, special Olympics, music, baseball, soccer, drama, art, bowling, trips, outings, social clubs, wheelchair and adapted sports and much more.

WAIVER REGISTRATION/BROCHURE WAIVER & RELEASE IMPORTANT INFORMATION The Carol Stream Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for the above listed programs/activities must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. 40 Years for Conservation Foundation

Online: Browse the full activity guide online, register for activities and pay by credit card at csparks.org In person: Complete a registration form and pay by cash, check or credit card at Simkus Recreation Center. Payment Full payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/ AMEX/DISC). Make checks payable to the Carol Stream Park District. There is a $20 fee for returned checks.

Program Capacity All programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits will be issued.

Waiting Lists Once the maximum enrollment is met for a class, a waiting list will be formed. We will create additional classes when feasible and you will be contacted. Refunds will be processed after the first class.

Insurance The Park District does not carry medical or accident insurance for program participants or park patrons. Please review your personal health insurance plan to be certain that you and your family have proper coverage.

Financial Assistance We offer financial assistance/scholarships for qualified Carol Stream Park District residents. Confidential applications are available at the Simkus Recreation Center.

Satisfaction Guaranteed! Our goal is your satisfaction! If you are not completely satisfied with any Carol Stream Park District program, class or event, please tell us! We will work to earn your satisfaction. If you are still not completely satisfied, we will transfer your registration to another program or session, or issue you a refund - that’s our guarantee!
