Congratulations to Bill Bonnema (pictured on the top left with his grade 5-6 basketball team), recently named the Carol Stream Citizen of the Year for 2009.
Bill has dedicated hundreds of hours of service to the Park District through the youth sports programs. He has proven his leadership as a member of our basketball and baseball planning committee, a baseball league coordinator, and as a coach and manager for six different teams this year. Bill is an enthusiastic volunteer who is always willing to share ideas to improve the Park District programs not only for his family, but for all of the children in our sports programs. His assistance has helped the recreation staff develop youth sports programs that evaluate, teach and inspire players. Dive in with the Dolphins! Join the Dolphins Swim Team The summer swim team is designed to accommodate every level from a novice swimmer to a highly competitive age group of swimmers. Each coach strives to develop the swimmers to their fullest potential, while preparing for the next level. Parent volunteers are needed to work at all swim meets. Assessments are also required for any of these levels if more than one session has been skipped. Siblings under the age of 6 may not attend evening class observation. Before & After School Care 2009-2010 School Year This program provides opportunities for all children enrolled to develop socialization and a positive self-image through experiences in a friendly, structured, and learning environment. The program provides a safe atmosphere where children develop problem solving skills and self-discipline through interaction and quality activities. Physical activities such as kick ball, freeze dance and playing at the playground is encouraged to help the children maintain a healthy lifestyle. Scheduled time is provided for homework and reading. We positively reinforce the importance of the Character Counts philosophy. Coral Cove Water Park Rental The Coral Cove Water Park is available for rentals during the summer on Fridays and Saturdays from 8:15 pm - 10:15 pm and on Sundays from 7:15 pm to 9:15 pm. We have packages for your small group of up to 50 or for a larger group up to 1500. Full payment is required to secure a date. Rentals may be canceled due to inclement weather at the manager’s discretion and can be rescheduled if dates are open.

3 Waiver REGISTRATION/BROCHURE WAIVER & RELEASE IMPORTANT INFORMATION The Carol Stream Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Carol Stream Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for the above listed programs/activities must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.

WARNING OF RISK Recreational activities are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers and injuries due to inclement weather, slip and falls, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities exist. In this regard, it must be recognized that it is impossible for the Carol Stream Park District to guarantee absolute safety.

WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK Please read this form carefully and be aware that in signing up and participating in the programs listed above, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and associated with these programs (including transportation services and vehicle operations, when provided). I recognize and acknowledge that there are certain risks of physical injury to participants in these programs, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in these programs against the Carol Stream Park District, including its officials, agents, volunteers and employees. I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. If registering on-line or via fax, my on-line or facsimile signature shall substitute for and have the same legal effect as an original form signature. Carol Stream Park District occasionaly takes photographs of participants in park district programs, classes or events for marketing purposes. Park District resdients are also welcome to submit their own photos to info@csparks.org.

Financial Assistance We offer financial assistance/scholarships for qualified Carol Stream Park District residents. Confidential applications are available at the Simkus Recreation Center.

We will work to earn your satisfaction. If you are still not completely satisfied, we will transfer your registration to another program or session, or issue you a refund. That’s our guarantee!
I strongly believe team sports offered by the Park District provide our children a great learning experience. Congratulations to Bill Bonnema (pictured on the top left with his grade 5-6 basketball team), recently named the Carol Stream Citizen of the Year for 2009.
Bill has dedicated hundreds of hours of service to the Park District through the youth sports programs. He has proven his leadership as a member of our basketball and baseball planning committee, a baseball league coordinator, and as a coach and manager for six different teams this year. Bill is an enthusiastic volunteer who is always willing to share ideas to improve the Park District programs not only for his family, but for all of the children in our sports programs. His assistance has helped the recreation staff develop youth sports programs that evaluate, teach and inspire players.
