TO OUR WHOLE COMMUNITY:

If there is one thing I can say about the past year, it’s that it tested our agility, emphasized our relevance, and captured our character as an organization. We faced challenges, made progress, celebrated success, and grieved losses. But today I write in gratitude: proud of our accomplishments, confident in our strategy, and, frankly, inspired by our extraordinary staff. And although our communities are still beset by the disruptions, uncertainties and tragedies created by the COVID-19 pandemic, the world of tomorrow is brimming with possibilities.

Over the past year, Cascadia Health has responded to the needs of our community—from crisis intervention and housing to recovery and wellness supports, we continued our path to integrate care more fully for those we serve. This included welcoming over 120 individuals and families to our newest affordable housing site, Centennial Place Apartments; celebrating 30 years of crisis intervention through our Project Respond program; and working with industry partners to advocate for various policy priorities during the 82nd Legislative Session, including securing a 3.4% costof-living rate increase (annually over the next two years) and nearly $85 billion in new facility infrastructure, among other investments to support service delivery, capacity expansion, and workforce stability.

This year, through various grants awarded to Cascadia, we are looking to better integrate care and expand services. This includes a $6.25 million grant to implement OCHIN Epic Electronic Health Record software across behavioral health, synthesizing information from across services and helping us realize our goal of seamless collaboration between the many different providers across the organization. In addition, through an $8 million grant awarded to Cascadia last fall, we are looking to add 55 additional units of housing to support the increasing need in our community.

Finally, we looked internally. Cascadians—our dedicated staff—thrive on excellence and candor. We are a group of individuals who work together to accomplish something beyond ourselves. And to ensure our continued cultivation of an environment where people want to work, we listened, focused on feedback, and made strides toward improvements through our Workplace Excellence Initiative, including addressing themes surrounding wages and benefits, communications, workload, training, recruitment and retention, leadership, tools and resources, and equity.

Our work is never done and impossible to capture in a single report. But as we envision what’s next for those we serve, for our community, and for our employees, we are emerging stronger and digging deeper: emerging as a stronger organization, a stronger leader, and a stronger resource— as well as digging deeper by forging ahead on plans to provide more housing, more health, more hope, and more love for our whole community.

As both a Certified Community Behavioral Health Clinic (CCBHC) and a Federally Qualified Health Center Look-Alike (FQHC-LAL), Cascadia Health believes that an integrated approach to care is the best way to serve our community. This includes providing integrated mental health, substance use treatment, primary care, crisis intervention, peer wellness, skills training, housing support, and more to individuals and families. Our clients can find the care they need through one team and often at one location, regardless of their ability to pay.

MENTAL HEALTH TREATMENT

Cascadia offers evidence-based, trauma informed mental health services for people of all ages, including specialty mental health services for those in the correction system and programs for those with chronic and severe mental health challenges.

ADDICTION RECOVERY

We offer treatment, recovery and prevention support for alcohol, drug, and gambling addiction. Services include counseling, groups and medication management.

PRIMARY CARE

Offering primary care gives us the opportunity to look at our patients’ whole health care needs, including disease and medication management, family planning, vaccines and immunizations, urgent care, and more.

COMMUNITY SUPPORTS

Creating stronger communities through supported employment, education, street outreach, community reintegration, licensed residential, permanent supportive housing, independent housing, and community wellness programs.

CRISIS INTERVENTION

Our Urgent Walk-In Clinic, mobile crisis unit, and respite programs support community members facing crisis and provide support and safety.

PEER WELLNESS

Peer Wellness Services are provided by individuals with lived experiences of mental health and/or substance use challenges.

OUR IMPACT | INTEGRATED CARE

“At Cascadia, we know that in order to care for someone with severe and persistent mental illness (SPMI), you have to take care of their whole body. Sadly, those with SPMI die on average 25 to 30 years sooner than those without, and they’re not dying from complications from their mental health. They die from ailments like diabetes and high blood pressure; they die because their physical health needs have been unmet for so long. It is essential to take care of the whole person. That leads to longer lives and better relationships. It means better health. And that’s why it’s so important.”

OUR IMPACT | INTEGRATED CARE

Cascadia Health has engaged in a variety of projects to help drive integrated care efforts through the Certified Community Behavioral Health Clinic demonstration program (2017 – present), as well as through a series of additional expansion and advancement grants. This includes adding primary care services to our Clackamas Health Center in January 2021 and multiple pilot programs.

WHAT’S IN A NAME | CASCADIA HEALTH REBRANDS

In line with Cascadia’s long journey toward integrated care, the organization officially changed its name to Cascadia Health in June of 2022, highlighting our certification and designation as a CCBHC and FQHC-LAL, respectively.

The new name allows Cascadia to continue its work in the community behavioral health space and present itself as a truly integrated provider offering a person-centered approach to whole health for those we serve.

OUR IMPACT | INTEGRATED CARE

ONGOING EFFORTS: CCBHC-IMPROVEMENT & ADVANCEMENT GRANT

Last September, Cascadia was awarded a four-year SAMHSA CCBHC-Improvement and Advancement Grant. This funding will help us continue to provide integrated healthcare, with a focus on improving care for individuals with co-occurring mental health and substance use disorders.

FQHC PROGRAM UPDATE

In 2021, Cascadia Health was awarded Federally Qualified Health Center Look Alike (FQHC-LAL) status by the Health Resources & Services Administration (HRSA). Since this time, Cascadia has extended primary care services to each of its four health centers, including Clackamas Health Center in March 2022, and Plaza Health Center in September 2022. This past May, 2022, Cascadia held a successful site visit with HRSA representatives resulting in positive feedback and setting the stage for continuing our FQHC journey. (Every two years, HRSA will complete a site visit to provide oversight and ensure all federal requirements are followed.)

340B DRUG PRICING PROGRAM – PASSING SAVINGS TO OUR COMMUNITY

In 2023, Cascadia Health began participating in the 340B drug pricing program. The program requires pharmaceutical manufacturers participating in Medicaid to sell medications at discounted rates. Cascadia can then use the savings to invest back into services and the community, among other benefits.

30 YEARS OF RESPONDING TO THE CALL

Since the early 90s, a dedicated team of crisis responders have been answering the call to assist people experiencing mental health emergencies. A cornerstone program, Project Respond has met individuals and families where they are, helping them regain a sense of control and connecting them with follow-up support 24-hours a day, seven days a week.

In 1993, Project Respond was initially tasked with outreach crisis work limited to downtown Portland. The last 30 years have seen remarkable expansion in line with the city’s evolving needs. Today, Project Respond is a many-tiered effort comprised of specialized teams. This has included a partnership with the police, who will call on Project Respond to help in circumstances when mental health issues are present.

In addition, Project Respond includes the following programs:.

• In the past year, Project Respond answered 5,114 calls from 2,373 individuals

OUR IMPACT | HOUSING

Cascadia brings safe, affordable housing to those who need it most. For 30 years, Cascadia has been a leader in developing and managing safe and affordable housing for Oregon residents with mental illness and addictions. Cascadia owns, operates and/ or manages over 800 units or vouchers of housing in sites operating as licensed residential, permanent supportive housing, and independent housing.

HOMES + HOPE

In fall 2022, the Oregon Health Authority awarded Cascadia four housing grants totaling $8 million. The grants will add about 55 units, including three Licensed Residential properties (about 15 total units) and one Supportive Housing property (about 30-40 units). Over the past year, Cascadia has worked to purchasev and renovate properties with the grant dollars. Completion dates are still tentative.

5 YEARS AT GARLINGTON

Named in honor of the late Reverend Dr. John W. Garlington, Jr. and Mrs. Yvonne Garlington, who both championed social justice in Portland, Garlington Health Center began serving clients as Cascadia’s first fully integrated care center in September 2018. As we celebrate Garlington Health Center’s vital community role over the last five years, let’s look back on some of the other milestones in the health center’s history.

2018 | GARLINGTON CAMPUS OPENS

• At a moving ribbon-cutting ceremony attended by 250 federal and county officials, employees, and community partners, Cascadia celebrated the Garlington Campus (including the health center and Garlington Place Apartments) opening on Friday, Sept. 21, 2018.

2019 | A NEW PHARMACY & EXPANDED WELLNESS

• In June 2019, Cascadia was awarded the Spirit Mountain Community Fund grant for a new Garlington wellness program. In September, we celebrating the opening of Genoa pharmacy onsite.

2020 | GARLINGTON HEALTH CENTER USED FOR ADMINISTERING COVID-19 TESTS

• Cascadia Health’s COVID Response Team worked hard during the first months of the pandemic to offer COVID testing to everyone who needed it. Garlington proved an ideal space to provide PCR tests to Cascadians and the community.

2021 | FQHC LOOK-ALIKE STATUS

• Look-alike status was Cascadia’s first step in attaining its full FQHC designation, which means our qualifying health centers can continue to expand access to primary care services and increase medical outreach to high-risk populations.

2023 | CELEBRATING FIVE YEARS OF SERVING THE COMMUNITY

• Five years into its time in NE Portland, Garlington Campus has helped hundreds of clients through its suite of wraparound, personcentered services. Thank you to the staff who continue to make Garlington a vital community pillar.

HOMES IN THE HEART OF EAST PORTLAND

In June 2023, Cascadia Health celebrated the official grand opening of its newest affordable and supportive housing site, Centennial Place Apartments. Located in the heart of East Portland, Centennial Place includes 71 apartments and houses over 120 individuals and families with little or no income.

“East County is a wonderful place to live and has a vibrant and engaged community,” said Multnomah County Commissioner Lori Stegmann who attended the event. “I’ve focused a lot of my energy on addressing housing stability and economic development, so it always brings me joy to celebrate not just a new

building but beautiful new homes for East County residents.”

Cascadia Health began constructing Centennial Place in June 2021 and welcomed residents in October 2022. “This project was all about partnership, community, and working together,” said Superintendent James Owens.

“The Centennial School District is proud to have had a long-term partnership with Cascadia Health, providing health services to Centennial students and families. We are grateful to be able to deepen this partnership with the development of Centennial Place.”

In addition, Centennial Place features environmentally sustainable elements that benefit the health of everyone, including solar power, energy star appliances, efficient heating and cooling, and more. As a result, Centennial Place will receive Earth Advantage platinum certification—the highestlevel certification through Earth Advantage—demonstrating Cascadia’s commitment to the health of residents, the community, and the planet.

THOUGHT LEADERSHIP & COMMUNITY ENGAGEMENT

ADVOCACY

During the one-month 2022 and full 2023 Legislative Sessions, Cascadia Health supported legislation to bolster behavioral health services that are more equitable, effectively prioritized, and better funded.

Most notable was Cascadia’s early support for House Bill (HB) 4004 during the 2022 short session, which required the Oregon Health Authority (OHA) to distribute grants to behavioral health care providers for staff compensation, workforce retention, and recruitment. For Cascadia, this meant:

• $7.2 million in workforce investments:

• 75% for compensation increases

• 25% for retention efforts such as benefits, workforce improvements, and more

In addition, funding increases and investments to support service delivery, capacity expansion, and workforce stability were obtained during the 2023 session, in addition to supporting OHA to deliver 2021 investments to providers. This included:

• A cost-of-living rate increase (3.4% annually over the next two years)

• $85 million for infrastructure expansion

• Investments to expand Community Mental Health Provider (CMHP) services

• Training opportunities

• Crisis line (988) investments, and more

LEGISLATION

Cascadia Health tracked over 200 bills during the 2023 session, supporting and testifying on proposed legislation, including (but not limited to):

• HB 2235 (passed), which would establish a task force to study workforce workload and burden.

• HB 2485 (failed), which would direct funding to support social work students in pursuit of the master’s degree.

• HB 2544 (failed), which would invest in licensed residential facilities serving individuals with substance use disorders or mental health concerns.

• HB 5019 (passed), which would appropriate funds for homelessness prevention and services across Oregon.

• Senate Bill 432 (failed), which would require the Oregon Health Authority to develop, implement and administer a program to support consumer engagement efforts aimed at increasing and optimizing consumer involvement in planning and decision making surrounding the access to, and delivery of, behavioral health services in Oregon.

As a nonprofit organization, Cascadia’s funding across most programs and services originates primarily from taxpayer dollars. These funds come through contracts at the federal, state, county, and city levels. Legislative decisions across all levels of government greatly impact the work we do.

In addition, we also rely on generous donor contributions. These funding streams allow us to fund programs, support areas of greatest need, and engage in research that contributes to delivering whole health care.

CULTURE OF CARING | 2022 GALA

Cascadia’s 6th annual Culture of Caring Gala was presented virtually and broadcast locally on KGW on October 20, 2022. The event celebrated Cascadia’s mission, highlighted clients and staff, and honored exceptional community members for their contributions to health, housing, and civic engagement. In addition, the event featured musical performances by Pink Martini and China Forbes. The evening festivities raised more than $350,000!

HEART 2023

Each year, Cascadia’s HEART events explore challenges unique to women in our community while working to reduce the stigma of mental illness and celebrating stories of hope and resilience. A combination of in-person and virtual events, HEART 2023 featured virtual storytelling from Baby Blues Connection, short films from Period.org, collage-making, writing workshops with Write Around Portland, a podcast with OHSU’s Center for Women’s Health exploring mental health and reproductive equity, movement and meditation from Barre3, and a showcase of client art featured at TwentySix Café.
