It’s a heart-wrenching fact: individuals who struggle with severe and persistent mental illness die 20 to 30 years sooner than those without mental illness. Just as distressing, research shows these individuals’ deaths are typically not due to mental illness, but to other health problems — hypertension, asthma, diabetes — that haven’t been adequately treated and that can be aggravated by poor health habits.

Cascadia looks at the whole person and believes that an integrated health care approach is the best way to serve our clients and greater community. Cascadia’s whole health care model delivers integrated mental health and addiction services, primary care, and housing to promote hope and support the well-being of the communities we serve. We envision a community where everyone benefits from whole health care, experiences wellbeing, and has a self-directed, connected life. For 40 years, Cascadia has been the community health and housing safety net provider for Oregonians of all ages experiencing mental health and addiction challenges, trauma, poverty, and homelessness. The COVID-19 pandemic has created significant implications for our community’s health. We face an unprecedented increase in demand for health care services, and supporting the whole health of our community is crucial — now more than ever. Cascadia serves community members impacted by significant adversities. The people we serve can rely on Cascadia’s continuity of care, providing a calming port in the storm. Our mission helps to address health disparities and inequities among underserved communities, including Black, Indigenous, and People of Color. We remain committed to serving our community as we navigate the uncertainties of the future. OUR WORK Programs & Service Impacts July 1, 2020 – June 30, 2021 Cascadia’s whole health care model empowers individuals as they work toward their goals to achieve recovery. From prevention, to healthy lifestyle education, to intensive treatment when needed, our services encompass a systemic approach to health management for the adults, children, seniors, and families we serve. We meet our clients where they are and provide solutions that take into account their experiences and resources.

In 2021, Cascadia was awarded FQHC (Federally Qualified Health Center) designation for our Garlington and Woodland Park Health Centers.

1,019 individuals served by our Child & Family programming Last year, 17 clients engaged in ParentChild Interaction Therapy (PCIT), which focuses on supporting parents in managing behavioral difficulties with children through coaching. This program supports parents in learning and applying skills proven to help children feel calm and helping parents remain consistent with effective approaches to discipline. Our Child & Family team manages partnerships at eight different Portland Public Schools. Cascadia also provides resources at SMYRC (Sexual and Gender Minority Youth Resource Center), a drop-in center for queer and trans youth ages 13-24. Our clinician is available for youth who might have difficulty accessing therapy through traditional

routes. This is especially beneficial for transgender youth who may need a therapist’s referral for gender-affirming medical care. A clinician’s assessments and letters of referral can be life-saving mental health care for these youth.
Enhanced Care Outreach Services Cascadia provides mental health services for clients who have both physical and mental health challenges, and who have had failed placement in aging and disability facilities due to behavioral issues. We also provide services to clients in

Individuals struggling with mental illness are much more likely to have a diagnosis of a chronic illness, like hypertension, COPD, or chronic pain, which is predictive of more emergency department and inpatient hospital visits.
Due to the COVID-19 public health emergency, we have had to limit in-person meetings and travel to facilities. Our clients were increasingly isolated due to facility lock-downs that limited visits from family and friends, decreased opportunities for activities, and created health uncertainties. Cascadia adapted to these circumstances by increasing collaboration with facilities and other agencies, adding telehealth services, and sending mailings to clients and facilities.
OLDER A DULTS The older adult population is growing. By 2030, older adults are expected to make up 21% of the U.S. population. Older adults have unique mental health challenges and are an underserved population. We work closely with community partners, including longterm care facilities, primary care providers, and aging and disability services, to serve older adults. Medically Complex Program

This work was funded by the Cambia Health Solutions. Our Diabetes Integration Project (DIP), focused on staff training and education around diabetes integration, as well as innovation and care coordination to support clients with diabetes and SPMI was funded by CareOregon.

clients engaged in this work Approximately 20% percent of Cascadia clients with a mental health diagnosis also have diabetes or prediabetes. Cascadia implemented a diabetes self-management program using a 12-week group skills training and peer support format. The group was facilitated by a clinician and peer wellness specialist, and it provided individuals with co-occurring diabetes and mental health concerns with information and skills that integrated physical and mental health, addiction treatment, and oral health.
The Emergency Department utilization work will continue to be supported through funding by SAMHSA’s two-year CCBHC Expansion grant.

Peer staff share their lived experience, knowledge, and compassion with clients that supports them in their recovery. Cascadia also provides peer wellness and peer support training in two cohorts each year. While the pandemic impacted the number of people going through this training, we are back on track to pre-pandemic levels. Our renowned training curriculum is being used internationally.

CRISIS SERVICES Project Respond Cascadia’s Project Respond is committed to supporting individuals in mental health crisis in the community and believes that the most appropriate response is trauma-informed and clinical in nature. Project Respond provides a spectrum of services within the community in order to reduce or alleviate pre-crisis and crisis situations. These services include 24/7 mobile crisis response, library services, shelter services, and the Behavioral Health Unit of the Portland Police Bureau.

HOUSING Programs & Service Impacts July 1, 2020 – June 30, 2021 Cascadia is a leader in developing and managing safe and affordable housing to Oregon residents with mental illness and addictions. Cascadia owns, operates and/or manages licensed residential, permanent supportive housing, and independent housing. We bring safe, affordable housing to those who need it most, and believe that housing is healthcare. An important part of our equity work is to actively recruit companies qualified with MWESB ownership (Minority, Women, Emerging Small Business) to provide property-related services within our housing portfolio. Cascadia Housing, Inc. vended 21% of its services to MWESB companies.

This decline is due to several factors, including implementing additional training and support, focusing on retention efforts, developing a collaborative mindset focused on compassion and empathy, and supporting residents’ progress.

In June 2021, we began construction on Centennial Place Apartments, a new affordable housing project at 3750 SE 164th Avenue. The development will have 71 units (15 studios, 32 one-bedroom, and 24 two-bedroom) of affordable apartments. Eighteen units will have rental preference for people with mental illness and will be affordable to individuals earning 0-30% of the Area Median Income (AMI). Fifty-three units will be affordable to households earning up to 60% of the AMI, with a preference for households with a student enrolled in the Centennial School District. Households who are homeless or at risk of becoming homeless will receive the highest preference. All residents will be served by a part-time resident services coordinator and have access to Cascadia services.

HOW WE ARE FUNDED Cascadia Behavioral Healthcare and Affiliates finished the fiscal year 2021 with a strong financial position that included a healthy operating cash position and $5 million in cash reserves.

As a nonprofit safety net organization, Cascadia’s funding, across most programs and services, originates primarily from taxpayer dollars. These funds come through contracts at the federal, state, county, and city levels. Legislative decisions across all levels of government greatly impact the work we do.
Revenues for Cascadia Behavioral Healthcare and affiliates for fiscal year 2021 were $84.5 million, which reflects an increase due to non-recurring payments under COVID-19 relief programs, a retroactive residential program rate increase, and the reinstatement of the CCBHC program funding by the State of Oregon.

We also rely on generous donor support and grant opportunities. These funding streams allow us to fund new programs, support areas of greatest need, and engage in research that contributes to delivering whole health care.
Through corporate and individual giving, we raised nearly $1.1 million.

This includes integrating trauma-informed primary care into our existing array of mental health, addiction, wellness, and permanent affordable housing programs.
