Building Families One Healthy Baby at a Time At Reproductive Medicine Associates of Florida, Dr. George Patounakis and his team help couples fulfill their dreams of being a parent.

C

reating a family is a dream for many, but that dream isn’t always easily achieved. In fact, one in eight people struggle with

infertility. The RMA Network has been at the forefront of research, innovation, and patient-focused fertility treatments for almost two decades. Dr. George Patounakis, who began seeing patients at Reproductive Medicine Associates of Florida on Dec. 11, 2017, provides personalized patient care based on a proven method from RMANJ, where success rates are 20 percent higher than the nation-

This success is based on a three-prong approach: science, success and support. “We’re truly committed to helping the patient build their family one healthy baby at a time,” Dr. Patounakis says. “We also work with families of all types to help them fulfill their dreams of being a parent.” How it Works RMAFL thrives on its patientfocused care philosophy that incorporates the latest scientific research and breakthroughs as well as industryleading expertise to provide patients with the most positive experiences possible. Each patient receives an individualized treatment plan geared toward providing the best possible chances of starting their own family. Dr. Patounakis is extremely hands on throughout the process. Every time the patient visits the of-

fice, they see him. He performs ultrasounds in the mornings and stops to chat during blood work appointments. Most importantly, he is always accessible should they need anything along the way. “We’re not just about using standardized protocols,” Dr. Patounakis says. “We will tailor each plan to each patient, and not just the clinical aspect but also in the lab.” Right there on the premises, technicians can fertilize eggs, culture embryos and perform biopsies to ensure they are genetically normal. Using the genetic results, they are then able to select one single embryo that has the highest chances of leading to the desired result: a live birth. “Overall, we are both high tech and high touch,” Dr. Patounakis says. MEET THE DOCTOR George Patounakis, M.D., Ph.D., F.A.C.O.G., is the lead physician of Reproductive Medicine Associates of Florida. After earning his bachelor’s degree, master’s degree and doctorate in electrical engineering, Dr. Patounakis’ passion shifted to health.

RMANJ, Dr. Patounakis dedicates himself to patient success. While RMAFL consistently pursues the desired outcome of each patient, the facility is willing to share in the risk of failure. “We feel so strongly about helping people build their families that we have a shared risk program that takes out the financial risk of going through treatment but not getting the desired result of a live birth,” Dr. Patounakis says. Care Share is a fertility care refund program that provides hopeful parents with reassurance and avoids the financial risk should they require alternative means of family building. A Circle of Support Along with his staff, Dr. Patounakis understands that there is not one single answer to every infertility case, so patients can expect an unparalleled personal degree of attention and care throughout their treatment. They can also expect a full range of emotional support. “One of the deterrents to being successful is that people get emotionally burnt out,” Dr. Patounakis says. “A large part of my job and the job of my staff is to make sure that we provide emotional support to give them the energy to keep moving forward.” In addition, a private Facebook group called FertiliTalk is a resource that was created by patients for patients. Members include individuals who are struggling with infertility

He went on to earn his M.D. from Rutgers — Robert Wood Johnson Medical School before completing his internship and residency in obstetrics and gynecology at Thomas Jefferson University Hospital. Dr. Patounakis pursued his fellowship training in reproductive endocrinology and infertility at the National Institutes of Health in Bethesda, Maryland. While there, his research focused on cholesterol transport in steroidogenic cells using live-cell microscopy, IVF outcome predictors and genetic influences on infertility. In addition to being the medical director for RMAFL, Dr. Patounakis is an assistant professor of obstetrics and gynecology with the University of Central Florida College of Medicine. His research interests include statistical IVF cycle modeling, fertility preservation and diminished ovarian reserve. He also serves as an ad hoc reviewer for Fertility & Sterility and the Journal of Assisted Reproduction and Genetics.

People Serving People Matthew’s Hope Ministries has provided vital resources for homeless individuals in the community for nearly a decade. By Lyndsay Fogarty

M

atthew’s Hope Ministries is calling on the community to help restock its food pantry as an influx of requests for help have quickly left the shelves nearly bare. Since 2010, the faithbased nonprofit organization has worked to successfully move people from homelessness to a life of independence and self-sustainability through a variety of assistance options, programs and resources. Based in Winter Garden, volunteers connect Matthew’s Hope guests with the resources and services they need to get back on their feet. They also offer showers, haircuts, clothing, shoes, hygiene products and so much more. The Garden of Eatin’ food pantry serves the most basic food needs of every guest who arrives between 8:30 a.m. and 1:30 p.m. for Tuesday Outreach Services Day each week. Guests are provided with $5 in Matthew’s Hope “money” to spend in the pantry, and they are invited to participate in various work opportunities through the Moving Forward Program to earn more.

Matthew’s Hope has seen a 21% jump in the number of people served this spring. Volunteers welcomed 77 new individuals on top of the 300 to 400 people that are served on a regular basis. Many are the working homeless who can’t find affordable housing and aren’t paid a high enough wage (or provided the work hours needed) to support themselves and their families. Due to the increase in need for the food, hygiene and clothing pantries, the shelves aren’t as full as they once were. Help is needed to restock so the organization can continue providing this much-needed service to all of those who need it within the community. Some of the most needed food items include Vienna sausages, Spam, canned chicken, canned ham, canned pastas, canned fruits, chili, Ramen noodles, spinach, peas, baked beans and potatoes. Founder Scott Billue asks residents to consider participating in a BOGO (buy one meal for your family and also get one for a Matthew’s Hope family), donating

monetarily or organizing a food drive with your church, school, office, civic group or neighborhood. Every contribution makes a difference to individuals and families who have found themselves homeless right here in our community. One of Luke’s most precious moments was his first experience with the beach in Waimanalo, Hawaii during our visit with family to celebrate his first miracle birthday. We can tell that he loved the wind on his face, the sand on his feet and floating with daddy in the ocean waves. What is your favorite story to tell about Luke? I love to share how God answered the prayers of his parents who didn’t know if they were ever going to have a baby after 10 years of marriage, and how Luke surprised them with his gift of life. Luke’s name was chosen 10 months before he was born, but unbeknownst to his parents, his name would fit his journey perfectly. The name Luke means “light giver” and his middle name is “Kekoa,” which in the Hawaiian language means “courageous” or “warrior.” They call him their “little Warrior of Light.” Luke’s smile radiates with joy, and his blowing kisses say, “I love you”, and his hugs of compassion and affection can heal a wounded soul. Ingredients • 2 oz. pumpkin pie vodka • 2 oz. pumpkin cream liqueur • 1 oz. salted caramel vodka • 3 oz. unsweetened vanilla almond milk • Graham crackers • Caramel (optional) Directions Pour first four ingredients into a shaker with ice. Shake well and strain into a martini glass with a graham cracker crumb rim and caramel swirl if desired.

Heavy on the fall flavors, with a sweet and savory twist, this bar snack is a tasty complement to your happy hour cocktail. Ingredients • 2 tablespoons brown sugar • 2 tablespoons maple syrup • 1 1/2 teaspoon pumpkin spice mix • 1 tablespoon butter or margarine • 1/2 cup chopped pecans, optional • 5 cups popped popcorn Directions In a large saucepan or pot, heat brown sugar, maple syrup and pumpkin pie spice mix over medium heat. Cook, while stirring, for 3 minutes or until sugar is dissolved and mixture is bubbling. Stir in butter until melted and well blended. Add pecans, if desired, and popcorn and stir until well coated. Allow mixture to cool before serving. Serve immediately or store in an airtight container. 18

Non-surgical spinal decompression therapy is a painless and noninvasive procedure that helps patients return to normal function and get back to living a pain-free life. This gentle, proven therapy is effective in the rehabilitation of spinal disc injury and is currently one of the safest and most reliable therapies available. Patients who are experiencing a pinched nerve, sciatica, neck pain, lower back pain, or symptoms related to damaged, bulging or degenerative discs could benefit a great deal from decompression therapy. It can also be used to treat syndromes of the lumbar and cervical spine. During a spinal decompression therapy treatment, a patient lies on a therapeutic table and is strapped to a padded harness. They are monitored by a chiropractor as the device gently stretches the spine to relieve pressure on the spinal discs.

Throughout the process, fluid from the outside of the disc is sucked back into the disc. Once the disc rehydrates, and the size of the disc space gets larger, there’s more space for nerves to exit the spine. The nerve will begin healing when it’s no longer being compressed or pinched. The number of required treatment sessions varies among patients, depending on the severity of the injured or diseased disc. Sometimes patients are better after only four treatments, and other times it may take 20 treatments. Every individual is unique. Not everyone is a candidate for spinal decompression therapy; for example, individuals with artificial discs or conditions that compromise the structure of the spine. I’ve found in my practice that many clients suffer very specific injuries to specific parts of their bodies. To support subjective complaints of pain, most crash victims undergo various diagnostic studies designed to elucidate the existence of structural tissue damage as a result of a crash. These include xrays, MRIs, EMGs and CT scans. The results of these tests are remarkably similar in many cases. For instance, when clients complain of neck pain, MRIs often show herniated discs and other disrupted tissue at the level of C3-4 and C4-5. The same is true for clients who complain of back pain, where MRIs often show herniated discs at L4-5 and L5-S1. These are the most frequent injuries because of the seat’s design and the use of a shoulder and lap belt combination. In a collision, the body is lurched forward until the seatbelt stops you, at which point your body is then thrust backwards into the seat. The head whips forward then backwards, causing whiplash. Due to the height of the seat and headrest, and because the shoulder harness doesn’t prevent movement of the head, the rapid flexion of the neck seems to cause injury

in the same location across patients. Additionally, as the hips are jolted forward, the lap belt holds you in place until the seat catches up to you, causing a traumatic impact with your low back and injury to your spine. Crash victims often seek the care of spine specialists in the form of chiropractic, orthopedic or neurosurgical disciplines. An intense course of chiropractic manipulation, anti-inflammatory medication and physical therapy can reduce the pain and loss of function caused by the crash. However, depending on your pre-existing physical condition, the size of the vehicle you’re riding in, the forces generated in the crash and a host of other factors, the injuries could be significant enough to require surgery. After a course of chiropractic care

and physical rehabilitation, a physician may recommend a series of injections that include anti-inflammatory medication that can be injected precisely at the injury point to reduce pain and swelling. If these therapies fail to relieve the pain sufficient enough for the patient to return to their pre-injury lifestyle, surgery may be warranted. In that case, specialists utilize various techniques and innovations to perform invasive surgery causing the least amount of tissue disruption, with the most common being fusions. If you’re injured in a motor vehicle crash, it’s imperative that you see a physician or present to a walk-in clinic within 14 days. Otherwise, you will forfeit $10,000 in benefits designed to pay your medical bills.

3D Mammograms Bring New Dimension to Cancer Detection How this new technology has made it possible for mammograms to better detect cancer. By Leena Kamat, MD

ince the 1960s, mammograms have been the best way to detect breast cancer early. Still, the technology has had its limitations. With 2D digital mammography, a potential trouble spot could appear as just that — a spot. If the radiologist sees a zone of density and can’t be sure what it is, the patient must return to get more images taken or undergo a biopsy. If the radiologist had the ability to look both beneath and above the spot, she could have seen whether the dense area in the breast was a harmless clump of breast tissue or whether it was a dangerous tumor. Now, she can do just that with the help of 3D mammography. 3D mammography allows you to scroll through multiple levels and see if a potential problem spot is regular

tissue or, possibly, a tumor. It also allows radiologists to find small tumors that could be hiding in denser areas of the breast. These areas appear as white spots on mammograms and can sometimes block our view of tumors, which can be disguised as white spots. 3D mammography gives us a view through the tissue, making it possible to catch something that would have been otherwise obscured.

How 3D Mammography Helps Women with Dense Breasts

A woman’s breasts are composed of several kinds of tissues, including fat, milk ducts and supportive tissues. Different women have different amounts of each, and they change over time. A woman who has more dense tissue than fatty tissue is said to have “dense breasts.” Traditional 2D mammography has limitations for women with dense breasts because dense tissue appears as white on its images. In 3D mammography, the radiologist can get a better idea of what these spots truly are by looking above and below them.

Dealing With Dementia Use these tips to communicate more effectively with a loved one who is living with Alzheimer’s disease or dementia.

A

lzheimer’s disease (AD) is on the rise. All forms, from classic to late onset to young onset (which has seen an 83% increase between 2013 and 2017), drain family resources, energy and, all too often, joy. At this time, there is no meaningful treatment for AD. There are, however, any number of interventions that assist caregivers for the persons living with AD (PLwAD) with maintaining a peaceful atmosphere that fosters the sense of well-being for both the person and the caregiver. The voyage isn’t without rough waters, but it can be a voyage that brings joy and hopefulness to all involved. Working with a PLwAD is a complex situation that cannot be fully addressed in one article. Highlights of maintaining that crucial peacefulness include understanding that this is a progressive disease. What’s happening today will change - maybe as early as tomorrow. What works today to help the person be successful and feel secure will change over time. Interventions that are

attempted should not be given up on too soon. Keep trying, gently and with small adjustments to your approach. There are several key highlights for maintaining a serene atmosphere. This includes communication, understanding how to identify and reduce anxiety, providing purposeful tasks and activities, and having a skilled team of professionals who the caregiver can call on for help. Never argue with the PLwAD. It will do no good and almost always results in escalation of anxiety and confusion. Step into the person’s version of reality and work to understand their concerns. Often, the PLwAD will think the caregiver has taken something of theirs. If you thought someone stole your wallet, you would be upset, too! Acknowledge that and offer to help look for it. Consider having duplicates or triplicates of the exact same item that seems to get lost. Instilling a routine can help, too. Always place wallets and other items (shoes, bags, etc.) in the same place. Reassure, don’t lecture. Talk about what the person does remember instead

of “reminding.” Anxiety fuels the confusion that is always present in PLwAD, so speak in a way that eases the anxiety. The PLwAD will use the language in the way they understand it. Speak clearly and simply, using a tone of voice that is kind. As the PLwAD’s brain changes, their abilities will change, and they will need more help to accomplish the daily tasks of living. Lay out clothing, talk the person through a task, understand when a given task is too complicated and break it down into simpler steps. Caregivers are at real risk for developing illnesses themselves. As many as 60% of caregivers end up dying before the person they care for. Remember to take time for yourself every single day. Get enough sleep, exercise and eat a plant-laden, nutrient-dense diet. Care for yourself as carefully as you do your person. Alzheimer’s disease is a complex voyage that cannot and should not be navigated alone.

What You Need to Know About Colorectal Cancer Courtesy of American Cancer Society

F

ollowing the devastating news of the death of the world’s beloved Black Panther, Chadwick Boseman, from colorectal cancer, many were left asking why ­— and how. What most don’t know is that Boseman sat at the intersection of where colorectal cancer rates are among the highest and rising the fastest. He was a young man. And he was a Black man. Among the highest rates of colorectal cancer of any ethnic group in the U.S., African Americans are 20% more likely to get colorectal cancer and 40% more likely to die from it. Black men have the highest incidence rate. In addition, rates of colorectal cancer in younger age groups is rising. In 2020, 12% of cases will be diagnosed in people under 50 — about 18,000 cases. Since the mid-

1980s, adults age 20-39 have experienced the steepest increase in colorectal cancer rates. The American Cancer Society recommends those with average risk begin regular screening at age 45. Those with higher risk should consider — with their physician — earlier screening. Higher risk factors include: • Family or personal history of colorectal cancer or certain types of polyps • Personal history of inflammatory bowel disease (ulcerative colitis or Crohn’s disease) • Known family history of a hereditary colorectal cancer syndrome such as familial adenomatous polyposis (FAP) or Lynch syndrome (also known as hereditary non-polyposis colon cancer)

• Personal history of radiation to the abdomen or pelvic area to treat a prior cancer Anyone with concerning gastro-intestinal symptoms, such as a change in bowel habits that lasts for more than a few days, rectal bleeding, blood in the stool, cramping or abdominal pain, weakness and fatigue, or unintended weight loss, should consult with their doctor. Screening can prevent colorectal cancer by finding and removing growth, called polyps, in the colon and rectum before they become cancer. It can also find colorectal cancer early, when it’s small, hasn’t spread and may be easier to treat. When found early, the five-year relative survival rate is 90%.

or those who like to get outdoors, stretch their legs and burn some calories, the Florida heat can be a serious challenge. We’ve got some rules of the road to help you create an exercise routine that will keep you safe, motivated and cool as a cucumber.

1. Pick the Right Time of Day: Unless you’re conditioning for an event that takes place in the daytime heat, avoid exercising from 10 a.m. to 5 p.m. — the hottest part of day. Exercise outside in the early morning to avoid the heat and the harmful rays of the sun while crushing your workout. If you’re getting a move on at night, wear bright clothing, move with/against 32

traffic (depending on the exercise) and wear reflectors or lights on your hat, torso or ankles. 2. Stay Hydrated: Before you go out, drink a glass or two of water. Bring a water bottle with you, or even a hydration pack, and take a drink every 15 minutes - even when you’re not thirsty. If you’re thirsty, you’re already dehydrated! When you’re done with your workout, cool down with a few more glasses of water. 3. Dress for the Occasion: Wear breathable, light-colored clothing. It helps reflect heat, increase your visibility to motorists, and moisture-wicking

material helps with sweat. Don’t forget to protect your melon, too! Wear a hat to protect your head, face and neck from the sun or a bike helmet with a sun visor if you’re putting the pedal to the metal. 4. Stay in the Shade: If you can, choose shaded trails or pathways to stay out of the sun. The West Orange Trail is a great spot to walk, run and ride bikes under shady trees. 5. Listen to Your Body: If you’re feeling dizzy, nauseous or faint, stop immediately. Heat exhaustion and heat stroke are real dangers and can happen to anyone.


COVID-19: Increasing Screen Times in our Pediatric Population By Mark Ossi, OD, Community Health Centers, Inc.

oday’s kids are growing up in an evolving world that revolves around computers. Add the current pandemic, and it’s no surprise surveys reveal that screen time, as reported by parents, has gone up 5-fold between pre-pandemic and now. So, what’s the big deal? When we look at something up-close, the eye muscles contract to bring that object into focus. This “focusing power” is called accommodation. The issue of eyestrain arises when that accommodation system is put under continual stress associated with extended screen time. With time, the eye muscles fatigue and lose their ability to function optimally, leading to headaches, blurry vision and sore eyes. In children, these complaints can easily be overlooked, especially when a basic visual acuity exam determines the child is 20/20 and the provider doesn’t do additional supplementary testing. Under these circumstances, a child could benefit from a pair of prescription reading glasses. The blue light emitted by screens plays a significant role in sleep. It suppresses the release of melatonin, a hormone that orchestrates the circadian rhythm. In the pediatric population, sleep is especially crucial given the role it plays in the development of the child cognitively and physically.

There are several methods to combat the potential sleep disruption of blue light. The most obvious is limiting screen time before bed, preferably discontinuing use at least two hours before bedtime. A second method is incorporating “blue-blocking” lenses in glasses. They filter the amount of blue light that is transmitted through the lenses of the glasses, reducing exposure and sleep disruption. Another alternative is installing apps on your child’s electronic device that act as a blueblocking filter. Lastly, the increasing role of electronics in children’s lives is increasing the prevalence of dry eyes. Studies have shown electronic use decreases the amount of blinking we do. Because the blinking mechanism is connected to our tear film, reduced blinking causes tear film instability and dry eyes. This leads to a cascade of events that promote inflammation on the surface of the eyes, manifesting in a variety of symptoms such as burning, itching, redness, tearing and fluctuating vision. Whether we like it or not, electronics have become a big part of our children’s lives. If your child complains of any of the symptoms discussed, or you notice a decline in school performance when transitioning to electronic learning, consider seeing an eye doctor for further testing that may help alleviate possible eyestrain linked to prolong screen time.

Orlando Science Center

The museum will be open Monday-Tuesday and Thursday-Saturday from 10 a.m. to 5 p.m., Sunday from 12-5 p.m., and it will be closed each Wednesday for deep cleaning. While all programming is currently still being offered, staff will be present in all exhibit halls to facilitate the experience and to sanitize between guests. Nemours Health Care Heroes Virtual Celebration

How your support of this annual fundraiser for Nemours Children’s Hospital will positively impact families in our community. By Lyndsay Fogarty

T

he COVID-19 pandemic has changed our sense of normal. But for one Central Florida mother, “normal” changed for her family in June 2019. It was then that Timidigi “Timi” Tadawei faced a harrowing diagnosis: her 2-year-old daughter, Christabel, had stage two kidney cancer. Living in Nigeria at the time, the family spent several frustrating months trying to find the right care for their daughter. In September 2019, they settled with family in Central Florida, where Timi found hope at Nemours Children’s Hospital in Lake Nona, Medical City. As COVID-19 began to spread in our communities in March 2020, Timi found herself in need of additional resources with six more months of Christabel’s chemo treatment on the horizon. A dedicated Nemours social worker connected her with support made possible through the Nemours Patient Impact Fund. Funded by the generosity of corporate and private donors, this fund provides access to vital services that directly enhance and support the patient and family experience at Nemours Children’s Hospital. This includes insurance and medication support, meals, transportation, translation services, chaplain services, technology advancements and more. It may come as a surprise that more than 70% of patients at Nemours Children’s Hospital are Medicaid re46

cipients or uninsured. Many have been disproportionately impacted by the COVID-19 pandemic, including Timi’s family. Even with months of added pandemic fears, strict safety protocols and the need for personal protective equipment, nothing could dampen the celebration when Christabel rang the bell to signal the end of her yearlong cancer treatments in August. The pandemic has changed the way that nonprofits connect with the community to raise vital funds to help patients like Christabel. The annual Nemours fundraising gala is known as one of the best parties with a purpose, but the on-going effects of the pandemic have required Nemours to think differently. Instead of a black-tie affair in a hotel ballroom, Nemours is bringing the celebration directly into supporters’ homes through the Nemours Health Care Heroes Virtual Celebration. On Nov. 7, Joey Fatone (*NSYNC, “Dancing with the Stars”) and Nancy Alvarez (WFTV Eyewitness News anchor) will co-host an interactive online experience featuring stories of hope and healing. Expect exciting entertainment from Dance It Studio’s ballroom dancers and musical performances from local band, Audio Exchange, and Alyssa Raghu (“American Idol”). Knowing that both systems strive to bring first-class, award-winning and innovative care to our community helps me sleep at night, especially during Breast Cancer Awareness Month. Both systems provide opportunities for men and women to learn how to self-examine and have access to mammograms. We should always take the opportunities for ourselves and our loved ones to take preventive measures for our health. With all that is available in West Orange County, there is no excuse.

Our local health care systems, including AdventHealth and Orlando Health, were and continue to be prepared to handle this crisis while still delivering quality care to others in need. The West Orange Chamber is proud to call both systems Community Trustees, the highest-level member investors demonstrating their support and partnership within our community. They were both of tremendous support when the chamber home was renovated, and they continue to support us through virtual event sponsorships during this pandemic as well as serving on our board of directors. Their board representatives are the presidents of hospitals within West Orange: Mark Marsh, President/CEO of Orlando Health-Health Central Hospital and Horizon West Hospital and Carl Cupper, CEO AdventHeatlh Winter Garden. This is how I know that we are in good hands. Both men, along with their talented teams, share valuable information with us ranging from their growing list of services to protocols on staying safe during this pandemic to community programs that enhance our quality of life. Since its opening in 2016, AdventHealth Winter Garden has seen tens of thousands of patients, bringing urgent, life-saving care close to home for residents of West Orange

County. A major expansion of the Winter Garden campus is underway with the construction of a 100-bed inpatient pavilion. A variety of primary and specialty care is available at the adjacent medical office building. Just north of us, AdventHealth Apopka includes 120 private inpatient rooms, expanded surgical services, advanced surgical suites and a catheterization lab. Orlando Health – Health Central Hospital is a Joint Commission accredited, 253-bed hospital with services ranging from primary and specialty care to intensive and emergency care. Orlando Health – Health Central Hospital has an onsite, 60-bed emergency department, and its Horizon West location has a 24-bed emergency room. In early 2021, the Horizon West facility will include a capacity of 120 beds. Dr. P. Phillips Hospital, a full-service medical facility, also has a 47-bed emergency eepartment and all facilities are staffed by physicians who are board certified in emergency medicine.
