Hospitals May Get Accredited Even With Poor, Unsafe Care

E Full practice authority for advanced practice nurses now law SPRINGFIELD – Full practice authority for advanced practice nurses is now law under legislation cosponsored by State Senator Heather Steans (D-Chicago). “With the increasing shortage of doctors in rural areas, this new law seeks to provide patients with greater access to care,” Steans said. “Advanced practice nurses that meet the full practice authority certification requirements will now be able to prescribe medication and administer local anesthetics. This law updates an outdated system and recognizes the skills that many advance practice nurses possess.” To obtain full practice authority status, advanced practice nurses will be required to complete 4,000 hours of clinical experience and 250 hours of continuing education or training. Once this has been completed, advanced practice nurses will be able to practice without a written collaborative agreement from a doctor. The law takes effect immediately.

arlier this year an Illinois woman sent an email telling me of the poor care her husband received at a large Chicago hospital. After six weeks of fighting for his life, he died. “I wish you could see his records and all the infections and surgeries he had,” she told me. “I’ve been wanting to do something about the care he got and just didn’t know who to go to.” I return to this topic from time to time because everyone is vulnerable to hospital mistakes. Most people think of hospitals as safe, loving places that advertise their miracle cures on TV and build new wings to house the latest technology. How can things go wrong? But they do, and readers of this column have been eager to share their experiences. Increasingly, it’s becoming clear there are few places patients and their families can turn for help in avoiding bad care and equally important there’s almost no guidance from state or the federal regulators. When news outlets try to write about unsafe hospitals, they run into a “veil of secrecy that protects the industry” as one reporter in Michigan put it. Trudy Lieberman The Wall Street Journal has just published a fine story that begins to lift that veil. Not many Americans know that the federal government has turned over the task of accrediting most of the nation’s hospitals to a private organization called the Joint Commission, which is funded by hospitals and governed by a board of directors some of whom are executives in the health systems it accredits. Medicare requires hospitals to comply with safety standards, and they can use state inspections or hire a government-approved accrediting body to show they are in compliance. Most have chosen the latter. Hospitals have a lot of skin in this game paying the Commission an annual fee between $1,500 and $37,000 depending on how big they are, the Journal reported. They also pay for the inspections, which in 2015 cost on average $18,000. The Joint Commission awards a “Gold Seal of Approval” to those facilities that meet its standards. Maybe you’ve seen them when you’ve visited a hospital. It encourages hospitals to use those accreditations in their marketing activities, even providing them with a publicity kit. The Commission has also created

a patient brochure with this reassuring message, “Whenever and wherever you receive health care, look for The Joint Commission Gold Seal of Approval.” However, the Journal found that those seals of approval can be misleading. Using information from inspection records, reporters found that in 2014 some 350 hospitals had Joint Commission accreditation even though they were in violation of Medicare safety requirements, and 60 percent of them had safety violations in the preceding three years. It seems that most patients facing an operation or a hospital stay would want to know that. Dr. Mark Chassin, president and chief executive of the Joint Commission, told the Journal that his organization doesn’t routinely withdraw accreditation of hospitals with safety problems because its focus is less on regulating or penalizing and more on preventing problems. And this brings us back to that age-old problem: Should a government regulate business, including hospitals which have become very big businesses - or should it provide information so buyers, or in this case patients, can compare the offerings and make decent choices? Opponents of regulation argue that providing information is best. In the case of hospitals, though, patients have neither regulation nor information. The Journal’s reporting shows that the Joint Commission’s inspection process often lets hospitals keep their accreditation even as they deliver poor care. At the same time, the Joint Commission has continually refused to make its hospital inspection reports public. So if patients are supposed to do their homework before undergoing health procedures, it’s a fair question to ask: How should they do that? Earlier this year the Centers for Medicare & Medicaid Services proposed a rule that would have required the Joint Commission to make its inspection records public. The agency cited serious concerns about the Commission’s ability to identify safety problems. The Commission opposed the regulation, arguing it would make its work harder. Hospitals opposed it, too. CMS heard their pleas and withdrew the proposal leaving patients in the same information vacuum they were in before. Earlier this year Ashish Jha, a professor of health policy at the T. H. Chan School of Public Health at Harvard, told me he had no idea which hospital in New York City had the lowest risk of safety problems. At best he could only make a rough guess. He told the Journal its findings show “accreditation is basically meaningless – it doesn’t mean a hospital is safe.” What kind of information would you like to see about safety and quality of care at your local hospital? On October 14, Back 2 School Illinois (B2SI) and the Chicago Park District will host the 4th Annual POWER UP! Boot Camp at West Lawn Park, located at 4233 W. 65th Street. At this free event, two hundred Chicago kids aged 6 to 12 years old will spend the day learning about the importance of a healthy, active lifestyle through physical activity and smart food choices. For over a decade, B2SI has distributed their signature Back 2 School Kits to ensure that all Illinois students have the tools they need to prepare them academically, and to develop the confidence they need for successful futures. This high-energy event will lead kids through five stations focusing on fun physical activity, nutrition, teamwork training, and mind exercises – all designed to demonstrate how diet and fitness habits can impact overall health. “Though Back 2 School Illinois is most widely known for its school supply distribution programs, the POWER UP! Boot Camp addresses another important part of a child’s education,” says Matthew Kurtzman, B2SI CEO. “Success in the classroom requires a habit of healthy lifestyle choices.” Parents are invited to attend a 30-minute welcome session that will provide information on the children’s activities for the day, along with plans on how to apply the fitness and nutrition instructions beyond the Boot Camp.

CHICAGO BAR ASSOCIATION SENDS LAWYERS INTO THE CLASSROOM TO ADDRESS YOUTH VIOLENCE

The Chicago Bar Association’s Interfaith Committee has kicked off its Restorative Justice Program for schoolchildren designed to address youth violence by introducing students to information and strategies that promote peaceful conflict resolution. The eight-week program is conducted by CBA member volunteers as well as faith and community leaders and educators. The curriculum introduces students to the concepts of civics and restorative justice and explores the methods for resolving disputes, ranging from traditional legal proceedings in court to peace circles and peer juries. The program is being sponsored this year by the Big Shoulders Fund, which provides support to Catholic schools in the neediest areas of inner-city Chicago. It is taking place at Leo Catholic High School and St. Sabina Academy. “This program utilizes the special skills of lawyers to bring a comprehensive message of nonviolence straight into the classroom,” said Ret. Judge Thomas Hogan, who chairs the CBA’s Interfaith Committee and administers the program. “We give students reallife examples and practical solutions to help them resolve conflict peacefully.”

Cook County Commissioner Jesus “Chuy” Garcia Endorses Chris Kennedy for Governor

Cook County Commissioner Jesus “Chuy” Garcia endorsed Chris Kennedy as the next Governor of Illinois. During a press conference at New Era Windows cooperative, an employee-owned manufacturing plant on Chicago’s southwest side, Garcia thanked Kennedy for his commitment to challenging the status quo and bringing fundamental change to Illinois. “There is only one person running for Governor who has a chance to defeat Bruce Rauner and be the progressive leader our community deserves: Chris Kennedy,” Garcia said. “I trust Chris Kennedy to deliver on the key issues facing our community.” Garcia continued, “[Kennedy] supports giving low-to-middle income students free college tuition for at least two years at Illinois community colleges and trade schools. Chris will fight to raise the minimum wage to $15 an hour and he believes health care is a basic human right. That’s why he has signed on to one of the main tenets of the Bernie Sanders platform: Medicare for All.” Kennedy thanked Commissioner Garcia for his endorsement saying, “I am honored that Chuy Garcia has joined the Kennedy-Joy campaign.

CommonWealth Edison Company (ComEd) Solar Spotlight program hosted a two-day immersion session and brought together 50 Latino high school students to help design two solar-powered fashion designs for the Latino Fashion Week. The Solar Spotlight program, which is focused on Science, Technology, Engineering, Arts and Math (STEAM), honored the Hispanic Heritage Month through hands-on student activities on solar energy and solar panels. Fidel Marquez, senior vice president of ComEd’s Governmental and External Affairs, explained that ComEd partnered with Latino Fashion Week so local students could have the opportunity of designing a dress and blazer lit up with LED lights, which were solar-powered with a battery back-up. “Fashion is clearly a very visual art and it makes it fun for the students so engineering doesn’t have to be boring,” Marquez said. “This lets students know how you can apply STEAM in a very fun way.” High school student Esmeralda Facio said she was hesitant about joining the program but decided to participate because the incorporation of fashion design and engineering was something new.

“At first I thought it would be boring but the experience was really good,” Facio said. “I like how ComEd made it fun and how we were able to design a dress and incorporate our cool [Latino] heritage.” Marquez continued to explain that statically the percentage of Black and Latino students entering careers in Science, Technology, Engineering and Math (STEM) is small, but he says STEM careers provide great stability for students to take care of their families. “We believe through programs like Solar Spotlight, we at least make the introduction of the possibilities of STEM,” he said. “When ComEd thinks of STEM we see this program as the future of our company. Hopefully, these students will replace us one day but we have to find creative ways to make them excited.” ComEd engineers will serve as volunteer mentors to help the students learn more about solar energy and creative ways to improve public spaces using technology, according to a ComEd press release. The dress and blazer filled with LED lights will also be on display at the Mexican Museum of National Art and the Puerto Museum of Art in the coming weeks. S. as Communities Take Action Against It By Jane Kennedy (TriceEdneyWire.com) - The prevalence of diabetes in the U.S. climbed to a new high of 11.6 percent last year, up from 10.6 percent in 2008, according to a new report by Gallup and Sharecare, titled “The Face of Diabetes in the United States.” “If the diabetes rate had held steady at its 2008 level rather than increased, 2.5 million fewer U.S. adults would have the disease today. Hospitals, health systems, employers, community leaders and other champions of population health are implementing both inpatient and outpatient solutions to address this rising epidemic,” said the report released this summer. Without differentiating between type 1 and type 2 diabetes, the report analyzes diabetes prevalence across key demographics, occupations, and regions. Neither does it take into account whether those surveyed were undergoing treatment. Rather, the new numbers are based on the answer to the question, “Has a doctor or nurse ever told you that you have diabetes?” “By shedding light on where diabetes rates are higher or lower than the national average by a statistically significant margin, these data can be used to more effectively target investments and customize programs that address specific populations who are at greatest risk,” the report states. Among the key findings: The analysis shows that prevalence spikes in the South, with a 12.8 percent diabetes rate in 2016. Previous research by Gallup and Sharecare shows that people in southern-based states and communities have “some of the highest prevalence rates in the nation.” Seven of the 10 states and communities with the highest diabetes rates are Alabama, West Virginia, Mississippi, Tennessee, South Carolina, and Arkansas each having more than 14% of their population reporting having

been diagnosed with diabetes. The demographic insights show a slightly higher prevalence of diabetes among women versus men and a higher prevalence rate among Blacks as compared to other racial and ethnic groups. It also reveals a direct correlation between age and higher diabetes prevalence: Seniors age 65 and older have an alarming 23.6 percent prevalence rate. Income and education levels have an inverse relationship with diabetes prevalence of the disease “falls as education and income levels rise.” Additionally those who are divorced, separated or widowed have much higher prevalence of diabetes than those who are single, in a domestic partnership, or married. Observing the analysis, Sharecare Vice President Sheila Holcomb said objectives for diabetes care must gravitate from just treatment to lowing its prevalence by holistically “slowing the progression of the disease”. In that regard, community-based organizations, non-profits, government agencies and hospitals have formed partnerships promoting healthier lifestyles to abate the rise of nutrition related diseases such as diabetes. One such project, Partnering4Health, has been funded for three years by the U.S. Centers for Disease Control and Prevention (CDC) “to promote healthy lifestyles and decrease morbidity and mortality due to costly chronic diseases,” according to the executive summary of a report on the partnership. “The aim of each funding initiative was to stimulate communities’ adoption of policies, systems, and environments (PSE) that would make communities healthier and promote individuals’ healthy choices.” In a nutshell, the U. S. Department of Health and Human Services reported last year that “African Americans are almost twice as likely to be diagnosed with diabetes as non-Hispanic whites. In addition, they are more likely to suffer complications from diabetes, such as end-stage renal disease and lower extremity amputations.” The Partnering4Health project has resulted in a 3 percent reduction in the rates of death and disability due to diabetes, heart disease, and stroke. But, the Gallop report reveals the necessity for persistence in dealing with the daunting issue of diabetes in the Black community through continued education and information. Comprehensive inpatient glycemic and outpatient diabetes management programs can and do change the trajectory of the disease, and help those with diabetes better manage their condition and live a higher quality of life.”

What To Do If You See A Suspicious Credit Card Transaction (NAPSI)—You scan your monthly credit card bill, only to find a suspicious transaction. Don’t panic. Fortunately, credit card companies largely shoulder the financial burden of unauthorized credit card charges. By law, you’re only on the hook for $50, and it’s unlikely you’ll even be dinged for that much. Now it’s time to take action through these four steps. Let Your Issuer Know, And Fast If you find a peculiar transaction, call your credit card issuer right away to report it—that’s if your issuer didn’t alert you first. Your issuer will ask you to verify the most recent transactions to make sure no other suspicious activity was recorded. Your issuer will close your current credit card and issue another one with a new number within days. If your account has any authorized users on it, the issuer may send them new cards, too. Confirm that the new card is linked to the payment history of the old card account, so they appear as one on your credit history. Update any recurring bills with the new card number. Stay Vigilant When you place a fraud alert on your credit report, you’ll get a free report as well. This doesn’t count toward your annual free credit report under federal law. Check your credit report for

any unauthorized accounts and for the accuracy of your personal information. If anything looks funny, report it to the credit bureaus. File a police report if you find an account that you didn’t open. Often, companies that experienced data breaches will offer free credit monitoring to victims. Use it. Be Proactive In the future, use best practices with your credit card. Opt for two-factor authentication when available while shopping online to make it more difficult for scammers to take over your accounts. Consistently update your online passwords to protect your financial information from data breaches. Always check the security credentials of online retailers before making a purchase with your credit card. Be On The Lookout Regularly and carefully check your recent transaction history for any fraudulent charges. A fraudster only needs your card number to make purchases, so fraud can happen even if your card is in your wallet. Are You Saving Enough? Consider Building These 4 Savings ‘Buckets’

Perhaps you’ll need to replace your timing belt, or maybe you’ll need a new clutch. You don’t know when this will happen. You don’t know how much money this may cost. But you know that at some indeterminate point, you’ll need to repair your car. If you’re a homeowner, you’ll eventually need to replace your furnace or buy a new dishwasher. If you’re not prepared to cover these costs, you may need to start increasing your savings. Consider creating an “irregular bills fund,” to which you contribute money to help cover repairs, maintenance and other irregular expenses. 3. Emergency Fund Imagine that you and your spouse both lost your jobs at the same time. How long could you live on your savings? Your answer may depend on the size of your emergency fund. An emergency fund is money that you set aside for unpredictable, once-in- a-lifetime emergencies, such as job loss or a health emergency. It should cover at least three months’ worth of living expenses, according to the Financial Industry Regulatory Authority (FINRA). In 2015, 54 % of U.S. households didn’t have enough savings to cover three months of expenses, according to the FINRA Investor Education Foundation’s To start building an emergency fund, consider opening a separate savings account. Make a “payment” into this account every month, as if you’re paying a bill. By housing your emergency fund in a separate account, you might signal to yourself that this fund is reserved only for dire situations. 4. Financial Goals The CFPB also recommends setting aside enough money to help pursue your goals. If you hope to retire at age 62, attend graduate school or start a small business on the side, consider putting away money that can help you build these goals. Consider keeping this money separate from the rest of your savings. By building “buckets” for every savings goal, you’ll be able to monitor whether or not you’re on track. When you’ve built a comfortable savings nest, you may be better prepared for life’s curve balls.
