Quiz Program “THE SURGICAL BRAINS” at Vaatsalya Hospital, Vizianagaram Vaatsalya Hospital Vizianagaram conducted a quiz program in General Surgery - “The Surgical Brains” for the duty doctors of Vaatsalya hospital, Undergraduate students and interns of Maharaja Institute of Medical sciences [MIMS], in the Hospital’s conference hall on 25th August 2011. This was the day he took his 11 year old son Mehboob, who was suffering from fever for the last 5 days, to a Paediatrician near Sedam, 35 Kms away from Shirolli. There was bad news awaiting for Mr.Rahul Sab Mulla as the doctor informed him that the boy’s condition is very

critical as both his kidneys had failed and not working. What is diabetes? Our body produces glucose or commonly known as “sugar” from the digestion of carbohydrate consumed through food. Cells and tissues take up sugar from blood and break it down to get energy. In some individuals, sugar is not taken up efficiently resulting in the accumulation of sugar in the blood. This condition is known as “Diabetes”. How does one get diabetes? Carbohydrate is partially digested in the mouth and completely in the small intestine. The sugar produced from digestion is absorbed by the blood and circulated throughout the body to supply sugar to the cells and tissues. Cells need “insulin”, a hormone secreted by pancreas. Sugar can’t enter the cell because the cell is like a locked house. Just like one would need a “key” to enter the house cells also need a key to let the sugars inside the cell and that “key” is insulin. In normal individuals, there

renganathan@vaatsalya.com is enough insulin in the body for the cells to absorb glucose. In diabetic individuals, the insulin levels might be low or sometimes insulin is unable to “unlock” the cells for the entry of sugar. In both situations, sugar levels increase causing diabetes.

What are the different types of diabetes? There are three different types of diabetes—Type 1, Type 2, and gestational diabetes. Type 1 diabetes is also known as “Insulin-dependent diabetes or Juvenile diabetes”. In Type 1, a portion of pancreas which produces insulin is destroyed by patient’s own immune system leading to non-availability of insulin for sugar uptake by cells. Hence the Type 1 diabetes patients need to take insulin life-long. Type 1 occurs in people younger than 20 years. Type 2 diabetes is also known as “Non-insulin dependent diabetes or adult onset of diabetes”. In the

early stages of Type 2 diabetes, patient’s pancreas produces enough insulin; however, over time the insulin producing cells of pancreas are destroyed leading to insulin deficiency. Type 2 diabetes can be controlled in the early stages with oral anti-diabetic drugs, but if the sugar levels are uncontrolled then Type 2 patients also might need insulin supplements. About 90% of the diabetes is of the Type 2 and the rest Type 1. Gestational diabetes occurs in women during pregnancy. This is a temporary condition which would revert to normal after delivery. However, women who suffered from gestational diabetes have a 50% chance of developing Type 2 diabetes later.

Is there a cure for diabetes? No, there is no cure for diabetes. The oral medications and insulin only help controlling sugar levels and does not cure them of diabetes.

Who is susceptible to Type 2 diabetes? The risk factors for Type 2 include obesity, ethnicity, and age. Type 2 usually is found at about 45 years of age; however Indians are more susceptible and is not uncommon to find Type2 diabetes in people 35 years of age. If any one of the parents was diabetic then their children have a higher probability of being diabetic as well. What are the symptoms of diabetes? Excessive thirst, frequent urination, weight loss, and constant hunger are some of the symptoms. Presence or absence of diabetes can be confirmed only through a blood test.

• Math – Math is the calculator functionality of the OpenOffice. org suite providing functions and operators to create formulas.

• Base – Base is similar to MS Access. Data can be stored in various database file formats. The benefits and main advantage of OpenOffice.org is, of course, the fact

that it is free. Installing OpenOffice. org is a simple procedure. You can install all components in the suite in a single operation. The interface is similar to that of MS Office, and even advanced Office users will find almost everything they’re used to: templates, collaborative features, macros, and even a programming language. OOo, as it’s known, lets you open and save documents in formats as diverse as MS Office formats, PDF, HTML, and XML. It can also import files from those formats, as well as Word Perfect and others.

Patient Safety – Assessing patients for risk of developing pressure ulcers.

During the course of working in a hospital, some of our nurses, technicians, housekeepers and other employees accidentally prick themselves due to which they face a risk of contracting infections from the patient. From Sept 2011, in ten of our hospitals our Nursing Superintendents have started monitoring the number of needle stick injuries that occur in their hospital and training the employees to report the accidental pricks so that we can provide immediate treatment to minimize the risk of infection to our employees. Good techniques to prevent accidental needle pricks are also being taught to our employees.

When sick patients stay for several days in hospitals, they face a risk of developing ulcers on their body due to pressure on their skin from the weight of their body – called pressure ulcers. An important responsibility of the hospital employees in general and the nursing staff in particular is to take preventive measures to safeguard their patients from developing pressure ulcers. From Sept 2011, in ten of our hospitals all our nursing staffs are being trained to assess all admitted patients for the risk of developing pressure ulcers by using Braden scale and Braden score, used by hospitals all over the world. The main aim of this initiative is to spread the awareness about the effects of stress levels, importance of stress management and techniques to reduce, prevent and cope with stress.
