Expert Interview - All you wanted to know about Health Insurance We have recently renewed Group Health Insurance Policy with National Insurance Co to extend the healthcare services to all our employees and their dependents. Dr.Talha Siddiqi has been associated with health insurance industry, before being part of vaatsalya family. Here he will answer some of the commonly asked questions on employee health insurance. Mansi: First, Who are covered in this policy?

Talha: Presently, those employees who have completed six months in the organization or whoever is confirmed. Six members of a family are covered under this plan. The Employee, his/her spouse (Husband or Wife), Father and Mother who are aged 75 or below and first two kids aged 25 or below. Mansi: So, Which are the hospitals where one can take treatment? Mansi: What is the difference between cashless and reimbursement process? Talha: In Cashless, the patient does not pay the hospital bill except for the non medical expenses. In case of Reimbursement, Patient pays for the hospital expenses and later gets reimbursed by Mediassist upon verification of the documents. Mansi: Does policy cover the incurred expenses on Outpatient services? Talha: The policy covers only the Inpatient admission with at least 24 hours stay in hospital. Expenses related to outpatient services and admissions only for diagnostic purpose are not covered. Mansi: Tell me about the Sum Insured and room rent cap? Talha: Each employee is covered with a family floater of 1 lac sum Insured. The room rent is restricted to maximum of Rs.1000/- per day and ICU with Rs.2000/Mansi: If somebody is diabetic can he avail the health insurance benefits. Talha: Yes, the policy covers all the pre existing diseases and conditions except for

Sexually transmitted diseases, congenital defects, mental disorders and self injuries. Mansi: Whether Maternity and child birth expenses are covered?? Talha: Yes. Maternity benefits are covered with a maximum of Rs 25000/- in case of normal delivery & Rs 30000/- in case of Caesarian sections. Maternity benefits are extended only to first two children only. Mansi: What is the co-payment paid by the patient? Talha: Co-payment is not applicable to employee but if a dependant ie father, mother, spouse or children is admitted they have to pay 15% of the total bill as copayment. For instance, if the final inpatient bill is Rs.10000/- then the patient will have to pay Rs.1500/-. Mansi: What are the other features of the policy? Talha: Upon analyzing the previous year’s claims and suggestions from the insurance company, 1. The expenses for the treatments of Cataract, Piles & Fistula, Tonsillitis, Open Appendectomy, Septoplasty, and Closed reduction of bones, are payable only upto Rs 25000/2. Expenses on Open Hysterectomy are covered with the maximum of Rs.30,000

excluding service tax. 3. The maximum expense on each admission is restricted to a maximum of 60% of the balance sum insured. Mansi: Is the 60% restriction applicable on cardiac surgeries also. Talha: The 60% restriction is not applicable for the expenses on Cardiac Surgeries, Organ Transplants and Cancer related treatment. The expenses are covered up to the maximum of balance sum insured. Mansi: Is there anything you would like to share. Talha: I would like to request all the employees to kindly use the health insurance benefit diligently and verify the Inpatient bills before discharge with itemized bills. Get the preauthorization before hospitalization in case of planned admissions to avoid inconvenience. Mansi: Last question, whom to contact in case there are any problems /queries? t is recognised world over today that the specialists of various disciplines are overworked professionally which make them unable to meet the growing need of critical care. In modern medicine, despite the

organ specific support. It’s not only about nonavailability – many die because they cannot reach higher centres or many cannot afford the expensive critical care in urban cities. A critical care centre within the town

bridges this gap. A team dedicated for such kind of care is the need of hour where they can actually do the job of babysitting when the primary physician has to attend to many outpatients. This is where the critical care specialists called intensivists come into the fore. Today let it be surgery case of perforation with septicemic shock or physician’s case of Pneumonia with Acute respiratory distress syndrome (ARDS) – intensivists manage it smoothly. Similarly, let it be neonatologist case or obstetrician’s case of Pregancy induced Hypertension (PIH) with organ failure or neurologist’s case of subarachnoid hemorrhage – primary clinicians can rest peacefully. Intensivists are trained in all aspects right from ventilator, dialysis, managing blood pressure and doing the most difficult procedures under stressful circumstances. Intensivists just do not have to manage the patients, we also have to manage the family. Counselling the family from time to time in patients who have unpredictable prognosis is a skill learnt by experience. It’s a challenge to do that and at the same time being sensitive to family’s expectations and their financial situations. The advantages of having round the

clock critical care support are manifold 1. High Survival Rates 2. Drastic reduction in cost to patients, referral to expensive higher centres are avoided. 3. Low complication rates 4. Time saved for the primary clinician which has bearing on improving the outcome on noncritical cases. 6. Training of local paramedical staff Critical care is an emerging specialty by itself. C

hronic kidney disease (CKD) also called kidney failure is a condition in which the kidneys lose the ability to remove waste and excess water from the bloodstream. The most common causes of chronic kidney disease are diabetes and high blood pressure. In the early stages of CKD, there are no obvious symptoms. The disease can progress to complete kidney failure, also called end stage renal disease. The main goal of treatment is to prevent progression of CKD to complete kidney failure.The best way to do this is to diagnose and control the underlying cause. NORMAL KIDNEY FUNCTION The kidneys function to remove wastes and excess water from the blood. Not only this, kidney maintains normal blood pressure. Further, it produces hormones like Vitamin D and Erythropoietin which is necessary for Bone – Mineral Metabolism and Hemoglobin respectively. In short, kidney maintains the chemical balance in body and many vital body functions are dependent upon the proper functioning of the kidneys. For the filtering process to occur properly, the blood pressure and blood flow to the kidneys must be adequate. If the arteries leading to the kidney are diseased, the filtering process will be affected. The nephrons, including the glomeruli and the tubules, must be healthy, and the path from the nephron to the urethra must not be blocked. When the kidney filters are working properly, the result is aproper balance of fluids and chemicals in the body. If an imbalance occurs, many critical bodily functions can be affected, possibly producing symptoms associated with kidney disease

CHRONIC KIDNEY DISEASE SYMPTOMS Most people with chronic kidney disease do not have symptoms until the kidney function is severely impaired. The symptoms are mild and nonspecific which include Fatigue, Nausea, Generalised Weakness, Swelling of Feet, Increased frequency of urine at night, Mild loss of appetite In later stages, one develops difficulty in breathing, and severe loss of appetite

Avoid Drugs harmful to kidney – Drugs like NSAIDs (Pain Killers commonly used over the counter), Antibiotics like Amikacin, Gentamicin can be harmful to kidney. Also many medicine dosages have to be modified in Chronic Kidney Disease. Please consult Nephrologist before taking any medicines.

DIAGNOSIS Serum Creatinine. Based on this, there is a formula to calculate percentage functioning of your kidneys Urine tests — The presence of albumin or protein in the urine (called albuminuria or proteinuria) is a marker of kidney disease. Even small amounts of albumin in the urine, called microalbuminuria, may be an early sign of chronic kidney disease in some people, particularly those with diabetes and high blood pressure.
