TO THE SENIOR ACTIVIY CENTER

DEAR MEMBER, Welcome to the Southlake Senior Center Family! The City of Southlake is proud to have you as a member of the Southlake Activity Center located in Legends Hall at The Marq Southlake. Our team is committed to providing you with outstanding services and experiences. Whether you are looking to join a fun fitness class, learn a new skill, socialize with friends, enjoy lunch or just have a nice place to hang out, the Southlake Senior Activity Center is the place for you! Legends Hall serves as the location for the Senior Activity Center as well as a rental venue capable of hosting conferences, corporate meetings, weddings, banquets, birthday parties and so much more! We look forward to serving our guests and seniors in the best way possible. In this Member Handbook, you will find a full listing of amenities, hours of operation, program information and facility rules and guidelines. Please familiarize yourself with this information so you can take advantage of all we have to offer. As we reopen the Senior Activity Center, the safety and well being of members, guests and staff are our top priority. With that mind, we are committed to reopening with a carefully planned and phased approach. Each of us will play a role in making the Senior Activity Center a safe experience for all. MEMBERSHIPS Membership Options Memberships are open to individuals 55 years old and better.

• Southlake resident membership: Complimentary • Non-resident membership: $75 annually or $6.25 per month (auto-draft) • 10% military discount available with proper ID • Each membership receives one complimentary 55yr+ guest pass per month. Monthly Membership Auto-Draft Authorization An automatic payment utilizing an electronic funds transfer is required for all monthly memberships.

• Auto-draft authorization can be set up with a debit card, checking, or savings account. • Debit will occur monthly on the day of original purchase. • Members may cancel memberships with a minimum of 7 days’ notice. • Memberships are non-transferable and not eligible for a freeze after the date of purchase. Membership Cancellation A Customer Adjustment Request (CAR) form must be submitted at least seven business days prior to

the next auto-payment date in order to stop the payment from being processed. All passes listed on the cancellation form shall be set for non-renewal for the next billing date. Membership may continue to be used up and through the date already purchased. Prorated refunds are not issued for memberships. • Insufficient fund fees will be assessed if auto-draft payments are returned. Annual Memberships are paid in full at the time of purchase and are non-refundable. • Annual memberships may be suspended in the event a member is prevented from utilizing his or her membership. Members may freeze a membership for a minimum of one month by completing a Customer Adjustment Request form. • Once a member is ready to return to the facility, he or she must complete another Customer Adjustment Request form a minimum of seven business days prior to the desired return date. • The amount of time the membership was frozen will be added to the expiration date of the annual membership.

INFORMATION General Information Members and guests are asked to adhere to and respect the following guidelines: • Members must be 55 years of age or older to participate in any programs, special events and trips with the Senior Activity Center. • All members are required to check in at the Senior Activity Center front desk before proceeding to their activities. • Proper etiquette, courtesy, dignity and respect for others are expected at all times. • All injuries on premises must be reported immediately to a team member. • Smoking, electronic cigarettes or use of any tobacco product is prohibited except in designated areas. The use of alcohol and illegal drugs are strictly prohibited on the premises of the facility. • The Senior Activity Center is not responsible for lost, stolen or damaged items. • Pets are not allowed except for service animals. • Members and guests are required to keep the safety and well-being of everyone—including other members and employees—in consideration while contagious with the any virus or infection. Members should not attend activities or trips when they are sick. Members should wash their hands often, especially when handling playing cards, dominoes, bingo cards and other game pieces. • Members must be fully independent and able to take care of their own needs. This includes, but is not limited to, eating and restroom needs. If the member is unable to attend independently, then a caretaker must be present with the member at all times.

Classes • Classes are open to registered members of the Senior Activity Center. • Registration for classes will open two weeks prior to each month of classes for Southlake residents and one week prior to the new month for non-residents. • Please scan your membership card at the front desk before each program. • Classes may be cancelled due to insufficient interest or weather conditions/unforeseen circumstances. • Proper attire is required for the activity in which you are participating. The City of Southlake reserves the right to define inappropriate attire when necessary. • Personal belongings must be kept with the member or locked in his or her car. No items may be left with an employee at the front desk. • If you are registered for a class and cannot attend, contact 817-748-8315 as soon as possible so we can contact the waitlist.

Trip/Travel Information • Trips are open to members of the Senior Activity Center. • Each member must pay all fees in order to confirm registration. • If you are registered for an event and cannot attend, contact the Senior Activity Center at 817-748-8315 as soon as possible so we can contact the waitlist. • Seniors must comply with the designated departure and/or arrival times. Seniors who are not at the designated departure location and/or not present at the scheduled time may be left behind and will not receive a refund.

Refund Policy All trips and event fees are non-refundable unless one of the following occurs: • Member cancels five business days prior to the event. • The Senior Activity Center or venue cancels the event. • The spot is replaced by someone on the waitlist.

Solicitation • You may not solicit in person or in writing on the premises. City of Southlake retains sole discretion to determine whether a member or guest is engaged in soliciting.

Personal Conduct No guest shall behave in such a manner as to jeopardize the safety, health or well-being of themselves, team members, other guests or the facility. Such behavior, including abusive or profane language, may be grounds for expulsion: • Repeated and/or intentional disobeying of Senior Activity Center policies. • Harassment, verbal or physical threats, or abuse of another individual. • The use of obscene or profane language and gestures.

Right of Removal • Team members reserve the right to remove anyone from the facility who is found in violation of the above rules or is otherwise disruptive to other guests. • A member or guest who violates established rules, policies or procedures is subject to disciplinary action up to and including expulsion.

Emergency Action Plan A primary objective of the City of Southlake is to provide the safest possible parks, facilities and programs for the public’s use and enjoyment. Therefore, each team member at Legends Hall and the Senior Activity Center has received a copy of the Emergency Action Plan and has participated in training and drills conducted to familiarize themselves with the plan and the necessary procedures during an emergency situation. If an emergency occurs, team members will direct all guests based upon the nature of the emergency to ensure their safety and welfare.
