Loan Closet
Serving: Stoughton, Towns of Albion & Dunkirk
Equipment such as wheelchairs, commodes, walkers and other items may be checked out on a short-term basis for a minimum donation of $5.00 to those residing in our case management service area (Stoughton, Town of Albion and Town of Dunkirk).
The Case Management program links older adults with services available to help them remain as independent as possible in their homes and community. Case Managers coordinate, evaluate and advocate for such services (e.g. medical, legal, housing, food financial, and transportation options) to meet each individual’s needs and preferences. Referrals to Case Managers remain confidential. Case Managers share information only with necessary provider agencies for the purpose of providing services and only with the signed consent of the care recipient. Persons may remain anonymous when making a referral for another person. An appointment may be made at the Senior Center or in your home. If you live in Dunn, Pleasant Springs or Rutland case management services are provided by the McFarland Senior Center, (608) 838-7117. _________________________________________
Nutrition Site The Senior Center serves lunch every weekday at 12:00 PM on a donation basis to people over 60 years of age. Participants must make a reservation by 1:00 PM the day before the intended visit. _________________________________________

Foot Care Clinic Stoughton Hospital Home Health offers monthly foot clinics at the Senior Center. Check our newsletter or website www.ci.stoughton.wi.us/senior for specific dates and rates. Participants must contact the Senior Center to make an appointment. _________________________________________
Blood Pressure Screens Blood pressure screening is available each month. Check our newsletter or website www.ci.stoughton.wi.us/senior for specific dates and rates. No registration needed. _________________________________________
Support Groups The Senior Center hosts the following support groups: Caregiver, Grief, Low Vision, Multiple Sclerosis, Older Adult Alcoholics Anonymous, Al-Anon and Parkinson’s disease. Please see pages 20 – 21 for more information, along with other area groups.
Home Delivered Meals Meal delivery is available to homebound older adults in the Stoughton School District every weekday. Participants must call before 1:00 PM the day before to request meals to be delivered at home.

FINANCIAL SERVICES_______________________________________________ Care Wisconsin’s Partnership Program coordinates health and long-term care, including all medical care, for eligible Dane County seniors. Partnership covers all prescription drugs, nursing care, transportation, in-home care, hospitalizations and other services. The Partnership care team—including a care manager/social worker, a registered nurse, and a nurse practitioner—coordinates with local doctors and service providers to support area enrollees. Funding is provided by Medicaid and Medicare for eligible enrollees. Caring for the Caregiver The Area Agency on Aging of Dane County coordinates the National Family Caregiver Support Program. The Caregiver Program provides referrals for support services, help with decision-making and problem-solving related to caregiving, personal care training, a free monthly newsletter filled with caregiving events and useful caregiver information. We also coordinate the Powerful Tools for Caregivers workshop series! For more information please contact Dane County AAA Caregiver Coordinator at (608) 261-5679. Grant funds are available for respite, home chore, lifeline and transportation costs. Community Action Coalition The Community Action Coalition (C.A.C.) provides financial assistance to residents in Dane County in need of security deposits or eviction prevention funds. C.A.C. only assists those households seeking new housing due to eviction, non-renewal of lease, a change in family size, or another emergency situation. Contact a Housing Counselor a(608) 246-4730. To be eligible, a patient can be hospitalized at Stoughton Hospital or another hospital and then enter the inpatient rehabilitative care program. Care Wisconsin Care Wisconsin’s Partnership Program coordinates health and long-term care, including all medical care, for eligible Dane County seniors. Partnership covers all prescription drugs, nursing care, transportation, in-home care, hospitalizations and other services. The Partnership care team—including a care manager/social worker, a registered nurse, and a nurse practitioner—coordinates with local doctors and service providers to support area enrollees. Funding is provided by Medicaid and Medicare for eligible enrollees. Agrace HospiceCare Inc. is a nonprofit, community-based hospice dedicated to providing physical, emotional and spiritual support to patients and families dealing with life-limiting illnesses. Agrace offers comprehensive inhome, inpatient, respite, and residential care, and also provides hospice services in skilled nursing and assisted living facilities throughout Agrace’s service area. Mobile Outreach to Seniors Team (M.O.S.T) M.O.S.T counsels older adults who are experiencing changes and emotional stresses. The goal is to improve life satisfaction and to resolve areas of unrest or conflict. M.O.S.T. also offers the following services: Care plans that focus on the individual’s concern Supportive counseling and education Advocacy Support for the caregiver Partnership with senior centers for community services An appointment may be held at the Senior Center or in the resident’s home from Mondays –Thursdays, 8:30 AM -4:30 PM. Telephone Reassurance, (608) 268-9641 Screened and trained volunteers make scheduled phone calls to promote interaction, reduce loneliness and to for medication or appointment reminders. There is an annual, flat fee to cover minor expenses such as background checks. The maximum fee is $100 per year; qualified low-income individuals receive a discount. There is no limit on the number of volunteer services a client may use although the level of services does depend on matching the client with volunteers able to perform the service. ____________________________________________

Peer Support Program The Peer Support Program is a proactive approach intended to engage older adults who are experiencing isolation and/or loneliness. The project empowers trained volunteers to use their skills to interact with at-risk older adults, while offering compassionate outreach. They are good listeners who bring a little of the outside world into that of their peer, enrich their lives via friendship, and may even foster an increase in independence.
