News: Case Management Information How Long Should I Keep This? by the GWAAR Legal Services Team
Now that tax season is well behind us and summer cleaning is ahead – it is a good time to go through old records and determine whether it is time to fire up the shredder or hold on to the records a bit longer. Below are general recommendations on how long certain documents should be stored. Keep for Less than 1 Year: · ATM, bank-deposit, and credit-card receipts until you reconcile them with your monthly statements. Once you've done that, shred the paper documents (to avoid ID theft) or securely trash electronic files unless you need them to support your tax return. · Insurance policies and investment statements until new ones arrive. · Pay stubs to verify W-2 at the end of the year. Keep for More than 1 Year: · Loan documents (until the loan is paid off). · Vehicle titles until you sell the vehicles. · Investment purchase confirmations in stocks, bonds, mutual funds or anything else. Keep until you sell the investment so you can establish your cost basis and holding period. If that information appears on your annual statements, you can keep those instead.
decrees, Social Security cards, and military discharge papers. · Defined-benefit plan documents, estateplanning documents, life-insurance policies, and an inventory of your bank safe-deposit box. · Annual tax returns. Sources: http://www.consumerreports.org/taxes/how-long-to -keep-tax-documents/ https://www.bankofamerica.com/deposits/manage/how-long -to-keep-documents-before-shredding.go.

Case Managers schedules do vary at the Stoughton Area Senior Center. Please call ahead to schedule appointments. 873-8585
Keep for 7 Years: · Supporting tax documents (W-2, charitable donations receipts, etc.) Keep Forever · Essential records such as birth and death certificates, marriage licenses, divorce

News: Case Management Information Planning Your Doctor Visit Source: nihseniorhealth.gov
How well you and your doctor talk to each other is one of the most important parts of getting good health care. Unfortunately, talking with your doctor isn't always easy. In the past, the doctor typically took the lead and the patient followed. Today, a good patient-doctor relationship is a partnership. You and your doctor can work as a team. Creating a basic plan before you go to the doctor can help you make the most of your visit. Make a List of Your Symptoms Talking about your health means sharing information about how you feel. Sometimes it can be hard to remember everything that is bothering you during your doctor visit. Make a list of your symptoms before your visit so you won’t forget to tell the doctor. Symptoms can be physical, such as pain, fever, a lump or bump, unexplained weight gain or loss, change in energy level, or having a hard time sleeping. Symptoms can also involve your thoughts and your feelings. For example, if you are often confused, or if you feel sad a lot. Be specific. Your list should include: · what the symptom is · when it started · what time of day it happens and how long it lasts · how often it happens · anything that makes it worse or better anything it prevents you from doing. List Your Medications Your doctor needs to know about ALL the medications you take, along with how much you take and how often. The doctor needs the full list because sometimes medicines cause problems when taken together. Also, sometimes a medicine you take for one health problem can cause another health problem to get worse. Medications include: · prescription drugs · over-the-counter (non-prescription) drugs · vitamins, herbal remedies or supplements
· ·
laxatives eye drops.
Do You Use Assistive Devices? Be sure to let your doctor know if you use any assistive devices to help you in your daily activities. Assistive devices can help you see, hear, stand, reach, balance, grasp items, go up or down stairs, and move around. Devices used by older adults may include canes, walkers, scooters, hearing aids, reachers, grab bars, and stair lifts. What Are Your Everyday Habits? Be prepared to tell your doctor about where you live, if you drive or how you get around, eating and sleeping habits, what activities you enjoy, what your sex life is like, and if you smoke or drink alcohol. Be open and honest. It will help your doctor to better understand your medical conditions and best treatment choices for you. Any Life Changes? Your doctor needs to know about any life changes that have occurred since your last visit because they can affect your health. Examples of life changes are divorce, death of a loved one, or changing where you live. Your list should include all your life changes but can be brief, such as "had to sell home and move in with daughter." Any Other Medical Encounters? Write down and tell your doctor if you had to go to the emergency room, stay in the hospital or see a different doctor, such as a specialist, since your last visit. It may be helpful to bring that doctor's contact information.

Dance Lessons Coming This Fall Dancing provides good exercise with many health benefits. We are exploring the possibility of offering dance classes on Saturday or Sunday afternoons this fall. We will need to have a minimum of 8 people that have shown interest by August 1 to secure our instructor and location. This class will be open to couples as well as singles. The cost would be $47 per person for a 6 week class. That’s a little less than $8 for a one -hour class each week! While we will only work on 2-3 dances in a 6 week session, some dance choices include Swing, Salsa, Foxtrot, Waltz, Tango and Country. Those registered as couples are not required to switch partners during class. Singles can switch among themselves. Please contact Tricia Nicoll 873-8585.to secure your spot and state your preference as to which day works best for you and what dances you are most interested in.

ð Thurs., July 7, 10:00-11:00AM. John Buetel moved to this farm house in 1979 and shortly found out that the original part of the house under the clapboard was a Norwegian log cabin. John built a small pond with a 30 ft stream and a small waterfall. Dwarf and weeping conifers along with perennials and a little touch of formality with a trimmed boxwood hedge. Also many raised beds. ð Thurs., July 14, 10:00-11:00 AM A shaded urban garden at the home of Jim Ottney and Jay Hatheway, the Senior Center’s “backyard neighbor.” The lot is filled with a mix of seasonally changing perennials and colorful annuals; plus water features, garden art and small seating areas. Access requires ability to walk an unevenly sloped pathway. ð Thurs., July 21, 10:00-11:00 AM Join Peggy Sharpe for a Concentric condo garden tour in Venevoll. This garden has really developed over the last 15 years. She has many shaded plants, a butterfly garden area, an Asian garden area and a beautiful array of lilies. ð Thurs., July, 28, 10:00-11:00 AM Judy Crandall and her husband have been gardening on their acre property for over 25 years in Albion. View many shade gardens, over 200 different cultivars of hostas and all the accompanying shade companion plants. They have many flowering shrubs & dwarf conifers. Addresses for the tours can be picked up at the Center or call 873-8585.

Lisa’s
Kitchen Corner Staying Hydrated and Staying Healthy
When the temperatures rise, getting enough to drink is important whether you’re playing sports, traveling or just sitting in the sun. Keeping the body hydrated helps the heart more easily pump blood through the blood vessels to the muscles. And, it helps the muscles work efficiently. Dehydration can be a serious condition that can lead to problems ranging from swollen feet or a headache to life-threatening illnesses such as heat stroke. What does being well hydrated mean? The amount of water a person needs depends on climatic conditions, clothing worn and exercise intensity and duration, said John Batson, M.D, sports medicine physician with Lowcountry Spine & Sport in Hilton Head Island, S.C. A person who perspires heavily will need to drink more than someone who doesn’t. Certain medical conditions, such as diabetes or heart disease, may also mean you need to drink more water. People with cystic fibrosis have high concentrations of sodium in their sweat and also need to use caution to avoid dehydration. And some medications can act as diuretics, causing the body to lose more fluid. Thirst isn’t the best indicator that you need to drink. “If you get thirsty, you’re already dehydrated,” Batson said. He also said the easiest thing to do is pay attention to the color of your urine. Pale and clear means you’re well hydrated. If it’s dark, drink more fluids. For most people, water is the best thing to drink to stay hydrated. Sources of water also include foods, such as fruits and vegetables which contain a high percentage of water. It’s also best to avoid drinks containing caffeine, which acts as a diuretic and causes you to lose more fluids. For more information: http://www.heart.org/ HEARTORG/. 14
