National Volunteer Appreciation Week April 10-16, 2016 A special ‘thank-you’ to all of our volunteers for their continued service and support of the Senior Center. We pay special tribute to them the week of April 10-16 during National Volunteer Appreciation Week; however we recognize their efforts throughout the year. Elizabeth Andrew said it best ‘Volunteers do not necessarily have the time; they just have the heart. We appreciate all of the time and energy our volunteers have given. In honor of those volunteers we will be providing opportunities to recognize and thank our volunteers throughout the week at the senior center. Our Senior Center’s case managers are devoted to coordinating, evaluating and advocating for such services to meet each individual’s needs and preferences. Call 873-8585 to schedule an appointment. If you live in Dunn, Pleasant Springs or Rutland case management services are provided by the McFarland Senior Outreach, (608) 838-7117.

News: Case Management Information Transitions of Care Means Success at Home by Julie Stenbroten, RN, Home Health Manager, Stoughton Hospital Home Health
“Transitions of care” refers to the movement of patients between health care practitioners, hospital settings, and home as their condition and care needs change. In today’s changing health care, we are challenged to transition patients from the hospital to home with excellent patient outcomes. Unfortunately, these transitions have not always been smooth. Factors that contribute to unsuccessful transition include poor communication, lack of awareness of community resources, insufficient patient education and reduced support systems. Stoughton Hospital has implemented a transitional care program that is aimed at improving patient experience, reducing readmissions and keeping health care costs down. Comprehensive discharge planning in the hospital begins usually within 24-48 hours of admission. Patients are assessed for risks that may limit their ability to perform necessary aspects of self-care. Risk factors include limited social support, multiple emergency room visits or hospital admissions, multiple chronic conditions and financial stress. Patients play a major role in their successful transition home and fortunately they have access to a highly supportive healthcare system to aid in their success. The Stoughton Hospital Home Health Department is one example of in-home support that can ease the transition home for our frailer, older adults. Whether a patient is admitted to Home Health or simply receiving the complementary care visit of a Home Health RN, a Home Health Registered Nurse will visit the patient within 24-72 hours post discharge. Key areas are addressed during this visit, including but not limited to: an in-depth review of the patient’s prescription and over-thecounter medications and how to manage them, home safety, fall risks, available food and meal preparation, and follow-up appointments with the patient’s physician. Social, family and community 4
supports are also evaluated along with ways to increase these supports. A proactive patient will accept this help, follow doctor’s orders, be attentive to symptoms of concern, and practice good communication with their health team. Through this process, it’s vitally important that the patient concede when they need extra help in the home. A little bit of help with cleaning, bathing, or errands will go a long way toward the long term goal of improved health. With the nurse, the patient can strategize how to best use community supports to make the transition back to home a success. Overall, it is our hope to empower patients and families towards greater self-care and longevity with increased support through all care settings. If you’re not eligible for premium-free Part A, you can buy Part A and Part B. If you’re not getting Social Security benefits, you won’t get any enrollment information from Medicare. You’ll need to call Social Security at 1̻800̻772̻1213 at least 3 months before you turn 65 if you want Medicare to begin the month you turn 65. You can also apply online at www.socialsecurity.gov or make an appointment at the Madison Social Security office t 1-866-7702262. TTY users should call 1̻800̻325̻0778. If you worked for a railroad, contact the RRB to sign up. After you enroll, you’ll receive your Medicare card.

News: Case Management Information Wisconsin Offers Non-Expiring IDs to Seniors by the GWAAR Legal Services Team The Wisconsin Department of Transportation now offers non-expiring identification (ID) cards to Wisconsin residents. To qualify, an individual must be (1) a Wisconsin resident, (2) a U.S. citizen, and (3) age 65 or over. The non-expiring ID cards give seniors the opportunity to have a state ID for voting purposes—without requiring seniors to get a new picture taken every eight years. The non-expiring ID will have the same appearance and security features as a standard expiring ID card, but in place of the expiration date it will indicate that it is “non-expiring.” However, seniors should carefully consider whether the non-expiring ID is right for them. First, to obtain a non-expiring ID, a senior will need to surrender his or her driver license. When an individual surrenders his or her driver license, they are ineligible to drive in any state. Second, the non-expiring IDS are not “REAL ID”- compliant. REAL ID cards must be renewed every eight years. The REAL ID Act sets minimum security standards for sources of identification. Federal agencies cannot accept ID cards from states that do not meet the minimum security standards. For example, if you plan to visit a federal facility or a military base, you will need an ID that meets the act’s standards. According to the official Website of the Department of Homeland Security, REAL ID requirements do not apply to the following: entering federal facilities that do not require ID, voting, applying for or receiving federal benefits, being licensed to drive, accessing health or life-preserving services, or participating in law enforcement proceedings or investigations. The non-expiring IDS are currently free of charge and serve as legal identification for voting

Getting adequate nutrition can be a challenge as you get older. With age, the number of calories you need begins to decline. Every calorie you consume must be packed with nutrition in order to hit the mark. "As we get older, the body becomes less efficient at absorbing some key nutrients," says Katherine Tucker, RD, PhD, chair of health sciences at Northeastern University in Boston. In addition, the ability to taste food declines, blunting appetite. Some foods become difficult to chew or digest. Several key nutrients in particular may be in short supply as you get older. Here are the top vitamins and nutrients to look out for -- and how to get enough. Eat more foods rich in B12. The richest sources include fish, meat, poultry, eggs, milk, and milk products. Talk to your doctor about whether you should take a B12 supplement. Folate/Folic Acid Eat fortified breakfast cereals or plenty of fruits and vegetables.Calcium plays many roles in the body. But it is most important for building and maintaining strong bones. Help yourself to three servings a day of low-fat milk and other dairy products.Vitamin D helps the body absorb calcium, maintain bone density, and prevent osteoporosis. Many foods are fortified with vitamin D, including cereals, milk, some yogurts, and juices. Many Americans fall short on vitamin D, which is mainly produced by the skin when exposed to sunlight. As always check with your personal doctor.

Senior Center Nutrition Program: Meals are provided by Dane County through Consolidated Food Services. They are packed, delivered and served by Senior Center staff and volunteers. If you are a vegetarian or have special dietary needs, talk with Lisa regarding your meal options. Home Delivered Meals: Meal delivery is available to homebound older adults in the Stoughton Area School District every weekday. The Home Delivered Meal menu for the month is on the following page. Nutrition Site: The Senior Center serves lunch every weekday at 12:00 PM. The Nutrition Site offers meals from the menu on p. 15, as well as additional meal choices on Tuesdays & Thursdays (see below). See page 2 for transportation options. Cost of Meals: Individuals 60 or older pay by donation. Suggested donation is $4.00. Individuals under the age of 60 must pay the total cost: Site: $7.50 Delivered Meal: $7.75. Please be reminded that, if at all possible, cancellations/reservations should be made by 1:00 the previous working day. Any cancellations made on the day of, are still subject to payment to the county.
