Case Management Stoughton, Towns of Albion & Dunkirk The Case Management program links older adults with services available to help them remain as independent as possible in their homes and community. Our Senior Center’s case managers are devoted to coordinating, evaluating and advocating for such services to meet each individual’s needs and preferences. Call 873-8585 to schedule an appointment. News: Case Management Services Med D Q’s & A’s:

What is the donut hole and how do I get out? My drug costs at the pharmacy suddenly increased and my pharmacist said it’s because I’m in the coverage gap. What is the coverage gap and how do I get out? The coverage gap- or as it’s more commonly known, the doughnut hole, is one of the four coverage periods people with Medicare Part D can be in during the year. Many people find out they’ve reached the doughnut hole when they unexpectedly get a big bill at the pharmacy. You reach the doughnut hole after you have gone through the deductible and initial coverage periods. This year, you enter the doughnut hole when your total drug costs reach $2,840. Your total drug costs are what you and your plan have spent on covered prescriptions since the start of the year. In 2012, you enter the doughnut hole when your total drug costs reach $2,930. Your costs at the pharmacy temporarily increase when you’re in the doughnut hole. In the past, people were usually responsible for the full cost of their prescriptions during this period. However, because of health reform, people started getting discounts this year on drugs they bought while in the doughnut hole. In 2011, people in the doughnut hole get a 50 percent manufacturer’s discount on covered brand-name drugs and a 7 percent government subsidy on covered generic drugs. These discounts will increase every year until 2020, when the doughnut hole will be gone.

Next year, in 2012, the discount on covered brand-name drugs will stay the same (50 percent) and the discount on covered generics will go up to 14 percent. So, how do you get out of the doughnut hole? In 2011, you get out of the doughnut hole and into the fourth coverage period (catastrophic coverage) when you have spent $4,550 out of pocket on drugs since the start of the year. In 2012, this amount will increase to $4,700. This amount includes what you’ve paid out of pocket, but it does not include your premium or anything your plan paid toward your drugs. People reach the doughnut hole at different times. Some may never enter the doughnut hole while others reach it very early in the year. Whether you get to the doughnut hole depends on your drug costs. There are ways to get help paying for your drugs in the doughnut hole: If you belong to a Medicare private drug plan but cannot afford your medications under this coverage—whether or not you have Extra Help (federal assistance that helps with the costs of the Medicare drug benefit) or get help from a state prescription assistance program—there are a few options that might help, especially during your plan’s coverage gap (“doughnut hole”): 1. Ask your doctor:  About generics. Generic drugs are almost always less expensive than brand-name drugs and might be more affordable for you. In most cases, generic drugs work the same as the brand-name versions.  For samples of your medication. This is a temporary solution, as your doctor may not be able to provide samples for very long.

News: Case Management Services While you are using the samples, you should explore other options for getting your drugs covered, such as asking for an exception (see suggestion 3 below) or switching drug plans. 2. If you have Extra Help, find out if your drug plan has a mail-order option.  If you have Extra Help, you may be able to get a 90-day supply of your prescription for the same amount of money that you would normally pay for a one-month supply.  Keep in mind that with mail order, it will take longer for your drugs to reach you than if you were to go to the pharmacy yourself. You must plan ahead when filling your prescriptions by mail. 3. Request that your plan put your drug in a lower "cost tier" for you. To do so you will have to have your doctor submit a formal request to your plan to ask for an "exception" to the formulary. Be aware that you cannot ask for a "tier exception" when the drug you need is in a "specialty tier" (often the most expensive drugs); or you want to access a brand-name drug at the same co-pay tier as a generic drug.

Cost-of-Living Adjustment (COLA) Information for 2012 Monthly Social Security and Supplemental Security Income (SSI) benefits for more than 60 million Americans will increase 3.6 percent in 2012. The 3.6 percent cost-of-living adjustment (COLA) will begin with benefits that nearly 55 million Social Security beneficiaries receive in January 2012. Increased payments to more than 8 million SSI beneficiaries will begin on December 30, 2011. There is a phone scam that was reported by one of our local seniors. She received a phone call and was told that because of the increase in the Social Security benefit next year that her bank account information was needed. She provided her checking account number because her Social Security check is deposited directly. She realized as she hung up that she shouldn’t have given out the information and went directly to her bank and her checking account was closed. Never provide any personal information to someone who calls you on the phone.

Lisa’s Kitchen Corner Eating Well As You Get Older Includes Milk Low-fat or fat-free milk and milk products should be among the foods you choose every day. These products provide calcium and vitamin D to help maintain strong bones. They also provide protein, potassium, vitamin A, and magnesium. Low-fat or fat-free milk, cheese, and yogurt are good options. If you can’t drink milk, be sure to have other products that contain the nutrients that milk provides. Some cereals and juices are fortified with extra calcium and vitamin D. Salmon, sardines and mackerel are good sources of vitamin D. Protein helps build and maintain muscle, bones, and skin, and you should include some protein in your diet every day. Meats and poultry are sources of protein, B vitamins, iron, and zinc. When buying meats and poultry, choose lean cuts or low-fat products. They provide less total fat, less saturated fat, and fewer

Be Kind and Don’t Share the Flu This Season Hand washing is one of the most significant methods of reducing the spread of influenza infection. If you do it properly, it is also one of the easiest methods to protect you against the flu. In order to catch the flu, you must come in contact with droplets that contain the virus. If those droplets are on your hands and then you touch your nose or mouth, you could contract the virus; however, washing hands with soap will remove much of the flu virus.

Community News Walking The Local Halls Are you looking for an opportunity to get out of the house and get exercise during the colder weather? If so, consider walking the halls of River Bluff School. At no charge, River Bluff will be open to the public from Nov. 14 to March 16th Monday-Friday, 5:00 - 7:00 pm. It will be closed on 11/24, 11/25, 12/26, 1/2. For more details call the Stoughton High School Athletic Office at 877-5620.

CHRISTMAS DAY MEAL The Stoughton VFW will be preparing their Annual Christmas Day Meal on Dec. 25th from 11:00 AM to 1:00 PM at the VFW. If you are homebound you may have a meal delivered to you by calling the Senior Center. If you are in need of a ride to the VFW for the meal, you may also call 873-8585. Thank you to the VFW.

Volunteers Needed...  

To coordinate the monthly Saturday night card party. The time commitment would be 3 to 4 hours the fourth Saturday of the month. For fund raising committee to help plan and complete several large fund raisers for the Senior Center. Time commitment would be 5 to 6 meetings and the time for the actual event. Call Cindy 873-8585. Come in or call on Tuesday mornings from 9:00 AM to 11:00 AM, where you can ask your questions directly to a computer specialist.

Agrace HospiceCare to Offer Two Grief Support Opportunities In December, Agrace HospiceCare (formerly known as HospiceCare Inc.), will offer a recurring support group and provide an introductory session for those grieving the death of a loved one. All of the sessions are open to the public and are provided without charge. All groups are held at the Don & Marilyn Anderson HospiceCare Center, 5395 E. Cheryl Parkway, Madison. Bridges Weekly Support Group is held every Wednesday from 9:30 a.m. to 11:00 a.m. This ongoing group is for adults who are grieving the death of a loved one. Pre-registration is not required. Introduction to Grief is an informative session for adults that offers suggestions on how to navigate the grieving process and provides information about Agrace HospiceCare grief support groups. Tips to Manage Holiday Stress 1. Acknowledge your feelings. If someone close to you has recently died or you can't be with loved ones, realize that it's normal to feel sadness and grief. 2. Reach out. If you feel lonely or isolated, seek out community, religious or other social events. 3. Be realistic. As families change and grow, traditions change as well. Choose a few to hang on to & be open to creating new ones. 4. Set aside differences Accept family members and friends as they are, even if they do no live up to your expectations. 5. Stick to a budget. Decide how much money you can afford to spend; and stick to your budget. 6. Plan ahead. Set aside specific days for shopping, baking, visiting friends, etc. to prevent last minute trips. 7. Learn to say no. Saying yes when you should say no can leave you feeling resentful & overwhelmed. 8. Don’t forget healthy habits. Don’t go overboard on sweets, cheese or drinks; continue to get plenty of sleep & physical activity. 9. Take a breather. Make some time for yourself! 10. Elder Friends: Reaching out to Peers As we get older some of us feel lonely, depressed, forgetful or find life changes too difficult. Often all that is needed is a friendly listener; someone who can give a little help over the rough spots. Through the Peer Support Program of Dane County, these individuals can be matched with an Elder Friend. Elder Friends are volunteers who meet with a peer to help enrich their lives with sensitivity and practical assistance. The program is currently seeking more volunteers to fill our Elder Friend needs. What do you get from being an Elder Friend?  Lasting friendships.  A fuller and more enriched life for you and your Elder Friend.  Training on issues specific to older adults and training on when and whom to make a referral for further assistance.  Ongoing support from the Peer Support Coordinator and Case Managers. What is required to be an Elder Friend?  Elder Friends are asked to maintain a 1 time per week relationship (in person or by phone).  Elder Friends need to have good communication and listening skills.  Elder Friends need to be non-judgmental and have respect for others. News: COA & Community Commission on Aging Meeting Highlights for November 1, 2011      

All lighting fixtures where replaced with energy saving fixtures and bulbs this month, through a grant written by the City’s Planning Department. Fund raising letter was discussed, still needing to accomplish our 2011 requirement of $29,500. Budget for 2012 still being discussed at city council. Hoping for approval soon so we can post the assistant position. Discussion of special gift fund monies, questions have come form City Council of what that money is intended for. State and Federal Legislative issues affecting older adults reported on and information distributed. Discussion on potential expansion of our brat fest to be included in the Madison Brat Fest.
