Case Management Stoughton, Towns of Albion & Dunkirk The Case Management program links older adults with services available to help them remain as independent as possible in their homes and community. Our Senior Center’s case managers are devoted to coordinating, evaluating and advocating for such services to meet each individual’s needs and preferences. Call 873-8585 to schedule an appointment. News: Case Management Services Not All Health Care Is Created Equal Printed with permission from Centers for Medicare and Medicaid Services (CMS) and State Health Insurance Assistance Program (SHIP)

The quality of health care professionals, services, facilities and plans can greatly impact a patient’s health. Medicare beneficiaries have choices when it comes to the quality of their providers. That’s why the Centers for Medicare & Medicaid Services just launched the Quality Care Finder designed to help beneficiaries and their caregivers find better health care options. Patients and their caregivers can get started at: www.Medicare.gov/QualityCareFinder to find health care providers, facilities, health and drug plans, and equipment suppliers, and make “apples-to-apples” comparisons of their quality. Available resources for consumers include: Hospital Compare: Compare Medicarecertified hospitals locally and throughout the country based on the quality of their care. Nursing Home Compare: Find Medicarecertified nursing homes and the special services each nursing home offers, like dementia care, ventilators or rehabilitation. Then compare their star ratings and the quality of care they give. Home Health Compare: Find Medicarecertified home health agencies based on services like skilled nursing care, physical therapy, speech therapy and home health aides. Then, compare each home health agency based on the quality of their care.

Medicare Plan Finder: Get detailed, personalized information about the cost and benefits of available Medicare health and drug plans, and compare the quality of the services they provide. Dialysis Facility Compare: Find Medicarecertified dialysis facilities and their services. Then, compare each facility based on quality of care. Physician Compare: Find doctors based on medical specialty, clinical training, foreign languages spoken, and more. Check to see if a doctor accepts the Medicare-approved amount as full payment.

The Centers for Medicare and Medicaid Services: Consumer Alert The Centers for Medicare and Medicaid Services (CMS) has recently become aware of a website that has the appearance of being an official government website for the Pre-Existing Condition Insurance Plan. This new website - http:// preexistingconditioninsuranceplan.com is not maintained by any government programs and consumers are strongly urged not to submit any personal information requested by this website under the assumption that it is a government website. CMS is taking the appropriate steps to protect consumers from being misled.

News: Case Management Services The Pre-Existing Condition Insurance Plan made available through the Affordable Care Act makes health insurance available to people who have had a problem getting insurance due to a pre-existing condition. The Pre-Existing Condition Insurance Plan:  Covers a broad range of health benefits, including primary and specialty care, hospital care, and prescription drugs.  Does not charge you a higher premium just because of your medical condition.  Does not base eligibility on income. Individuals interested in the new federally backed pre-existing condition insurance program should visit: www.pcip.gov or call 866-717-5826.

FoodShare Program Changes Effective October 1, 2011 Beginning on October 1, the Foodshare program will have new income limits and allotments and the standard household deduction amounts will increase. The Maximum Excess Shelter deduction, for example, will increase to $444. This enables more people to qualify for food share at slightly higher food share benefits per month. For questions related to Food Share, please call your benefit specialist or local senior center. Please always feel free to see a Stoughton Senior Case Manager for more assistance about food share or call Second Harvest Food Bank Helpline at 1-877366-3635 for further information and assistance.

MARK YOUR CALENDARS! Donations for Case Management We are asking for donations to help offset the cost of Case Management services here at the Stoughton Area Senior Center. Our case managers help individuals and families with a variety of services including home services, food programs, loan closet, mental health services, Medicare and Medicare D questions, housing and financial assistance. If you or some one you care about has received help from our case managers please consider making a donation to the Senior Center on behalf of the case managers. Starting in calendar year 2011, there will be a change in the dates of the Open Enrollment Period for Medicare Part D and Medicare Advantage Plans. The new dates for changing your Medicare Part D and Medicare Advantage plans will be October 15 – December 7, 2011 for plans to be effective January 1, 2012. You will not be able to change plans for the next year after December 7, therefore please plan accordingly. To find a new plan that will work for you, go to the Plan Finder on the Medicare website – www.medicare.gov. Lisa’s Kitchen Corner Food Safety Food safety is very important for older adults because they are at increased risk of food borne illness. As we age, our immune systems become weaker. We have less stomach acid to control bacteria and our kidneys, which help filter bacteria from the blood, also weaken. Infections from food borne illness can be hard to treat and can recur in older adults. In addition to following good food safety habits, there are certain foods that older adults should not eat:  Raw, rare or undercooked meats and poultry (rare hamburgers, carpaccio and beef or steak tartare)  Raw fish (including sushi, sashimi, ceviche and carpaccio)  Undercooked and raw shellfish (clams, oysters, mussels and scallops)  Refrigerated smoked seafood  Unpasteurized dairy products ("raw" milk and cheeses)  Some fresh soft cheeses (Brie, Camembert, blue-veined varieties and Mexican-style queso fresco) unless made with pasteurized milk  Raw or undercooked eggs (soft-cooked, runny or poached)  Raw sprouts (alfalfa, clover and radish)  Deli salads  Unpasteurized fruit and vegetable juices  Refrigerated pate or meat spreads. Some ready-to-eat foods require reheating before use. These foods include hot dogs, luncheon and deli meats and fermented and dry sausages. Throw away packaged items once the "use-by" date has passed. If you think you have a food borne illness, contact your health-care provider.

Maple Pumpkin Muffins 2 c. all-purpose flour ¾ cup plus 2 tablespoons packed brown sugar, divided 2 teaspoons baking powder 1 teaspoon ground cinnamon ½ teaspoon baking soda ½ teaspoon pumpkin pie spice ¼ teaspoon salt 2 eggs 1 cup canned pumpkin ¾ cup evaporated milk ¼ cup vegetable oil 3 tablespoons maple syrup, divided ½ cup chopped pecans or walnuts 1package(3ounces)cream cheese, softened Topping: ¼ cup chopped pecans or walnuts 2 teaspoons brown sugar In a large bowl, combine flour, ¾ c. brown sugar, baking powder, cinnamon, baking soda, pumpkin pie spice and salt. Whisk the eggs, pumpkin, milk, oil and 1 tablespoon syrup; stir into dry ingredients just until moistened. Fold in nuts. In a small mixing bowl, beat cream cheese and remaining brown sugar and syrup until smooth. Gently stir into batter until mixture appears swirled. Fill greased or paper lined muffin cups about ¾ full. Combine topping ingredients; sprinkle over batter. Bake at 400 degrees for 20-25 minutes.

NUTRITION PROGRAMS All meals are provided by Dane County. Consolidated Food Services. They are packaged, delivered and served by Senior Center volunteers. If you have special dietary needs, please call and talk with Lisa regarding your meal options. Nutrition Site: The Senior Center serves lunch every weekday at 12:00 PM. Menu Pg.15 Home Delivered Meals: Meal delivery is available to homebound older adults in the Stoughton Area School District every weekday. Cost of Meals 2011: Individuals 60 years or older pay by donation. The minimum suggested donation is $3.00/meal. Individuals under the age of 60 must pay the total cost of the meal. Meal Site: $6.10 Delivered Meal: $6.35.

Community News November is National Family Caregiver Month It is a time to thank, support, educate, and celebrate family caregivers! November is the nationally recognized family caregiver month that seeks to draw attention to the many challenges facing family caregivers, advocate for stronger public policy to address family care giving issues, and raise awareness about community programs that support family caregivers. For more information see page 7.

Computer “newbies” and experienced computer users are welcome to use our Senior Center Computer Lab and WiFi Internet. Please call 873-8585 to inquire about our private beginner computer classes. Come in or call on Tuesday mornings from 9:00 AM to 11:00 AM, where you can ask your questions directly to a computer specialist.

Walking The Local Halls Are you looking for an opportunity to get out of the house and get exercise during the colder weather? If so, consider walking the halls of River Bluff School. At no charge, River Bluff will be open to the public from Nov. 14 to March 16th Monday-Friday, 5:00 - 7:00 pm. It will be closed on 11/24, 11/25, 12/26, 1/2. Yes ...You Can Join Us! If you are 55 and older , feel free to join us anytime! There are no membership fees. You do not have to live in the city of Stoughton to participate in the activities or programs at the Stoughton Area Senior Center. There are residency requirements for certain services such as case management services, loan closet items, transportation and meals on wheels. We do ask that you sign up for our scan card for the center. For additional information, please contact us.

She is a student at University of Wisconsin -Madison. She lives on the East side of Madison with her fiancé, Dan, and her two cats. She does her studies in Social Work part-time at the UW with classes only on Saturdays. This allows her time to work her regular job, intern here, and plan her upcoming wedding. She will be married next May in Madison while she has a one week break from her classes! She enjoys gardening, baking, reading, documentaries, laughing, and rummaging through antique stores. Welcome!

Thanksgiving Day Meal and Volunteers We are once again partnering with Stoughton Hospital, On Track Communications and McGlynn Pharmacy in providing a noon meal for homebound individuals 60 years and over who live in the Stoughton school district. If you are home bound and are in need of a Thanksgiving Day meal with all the trimmings, please call the Stoughton Senior Center by November 18th. News: COA & Community Commission on Aging Meeting Highlights for October 5, 2011      

Possible reductions at Senior Center with 2012 budget. Assistant director hiring put on hold until after budget is finalized, reductions to position are possible. Fund raising letter will be going out at the end of November to help realize goal for 2011 of $30,000. Committees will resume meeting schedules now that Cindy is back. A Message From Our Director November is the month in which we give thanks. Even though we find things that could be better, or could have happened differently, we are still able to find things that are good and that we are thankful for. The economic climate is one that we wish could be different, but the Senior Center is thankful for all of the support we receive from volunteers and donors. Our fund raising goal for 2011 is $29,500. We are well on our way to that goal, but need some help in finishing that by the end of December. We will be sending a donation letter out to all community members at the end of November. We appreciate any donations that can be sent to support the Senior Center. If you have any questions or would like to discuss fund raising for the Senior Center, please contact me here at the Center. It's up to Madea, with the help of the equally rambunctious Aunt Bam, to gather the clan together and make things right the only way she knows how: with a lot of tough love, laughter, and the revelation of a long-buried family secret.

The Norwegian Language Class is looking for more members. The class meets on Wed. mornings from 9:00AM to 11:00AM. Cost is $10 per class, paid directly to the teacher. Komm til vår klasse.

Christmas Floral Class Are you interested in taking a 3 session floral design class at the Senior Center? Cost would be $15.00 for the class and flowers. The class will be meeting Tuesday afternoon at 1:00 pm. Nov. 1, 8 and 15. There would be a few props that the student would need to supply.
