Case Management Stoughton, Towns of Albion & Dunkirk The Case Management program links older adults with services available to help them remain as independent as possible in their homes and community. Our Senior Center’s case managers are devoted to coordinating, evaluating and advocating for such services to meet each individual’s needs and preferences.

TURNING 65?

7 COMMON QUESTIONS AND ANSWERS ABOUT MEDICARE 1-HOW DO I ENROLL IN MEDICARE? To enroll in Medicare, most people need to contact Social Security directly. Do this before your 65th birthday to avoid a lapse in health coverage.

Some people-including early retirees receiving Social Security and those with Social Security disability or Railroad Retirement benefits-are automatically enrolled in Medicare when they become eligible.

News: Case Management Services You can enroll online, at your local Social Security office, or by calling 1-800-772-1213 (TTY 1-800-325-0778) 2. WHEN DO I HAVE TO ENROLL? You have a 7-month window around your 65th birthday to enroll. This is called the Initial Enrollment Period.

After that, you can still enroll in Medicare, but you may have to wait for a designated special/general enrollment period to do so. Learn more about when to join.

3. WHAT IF I’M STILL WORKING AND HAVE INSURANCE? DO I NEED TO ENROLL AT AGE 65? If you are working past age 65 and have insurance from your job (or your spouse is working and you get health coverage that way), you should contact your plan and review how your coverage will change before deciding whether you need to enroll in Medicare now.

National Council on Aging has a free website: My Medicare Matters . This site offers some helpful tips for what to consider if you have other health insurance besides Medicare.

4. DOES MEDICARE COVER? Medicare covers a wide range of medically necessary screenings, supplies, and procedures.

Some are free, including many preventive services.

The Medicare & You handbook which Medicare sends out every year to beneficiaries, provides a summary of many covered services.

For more specific coverage details, Medicare.gov contains a helpful online search tool where you can find whether a test, item or service is covered.

You also may speak with your doctor, or you can contact Medicare directly at 1-800-MEDICARE (1-800-633-4227) to verify coverage based on your personal situation.

5-I’M GOING ON A CRUISE/OVERSEAS TRIP. WILL MEDICARE COVER ME IF I NEED EMERGENCY CARE WHEN I’M AWAY? 4

Medicare covers health care for people when they are physically located within the 50 US states, District of Columbia, and US territories.

In rare cases, Medicare Part A may cover medically necessary services in a foreign country when a foreign hospital is closer than a US facility. Learn more about these exceptions.

In some cases, Medicare Part B may cover medically necessary health care while on board a cruise ship within the territorial waters adjoining the US. Generally Medicare won’t pay for services you get when a ship is more than six hours away from a US port.

If you have a supplemental insurance policy, you should check to see if it covers medical care abroad. Otherwise, it is recommended to purchase travel insurance before your trip, which can help to offset the costs of overseas emergency care if it’s needed.

6. HOW DO I PAY FOR MY MEDICARE? If you are receiving Social Security benefits at the time you enroll in Medicare, your Part B premium will be deducted automatically from your monthly check.

If you have not begun to receive Social Security yet, you receive a quarterly bill for your premium, which you can pay several different ways.

7. HOW CAN I FIND OUT HOW MUCH I’VE PAID FOR MEDICARE SERVICES TIS YEAR? Medicare offers a free, confidential online service for people to access information about their benefits and services, including how much they’ve paid for services. See how to create an account.

You also can contact Medicare directly at 1-800MEDICARE (1-800-633-4227) with any questions about enrollment, payments, claims, coverage issues, and more.

What Is Osteoporosis? Article reprinted with permission from Nutrition.gov and NIH Senior Health A Bone Disease Osteoporosis is a disease that thins and weakens the bones to the point that they become fragile and break easily. Women and men with osteoporosis most often break bones in the hip, spine, and wrist, but any bone can be affected. You can't "catch" osteoporosis or give it to someone else.

How Bone Loss Occurs Bone is living tissue. Throughout our lives, the body breaks down old bone and replaces it with new bone. But as people age, more bone is broken down than is replaced.

The inside of a bone normally looks like a honeycomb, but when a person has osteoporosis, the spaces inside this honeycomb become larger, reflecting the loss of bone density and strength. The outside of long bones -- called the cortex -- also thins, further weakening the bone. In fact, the word "osteoporosis" means "porous bone."

Osteoporosis is often called "silent" because bone loss occurs without symptoms. People may not know that they have osteoporosis until a sudden strain, bump, or fall causes a bone to break. This can result in a trip to the hospital, surgery, and possibly a long-term disabling condition.

Prevention and Treatment The good news is that osteoporosis can often be prevented and treated. Healthy lifestyle choices such as proper diet, exercise, and treatment medications can help prevent further bone loss and reduce the risk of fractures. Contact your physician for further information.

NUTRITION PROGRAM INFORMATION All meals are provided by Dane County through Consolidated Food Services. They are packaged, delivered and served by Senior Center volunteers. If you are a vegetarian or have special dietary needs, please call and talk with Lisa regarding your meal options.

Nutrition Site: The Senior Center serves lunch every weekday at 12:00 PM. See menu page 15.

Home Delivered Meals: Meal delivery is available to homebound older adults in the Stoughton Area School District every weekday.

Cost of Meals: Individuals 60 years or older pay by donation. The minimum suggested donation is $3.00/meal. Individuals under the age of 60 must pay the total cost of the meal. Meal Site: $6.15 Delivered Meal: $6.40. Please be reminded that cancellations and reservations should be made by 1:00pm the previous working day. Any cancellations made on the day of, are still subject to payment to the county.

Recipe of the Month: Printed from Cooks.com Cheesecake Base: 1 ¼ c. graham cracker crumbs ¼ c. butter, melted Filling: 2 (8oz.) pkgs. cream cheese, softened 1 can Pillsbury Ready to Spread Cream Cheese Frosting Supreme 2 tablespoons flour 1 teaspoon vanilla 2 eggs Topping: 1 cup sour cream ¼ cup powdered sugar 1/2 teaspoon vanilla Heat oven to 350 degrees. In a small bowl, combine graham cracker crumbs and melted butter; press into 9 inch pie pan or bottom of 8 inch square pan. In large bowl, blend filling ingredients; beat I minute at highest speed. Pour over crust. Bake for 35-45 minutes or until light golden brown. Cool. In a small bowl, stir together topping ingredients until well blended. Spread over top of cheese cake.

Chill. Store in refrigerator.

Afraid of Falling? Take a Stepping On Class….

Reduce your falls risk by registering now for “Stepping On,” an evidence based 7-week, 2-hour class series for older adults. In a recent study, seniors who completed “Stepping On” experienced a 31% reduction in falls; and in Dane County, data supports 50%. This will be held on Tuesdays, October 8 thru November 19, at 3:00 to 5:00 PM sponsored by the Stoughton Area Senior Center, 248 W. Main St. This series is led by a medically trained professional. They will examine medications, home safety, balance, vision and more. Learn simple and fun balance and strength training exercises. The program builds confidence, reduces falls, and prevents hospital stays. Total cost $35.00/person for instruction and materials.

Parkinson’s Disease Exercise Class Instructed by Emily Devine, Stoughton Hospital DPT Designed for individuals diagnosed with Parkinson’s Disease. This class is based on current research targeting specific Parkinson’s Disease movement deficits and focuses on improving function, mobility, mood, quality of life, and fall reduction in a fun atmosphere. This six-week class will be every Friday starting Sept. 20 through Nov. 1 (no class Oct. 4) from 1:00 pm to 1:45 pm at the Stoughton Senior Center. News: COA & Community Commission on Aging Meeting Highlights for August 6, 2013 News  Welcomed David Kneebone as COA’s official City Council representative.  NCOA received our accreditation workbook; onsite peer review will be scheduled soon.  Thanks to Wood Shop for making us a new, larger contribution box.

City’s Budgeting for Outcomes: The City Results Teams’ prioritization of City service offers is underway. Soon Department Head’s will have a chance to pitch their recommendations.

Senior Center Policy Manual: Discussed and approved final Introduction, Service Animal and Program Registration Policies of Policy Manual.

Action plans: COA motioned to approve the COA orientation packet to include participant welcome packet materials, Annual Report, COA bylaws, COA committee synopsis, budget, City’s Code of Ethics and Conflict of Interest policy.

Peer Support of Dane County: Help Needed in Stoughton Peer Support of Dane County connects older adults with volunteers to provide needed support through friendly home visits. We need volunteers who have an interest in helping to keep older adults in their own homes by providing emotional support to promote wellness and reduce social isolation. This is a flexible opportunity for adults 55+ in the Stoughton area, mental health or social work experience is helpful but not necessary.
