PROGRAMS___________________ Social Events & Activities The Stoughton Area Senior Center promotes activities that provide opportunities for growth, life-long learning, leadership, intergenerational interaction, and socialization. A complete list of our scheduled activities may be found in our monthly Yahara Senior News newsletter, which is available at the Senior Center or our website. Many events and activities are free, and scholarships are available through the Friendship Fund if needed. Below are some of the social events and activities offered.

Loan Closet

Serving: Stoughton, Towns of Albion & Dunkirk

Equipment such as wheelchairs, commodes, walkers and other items may be checked out on a short term basis for a minimum donation of $5.00 to those residing in our case management service area (Stoughton, Town of Albion and Town of Dunkirk). ______________________________________

The Case Management program links older adults with services available to help them remain as independent as possible in their homes and community. Our Case Managers coordinate, evaluate and advocate for such services (e.g. medical, legal, housing, financial, food and transportation options) to meet each individual’s needs and preferences. Referrals to Case Managers remain confidential. Case Managers share information only with necessary provider agencies for the purpose of providing services and only with the signed consent of the care recipient. Persons may remain anonymous when making a referral for another person. An appointment may be made at the Senior Center or in your home. If you live in Dunn, Pleasant Springs or Rutland townships, case management services are provided by the McFarland Senior Center, (608) 838-7117. ___________________________________________

Nutrition Site The Senior Center serves lunch every weekday at 12:00 PM on a donation basis to people over 60 years of age. Participants must make a reservation by 1:00 PM the day before the intended visit.

Foot Care Clinic Stoughton Hospital Home Health United offers monthly foot clinics at the Senior Center. Check our newsletter or website www.ci.stoughton.wi.us/senior for specific dates and rates. Participants must contact the Senior Center to make an appointment. ___________________________________________

Blood Pressure Screens Blood pressure screening is available each month. Check our newsletter or website www.ci.stoughton.wi.us/senior for specific dates and rates. No registration needed. ___________________________________________

Support Groups The Senior Center hosts the following support groups: Caregiver, Low Vision, Multiple Sclerosis, Older Adult Alcoholics Anonymous, Al-Anon, and Parkinson’s Disease. Please see pages 20 – 21 for more information, along with other area support

Home Delivered Meals Meal delivery is available to homebound older adults in the Stoughton School District every weekday. Participants must call before 1:00 PM the day before to request meals to be delivered at home.

Community Options Program

Care Wisconsin’s Partnership Program coordinates health and long-term care, including all medical care, for eligible Dane County seniors. Partnership covers all prescription drugs, nursing care, transportation, dental needs, in-home care, Hospitalizations and other services. The Partnership care team—including a care manager/social worker, a registered nurse, and a nurse practitioner—coordinates with local doctors and service providers, and is located in Stoughton to support area enrollees. Caring for the Caregiver The Area Agency on Aging of Dane County coordinates the National Family Caregiver Support Program. Community Action Coalition The Community Action Coalition (C.A.C.) provides financial assistance to residents in Dane County in need of security deposits or eviction prevention funds. C.A.C. only assists those households seeking new housing due to eviction, non-renewal of lease, a change in family size, or another emergency situation.

Stoughton Hospital Cardiac Rehabilitation & Wellness Center 900 Ridge Street, (608) 873-2314 Phase 3 Cardiac Rehab Program Once participants have finished their Cardiac Rehab (Phase 2 Program), they can continue to exercise at Stoughton Hospital under the supervision of medical professionals through the Phase 3 Cardiac Rehab Program. Primary Prevention Program Those who have not participated in a previous Cardiac Rehab Program but have multiple cardiac risk factors and need to exercise in a safe environment can join the Primary Prevention Program.

Peer Support Program Elder Friends: Reaching Out to Peers The Peer Support Program is a proactive approach intended to engage older adults who are experiencing isolation and/or loneliness. The project empowers trained older adult volunteers, called Elder Friends, to use their skills to interact with at-risk older adults (peers helping peers) while offering compassionate outreach. Elder Friends are good listeners who bring a little of the outside world into that of their peer, enrich their lives via friendship, and may even foster an increase in independence. Care Wisconsin Transportation (608) 241-5230 for program enrollment Private pay service is available to Dane County seniors (55 or older) and adults with disabilities needing rides to and from medical, dental or other therapeutic appointments. Cost is $3/mile and billed by mail at the beginning of the month for the previous month’s rides. (608) 280-7019 Volunteers escort veterans, who have no other means of transportation to their doctor appointments. Passengers must be ambulatory and a veteran. There is no charge, but reservations are at on a first come, first serve basis. Appointments must be made between9:00 am – 2:00 PM.
