Case Management Stoughton, Towns of Albion & Dunkirk The Case Management program links older adults with services available to help them remain as independent as possible in their homes and community. Our Senior Center’s case managers are devoted to coordinating, evaluating and advocating for such services to meet each individual’s needs and preferences. Call 873-8585 to schedule an appointment. News: Case Management Services MEDICARE AND OPT-OUT DOCTORS Doctors do not have to accept Medicare. An opt-out doctor does not accept Medicare at all. Opt-out Doctors can charge Medicare patients whatever they want. They don’t submit any health care claims to Medicare. They are not subject to the Medicare laws that limit the amount that doctors can charge. When you see an opt-out doctor, you pay the entire cost of your care, unless it’s an emergency or you need urgent care. Generally, Medicare does not pay for health care services from an opt-out-doctor. If your doctor has officially opted out of Medicare, he/she must have you sign a private contract to receive care from a doctor who has opted out of Medicare. If you sign this contract, you are responsible for the full cost of services you receive from the opt-out-doctor. You cannot get Medicare reimbursement for services received from an Opt-out doctor. If a doctor has opted out of Medicare and does not ask you to sign a private contract before providing you with Medicare-covered health care services, you will not have to pay anything for health care services received. Your doctor must refund you any money you’ve already paid for those services. If you have Original Medicare, the traditional Medicare program offered directly through the federal government, you will pay the least for doctor’s services if you see a doctor who accepts Medicare and takes assignment. That doctor accepts Medicare’s approved amount as full payment for health care services. Make sure your doctor accepts Medicare and takes assignment. You can also call 800-MEDICARE or visit www.medicare.gov to find doctors who accept Medicare and take assignment. If you have a Medicare Advantage Plan, also known as a Medicare private health plan,

you should see doctors within your plan’s network. You typically pay the least if you go to a doctor that is in the plan’s network. Check with your plan to see what rules apply. When you have weak hands/fingers, picking an egg out of a carton is easy if you first wet your fingers. If you find a drinking glass hard to hold onto, make it easier to grip by putting a terry cloth wristband around it. The terry cloth will absorb moisture and provide a better surface to hold the glass securely. If you have a diminished sense of smell or taste, put the dates on leftovers before you put them in the refrigerator.

News: Case Management Services WELCOME NEW FRIENDS! SENIOR CENTER OPEN HOUSE

IN THE HOSPITAL? BE AWARE OF YOUR STATUS If you are on Medicare and go to the hospital, make sure of your status. Ask if you have been admitted or are there for observation. Hospitals are failing to admit seniors and putting their status as being observed. Why does it matter? It could matter to your pocketbook: Medicare requires a 3-day hospital admission to cover the costs of any posthospitalization nursing home stay. If you have been in the hospital under observation, Medicare will not cover the cost of time you may need to spend in a nursing facility. If you have not been admitted to the hospital for 3 days, the whole nursing home bill will be your responsibility. Also, it is unlikely your supplemental insurer will help. If you have been admitted to a hospital, the costs are covered by Medicare Part A (hospital insurance). If you are at the hospital under observation the costs fall under Medicare Part B (doctor’s services and outpatient care). For some patients, this can mean paying more out of pocket to cover hospital bills. Check your status each day you are at the hospital. If told you are under observation, talk to your doctor.

For the duration of 2013, the Center will host gatherings to share information about all we have to offer, and hear your ideas about additional programs and activities we might start in the future. Welcome New Friends Open Dates: June 18th, September 17th, November 19th at 6:30 pm. There are no membership fees or residency requirements to participate in the programs at the Stoughton Area Senior Center. There are age and residency requirements for certain services including case management services, transportation, and meals on wheels. Eat Right: Food, Nutrition, and Health Tips from the Academy of Nutrition and Dietetics Eating Right for Older Adults

Make half your plate fruits and vegetables. Eat a variety of vegetables, especially dark green, red and orange vegetables plus beans and peas. Fresh, frozen and canned vegetables all count. Choose “reduced sodium” or “no-salt-added” canned vegetables. Add fruit to meals and snacks. Buy fruits that are dried, frozen or canned in water or 100% juice, as well as fresh fruits.

Make at least half your grains whole. Choose 100% whole-grain breads, cereals, crackers, pasta and brown rice. Also, look for fiber-rich cereals to help stay regular.

Enjoy your food but eat less. Most older adults need fewer calories than in younger years. Avoid oversized portions. Try using a smaller plate, bowl and glass. Cook more often at home, where you are in control of what’s in your food. When eating out, choose lower calorie menu options. Choose dishes that include vegetables, fruits and whole grains. When portions are large, share a meal or take half home for later. Write down what you eat to keep track of how much you eat.

Be physically active your way. Pick activities that you like and start by doing what you can. Every bit adds up and health benefits increase as you spend more time being active. If you are currently inactive, start with a few minutes of activity such as walking. Gradually increase the minutes as you become stronger.

Switch to fat-free or low-fat milk, yogurt and cheese. Older adults need more calcium and vitamin D to help keep bones healthy. Include three servings of fat-free or low-fat milk, yogurt or cheese each day. If you are lactose intolerant, try lactose-free milk or a calcium-fortified soy beverage.

Vary your protein choices. Eat a variety of foods from the protein food group each week, such as seafood, nuts, and beans and peas, as well as lean meat, poultry and eggs.

Cut back on sodium and empty calories from solid fats and added sugars. Look out for salt (sodium) in foods you buy. Compare sodium in foods and choose those with lower numbers. Add spices or herbs to season food without adding salt. Make major sources of saturated fats such as desserts, pizza, cheese, sausages and hot dogs occasional choices, not every day foods. Switch from solid fats to oils when preparing food. Drink water instead of sugary drinks. Select fruit for dessert. Eat sugary desserts less often.

NUTRITION PROGRAM INFORMATION All meals are provided by Dane County through Consolidated Food Services. They are packaged, delivered and served by Senior Center volunteers. If you are a vegetarian or have special dietary needs, please call and talk with Lisa regarding your meal options. Nutrition Site: The Senior Center serves lunch every weekday at 12:00 PM. See menu page 15. Home Delivered Meals: Meal delivery is available to homebound older adults in the Stoughton Area School District every weekday. Cost of Meals: Individuals 60 years or older pay by donation. The minimum suggested donation is $3.00/meal. Individuals under the age of 60 must pay the total cost of the meal. Meal Site: $6.15 Delivered Meal: $6.40. Please be reminded that cancellations and reservations should be made by 1:00pm the previous working day. Any cancellations made on the day of, are still subject to payment to the county.

Community & Center News Ballroom Dance Class

Thursdays, June 6th, 13th, 20th and 27th from 3:00 to 4:00 pm at the old Yahara Elementary building gym, 900 W. Wilson Street, Stoughton. Join dance instructor Gwen Engel of Right Step instruction for Ballroom Dance techniques that will include the most popular slow dance, the Fox Trot, and the most popular fast dance, the Swing. Cost is $9.00 per person for each dance class. Call to register ASAP.

Strolling the Streets-Walking Group Join your fellow walking enthusiasts this summer to stroll the streets of Stoughton. We will meet on Tuesday mornings at 8:30 AM starting June 4. This group will meet at the Senior Center and walk to various destinations of interest around the city. This is a great opportunity to learn more about our community, while getting regular exercise! In June we will search for Luke Stoughton’s grave site, July will take us to East Side Park and the Historic park neighborhood, and in August we will visit the Lowell Park Community Gardens.

You'll enjoy movement sequences that enhance your range of motion, flexibility and coordination, and rediscover your natural ability to function with grace, less discomfort and more efficiency, whatever your age or current condition. Feldenkrais has much to offer those who are concerned with retraining or regaining their ability to move, balance, and increase ease of functional movement. We will offer two different time options for this eight-week class: 1:30 PM Tuesdays, beginning on June 4th, and 6:30 PM Wednesdays, beginning on June 5th. Cost is $5.00 per class. Smart Planning Series Make plans to attend this month’s session to learn how to use Smart Planning for Medical and Financial Power of Attorney. This session will be presented by a local expert and they will provide us with helpful tips and key points to consider on Thursday, June 27th at 6:00 pm.

We will be traveling to the Fireside Theatre to see this exciting comedy performance. The bus will pick us up at 9:30 AM at the Stoughton Senior Center and the show will be at 1:30 PM. Cost is $89.00 for the bus, dinner and show. Call 873-8585 to sign up by June 21.

News: COA & Community Commission on Aging Meeting Highlights for May 7, 2013 Center News: · Chose’s famous Coney Dog sauce and BBQ will be our Syttende Mai fundraiser this year; we appreciate our partnership with Tom Chose and the help of our many volunteers. · Staff offices are getting ergonomic ally updated. · We continue to receive positive comments about the outdoor digital sign. · Police Chief Leck provided staff with an inservice on building/personal safety. Accreditation · Staff is finalizing the Site Review Book, and once sent to the national committee we can expect an onsite review this Summer. · COA approved the new Evaluation Plan. · COA made recommendations to reference the City’s Disaster Plan in the case of a major city disaster and to be clear about the functions of the Emergency Planning Team. · Discussed goal to replace fire-proof file cabinets and an electronic file back-up system. Legislative Update: C. Heidenway shared the letter to the editor she submitted to the Oregon Observer, which requests the State Joint Finance Committee to consider accepting federal funds to support Family Care. June Highlights Sons of NorwayMandt Lodge Luncheon and Demonstrations Join us on a visit to the Sons of Norway- Mandt Lodge on Tuesday June 25th, at noon for a Norwegian luncheon and cultural demonstrations. Cost $15.00. The lodge is offering an Icelandic baked cod meal with potatoes, vegetables, cucumber salad, riskrem (rice pudding), bread/lefse, beverages and Norwegian baked goods. Several of the lodge members will be providing cultural demonstrations which may include rosemaling, Hardanger and others! The group will also share the history of the lodge, their building, and about Sons of Norway, a fraternal organization. The lodge is located just up the street at 317 South Page Street. Beginning this month we start monthly presentations about cultures around the world, given by individuals from those cultures who are current employees of Cummins, Inc. here in Stoughton.
