Revised National TB Control Programme DISTRICT TB OFFICER Dr. Ashish Chawala Civil Hospital, Ludhiana, District TB Centre 0161-2221660 99883-02708 dtopnldn@rntcp.org RNTCP was started in the state of Punjab in the year 2001 in a phased manner. All the districts were covered upto April, 2004. There are 134 TB Units (all 118 TB Units are aligned with NHM blocks). There are 284 Designated Microscopy Centres (DMCs) where TB is diagnosed by sputum smear microscopy by trained LTs. Patients having cough for 2 week or more are referred to these DMCs by Medical OfďŹ cers or their sputum samples are being transported to these DMCs. The diagnosis of TB is free. After diagnosis patient is referred to DOT centre which is nearest to his home for taking TB treatment. DOT Centre is located in each and every village of the state where ASHAs are giving treatment to these TB patients. DOT Centre are also available in all public health institutions apart from Private practitioners and community volunteers. There are 12216 DOT Centres in the state. An honorarium of Rs 1000/- for CAT-I and Rs. 1500/- for CAT II and Rs. 5000/- for MDR TB patients is being given to ASHA or community DOT Providers after completing of TB treatment. Treatment of TB and MDR TB is available free of cost. 9 Medical colleges of the state are also involved in the programme. NGOs are also participating in the programme as DMCs, sputum collection and transportation centres and DOT Centres. In Punjab, approximate 2 lac TB Chest symptomatic undergone sputum examination annually and above 40000 TB patients are put on treatment. The success rate of TB patients is 88% in the state of Punjab. For Multi Drug Resistant (MDR) TB patients Programmatic Management of Drug Resistant TB (PMDT) was rolled out in the state of Punjab in June, 2012 under which IRL Patiala was established where MDR TB is diagnosed in IRL Patiala. All the diagnostic tools for diagnosis of MDR TB are available as Solid, culture, Liquid culture, LPA and CBNAAT. The diagnosis and treatment of MDR TB is also free.

Financial assistance under State illness Fund through Punjab Nirogi Society is provided to cancer patients along with other life threatening diseases belonging to BPL families. 2. Mukh Mantri Punjab Cancer Raahat Kosh Society – Under this scheme 50.00 crores has been made available by Govt. of Punjab, for treatment of all cancer patients except Govt. employees and those having health insurance cover. An amount of upto 1.50 lakhs is made available for treatment of every cancer patient. Till date, sanctions worth about 53.55 crores have been accorded to hospitals for treatment of 4987 Cancer patients. 3. School children suffering from cancer are provided free treatment by Health Deptt. So far, 171 children suffering from cancer were referred to PGI, Chandigarh and 88 Children to Mohan Dai Oswal Cancer Hospital, Ludhiana Rs. 189.72 Lakhs has already been spent on them.

Janani Suraksha Yojana (JSY) is a safe motherhood intervention under the National Rural Health Mission (NRHM) being implemented with the objective of reducing maternal and neo-natal mortality by promoting institutional delivery among the poor pregnant women. The Yojana, launched on 12th April 2005, by the Hon'ble Prime Minister, is being implemented in all states and UTs with special focus on low performing states. 2. JSY is a 100 % centrally sponsored scheme and it integrates cash assistance with delivery and post-delivery care. The success of the scheme would be determined by the increase in institutional delivery among the poor families 3. The Yojana has identified ASHA, the accredited social health activist as an effective link between the Government and the poor pregnant women in l0 low performing states, namely the 8 EAG states and Assam and J&K and the remaining NE States. In other eligible states and UTs, wherever, AWW and TBAs or ASHA like activist has been engaged in this purpose, she can be associated with this Yojana for providing the services.
12. Implementation of PC & PNDT Act The Pre-Natal Diagnostic Techniques (Regulation and Prevention of Misuse) Act, 1994 (PNDT), as amended in 2003 to The Pre-Conception and Pre-Natal Diagnostic Techniques (Prohibition Of Sex Selection) Act (PC & PNDT Act), is a legislation to curb the abhorrent practice of sex determination and sex selection, which has its roots in India's long history of strong patriarchal influence in all spheres of life. This has translated into an obsessive preference for sons and discrimination against the girl child and women. It has spawned practices such as female infanticide, bride-burning and sati and lead to the neglect of the girl child in terms of nutrition, education, health care and her overall development. What is the punishment? A medical practitioner may get an imprisonment for a term that may extend to three years and with a fine that may extend up to Rs. 10,000 and on any subsequent conviction, he/she may get an imprisonment that may extend to five years and with a fine that may extend to Rs. 50,00020 The name of the medical practitioner would be reported to the State Medical Council by the Appropriate Authority for taking necessary

action, including removal of his/her name from the register of the Council for a period of five years for the first offence and permanently for the subsequent offence.21 Any person who seeks the aid of any establishments and professionals for conducting a prenatal diagnostic technique on any pregnant women for the purpose of sex selection would be punishable with imprisonment for a term that may extend to three years and with a fine that may extend to Rs. 50,000 and on subsequent conviction with imprisonment that may extend to five years and with a fine that may extend to Rs. 100,000. Who should one approach to lodge a complaint? A complainant can approach the designated Appropriate Authority of the State or district or sub district. The Appropriate Authority at the State level is a high ranking health department official above the rank of Joint Director of Health and Family Welfare. But there are also officials at local level in rural and urban areas who can be approached – the civil surgeon or the chief medical officer at the district level; the chief health officer or a ward health officer in a city; and the medical superintendent of the rural hospital in rural areas. As an ordinary person what can I do to help eliminate the practice of sex selection? i) Within your community or neighbourhood if you get to know of any sex selection being done, lodge a complaint to the Appropriate Authority listed in the annexure with appropriate evidence. ii) If you get to know about any doctor, radiologist, laboratory, clinic or any other person providing such services, lodge a complaint with the necessary evidence. For example, if you see board is not displayed in a clinic, you can inform the Appropriate Authority. iii) Be socially responsible and make others aware and spread the message about the illegality of the practice of sex selection and the implications of such a practice. Help to break common myths around issues such as increase in value of girls with decreasing numbers Whenever you go to a clinic be aware that: 1. Registered centres/clinics/laboratories must display their certificate of registration as well as the message that sex selection is illegal. If such a display is not there, it is an offence under the PC & PNDT Act. 2. The registration certificate of clinics/laboratories includes the number of ultrasound machines being operated there. 3. The clinic/centre should have a copy of the PC & PNDT Act. 4. Conduct of any genetic test or genetic counseling or a procedure involving imaging like ultrasound requires the use of a consent form for invasive tests or declaration in case of ultrasound tests. 5. Please inform the Appropriate Authority of your area if any violation of the above or of any other provisions of the Act take place. If there is no response from the AA you can complain to any NGO or go directly to the

AMA's Code of Medical Ethics Opinion 5.01 - Advertising and Managed Care Organizations A physician may provide medical services to members of a prepaid medical care plan or to members of a health maintenance organization which seeks members or subscribers through advertising. Physicians practicing in prepaid plans or managed care organizations are subject to the same ethical principles as other physicians. Advertising which would lead prospective members or subscribers to believe that the services of a named physician who has a reputation for outstanding skill would be routinely available to all members or subscribers, if in fact this is not so, is deceptive. However, the publication by name of the roster of physicians who provide services to members, the type of practice in which each is engaged, and biographical and other relevant information is not a deceptive practice. (II, VI)
Opinion 5.02 - Advertising and Publicity There are no restrictions on advertising by physicians except those that can be specifically justified to protect the public from deceptive practices. A physician may publicize him or herself as a physician through any commercial publicity or other form of public communication (including
any newspaper, magazine, telephone directory, radio, television, direct mail, or other advertising) provided that the communication shall not be misleading because of the omission of necessary material information, shall not contain any false or misleading statement, or shall not otherwise operate to deceive. Because the public can sometimes be deceived by the use of medical terms or illustrations that are difficult to understand, physicians should design the form of communication to communicate the information contained therein to the public in a readily comprehensible manner. Aggressive, high-pressure advertising and publicity should be avoided if they create unjustified medical expectations or are accompanied by deceptive claims. The key issue, however, is whether advertising or publicity, regardless of format or content, is true and not materially misleading. The communication may include (1) the educational background of the physician, (2) the basis on which fees are determined (including charges for specific services), (3) available credit or other methods of payment, and (4) any other nondeceptive information. Nothing in this opinion is intended to discourage or to limit advertising and representations which are not false or deceptive within the meaning of Section 5 of the Federal Trade Commission Act. At the same time, how-
ever, physicians are advised that certain types of communications have a significant potential for deception and should therefore receive special attention. For example, testimonials of patients as to the physician's skill or the quality of the physician's professional services tend to be deceptive when they do not reflect the results that patients with conditions comparable to the testimoniant's condition generally receive. Objective claims regarding experience, competence, and the quality of physicians and the services they provide may be made only if they are factually supportable. Similarly, generalized statements of satisfaction with a physician's services may be made if they are representative of the experiences of that physician's patients. Because physicians have an ethical obligation to share medical advances, it is unlikely that a physician will have a truly exclusive or unique skill or remedy. Claims that imply such a skill or remedy therefore can be deceptive. Statements that a physician has an exclusive or unique skill or remedy in a particular geographic area, if true, however, are permissible. Similarly, a statement that a physician has cured or successfully treated a large number of cases involving a particular serious ailment is deceptive if it implies a certainty of result and creates unjustified and misleading expectations in

prospective patients. Consistent with federal regulatory standards which apply to commercial advertising, a physician who is considering the placement of an advertisement or publicity release, whether in print, radio, or television, should determine in advance that the communication or message is explicitly and implicitly truthful and not misleading. These standards require the advertiser to have a reasonable basis for claims before they are used in advertising. The reasonable basis must be established by those facts known to the advertiser, and those which a reasonable, prudent advertiser should have discovered. Inclusion of the physician's name in advertising may help to assure that these guidelines are being met. (II)
Opinion 3.04 - Referral of Patients A physician may refer a patient for diagnostic or therapeutic services to another physician, limited practitioner, or any other provider of health care services permitted by law to furnish such services, whenever he or she believes that this may beneďŹ t the patient. As in the case of referrals to physician-specialists, referrals to limited practitioners should be based on their individual competence and ability to perform the services needed by the patient. A physician should not so refer a patient unless the physician is
conďŹ dent that the services provided on referral will be performed competently and in accordance with accepted scientiďŹ c standards and legal requirements. (V, VI)
