Dear Reader, Welcome to the winter edition of LIFE Magazine. With cold weather already upon us, it’s quite appropriate that this issue is focused on winter wellness. As the Executive Director of the beautifully renovated Lieberman Center for Health and Rehabilitation, I want to point out that February is Heart Health Month. Lieberman’s Center for Heart Health, in the Haag Pavilion for Rehabilitation, opened two years ago to meet the special rehab needs of individuals with cardiac disease. Led by a board-certified cardiologist at NorthShore University HealthSystem, this program focuses on the afterhospital care of individuals with congestive heart failure (CHF), those who have undergone coronary artery surgery or those with cardiac disorders that coincide with lung disease. In the last year, the Center for Heart Health provided cardiac rehab care to 59 patients who were admitted with a primary cardiac diagnosis. The clinical outcome results to date have been very positive: Data for Lieberman’s Center for Heart Health shows that through June 2014, no patients were re-hospitalized after 72 hours and only 1% of our patients required re-hospitalization after 30 days. To help people understand the critical need for a program devoted to heart health in a rehabilitation facility, Dr. Noel DeBacker, Medical Director of CJE SeniorLife and Lieberman Center, provides a medical update on congestive heart failure (CHF) and the benefits of specialized post-acute rehabilitation. He gives a brief explanation of CHF, its symptoms and how optimal treatment is holistic and mindful. I am also delighted to announce that Lieberman Center has been recognized and accredited by the International Commission on Accreditation for Rehabilitation Facilities (CARF), for its Inpatient Adult Rehabilitation Program as well as its Long-Term Person-Centered Care Program. Lieberman is the only skilled nursing facility in the Chicago area with a CARF-accredited inpatient rehabilitation program. Additional achievements in the past year include the introduction of dialysis, advanced wound care, infusion therapy, medicallycomplex orthopedic care and a Transitional Care Nursing Program that provides support, education and guidance to patients returning home after a rehab stay. Moving on from hearts to minds, January is Mental Wellness Month. In this issue of LIFE, you will find a very informative article on whether depression in older adults is a fact or a myth. Also, there’s an article on Linkages, a unique and innovative program for families who have adult children with disabilities. Finally, it is our honor and privilege to thank the thousands of individuals, donors, corporations, foundations, and government agencies…as well as the Jewish Federation of Metropolitan Chicago…who provide us with the necessary support to fulfill our critical mission of enhancing the lives of older adults. Medical Update Congestive Heart Failure and Post-Acute Rehabilitation

Congestive Heart Failure (CHF) is one of the most common reasons for admission to the hospital, and it is also the most common reason for re-admission to the hospital within 30 days after the person has been discharged. What is congestive heart failure? CHF is a condition arising when the heart is not able to circulate the blood properly, and the flow becomes sluggish. This results in backing up of fluid throughout the body, which leads to many of the characteristic symptoms. When there is an overload of fluid in the lungs, shortness of breath results. At first this occurs only with physical activity, but as time goes on, it can occur at rest. It can also occur at night and result in an attack of shortness of breath waking the person from sleep. It is generally easier to sleep with congestive heart failure when the head is elevated, so people often start using more pillows. Swelling of the ankles is another common symptom, although if this is the only symptom, it is more often the result of impairment in function of the veins in the legs. Other symptoms can include cough (especially with frothy sputum or blood), wheezing, dizziness or lightheadedness, frequent urination at night, weakness and mental confusion. The treatment of CHF involves careful monitoring of body weight because it is the best way to know if

fluid retention is occurring. Gaining or losing five pounds in a day is never due to fat loss, but instead to retention or loss of fluid, so daily weighing provides a key warning system. Salt (sodium) intake is also closely monitored because it results in further fluid retention. Medications such as furosemide (Lasix) are commonly used to help remove sodium and water from the body. Other medications may be used to reduce the load the heart has to pump against and to improve the heart’s ability to contract. Skilled nursing facilities providing post-acute rehabilitation are common destinations for people with CHF after discharge from the hospital. Some facilities have developed specialized units to meet the challenges that have contributed to hospital readmission rates. Lieberman Center has such a unit—the Center for Heart Health, where we provide close monitoring of salt intake and fluid balance, consultation with a Cardiologist, special nurse training to administer intravenous medications, exercise equipment to provide safe reconditioning and a number of other enhancements. in our spirit before we grow gray in our hair.” In essence, he was saying that just as day becomes night, growing older surely means growing weary, hopeless, and despondent…right? Wrong!
Q.
Is i t nat ural to e x p e r i e nc e dep r e ssion as w e ag e? There is a common misconception that older adults are more likely to be depressed than their younger peers; in fact the opposite is true. Depression is less prevalent among older adults, ranging from 1 to 5% of the population*. However, the rate of depression does rise among those with serious health problems and those living in institutional settings**. Depression is not a sign of weakness and it is not a normal response to getting older. It is a medical condition, like high blood pressure or diabetes, and it can and should be treated.

What c ause s dep r e ssion a mong ol de r ad ults ? Some of the difficult changes and adjustments of the later years can certainly contribute. After the loss of a spouse or friend, one’s independence or career, grief is normal and helps in the healing process—up to a point. When grief over a loss becomes unrelenting or disabling, depression may be in play. How can you tell if an older loved one is depressed—or recognize those symptoms in yourself? Red flags include: • Loss of interest in hobbies or other pleasurable pastimes. • Social withdrawal and isolation. • Noticeable weight gain OR weight loss, OR a marked change in appetite. • Sleep disturbances. • Loss of self-esteem. • Increased use of alcohol or drugs. • Fixation on death; suicidal thoughts or statements.
Older adults who deny feeling depressed may exhibit symptoms in disguised ways: • Preoccupation with aches and pains, both real and imagined. • Feelings of hopelessness/helplessness. • Irritability/crankiness. • Lack of interest in personal care. • Lack of concentration/uncharacteristic forgetfulness.

What sho ul d yo u d o i f an ol der fami ly member or f r i end i s di sp l ay i ng any sy mp toms of dep r e ssion? Open, caring communication is the best place to start. Speak simply and honestly about the changes you are seeing. If you find your family member willing to talk, then listen actively, without judgment. Make clear that it is not “wrong” to feel bad, but offer hope and a plan for helping. The first step should be a visit to one’s doctor to rule out medical reasons for one’s symptoms. Offer to assist in making an appointment and going along for support. Remember that a first conversation is just a beginning; hopelessness and a lack of motivation to follow through are symptomatic of depression. Talk show host Dick Cavett described his depression this way: “When you’ve got it, if there were a magic wand across the room that would make you happy and give you everything you want—it would be too much trouble to cross the room and pick it up.” Express your concern as many times and as long as it takes to connect, remaining supportive and nonjudgmental. If there is any concern about your loved one being suicidal, however, seek help without delay.

What ar e som e op t ion s f or t r eat i ng de p r e ssion ? Treatments include “talk therapy” (counseling with a trained therapist) and/or medication. Medicare covers approximately 80% of the cost of talk therapy; secondary insurance may cover the balance. CJE SeniorLife has been at the forefront of recognizing the mental health needs of older adults. We have offered counseling services since our inception in 1971—and we remain here to help. Our seasoned clinical social workers most often see clients at our Rogers Park and Deerfield locations. Arrangements can also be made for homebound individuals to receive services in their homes. The good news about depression is that it is eminently treatable. According to experts, most major depressive episodes are resolved within 6 months with appropriate treatment. No one, whether they are 18 or 80, has to live with depression as a life sentence. Life— however long we are blessed to have it—is meant to be lived! Linkages Support for Families of Adults with Disabilities By Nicole Bruce
I attend the Linkages support group because our 40-year-old daughter has a developmental disability. I have traveled from the south suburbs for over eight years to attend because there is nothing else like it. Once your child is older, there is no support system. The members of the group talk about everything going on with our children, and we serve as a sounding board for each other. Rosann, the facilitator, has a lot of information to share. It is a very close group that’s very welcoming to others—both women and men. We even have large family celebrations for the holidays and other occasions. The challenges that arise as one gets older can be complicated. But for aging adults who have children with disabilities, their concerns are even more complex since they often have growing financial, social, emotional and physical issues of their own. The gnawing worry about who will be around to care for their child with a disability can add even more stress. Statistics show that more than three-fourths of older adults with disabilities are aging in place with their parents, a number that will only grow during the “silver tsunami.” CJE is very proactive in addressing the concerns of the families and service providers in light of many recent policy changes affecting longterm supports for older adults and people with disabilities. CJE’s Linkages program supports older adults who have adult children with disabilities. It is unique because it is one of only a few programs like it in the country, and the only one in Chicago. Linkages was originally formed to reduce the potential institutionalization or homelessness of adult children with disabilities and to give support to their aging parents. New legislation, revisions and programs have created a “perfect storm” situation in which confusion and the need for more information abounds. This presents undue stress and challenges that are overwhelming for both families and service providers. These changes and implementations include the Affordable Care Act; the Illinois SMART Act, which caused major changes to Medicaid; the rollout of the Integrated Care Program; the Ligas, Colbert and Williams legislation; and the Governor’s Rebalancing Initiative, which is Illinois’ attempt to save money while also trying to expand and improve services. While these changes create the opportunity for improving quality of life and community living, they can also create fear of the unknown such as the possibility of limited choices and reductions in the quality of services and supports.

All seniors as well as parents of adult children with disabilities can meet with CJE’s Consumer Assistance Resource Specialists to learn more about the new Health Insurance Marketplace, Medicare plans, retirement, veteran’s benefits, real estate tax exemptions, financial management, and other government and community resources as well as longterm services for seniors and adults with disabilities. For CJE’s Linkages program there are a number of ways to connect. Information and referral services can be provided over the phone. There is a support group for parents of adults with disabilities, which focuses on more sensitive issues within a small group setting in an effort to reduce feelings of isolation and discuss the best ways to access community aging and disability resources in Illinois. As one parent in the support group commented, “I don’t know what I would do without CJE and the Linkages program. They have become family to me.” Linkages also presents a monthly information meeting for larger groups where guest speakers cover relevant topics such as the Americans with Disabilities Act, housing and financial independence for people with disabilities and services for people with mental illness. We hold social events twice a year, providing adults with disabilities and their families with an uplifting way to connect with one another. Fund Raising is a Calling
For some people, fund raising can seem scary or awkward. Not to me! While raising money to support CJE’s myriad of programs, such as Creative Arts Therapy and HomeDelivered Meals, I am energized—and proud—of the impact that CJE’s programs have on people’s lives. So asking for money that could improve a senior’s life is very important and not at all scary! Donor meetings are the highlight of my job. Mark Weiner, our President and CEO, and I strive to meet with as many donors as possible each year. It’s magical to personally meet a donor and learn why he or she feels a meaningful connection to CJE. I have heard many donors, some of whom are reaching retirement age themselves, reminisce how their grandparents depended on our services decades ago. These long-time givers are still thankful that CJE helped their loved ones age with dignity and grace. The heartfelt stories we hear are abundant and extremely powerful. Just recently: • An exhausted daughter cried tears of gratitude for our Adult Day Services. By providing her frail and memorychallenged mother with a safe place to go daily, the daughter had time to work, provide for her own sick husband, and yes, pick up her kids from school. • A devoted niece sent a tribute donation to CJE in honor of her great-aunt’s 87th birthday. She is thrilled that her aunt’s social calendar is so busy at CJE’s Weinberg Community that she also bought an answering machine for her aunt’s apartment just to “check in” and play telephone tag! • A 95-year-old grandma at Lieberman Center was devastated she couldn’t be at her grandchild’s destination wedding. Putting our knowledge of technology to good
use, we arranged to have her and other family members participate in the wedding via Skype. That’s what CJE is all about: Taking care of our “family” and making each person feel empowered. When we engage in conversations with our donors, it is an opportunity to make or strengthen a connection. It gives us the chance to inform a person about what’s happening today at CJE. It is a time to show donors how their support can inspire our mission to enhance the lives of seniors in our community. Yes, it is a tool to raise dollars, but we also receive resounding endorsements and create ambassadors for our good work. They, in turn, can share our stories with others in the Jewish and larger community. And so, the stories can continue, “L’Dor V’Dor.” As the Sr. Development Director, I want to personally thank our 2,500 donors for their generosity in Fiscal Year 2014. Through our annual campaigns and government and private grants, we raised over $5 million in donations which helps subsidize care so that many people pay little or nothing for critical services like Consumer Assistance, Home-Delivered Meals, Transportation and Creative Arts Therapy and so much more. In closing, I wouldn’t be doing my job if I didn’t ask you to help us sustain the future of CJE. In the words of the Jewish scholar Hillel: “If I am not for myself who will be for me? Yet, if I am for myself only, what am I? And if not now, when?” CJE wants to be here for you when you need us and with your support, we can also be here for future generations. CJE Joins the #GivingTuesday Movement!
Weinberg Community hosted a celebration of its long-time collaboration with Chicagoland Jewish High School (CJHS). Musical performances, refreshments and a good time was had by all.
Dedicated to Care. CJE SeniorLife had a big social media presence leading up to #GivingTuesday, a global day of giving held annually on the Tuesday after Thanksgiving to kick off the holiday season. As part of the movement, employees, volunteers and residents at several CJE locations posed for pictures with signs to inspire others to give by donating or volunteering on Tuesday, Dec. 2. The photos were shared on CJE’s social media channels, including Facebook and Twitter, as well as online at www.cje.net/givingtuesday.
On November 18, the Bernard Heerey Annex was dedicated at The Friend Center at Weinberg Community. The Annex features five new private suites and expanded, renovated space for Adult Day Services, a stimulating program for residents or community members with Alzheimer’s and related dementias. CJE SeniorLife is grateful for the generous support of the Bernard Heerey Family Foundation for this new space.
