LINDA Chapman of Pepper Pike is wearing her typical “work uniform”: black yoga pants and an asymmetrical yoga-style cotton/stretch jacket from Neon Buddha. Most of her colleagues at a young adult group home wear jeans to work, but the 50-something Chapman thinks she looks more professional in a nice, comfortable yoga outfit and notes her clients “are intimidated if I’m all dressed up.” After work, Chapman says her yoga clothes look perfectly appropriate for lunch or, with heels and funky jewelry, for a Saturday night out. “I love the yoga style,” she says. Here’s the kicker: She doesn’t even DO yoga! Tami Schneider, owner of Cleveland Yoga, gets that a lot. “Yoga clothes are comfortable, and they’re stylish, and they flatter most figures,” she says. This is largely because of new athletic fabrics used. As a yoga instructor, Schneider’s typical work uniform would be bootcut black yoga pants from lululemon with a tank top. “They have these little vests you can throw over a yoga top if you have to run into Whole Foods to pick up something for dinner,” she says. At Cleveland Yoga’s boutique, another big selling brand is “I love yoga,” which features T-shirts that proclaim “Love is where the heart is” or “Peace on earth.” “People buy those shirts because they want that statement to be felt by others who see it,” says Schneider. “They’re living the life.” Chapman’s favorite yoga brand, Neon Buddha, is a “wonderfully socially conscious company,” emphasizing fashion with ethical work standards and environment standards, adds Gail Rocco,

miraculous minutes:
finding beauty
in a hurry
by amy newman smith
SOMETIMES it seems like a dream, or perhaps a beautiful fairytale … lunch hours spent shopping for the perfect new outfit and afterwork outings for mani-pedis and facials! But in real life, with career and family taking focus, the time we allot ourselves all too often shrinks. The results are predictable: an everyday cycle of going for the quick and the easy, even if it doesn’t make us look – or, more importantly, feel – our best. The good news is, there are quick and easy things women can do to turn frazzled into fabulous.
face the world For starters, focus on your face; it’s the first thing people notice. Lisa Dottore, owner of Lisa Moran Ltd. clothing boutique, is quick to point out that “it’s more important to take care of yourself” than it is to chase the latest trends. Figure out a basic makeup routine that you can do in a few minutes, and make sure you do it every day. There may not be time in your morning for a full face, but mascara and lipstick can make a huge difference. Find products that do more than one thing in a simple step. For summertime, I love a tinted moisturizer that also includes sunscreen. Many companies make cleansers with built-in exfoliators that slough off dead cells at the same time as they remove dirt. Make the most of what’s out there to make the most of yourself.
pop and pizzazz To jazz up a wardrobe that may have fallen into the humdrum, start with some accessories you’ll use every day. Busy mother Yaffa Geller of Cleveland Heights channels her inner movie star with a glamorous pair of shades. You’ll look cool, plus your eyes (and the delicate skin around them) will thank you, she notes. Cleveland Heights resident Heather Price, who juggles two small children and her business as a Mary Kay sales director, recommends improving any outfit with a great bag. Find something you enjoy carrying … even if it’s stuffed with diapers and teething toys! With three small children at home, Rivky Weiss of

Cleveland Heights admits that it is hard to find time for even the most basic of beauty routines. As a pick-me-up, she swears by her favorite perfume. It takes just a second to apply a scent, and “When I have it on, I feel good about the world,” she says. Keeping your nails nicely filed and painted can give you the same kind of lift in just a little more time. Bonnie Chernikoff of Bonnie’s Goubaud boutique is a fan of investing in a fantastic coat. She recommends a pop of color in any outfit, and for fall and winter, you can find that boost in a colorful coat or wrap. Finally, line up a few fun and funky accessories that work with numerous outfits. Look for earrings, belts and necklaces that will ramp up your style in just a few extra seconds.
stay in style With less time for yourself, it’s important to shop and dress smarter, not more. University Heights resident Channa Ireland, who juggles family life and a job, says she makes sure to invest in a subscription for a fashion magazine that features on-trend, real-world looks. “If I find a look that I think will work for me, I save a lot of time when I go shopping because I know what I’m looking for,” she explains. You don’t have to chase all the trends to feel welldressed, says Dottore of Lisa Moran Ltd. Instead, she recommends women find one or two trends that speak to them and pair those with classic pieces. For example, this season it’s simple to pair a fitted T-shirt dress with tights and a necklace for an easy and feminine look. Every season has these looks; the trick is to find them. Chernikoff of Bonnie’s Goubaud advises women to ease the pressure of moving from one part of their day to the next by planning ahead. For example, pick a bottom that can carry you through a morning of carpool and errands to a luncheon meeting or dinner. Start with a sweatshirt on top, but put a cute and chic shirt underneath. of shoes and perhaps a different belt and jewelry; stow it in your car or at home, and you’ll move easily from one task to the next. Dress from the top down, Chernikoff adds. Shirts are a great place to add a little bit of ruffle or sparkle. You have to wear a shirt anyway, so make it something that will make you look and feel feminine. Last but not least, take a quick minute to look in a fulllength mirror. You may see a quick adjustment you need to make, but more importantly, knowing you look good when you walk out the door is the key to feeling confident all day long.
back to basics Nobody is recommending we bring back the corset, but all the same, a beautiful, confident outside starts with a fit inside. With deadlines looming and childcare and household tasks piling up, it’s hard to find time for ourselves but worth every minute. • Instead of decompressing in front of the TV or computer, consider spending some time with the Wii Fit. • Schedule a little in-home spa time. For example, Mary Kay sales

director Heather Price recommends rubbing cold whole milk on your closed eyes. The vitamin D and lactic acid will reduce puffiness and under-eye circles while moisturizing the skin. • Take the time to soak in the bath with scented salts or oils. While you’re in there, why not treat yourself to a quick scrub with a pumice stone or loofah to brighten up dull skin on your feet, knees and elbows. Just because the world won’t see it doesn’t mean it won’t make you feel great.L

BEYOND THE FACELIFT SURGICAL AND NON-SURGICAL OPTIONS
DR.STEVENGOLDMAN
Steven Goldman, M.D., FACS, FAAFPRS medical director Beachwood Plastic Surgery & Medical Spa 216-514-8899 www.drgoldman.com
In the past, a patient who came to a plastic surgeon seeking to erase signs of aging in the face had one basic option: the facelift. Today, a formative spectrum of surgical and non-surgical procedures allows each patient to develop a treatment plan that is tailored to his or her specific goals and needs.
before botox & restylane
before lower eyelid lift
after botox & restylane
after lower eyelid lift
On one end of this spectrum are the non-surgical enhancements, like skin care and the injectables. Medical-grade skin care products, such as Obagi, allow treatment of the texture of the skin, which historically was ignored with traditional facelifts. Skin treatments can minimize wrinkles, sun damage and age spots, smoothing and tightening the skin. Almost any patient concerned about signs of facial aging can benefit from skin care, with minimal financial expense and no real downtime. Like skin care, injectables require minimal downtime and are relatively inexpensive. When used skillfully in appropriately selected patients, they are very effective. This has lead to their incredible popularity. There are two basic types of injectables: botulinum toxin and fillers. Botox and Dysport are the two FDA-approved botulinum products; they are injected into muscles that make wrinkles in order to relax them. Fillers include Restylane, Radiesse, Juvederm and others; these can be used to diminish wrinkles. When injected directly into a wrinkle, the filler will plump it, partially erasing it. Fillers can also be used to sculpt and reshape the face, as in non-surgical rhinoplasty (nasal reshaping). Injectables are so effective, they are generally used instead of surgery in patients with early signs of facial aging and are used in conjunction with surgery to treat more advanced aging. Laser treatments can be used to remove unwanted hair, spider veins, pigmentation or wrinkles. The CO2 lasers are the most effective lasers used to erase wrinkles but historically have required general anesthesia and require prolonged healing time. Today, fractionated CO2 lasers use a special pattern of tiny dots on the skin to allow faster healing time. The ExelO2 is the newest fractionated CO2 laser. It is used in an office setting with just topical numbing medicine, no shots. Downtime is 3-4 days, so even patients with mild aging are candidates.
before neck lift
after neck lift
Surgery remains the most effective way to treat significant facial aging. It is often combined with non-surgical treatment like lasers and fillers, but contemporary surgical techniques also emphasize a spectrum of options. Some patients need only treatment of the eyelids and forehead. Some patients only need treatment of their lower face and neck. By tailoring the techniques to the patient’s needs and goals, benefit can be maximized and downtime minimized. Full facelifts produce the most dramatic results and are still the gold standard for patients who have significant aging. Today, patients deserve a thorough evaluation with an experienced, caring, and board certified plastic surgeon to determine what options are best for them.

Paul Mikhli, DDS Beachwood Dental 3690 Orange Pl., Suite 540 Beachwood, Ohio 44122 216-831-5661 www.BeachwoodDental.com
QUESTION Is teeth whitening safe? ANSWER When applied properly, whitening is very safe. The main concern is tooth or gum sensitivity. This is common with improper use. However, if directions are followed, discomfort is rare. Whitening does not weaken teeth as commonly believed. Be very wary of whitening kiosks in malls. These are not dental care providers who can explain the risks and benefits. QUESTION What are whitening options? ANSWER There are three main whitening options. Drug store kits offer good value. Downsides are lower concentration and generic application. The latter may lead to uneven whitening and increased sensitivity. The dental office options are custom take-home trays or in-office whitening. Custom trays are used for 14 consecutive days at home; they isolate the whitening gel to the teeth with minimal contact to the gum tissue.
The in-office option is a two-hour appointment that will whiten your teeth as you relax in the dental chair. This is great for people who need their teeth whiter on a short timetable or for those who don’t want the hassle of wearing trays for two weeks. QUESTION Can anyone whiten his or her teeth? ANSWER If you have bonding, crowns or veneers on your teeth, they will not change color and can end up looking darker than your brightened natural teeth. If you plan on needing some dental work on your front teeth, whiten first and then have the restorations match your whitened teeth. QUESTION How long will it last? ANSWER Depending on how much staining foods you eat (or if you smoke), it will last 3-6 months and sometimes longer. However, all options can be easily touched up at minimal expense and can provide a permanent solution for a brilliant smile.

before pearl laser treatment
Having a tan complexion has often been associated with a healthy appearance. Today we know that excessive exposure to ultraviolet light can have detrimental effects on our skin. Sun exposure over a period of time can cause accelerated damage, leading to areas of increased pigmentation, fine lines or wrinkling. Aging can also adversely affect the appearance of skin by degeneration of collagen and elastic fibers, leading to fine lines and wrinkles. At the Center for Plastic & Cosmetic Surgery, we can rejuvenate your skin using state-of-the-art technology with our new Pearl laser. As the first in Northeast Ohio to offer this new technology, we are expanding office-based treatments to include a Pearl facial rejuvenation laser to renew skin’s surface with quick, visible results and minimal downtime. The Pearl laser procedure combines the impact of resurfacing lasers with the gentleness and safety of non-invasive procedures to treat aging and sun-damaged skin. Pearl treats fine lines on the face and neck, uneven skin texture, and light and dark spots caused by sun damage with fewer treatments and less overall recovery than other technologies. It provides a healthy, pearl-like glow. The Pearl laser replaces the entire top sun-damaged layer of skin with new healthy skin. Simultaneously, it sends heat to deeper layers of the skin, stim-

ulating new collagen growth, filling out and decreasing fine lines and wrinkles. New collagen growth continues for 4-6 weeks post-treatment. The combination of new skin and decreased wrinkles results in a healthier, more youthful appearance. Unlike “fractional” treatments, Pearl treats the entire face, minimizing the number of required treatments. Typically, only 1-2 treatments are necessary to renew your skin. There is also the Pearl Fractional laser, a new voluminizing fractional laser perfected for photoaging in just one treatment. Pearl Fractional offers the best combination of patient experience and recovery time. Patients will see improvements to photo damage, tone and texture, as well as a noticeably smoother, brighter complexion. Fractional therapy describes a treatment that affects only a fraction or small percentage of the skin. With Pearl Fractional, laser pulses selectively remove columns of damaged skin, leaving surrounding skin intact. The treatment shortens healing time and stimulates the body’s own natural process for creating new collagen, plumping the skin from below, and reversing photoaging and other imperfections. Pearl Fractional laser treats deeper dermal imperfections, acne scarring, wrinkles and facial lines in one treatment. Pearl and Pearl Fractional laser treatments can be performed simultaneously to maximize your results with a single scheduled downtime.

BODY CONTOURING HYPE VS. RELIABLE METHODS

These days, patients who see me for consultations about body contouring often ask me what method of liposuction I use. Similar to the ads run by the prescription drug companies pushing up-to-the-minute drugs, new forms of liposuction are used as hot PR tools for many plastic surgery practices. The result is patients with many questions about which technique is the “best.” I think it’s great that people stay up-to-date on the many available procedures, but it’s important to separate the marketing hype from the tested and reliable practices.
before
after
There are a number of liposuction techniques currently in use. One is tumescent liposuction, which was introduced 15 years ago to rave reviews. It took some time before the tumescent liposuction system was widely accepted and used, but it has proven to be the standard by which other liposuction techniques are judged. The main benefits of the tumescent procedure are a significant reduction in bruising and recovery time and that it can be performed under a local anesthetic, if preferred. The next innovations in body contouring were the “ultrasonic” and “powerassisted” techniques. These proved to be of some benefit to the surgeon by making the task of performing the operation a bit easier but really provided little, if any, benefit to the patient.
before
after
The latest method to be introduced is laser-assisted liposuction. When you add the word “laser,” it implies advanced technology, which makes for great PR. Unfortunately, the scientific data to support its use are not there. At a recent cosmetic surgery symposium I attended in Miami, Fla., not a shred of evidence as to the efficacy of the laser liposuction machines was presented, despite multiple requests by the panel of experts. Considering the risk of skin burns associated with laser use and the lack of definitive science supporting it, I’ve chosen not to use this method at this time. Wanting to stay ahead of the curve on innovations, I have tried all of the aforementioned methods of body contouring. For now, I find that tumescent liposuction remains the standard for excellent outcomes with minimal risk, compared with the newer techniques. I base my surgical methods on science and results, not on the PR advantage it provides. As with any surgical procedure, the outcome is only as good as the surgeon performing it. When looking for a surgeon, choose wisely, based on experience and skill.

The Natural ® (Vector) Facelift
T
he Natural (Vector) Facelift® is a method of tightening the face in a manner that gives the most natural and relaxed appearance after surgery. This facelift technique tightens the facial tissue under the skin in multiple directions or vectors. The tissue is then returned to its original more youthful position. The skin is then gently positioned over the naturally arranged tissue layer, resulting in a smooth and supple appearance, not the severe, tightened look often seen in facelift surgery. The Natural (Vector)
Facelift® is time efficient, taking only a few hours. The tissue is handled with extreme care which minimizes bruising considerably and facilitates prompt healing. The average recovery period is 10-14 days; this allows the sutures to be removed and any negligible swelling or bruising to diminish. Our patients can return to work in 14 days, safely and with confidence. For the most part, only minimal discomfort is ever reported after this procedure because it is done in such a precise and gentle manner. Surgery is performed in a state-of-the-art outpatient Surgery center with specialized nursing staff and post operative care. The Natural Vector Facelift® is exclusive to The Cosmetic Surgery Institute.

QUESTION AND ANSWER
DR.SONJASTILLER
Sonja Stiller, M.D. 2000 Auburn Dr., Suite 130 Beachwood, Ohio 44122 800-660-VEIN www.veinclinics.com
QUESTION I have varicose veins. What should I look for when choosing a physician? ANSWER Varicose vein disease can be very complex, and each patient is unique. Therefore, it’s important to find a physician experienced in treating a wide spectrum of vein disease, from small spider veins to large ropey varicose veins to leg ulcers. In making your choice, consider the following: • The right doctor: Many health providers offer vein treatments – but not all are equally qualified. Patients should seek a phlebologist, or one who is uniquely trained to manage venous disorders. At Vein Clinics of America, physicians meet rigorous standards of training and education. They must complete a four-month preceptorship, followed by a year of intense mentoring. They are also required to become certified by the American Board of Phlebology.
• Insurance coverage: Patients should ask whether their provider’s coverage is accepted. Vein Clinics of America is in network with most major insurance plans and will work with you to maximize your benefits. • The best diagnostics: It is essential to find a specialist who utilizes ultrasound during diagnosis and treatment, since vein disease is not always visible on the skin’s surface. • Comprehensive treatment offerings: Patients should choose a physician who treats a wide spectrum of venous disorders. Particularly important is finding a specialist who is skilled in both sclerotherapy and endovenous laser treatment, a combined approach that ensures the physician can treat both spider and varicose veins. Start with a consultation to discuss your options with an experienced physician.

QUESTION What is the definition of obesity? ANSWER Traditionally, obesity was defined as more than 20% over ideal body weight, which takes into account age, height, sex and build. The National Institutes of Health define obesity as a BMI (Body Mass Index) of 30 and above. BMI is a person’s weight in kilograms divided by height in meters squared. It correlates strongly but not completely in adults with total body fat percentage. QUESTION Why is it especially important for a woman of childbearing age to be conscious of her weight? ANSWER This is a very important and little-known aspect of the problem of obesity. A woman is less likely to be able to successfully conceive if obese. Obesity also leads to proven increased risks during pregnancy,
including miscarriage, stillbirth, infant death, maternal death, gestational diabetes and infection. If obese, a woman is 600% more likely to be hospitalized during pregnancy and 40% more likely to bear an infant with a major birth defect. High blood pressure in pregnancy is very dangerous to the mother. It can cause death of the fetus or strokes in utero, also called cerebral palsy. Rate of high blood pressure during pregnancy is increased in obese mothers by 1,000%. “Obesity among women of childbearing age is at a crisis level,” says Dr. Jennifer L. Howse, president of the March of Dimes. “We are concerned because it means more women are overweight or obese when they become pregnant, and this can have serious consequences for both mother and baby.”

QUESTION There are around 70 million people in the baby boomer generation. What are the dental issues they face?
problems in one’s 40s or 50s rather than in one’s 60s or 70s, when one is retired, has more health issues, and is less able to proceed with treatment?
ANSWER Baby boomers are facing choices in their dental health that will affect them for the rest of their lives. The most pressing issues are aging teeth and old dentistry that is wearing out. For too long, most boomers have not faced their dental issues, and/or their dentists have not had the chutzpah or knowledge to present to them a comprehensive dental approach.
Occlusal disease, gum disease, and “The Aging Smile” are the main reasons for tooth loss to this population. Comprehensive dentistry is an approach that works, rather than a piecemeal approach that often has mixed results at best. The most common dental services for boomers are porcelain veneers, crowns, bridges, implants, cosmetic dentures, and performance mouth guards. Boomers don’t want a yellow, worn-down, aging smile, which I term “Ugly Smile Disease,” or USD. Today, a smile can be easily restored to a healthy, vibrant and beautiful state in just a few appointments after proper diagnosis and planning.
More often than not, the cheapest route was chosen, and that merely put off the inevitable. Wouldn’t it have been better to deal with mid-life dentistry

QUESTION Why is sunscreen important, and which ones should I use? ANSWER Sunscreen can help protect you from health and cosmetic dangers. Skin cancer is caused by ultraviolet radiation from the sun and tanning beds. Additionally, radiation damage is a main contributor to wrinkles and sunspots or “photoaging.” Skin cancer results from accumulated sun damage, and that damage accumulates more quickly if you are fair-skinned, because pigment in the skin acts as a natural sunscreen to block radiation. Sunscreens should be incorporated into daily moisturizers, particularly if you are prone to burning. Keep in mind that the SPF factor holds true only if

enough sunscreen is used. Most of the time, a thin layer is applied that gives less than half the value stated on the bottle. Also, SPF is a measurement of protection against UVB, so make sure that the sunscreen contains UVA blockers as well. Micronized zinc oxide and titanium dioxide are excellent blockers of both UVA and UVB. Other UVA blockers to look for include Parsol, Mexoryl or Helioplex. Many sunscreens break down in the sun and therefore need reapplication every 1-2 hours. Sunscreens come in creams, lotions, sprays and gels, and many are very easy to apply. Find one you like so that you’ll use it.

reflections of home
MARY and Bernie Lerner,
world travelers, nature lovers, art enthusiasts and gracious hosts, wanted a house that reflected who they are. In 2000, architect Bill Morris started designing their spacious 4-bedroom, 5-1/2-bath contemporary house sited on four 36

Slate-floored foyer displays artwork in the spacious Aurora home. Below, outdoor pavilion welcomes guests to front door.
They had no need for a formal dining room – “That’s not us,” Mary says – but they frequently entertain large crowds at concerts, discussion groups and art shows they host at their home. Most importantly, the empty nesters wanted the house to “be part of outside” with lots of trees and greenery, says Mary. The couple, who previously lived in a condo across the lake from their current home, moved into their new residence in September 2002. Morris’s 10,000-square-foot design plus additional meeting and guest space over the garage fulfills all the Lerners’ wishes and more. Despite its expansive size, the taupe-sided home with wood-shake roof nestles into a tree-studded rise of land. Flower-filled perennial gardens and shrubbery step up to a broad covered entry; a manicured lawn sweeps down to the lake; and an intimate freestanding pavilion with greenery and blooms welcomes visitors. Upon entering the front door and passing through the slate-tiled foyer, guests see a large framed poster advertising the musical “Anything Goes.” “They walk in, saying, ‘Wow, this huge house,’ and the first thing they see is that silly poster,” says Mary, laughing. “That sets the mood. They see we don’t take ourselves too seriously.” To bring the outdoors inside, Morris designed the house so its occupants are never far from a wide expanse of glass. In the high-ceilinged rooms, walls of windows provide continual views of flora and fauna.

The Lerners host dinner parties in this broad corridor, which takes the place of a formal dining room. Below, Mary’s cozy reading room looks out to perennial gardens.
Instead of a separate formal space for dining, Walden developer Manny Barenholtz suggested using a long, wide corridor that would also provide access to other areas of the house. For the last eight years, a traditional round wood table, which could be opened up to seat 14, sat on an Oriental rug in the white-oak floored, glass-walled dining corridor. This summer, the Lerners replaced the heavy table with a racetrackshaped glass one etched with a motif of branches. The eight-foot-long table sits beneath an alabaster-colored glass-and-metal light fixture, one of about 20 similar pendants in the home. All were designed by John Koncar, who is responsible for the

antique Chinese wood transoms, hand-carved with trees, branches and birds; these timeless transoms, over 100 years old, fit perfectly over the entries into the family room and into the study beyond. The galley kitchen is part of the family room. There are almost no upper cabinets, so while cooking or eating at the breakfast counter, the Lerners can enjoy outdoor vistas glimpsed through the glassy corridor behind the kitchen. Earth-toned granite counters, taupe laminate lower cabinets, oak floors and stainless steel appliances complement the open space. All the glassware and dishes are stashed in drawers beneath the kitchen counters. A large walk-in pantry nearby also stores bulky serving pieces. The Lerners’ favorite spot to sit and watch the birds, read the newspaper, and enjoy meals is at a round glass table with four upholstered dining chairs. One of Morris’s signature architectural elements, two large sheets of glass meeting at the corner, allows unimpeded views of the lake and breathtaking grounds. Aristotle Enterprises Inc. designed the exterior landscape, which is maintained by T.C. Hurt Landscaping Inc. Off the kitchen is a striking powder room, with mirrored walls and ceilings creating infinite reflections. Beyond the main living areas, a book-lined study with a wood ceiling houses scrapbooks of photos from the couple’s travels. On the other side of the dining corridor is Mary’s private retreat, a cozy room with a wood ceiling and glass walls on three sides. During the day, the room looks out on perennial gardens. 44

Near the bedroom, the laundry room features a wall of 16 portraits of Mary painted by street artists in various cities across the globe. Bernie also takes a photo of each artist painting Mary. The couple came to realize that each quick likeness reflected the ethnicity of the area they were visiting. In Peru, Mary has darker skin tones. In Oslo, she looks dreadfully solemn. In Paris, she’s got a sophisticated, literary air. Upstairs, built over the garage, are two enormous multi-purpose spaces, complete with mini-kitchenette, a painting studio for Bernie, a guest bedroom and two bathrooms that share a shower. The Lerners have held art shows there, hands-on art workshops and classes. The house is very large but with a well-designed traffic pattern; Mary insists that aside from the upstairs multi-purpose space, she and Bernie live in every room. The home they love is not only eminently livable; it’s the perfect backdrop for this couple’s unflagging interest in their community and the world around them.L
