Surgery Patient Checklist In order to make your surgery day a pleasant, stress-free experience, we have made a list of patient responsibilities. If you have any questions, please let us know.

Fill out your consent form BEFORE surgery day (please wait to fill out the last page until you come in).

Have your payment type arranged PRIOR to your surgery.

Acceptable payment types include:  Cash  Visa, Mastercard or Discover  Money order  Cashier’s check  Financing (Care Credit)  Debit Card (you MUST release your daily limit PRIOR to coming in for surgery) Unacceptable forms of payment include:  Personal check  Third-party check Be sure to tell Beyer Laser Center if you have insurance so we can check on surgery discounts. Reporting your insurance after surgery will NOT yield a discount.

Fill your prescriptions and get any additional medications (i.e. moisture drops).

Understand that punctal plugs are NOT included in your surgery price. They are generally covered by medical insurance. If you don’t have medical insurance, we can offer a cash price for the plugs.

Understand the terms for enhancements as printed on your Global Fee Sheet You must have a driver on the day of surgery.
