The information in this document is a guide of how to access your benefits, and it provides a high-level overview. Columbus Academy employees that work 30 or more hours a week are eligible for benefits on their date of hire. Benefits are available for employees and their eligible dependents. Please see the Business Office to confirm your eligibility and offerings.
Columbus Academy benefits are administered on a calendar-year basis, and all plans are effective January 1st of each year. An annual open enrollment period is offered to allow you to enroll in benefits, make changes or add eligible dependents without requiring a Qualifying Event. Once your selection has been made, it will remain in effect until December 31st of the upcoming benefit year. A Qualifying Event such as, but not limited to, divorce, marriage, birth or adoption must be reported to the carriers and the Business Office within 30 days of the event.

When filling prescriptions, you have the following copay obligations for up to a 31-day supply for retail, and up to a 90-day supply for mail order:

Specialty medicines require special handling, administration or monitoring of effectiveness and side effects. Your health plan has an enhanced feature to help you if you need to take a specialty medication. Features include:
• You will be limited to a 30-day supply per fill – this helps you not get more medication than you need if you experience side effects or need to change medicines/doses
• Specialty drugs need to be filled through either Accredo Specialty Pharmacy or Gentry Health Services. When your medicine runs low they will arrange for your next refill and they’ll send free supplies such as syringes and sharps containers, if needed.
Health Spending Accounts
Flexible Spending Account (FSA) - Health Equity
The Healthcare FSA allows you to pay for qualified medical, dental, and vision care on a pre-tax basis. Your FSA is a “use it or lose it” account, as a minimal amount carries over from year to year. Current IRS FSA Limits are $3,200, and $640 for carry-over. Projected 2024 IRS health FSA Limits are $3,300, and $660 for carry-over.
Dependent Care FSA (DCFSA) - Health Equity
Childcare is a significant expense for many families. Contributing to a dependent care flexible spending account (DCFSA) allows you to pay for eligible dependent care expenses with pre-tax dollars from your paycheck. The maximum annual contribution limit for those that are married and filing jointly or a single parent is $5,000 and the maximum for those that are married and filing separately is $2,500.
Health Savings Account (HSA) - PNC
The HSA is a bank account in your name that allows you to pay for qualified medical, dental, and vision expenses on a pre-tax basis. You must be enrolled in a HDHP to be eligible for the HSA. The IRS sets limits on the amount that you can contribute to HSAs each year; the 2024 limits are $4,300 if you are enrolled on your medical plan as an employee only and $8,550 if you are enrolled on your medical plan with any dependents. If you are over the age of 55, you can contribute an additional $1,000 per year. One great component of HSAs is that any unused funds at the end of the year continue to accumulate indefinitely until you use them – so the money is never forfeited.

* When you receive services from a Nonparticipating Dentist, the percentages in this column indicate the portion of Delta Dental's Nonparticipating Dentist Fee that will be paid for those services. The Nonparticipating Dentist Fee may be less than what your dentist charges and you are responsible for that difference.
M a x i m u m P a y m e n t – $1,500 per person total per calendar year on diagnostic & preventive, basic services, and major services. $1,500 per person total per lifetime on orthodontics.
M a x i m u m C a r r y o v e r – If at least one Covered Service is paid in a Calendar Year and the total Benefit paid does not exceed $750 in that Calendar Year, $250 will carry over to the next Calendar Year’s Maximum Payment. This amount will accumulate from one Calendar Year to the next, but will not exceed $1,500. If no Covered Services are paid during a Calendar Year, all accumulated carryover amounts from previous Calendar Years will be forfeited.
D e d u c t i b l e – $50 deductible per person total per calendar year limited to a maximum deductible of $150 per family per calendar year on all services except diagnostic and preventive services, emergency palliative treatment, sealants, brush biopsy, X-rays, and orthodontic services.
