Beginning to Put the Pieces Together When Caring for a Loved One With Dementia: An introduction from the Notre Dame Health Care Memory Care Team

W

e hear the terms “Alzheimer’s” and “dementia” very often these days, and too many of us have loved ones who are living with the effects of these conditions. We are working to open up discussion in our community to increase awareness and understanding. To start, dementia is a decline in brain function affecting at least one of the following: abstract thinking; judgment; complex language; complex physical or mental tasks; recognizing persons, places, or objects; and personality changes. Alzheimer’s is the most common type of dementia. Dementia attacks the brain asymmetrically, usually affecting the left side first. The left side of brain is

responsible for short term memory and language, while the right side retains rhythm and long term memory. As Dr. Lisa Genova—best-selling author of Still Alice—shared at our Educational Forum in June 2016, the emotional part of the brain remains unchanged, and so individuals with Alzheimer’s and dementia still experience the full range of adult human emotion. You can continue to have quality experiences with your loved one by living in the moment, laughing with them, reminiscing, reading, poetry, listening to music, and just holding their hand. Remembering the past is often soothing for individuals with dementia, and it’s ok to skip current

events and focus on memories from long-ago during your time together. Music can also be a helpful tool. Studies show that 98% of individuals with dementia recall music, poetry, and prayer. According to Music and Memory founder Dan Cohen in an interview with FastCompany, “Since our music, emotions, and memories are so connected, we have something that's very vibrant and alive regardless of whether we know where we are and who we are talking to.” It helps people “feel like themselves, so they're less likely to be frustrated that they can't communicate.” We agree that “… music is more effective than any drug…, and, of course, music also has none of the Dementia page 11

Self care can feel indulgent, yet in order to be the best you can be for your loved one, you must take care of your own health. oo many people who have conventional dentures don't wear them for the simple fact that they don't stay in place. P

hysical Therapy is a service available to people to develop, maintain and restore lost movement in order to maximize independence throughout life. Home Physical Therapy is primarily for those individuals that are homebound, unable to leave the home for therapy in an outside clinic. Often, in-home therapy is covered under your insurance. As a therapist entering a client’s home, our therapists look at all factors that may limit independence and safety, whether due to physical, environmental or social limitations. A Home Safety Assessment provides information on environmental factors that may impede safety and independence, putting clients at greater risk for falls such as narrow doorways, area rugs, stairs and inaccessible rooms.

A Therapeutic Evaluation helps to identify any physical limitations to one’s ability to function with regard to strength, balance and mobility. A key component during the evaluation is discussing difficulties with managing everyday tasks which occurs with the patient as well as family/caregivers and other healthcare professionals involved in the patient’s care. Once problem areas are addressed, it is the role of the physical therapist to develop possible solutions/treatments that will increase the client’s level of function, maximizing the safety of all involved, and preventing or limiting permanent disability. A treatment plan is always tailored toward a patient’s individual goals. Home Physical Therapy allows for

one-on-one treatment time which can directly influence the effectiveness of intervention. It allows for clients to participate in a program in the comfort and convenience of their own home, promoting compliance with the established program. Physical therapy in a patient’s home can be rewarding for both patient and therapist. By Catherine Walsh It’s common knowledge that most of our aging population would like to remain in the comfort of their own home for all their days. Whether your home is in a house, an apartment or a senior community, the services of home care professionals can make that a reality. What is Supportive Home Care and what services do they provide? Supportive Home Care can provide personal care, homemaking, companionship, transition care, shopping, errands, meal preparation, laundry and escort assistance to and from appointments. And they can provide respite for family members who may be doing some or all of the caregiving themselves. Many have an RN on staff who will provide an assessment of both the patient and their surroundings to create an individualized care plan.

Reverse mortgages: It's about time

R

everse mortgages often conjure dire outcomes. But the facts are just the opposite. With changes to the government-insured loan program, formally called a Home Equity Conversion Mortgage (HECMs), the risks to the senior homeowner has diminished, and, under many circumstances, results in a drastic cut in costs. Financial and estate planners, tax specialists, attorneys, religious leaders, housing counselors, and even the adult children of seniors are coming around to the fact that reverse mortgages give seniors financial options to remain independent in their own homes. Lori Trawinski, director of banking and finance at the AARP Public Policy Institute, writes, "We think reverse mortgages can be a useful tool for some people.” AARP does not endorse any reverse

mortgage lender or product, but wants its members to have the information they need to make an informed decision about these loans. I couldn't agree more. Everyone should understand the merits of a reverse mortgage, but not everyone should get one. Some of the many reverse mortgage benefits are: • No required monthly mortgage payments on borrowed funds, which allows for increased cash flow (but property taxes and insurance must remain current and the property maintained) • Access to tax-free cash if needed at a later date • Option to receive monthly cash for as long as one lives in the home • Ability to receive funds for home repairs or pay off other debts

ince Access NSM (former l y known Hudson Accessibility) began its business 35 years ago, there have been incredible advances in the science of accessibility and the types of products available to address the numerous needs of our aging population. And following the signing of the Americans with Disabilities Act some 26 years ago, there has been a dramatic increase in the quality and quantity of innovative products. For example, just a few short years ago, people with mobility issues had to have a ramp built out of wood or steel to gain access to their home, often calling for a large cash outlay. Today, there is a wide variety of modular ramp styles available. These ramps can be ordered in your choice of material including aluminum, wood or steel.

For added convenience, Access NSM has options for rent or purchase and offers an easy to use finance plan. Another area that has seen major technological advances is stair lifts. The earliest stair lifts resembled a personal elevator, often requiring major home renovations to accommodate them. Today's chairlifts feature compact, modular designs that can be customized to nearly any staircase without the need for stairway modification. Make your bedroom an oasis – try to keep out or minimize electronics. 4. Paint, draw or doodle - Using your brain for a creative artistic pleasure can be fun and challenging. 5. Listen or play music - Listening to music can lift your spirits but singing or playing an instrument is also an excellent way to stretch your brain in a new way. 6. Learn something new - Many colleges and senior centers offer engaging, low-cost lectures and classes for older adults. And there are many free webinars and classes that can be viewed online as well. 7. Write - Whether you are finally writing that novel you have been meaning to or in a private journal or a letter to the editor of your local newspaper, writing forces you to process your thoughts in a logical, particular way. 9. Socialize - Join a friend or family member for a regular activity such as exercising, going to the movies or other fun pursuit. 10. Volunteer - Volunteer at an animal shelter, school, library, hospital or an organization of your choice. Not only will you be helping the organization, you will feel invigorated and full of purpose.

What services are typically available?

What are some alternatives to assisted living?

Family, friends or home health services can sometimes provide needed assistance in one’s home. Another option might be moving in with adult children. Seniors who qualify for independent living may choose to move to a retirement community (housing, social activities, recreation

and some services, without the daily personalized support of assisted living). If ongoing skilled nursing care is required, a nursing home could be a better alternative. Staying in one’s current home while receiving outside help may seem to be the most attractive alternative, but it is not always the best solution for extended needs.
