Chronic heart disease (CHD) is a leading cause of disability as we age. Managing CHD goes beyond extending life—it’s about improving quality of life with dignity and comfort. People with CHD face physical, emotional, and social challenges, requiring comprehensive and compassionate care.

CHD includes conditions like congestive heart failure, coronary artery disease, heart valve problems, and arrhythmias; often leading to symptoms like shortness of breath, fatigue, chest pain, and fluid retention. Although medical advances have improved treatment, CHD remains a lifelong condition needing continuous care and support.

A patient-centered approach is vital for managing CHD. This means addressing each person’s unique needs, values, and preferences. Open communication with healthcare providers is essential to ensure informed decisions about medications, treatments, and lifestyle changes. As the disease progresses, it’s important to focus on maintaining dignity and comfort, and for many, that includes aging at home.

Only one entry per person. Photo copies are allowed but duplicate entries from the same person will not count. All information needs to be filled in - Name, address, phone number to be counted. Be fair - even if you love a certain restaurant, not all restaurants sell pizza, or serve breakfast.., make sure you vote for the best. All categories need to be written in. Writing one name on the top line and putting an arrow through every line will not count towards the vote. In the event of a tie, each winner will receive an award and recognition. Contest winners will be drawn at random and notified via phone. Winners will be published in an upcoming issue. Culinary Award Winners will be awarded a certificate suitable for framing, and winners will be announced in the April issue. Be sure to watch the April issue to see if your favorite won!
