GETTING STARTED The Connie Portal is where Connie authorized users can access applications and services. The Portal allows authorized users to access approved patient records for the purpose of treatment, operations, and payment. This guide provides step-by-step information on how to access the basic functions of the Connie Portal. Additional User Guides are available for specific applications and features that can be accessed within the Portal.

Click on the “Reset your password?” link, and follow the onscreen instructions


In order to improve security, Connie requires that all users set up Two-Factor Authentication for their portal account.​ For more information about setting up 2FA, see Connie Portal Two-Factor Authentication Guide



To conduct a patient search, users must provide at the minimum:

FIRST NAME

LAST NAME

DATE OF BIRTH

*Gender and SSN can be added to Patient Search to refine results​


The results of Patient Search will appear in order of most likely to least likely matches

You may select the 'Clinical Data' option to navigate patient records.


How to launch a user application

If launching an application for patients with whom users do not have an active relationship, the following message will appear: Selecting ‘cancel’ will return users to ‘Patient Search’

To proceed, users will be asked to enter a reason for attesting to the relationship
